하악지 시상 골절단술 이후 흡수성 나사를 사용하여 고정한 환자에서 술 후 안정성에 대한 연구 (STABILITY AFTER SURGICAL CORRECTION OF MANDIBULAR PROGNATHISM USING BILATERAL SAGGITAL SPLIT RAMUS OSTEOTOMY AND FIXATION WITH POLY-L/DL-LACTIDE COPOLYMER SCREWS ($BIOSORB^{TM}FX$))

  • This study was designed to assess skeletal stabilily after surgical correction of mandibular prognathism by bilateral saggital split ramus osteotomy(BSSRO) and fixation with absorbable screws. From July 2001 to September 2003, 30 patients with Class III malocclusion were treated with BSSRO and mandibular setback. They underwent fixation with Biosorb$^{TM}$ FX screws. Cephalograms were obtained preoperatibely, 2 or 3 days postoperatively, and about 12 months after the operation. Changes in the position of lower incisor tip, B point, and pogonion were examined on lateral cephalograms. The mean mandibular setback just after surgery was 10.6mm. 12 months after surgery, mean relapse at pogonion represented 17.9% and 15.1% at B point. Our results suggest that fixation of the bony segments with absorbable screws after BSSO may be used effectively in properly selected cases.

  • Macroglossia can create dental and skeletal instability after orthodontic treatment or orthognathic surgery for mandibular prognathism. In relevant literature, partial glossectomy is suggested for a good post-treatment prognosis. Most of the published partial glossectomy cases are two-staged surgery, because of concern about postoperative airway obstruction. As orthognathic surgical techniques and fixation method develop, however, concerns about postoperative airway obstruction have lessened. In this case, mandibular setback surgery and partial glossectomy were performed simultaneously, leading to stable recovery without any postoperative respiratory problems. After surgical technique to preserve the tongue tip, we achieved good outcomes without postoperative side effects of lingual hypoesthesia, pronunciation disorder and dyskinesia. We report this case with a literature review.

  • Introduction: To evaluate the skeletal stability after a bilateral sagittal split osteotomy (BSSO) setback of the mandible fixed with a biodegradable internal fixation device or metal internal fixation device. Materials and Methods: Thirty consecutive patients underwent mandibular setback via BSSO. Fifteen patients were fixed with a biodegradable internal fixation device or metal internal fixation device respectively. Posteroanterior (PA) and lateral cephalograms were taken preoperatively and at two days, 5.5 months and 14.5 months postoperatively. The relevant skeletal points were traced and digitized to evaluate the skeletal changes postoperatively. The relapse rates were analyzed and compared statistically. Results: There was no statistically significant differences in postoperative stability between the two groups.(P<0.05) Conclusion: The biodegradable internal fixation device may make an effective device alternative to a metal internal fixation device for setback BSSO.

Correlation between skeletal and dental changes after mandibular setback surgery-first orthodontic treatment: Cone-beam computed tomography-generated half-cephalograms

  • Objective: To investigate skeletal and dental changes after application of a mandibular setback surgery-first orthodontic treatment approach in cases of skeletal Class III malocclusion. Methods: A retrospective study of 34 patients (23 men, 11 women; mean age, $26.2{\pm}6.6years$) with skeletal Class III deformities, who underwent surgery-first orthodontic treatment, was conducted. Skeletal landmarks in the maxilla and mandible at three time points, pre-treatment (T0), immediate-postoperative (T1), and post-treatment (T2), were analyzed using cone-beam computed tomography (CBCT)-generated half-cephalograms. Results: The significant T0 to T1 mandibular changes occurred $-9.24{\pm}3.97mm$ horizontally. From T1 to T2, the mandible tended to move forward $1.22{\pm}2.02mm$, while the condylar position (Cd to Po-perpendicular plane) shifted backward, and the coronoid process (Cp to FH plane) moved vertically. Between T1 and T2, the vertical dimension changed significantly (p < 0.05). Changes in the vertical dimension were significantly correlated to T1 to T2 changes in the Cd to Po-perpendicular plane (r = -0.671, p = 0.034), and in the Cp to FH plane (r = 0.733, p = 0.016), as well as to T0 to T1 changes in the Cp to Po-perpendicular plane (r = 0.758, p = 0.011). Conclusions: Greater alterations in the vertical dimension caused larger post-treatment (T2) stage skeletal changes. Studying the mandibular position in relation to the post-surgical vertical dimension emphasized the integral importance of vertical dimension control and proximal segment management to the success of surgery-first orthodontic treatment.

