나선형 단층방사선사진촬영에서 촬영조건이 악골 단면상의 높이, 폭 및 인지도에 미치는 영향 (The effect of different radiographic parameters on the height, width and visibility of cross-sectional image of mandible in spiral tomography)

  • Purpose : To evaluate the differences in bone height, bone width, and visibility of posterior spiral tomographic images according to various exposure directions, image layer thickness, and inclination of the mandibular inferior border. Materials and Methods: Six partially and completely edentulous dry mandibles were radiographed using Scanora spiral tomography. Spiral tomography was performed at different exposure directions (dentotangential and maxillotangential projection), image layer thicknesses (2 mm, 4 mm and 8 mm), and at various inclinations to the mandibular border (+ 100, 00 and -10°). The bone height and width was measured using selected tomographic images. The visibility of mandibular canal, crestal bone, and buccal and lingual surfaces were graded as 0, 1, or 2. Results : The bone width at the maxillo-tangential projection was wider than at the dento-tangential projection (p < 0.05). The visibility of buccal and lingual surface at the maxillo-tangential projection was higher than at the dento-tangential projection (p<0.05). Thinner image layer thicknesses resulted in greater visibility of buccal and lingual surfaces (p < 0.05). Bone height was greatest in the -10° group, and at the same time the bone width of the same group was the narrowest (p < 0.05). The visibility of alveolar crest and buccal surface of the + 10° group was the highest, while the visibility of the mandibular canal was greatest in the 00 group. Conclusion: When spiral tomography is performed at the mandibular posterior portion for visualization prior to implant surgery, it is important that the inferior border of mandible be positioned as parallel as possible to the floor. A greater improvement of visibility can be achieved by maintaining a thin image layer thickness when performing spiral tomography.

Evaluating Measurements: A Comparative Study of Digital and Plaster Models for Orthodontic Applications in Mixed Dentition

  • This study aimed to assess the accuracy of tooth widths, intermolar widths, and arch lengths acquired through two intraoral scanners, including iTero Element Plus Series (Align Technology, Santa Clara, CA, USA) and Trios 4 (3Shape, Copenhagen, Denmark), specifically on mixed dentition. A total of 30 subjects were divided into 2 groups, each undergoing both alginate impressions and intraoral scanning using either the iTero or Trios scanner. The plaster models were measured with a caliper, while the digital models were measured virtually. In the iTero group, all tooth width measurements exhibited differences compared to the plaster values, except for maxillary left lateral incisors (p = 0.179), mandibular right (p = 0.285), and left (p = 0.073) central incisors. The Trios group did not display significant differences in any of the tooth width measurements. Intermolar width comparisons for both groups indicated differences, except for mandibular primary canine to primary canine values (p = 0.426) in the iTero group. Regarding arch length, the mandibular anterior, maxillary right, and left arch lengths in the iTero group demonstrated larger caliper values than those of iTero. Conversely, in the Trios group, all parameters showed smaller caliper values, especially in upper anterior, maxillary right, mandibular right, and mandibular left arch lengths with significance (p = 0.027, 0.007, 0.003, and 0.047, respectively). Despite the differences between the two groups, digital models might be clinically suitable alternatives for plaster models. Pediatric dentists should carefully assess these differences, as a comprehensive evaluation would result in precise orthodontic treatment planning and favorable outcomes for young patients with mixed dentition.

Three-dimensional evaluation of the mandibular condyle in adults with various skeletal patterns

  • Objective: Morphometric and morphological evaluation of the mandibular condyle in adults and to identify its correlation with skeletal malocclusion patterns. Methods: Cone-beam computed tomography scans of 135 adult patients were used in this study and classified into groups according to four criteria: (1) sex (male and female); (2) sagittal skeletal discrepancy (Class I, Class II, and Class III); (3) vertical skeletal discrepancy (hyperdivergent, normodivergent, and hypodivergent); and age (group 1 ≤ 20 years, 21 ≤ group 2 < 30, and group 3 ≥ 30 years). The morphometrical variables were mandibular condyle height and width, and the morphological variable was the mandibular condyle shape in coronal and sagittal sections. Three-dimensional standard tessellation language files were created using itk-snap (open-source software), and measurements were performed using Meshmixer (open-source software). Results: The mandibular condyle height was significantly greater (p < 0.05) in patients with class III malocclusion than in those with class I or II malocclusion; the mandibular condyle width was not significantly different among different sexes, age groups, and sagittal and vertical malocclusions. There were no statistical associations between various mandibular condyle shapes and the sexes, age groups, and skeletal malocclusions. Conclusions: The condylar height was greatest in patients with class III malocclusion. The condylar height and width were greater among males than in females. The mandibular condyle shapes observed in sagittal and coronal sections did not affect the skeletal malocclusion patterns.

