Scoring Methods of Polysomnography for Diagnosis of Sleep Apnea in Adolescents (청소년에서 수면 무호흡 진단을 위한 수면 다원 검사의 판독 방법)

  • Background and Objectives Respiratory scoring guidelines for children and adults have been used for evaluating adolescents both in the 2007 and 2012 American Academy of Sleep Medicine (AASM) scoring manuals. We compared the scoring methods of polysomnography used in these scoring manuals, where pediatric and adult scoring rules were adopted for the diagnosis of sleep apnea in adolescents. Subjects and Method 106 Korean subjects aged between 13 and 18 years were enrolled. All subjects underwent overnight polysomnography in a sleep laboratory. Data were scored according to both pediatric and adult guidelines in the 2007 and 2012 AASM scoring manuals. Results Both pediatric and adult apnea hypopnea index (AHI) using the 2012 method were significantly higher than those using the 2007 method. The difference in AHI compared between pediatric and adult scores with the 2012 AASM scoring system was markedly decreased from that with the 2007 method. There was a significant discordance in sleep apnea diagnosis between pediatric and adult scoring rules in the 2012 method. Conclusion Both pediatric and adult rules were used for the diagnosis of adolescent sleep apnea in the 2012 method. However, there was significant discordance in the diagnosis between pediatric and adult scoring guidelines in the 2012 AASM manual, probably due to different cut-off values of AHI for the diagnosis of sleep apnea in pediatric (${\geq}1$) and adult (${\geq}5$) patients. Further studies are needed to determine a more reasonable cut-off value for the diagnosis of sleep apnea in adolescents.

Effects of Manual Intervention and Self-Corrective Exercise Models of the General Coordinative Manipulation on Balance Restoration of Spine and Extremities Joints

  • The purpose of this study was conducted in order to analyze the effects of the manual intervention and self-corrective exercise models of general coordinative manipulation(GCM) on the balance restoration of spine & extremities joints with distortions and mal-alignment areas. The subjects were the members who visited GCM Musculoskeletal Prevent Exercise Center from March 1 2012 to December 31 2013 because of spine & extremities joints distortion and mal-alignments, poor posture, and body type correction. All subjects were diagnosed with the four types of the GBT diagnosis. And according to the standards of the mobility vs stability types of the upper & lower body, they were classified into Group 1(40 persons) and Group 2(24 persons). For every other day for three times a week, GCM intervention models were applied to all subjects for four weeks, adding up to 12 times in total. Then the balance restoration effects were re-evaluated with the same methods. The results are as follows. 1) Balance restoration effects of VASdp(Visual analysis scale pain & discomfort) and ER(Equilibrium reaction: ER) came out higher in GCM body type(GBT) II III IV of Group 1. 2) In case of balance restoration effects in Moire and postural evaluation areas, Group 1 was higher and cervical and scapular girdle were higher in Group 2. The balance restoration of the four GBT types was significant in all regions(p<.05), and the scapular girdle came out as high in the order of GBTII IV I. 3) In case of thoracic-lumbar scoliosis and head rotation facial asymmetric cervical scoliosis ribcage forward, the balance restoration effects of the upper body postural evaluation areas came out the highest in Group 1 and Group 2, respectively. The balance restoration effects of the four GBT types were significant in all regions(p<.05), and came out the highest in lumbar scoliosis GBTIII I, ribcage forward and thoracic scoliosis GBTII IV. 4) The balance restoration effects of the lower body postural evaluation areas came out higher in Group 1 and Group 2 for pelvis girdle deviation patella high umbilicus tilt and hallux valgus foot longitudinal arch: FLA patella direction, respectively. The balance restoration effects of the four GBT types were significant in all regions(p<.05), and came out the highest in pelvis girdle deviation GBTIII I and patella high-direction GBTIV II I. 5) The balance restoration effects between the same GBT came out significant (p<.05) in all evaluation areas and items. The conclusions of this study was the manual intervention and self-corrective exercise models of the GCM about the mal-alignment of the spine & extremities joints across the whole body indicated high balance restoration effects(p<.05) in spine & extremities joints in all evaluation areas.

Discriminant Validity of the Child Behavior Checklist for Ages 1.5-5 in Diagnosis of Autism Spectrum Disorder (자폐스펙트럼장애 진단에서 Child Behavior Checklist 1.5-5 유아 행동평가척도 부모용의 변별력)

  • Objectives: The purpose of this study was to verify the validity and clinical cutoff score of the Child Behavior Checklist for ages 1.5-5 (CBCL 1.5-5) for diagnosis of autism spectrum disorder (ASD). Methods: 44 ASD infants and 100 normal infants participated. T-test, discriminant analysis, receiver operating characteristic (ROC) curve analysis, and odds ratio analysis were performed on the data. Results: Discriminant validity was confirmed by mean differences and discriminant analysis on the subscales of Withdrawn, Attention problems, Internalizing problems, Externalizing problems, Total problems, and all Diagnostic and Statistical Manual of Mental Disorders (DSM)-oriented scales between the two groups. ROC curve analysis showed that Withdrawn, Attention problems, Internalizing problems, Externalizing problems, Total problems, DSM pervasive developmental problems, DSM attention deficit/hyperactivity problems, and DSM oppositional defiant problems significantly predicted ASD infants compared to normal infants. In addition, the clinical cutoff score criteria adopted in the Korean CBCL 1.5-5 for subscales of Withdrawn, Attention problems, Internalizing problems, Externalizing problems, Total problems, DSM pervasive developmental problems, DSM attention deficit/hyperactivity problems, and DSM oppositional defiant problems were shown to be valid. Conclusion: The subscales of Withdrawn, Attention problems, Internalizing problems, Externalizing problems, Total problems, DSM pervasive developmental problems, DSM attention deficit/hyperactivity problems, and DSM oppositional defiant problems significantly discriminated for the diagnosis of ASD.