하악 전방분절 절골술을 이용한 경미한 하악전돌증의 교정 (The Correction of Mild Mandibular Prognathism Using Mandibular Anterior Segmental Osteotomy)

  • Purpose: For a minor degree of mandibular prognathism, mandibular anterior segmental osteotomy (ASO), usually extracting the bilateral premolars, has been performed frequently to correct malocclusion of the anterior teeth. Preoperative planning using cephalometry and a dental model is very important for such a orthognathic surgery. Depending on the specific preoperative mock surgery with the dental model, ASO, with ipsilateral unitooth extraction, is defined to be feasible and performed for ten patients. The comparisons of its preoperative and postoperative analysis of clinical photographs, dental casts, and lateral cephalograms, for soft tissue profiles, skeletal and dental relationships are described in the following, and its clinical applications are noted. Methods: From March 1, 2004, to March 31, 2006, We performed 10 mandibular ASO by extraction of ipsilateral unitooth to improve their lower facial profiles and the lip relationships. Patient age ranged from 19 to 33 years, with a mean age of 25.6 years. Two were males and eight were females. Results: All patients were satisfied with aesthetic and occlusal changes postoperatively. Significant and persistent decrease in the SNB and interincisal angle were observed in the postoperative cephalometries. The soft tissue profiles also were improved and near Ricketts's esthetic line. Other combined procedures include nine genioplasties, two rhinoplasties, and one blepharoplasty. One patient complained of transient unilateral inferior mental nerve paresthesia. There were no other significant complications or relapses throughout the follow-up period(6-20 months). Conclusion: Mandibular ASO, extracting the ipsilateral unitooth, was performed for ten patients to correct mild mandibular prognathism. The amount of setback of the mandibular anterior portion was 2 to 3 mm, and satisfactory results were obtained combined with genioplasties.

악교정 환자의 악교정 수술전후 발음양상에 대한 비교연구 (The Comparative Study of Effect on Speech before and after Orthognathic Surgery of Patients)

  • The purpose of this study was undertaken to determine the effects of orthognathic surgery on speech. The hyposis stated herein is that functional behaviors of the dentofacial complex, such as speech production, may be adversely affected by deviations of a structural nature(especially, Class III malocclusion). Twenty adults with Class III malocclusion(13 female and 7 male) were studied preoperative, immediate postoperative and either 6 or 12 months postoperative lateral cephalograms. They had mandibular prognathism and had undergone mandible setback operation. The position of tongue, soft palate(Uvula), hyoid bone, respiratory track width, and pharyngeal depth were assessed on lateral cephalograms with 23 cephalometric variables, ANOVA, Paired t-tests and Pearson's product-moment correlation coefficient tests were used to evalute the operative changes in all cephalometric parameters. A experienced speech and language pathologists performed narrow phonetic transcriptions of tape-recorded words and sentences produced by each of the ninth patients and the recording tapes were analyzed by phonetic computer program(Computerized Speech Lab(CSL) Model 4300BI(U.S.A.)) These judges also recorded their ratings of each patient's overall consonants, hypernasality, hyponasality, and articulation proficiency. The results obtained are as follows; 1. There were significant changes in distance of posterior pharyngeal wall to tongue (TI-TW2, TS-TW3) after the surgery at 6 months postoperatively(each p<0.01 p<0.05). 2. The posterior tongue point(TI, TS, PPT) moved posteriorly after surgery and remained to its changed position at 6 months postoperatively(p<0.05). The displacement of tongue was correlated with the movement of mandibular setback amount(p<0.05). The hyoid bone moved posteriorly superiorly after immediate postoperative period. There was significant changes in hyoid bone movement after immediated postoperative period(p<0.05), but returned to its original position during the follow-up period(p>0.05) 3. The soft palate was displaced posteriorly superiorly after immediated operative period and remained to its changed position at 6 months postoperatively(p<0.05). ANS-PNS-SPT angle increasing, PPU-PPPo distance narrowing was showed after surgery, and remained its appearance 6 months postoperatively(p<0.05). 4. There were significant changes in formant value and squre diagram of vowel sound after the orthognathic surgery and the follow-up period. There were significant changes in /ㅅ/sound and posterior tongue sound. 5. The posterior movement of tongue and the posteriosuperior movement of soft palate was correlated with mandibular setback amount after orthognathic surgery. On the vowel squre diagram, the author found that the place of articulation after operation moved downward, backward, upward. 6. In assessing speech abnormalities, dental occlusion should be considered as a contributing factor. The vast majority of subjects with preoperative misarticulations eliminated or reduced their errors following orthognathic surgery. There was significant difference in speech impovement between pre- and postoperation.