하악관 위치 분석시 일반단층사진과 전산화단층사진의 비교연구 (COMPARATIVE STUDY OF THE MACROSCOPIC FINDING, CONVENTIONAL TOMOGRAPHIC IMAGING, AND COMPUTED TOMOGRAPHIC IMAGING IN LOCATING THE MANDIBULAR CANAL)

  • The purpose of this study was comparision of conventional tomography with reformatted computed tomography for dental implant in locating the mandibular canal. Five dogs were used and after conventional tomographs and reformatted computed tomographs were taken, four dentist traced all films. Mandibles were sectioned with 2mm slice thickness and the sections were then radiographed(contact radiography). Each radiographic image was traced and linear measurements were made from mandibular canal to alveolar crest, buccal cortex, lingual cortex, and inferior border. Following results were acquired : 1. Reformatted computed tomographs were exacter than conventional tomography by alveolar crest to canal length of -0.6mm difference between real values and radiographs. 2. The average measurements of buccal cortex to mandibular canal width and lingual cortex to mandibular canal width of conventional tomographs were exacter than reformatted computed tomographs, but standard deviations were higher than reformatted computed tomographs. 3. Standard deviations of reformatted computed tomographs were lower than conventional tomographs at all comparing sites 4. At reformatted computed tomography 62.5% of the measurements performed were within. : filmm of the true value, and at conventional tomography 24.1% were. 5. Mandibular canal invisibiity was 0.8% at reformatted computed tomography and 9.2% at conventional tomography. Reformatted computed tomography has been shown to be more useful radiographic technique for assessment of the mandibular canal than conventional tomography.

3-D CT를 이용한 악교정수술후의 하악 과두 위치와 하악폭경 및 하악각의 평가 (3-D CT EVALUATION OF CONDYLE HEAD POSITION, MANDIBULAR WIDTH, AND MANDIBULAR ANGLE AFTER MANDIBULAR SETBACK SURGERY)

  • The purpose of this study is to evaluate the change in condylar position, width, and angle before and after orthognathic surgery using 3-dimensional computed tomograph. Pre and posterative 3-D CT was taken on 38 patients and through axial, frontal, sagittal measurements and by 3-dimensional reconstruction, the changes in condylar postion, mandibular width and angle were analyzed and others such as the difference in gender, operation and fixation method, setback length and in relation with temporomandibular disorders were done together too. The results were as follows: The inward rotation of condyle in axial condylar angle, the forward movement of right condyle in sagittal anterior-posterior distance, the superior movement of both condyles in sagittal superior-inferior distance, the decrease in gonial angle, the increase in mandibular width, the decrease in distance between the axial coronoid process distance and the increase in the frontal intercondylar distance were statistically significant. There were no statistically significant changes in gender difference, however in the difference in operation method, change in the gonial angle was observed and there was more change in bilateral sagittal split osteotomy group compared to two-jaw surgery group. In the difference in fixation method, the decrease in axial coronoid process distance and the change in sagittal anterior-posterior distance were statistically significant. In the difference in setback, the increase in setback didn't relate directly with the increased change in condyle position. In the relation with temporomandibular disorder, changes in left axial condylar angle and axial coronoid process distance were statistically significant. Changes in condylar position could be observed after the orthognathic surgery but it doesn't seem to have much of a clinical importance. The orthognathic surgery is effective in decreasing the mandibular angle, and it is not related with the temporomandibular disorder.

성인 구순구개열환자의 교정치료 전략 (The orthodontic strategies for adult patients of cleft lip and palate)

  • Even the adult cleft lip and palate patient who has not had timely treatment during the growth period, can be treated with orthodontic treatment without the necessity of orthognathic surgery if only the patient is treated under correct diagnosis and fitting appliances. Initially, maxillary arch form is established by constructing trifocal circles. Posterior region can be expanded and derotated laterally with pentahelix and anterior teeth are aligned with Tiggle brackets and "ㄷ"-shaped spring. Thereafter, anterior and posterior regions are consolidated. Mandibular intercanine width should be adjusted to maxillary intercanine width which was unavoidably reduced. Mandibular anterior tooth extraction will be helpful to attain proper mandibular intercanine width and better anterior dental showing.