  • Breast cancer is known as a disease that occurs in a lot of developed countries. However, in recent years, the incidence of Korea's modern woman is increased steadily. As well known, breast cancer usually occurs in women over 50. In the case of Korea, however, the incidence of 40s with young women is increased steadily than the West. Therefore, it is a very urgent task to build a manual to the accurate diagnosis of breast cancer in adult women in Korea. In this paper, we show how using data mining techniques to predict breast cancer. Data mining refers to the process of finding regular patterns or relationships among variables within the database. To this, sophisticated analysis using the model, you will find useful information that is easily revealed. In this paper, through experiments Deicion Tree Naive Bayes analysis techniques were compared using analysis techniques to diagnose breast cancer. Two algorithms was analyzed by applying C4.5 algorithm. Deicison Tree classification accuracy was fairly good. Naive Bayes classification method showed better accuracy compared to the Decision Tree method.

Efficient Semi-automatic Annotation System based on Deep Learning

  • This paper presents the development of specialized software for annotating volume-of-interest on 18F-FDG PET/CT images with the goal of facilitating the studies and diagnosis of head and neck cancer (HNC). To achieve an efficient annotation process, we employed the SE-Norm-Residual Layer-based U-Net model. This model exhibited outstanding proficiency to segment cancerous regions within 18F-FDG PET/CT scans of HNC cases. Manual annotation function was also integrated, allowing researchers and clinicians to validate and refine annotations based on dataset characteristics. Workspace has a display with fusion of both PET and CT images, providing enhance user convenience through simultaneous visualization. The performance of deeplearning model was validated using a Hecktor 2021 dataset, and subsequently developed semi-automatic annotation functionalities. We began by performing image preprocessing including resampling, normalization, and co-registration, followed by an evaluation of the deep learning model performance. This model was integrated into the software, serving as an initial automatic segmentation step. Users can manually refine pre-segmented regions to correct false positives and false negatives. Annotation images are subsequently saved along with their corresponding 18F-FDG PET/CT fusion images, enabling their application across various domains. In this study, we developed a semi-automatic annotation software designed for efficiently generating annotated lesion images, with applications in HNC research and diagnosis. The findings indicated that this software surpasses conventional tools, particularly in the context of HNC-specific annotation with 18F-FDG PET/CT data. Consequently, developed software offers a robust solution for producing annotated datasets, driving advances in the studies and diagnosis of HNC.

  • The activity of fucosidase shows different value depending on disease, eg) fucosidosis and I cell disease are characterized by the absence or deficiences of .alpha.-fucosidase, and sera of ovarian cancer patients exhibited a statistically significant deficiency of .alpha.-L-fucosidase activity (Zielke et al., 1972; Kress et al., 1980; Barlow et al., 1981). For the purpose of diagnosis of these disease easily, the manual of test can be developed by preparing kits of hydrophobic-binding substrate of fucosidase that bind C18-column.

A Case of Intussusception with Acute Appendicitis

  • In children presenting to hospital with gastrointestinal symptoms, diseases such as intussusception and acute appendicitis require particular attention and careful examination. Early diagnosis and proper treatment are important because of possible severe complications such as peritonitis and death. Intussusception and appendicitis share similar clinical manifestations. More importantly, the presence of acute appendicitis together with intussusception in children is very rare. We describe an interesting case of a 38-month-old boy who presented with abdominal pain in the right lower quadrant. His vital signs were stable and laboratory test findings showed no specific alterations. We detected tenderness in the right lower quadrant. A computed tomography scan showed an ileocolic intussusception with no strangulation and diffuse wall thickening of the appendix trapped within the intussusception. The patient underwent an appendectomy and manual reduction.

  • We are developing computer integrated polysomnography system. This system integrates conventional polysomnography with computer for data management, automatic analysis, scoring, and data transmission. In the first stage, we have developed the signal interface and user interface for the manual scoring and data management. For the automatic scoring of sleep stage, we have developed the protocol and have applied the analytic method in its primitive form. In the second stage we will develope a partially automatic scoring system, and finalize the fully automatic system in the final third stage.

A Clinical Case Report on a Patient of Acute Disseminated Encephalomyelitis Using Korean Medicine (급성 파종성 뇌척수염에 대한 한방적 치료 치험 1예)

  • The objective of this study is to report the improvement of the patient with Acute Disseminated Encephalomyelitis (ADEM) with Korean Medical intervention. The patient, diagnosed as ADEM, was treated by acupuncture, moxibustion and herb medicine and wild ginseng distilled herbal acupuncture. We evaluated the consequences by checking Manual Muscle Test (MMT), Korean Version of Barthel Index (K-MBI). As a result, the patient improved significantly MMT and K-MBI inclined. In ADEM, the initial diagnosis is important, and through a Korean medicine, we were able to achieve meaningful treatment result. There is a limit on this study due to sufficient number of case, thus, further studies will be needed.

  • The new edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) is published by the American Psychiatric Association. The diagnostic systems for mental disorders have come under criticism for relying on presenting signs and symptoms with the result that they do not adequately reflect relevant neurobiological and behavioral systems. Finally, the National Institute of Mental Health (NIMH) in the United States has suggested the Research Domain Criteria (RDoC) to develop a research classification system based upon dimensions of neurobiology and behavioral aspect. The present review introduces the RDoC as a new reaseach framework.