하악 절치 근원심폭경과 법랑질 두께의 관계 (Relationship between mesiodistal width and enamel thickness in mandibular incisors)

  • 하악 절치부에서 보이는 총생은 하악 절치의 큰 MD/FL (mesiodistal/faciolingual) 인덱스와 관련이 있음이 보고되었다. 큰 MD/FL 인덱스를 보이는 절치에서 치간 삭제를 통해 근원심폭경을 줄여 MD/FL 인덱스를 줄이고 공간을 확보하는 방법이 정당화되기 위해서는 MD/FL 인덱스가 클수록 인접면 법랑질 두께가 더 두꺼워야 할 것이다. 본 연구의 목적은 하악 절치의 MD/FL 인덱스, 근원심폭경, 최대풍융부/치경부폭경비와 같은 치아의 형태적 요인과 인접면 법랑질 두께 사이에 상관관계가 있는지를 조사하는 것이었다. 발거된 하악 절치 40개에서 MD/FL 인덱스, 근원심폭경, 최대풍융부/치경부폭경비를 구하였으며, 근원심폭경을 구한 선상까지 순면을 연마하여 그 단면에서 법랑질 두께를 측정하였다. 치경부근원심폭경은 순면을 연마한 단면에서의 좌우측 백악법랑경계 사이의 폭경을 측정하였다. 이후 법랑질 두께와 MD/FL 인덱스, 근원심폭경, 최대풍융부/치경부폭경비의 상관관계를 조사하였다. 연구결과 하악 절치의 법랑질 두께는 편측에서 $0.75{\pm}0.07mm$였고, 근원심폭경은 $5.56{\pm}0.40mm$였다. 법랑질 두께와 근원심폭경 간에는 유의한 상관관계가 있었지만(R = 0.68), 법랑질 두께와 MD/FL 인덱스, 법랑질 두께와 최대풍융부/치경부폭경비 간에는 상관관계가 없었다. 이는 근원심폭경이 큰 치아에서는 치간 삭제를 더 많이 할 수 있지만, 동일한 근원심폭경을 갖는 절치들의 경우 MD/FL 인덱스가 큰 하악 절치 또는 최대풍융부/치경부폭경비가 커서 삼각형의 형태를 갖는 하악절치에서 치간 삭제를 더 많이 할 수는 없다는 것을 나타낸다.

Mandibular midline osteotomy for correction of bimaxillary transverse discrepancy: a technical note

  • Bimaxillary transverse width discrepancies are commonly encountered among patients with dentofacial deformities. Skeletal discrepancies should be diagnosed and managed appropriately with possible surgical corrections. Transverse width deficiencies can present in varieties of combinations involving the maxilla and mandible. We observed that in a significant proportion of cases, the maxilla is normal, and the mandible showed deficiency in the transverse dimension after pre-surgical orthodontics. We designed novel osteotomy techniques to enhance mandibular transverse width correction, as well as simultaneous genioplasty. Chin repositioning along any plane is applicable concomitant with mandibular midline arch widening. When there is a requirement for larger widening, gonial angle reduction may be necessary. This technical note focuses on key points in management of patients with transversely deficient mandible and the factors affecting the outcome and stability. Further research on the maximum amount of stable widening will be conducted. We believe that developing evidence-based additional modifications to existing conventional surgical procedures can aid precise correction of complex dentofacial deformities.

  • A study of a sample of 81 Korean adolescents was taken to determine the degree of correlation between mandibular anterior teeth size and the size of the canines and premolars. The correlation between the total mesiodistal width of the mandibular permanent incisors and that of maxillary or mandibular canine and first and second premolars was found to be 0.53 and 0.58, respectively. Further , regression constants were determined in an attempt to estimate the buccal segments from the mandibular incisors.

파노라마방사선사진에서 골형태 계측과 구내표준필름에서 구리당량치의 상관관계 (The correlationship between mandibular radiomorphometric indices in panorama and bone mineral density in Cu-equivalent image of intraoral film)

  • Purpose: To determine whether the mandibular radiomorphometric indices in panoramic radiography are correlated with the bone mineral density of Cu-equivalent images in intraoral film. Materials and Methods: The bone mineral density (BMD) of the mandibular premolar area was measured in the Cu-equivalent image of intraoral film. The Panoramic Mandibular Index (PMI) and Mandibular Cortical Width (MCW) were measured in panoramic radiographs of six dry mandibles, and the Pearson correlation between PMI, MCW, and BMD were tested. Results: There were no significant correlations between PMI and BMD (r = 0.280), nor between MCW and BMD (r =0.237). Conclusion: The results show that PMI and MCW were poor diagnostic indicators of mandibular BMD in the six dry mandibles used in this study. The correlationship between the mandibular radiomorphometric indices (PMI and MCW) and mandibular BMD needs to be researched further using large in vivo patient samples.