골격성 제3급 부정교합환자의 하악지 시상분할 골절단술후 하안면 폭경 및 고경의 변화에 대한 두부계측 방사선학적 연구 (A CEPHALOMETRIC STUDY ON CHANGES OF FACIAL MORPHOLOGY IN THE FRONTAL VIEW FOLLOWING MANDIBLE SETBACK SURGERY ( BSSRO ) IN PATIENTS WITH SKELETAL CLASS III DENTOFACIAL DEFORMITIES)

  • Purpose : The purpose of this study was to analyze the lower third facial changes in frontal view after mandibular setback surgery. Materials and Methods : In this study, fifteen subjects(6 males and 9 females) with class III dental and skeletal malocclusions who were treated with BSSRO(Bilateral Sagittal Split Ramus Ostetomy) were used. Frontal cephalometric radiographs were taken preoperatively and more than 6 months postoperatively, and hard tissue(H2-Hl) and soft tissue changes (S2-S1) were measured on vertical and horizontal reference lines. In 15 cases, changes which developed more than 6 months after surgery were studied. Results : The results were as follows. 1. In the facial height, hard tissue $decreased(2.46{\pm}2.76mm)$ with statistical significance(P<0.01), and soft tissue also $decreased(1.64{\pm}3.66mm)$. As a result, the facial height generally becomes shorter after sagittal split ramus osteotomy. 2. In the mandibular width, hard tissue $decreased(2.08{\pm}3.59mm)$ with statistical sgnificance(P<0.05), but soft tissue $increased (2.14{\pm}5.73mm)$ without statistically significant difference(P>0.05) postoperatively. 3. In the facial index, hard tissue $decreased(0.23{\pm}2.21%)$, but soft tissue $increased(2.41{\pm}3.46%)$ with statistical significance. Conclusion : One of the main purpose of orthognathic surgery is to achieve facial esthetics and harmony. In order to fullfill this purpose, it is important to carry out a precise presurgical treatment planning by estimating the changes of frontal profile after surgery.

하악골 전돌증 환자에서 하악지 시상분할골절단술후 금속고정판의 조기제거 및 기능운동이 턱관절장애에 미치는 영향 (THE EFFECT OF EARLY REMOVAL OF THE FIXATION PLATES AND ACTIVE MOUTH OPENING EXERCISE ON THE TEMPOROMANDIBULAR DYSFUNCTION AFTER MANDIBULAR SETBACK SURGERY)

  • Purpose: The effect of orthognathic surgery on the temporomandibular dysfunction has been controversial. The purpose of this study is to prove statistically that early removal of fixation plate at postoperative 2 weeks with active exercise of mouth opening could relieve preoperative temporomandibular dysfunction and reposition of temporomandibular joint. Patients and Methods: All 28 subject patients underwent mandibular setback with BSSRO in Kyunghee medical center by one surgeon. The fixation plates used for rigid fixation were removed at postoperative 2 weeks and we had the patients excercise active mouth opening with intermaxillary rubber rings for the guiding proper postoperative occlusion. Temporomandibular symptoms were checked and radiographs were taken before surgery, within a month after surgery, six to twelve months after surgery respectively. Results: The temporomandibular dysfunction symptoms were relieved after the surgery and the condyle was displaced inferior-posteriorly immediate after surgery and repositioned toward its original position during follow-up periods. Conculusion: Orthognathic surgery may benefit temporomandibular joint dysfunction by obtaining a postoperative stable occlusion and more physiologic neuromuscular function. The early removal of fixation plates after BSSRO could reposition the temporomandibular joint to physiologic position and relieve the symptoms of temporomandibular dysfunction by permitting movement of proximal segment.

골격성 3급 부정교합 환자의 하악골 후퇴술 시행후 안모변화에 대한 3차원적 연구 (Three-dimensional analysis of soft and hard tissue changes after mandibular setback surgery in skeletal Class III patients)

  • 하악 수술로 치료한 골격성 3급 부정교합 환자의 연조직 변화를 3차원적으로 분석하였다. 수술전과 수술후에 CT를 촬영하고, 연조직과 경조직을 각각의 임계값에 따라 segmentation였다. FH.평면, 정중시상면, PNS를 포함하는 전두면을 기준으로 공통 좌표계를 구성하고, 이 좌표계를 기준으로 술전, 술후 영상을 위치시켰다. 술후의 변화를 측정하기 위해 각각의 모형에 대해 전두면에 평행한 grid를 형성하였다. Grid내의 교점에서 골조직과 연조직 모형에 투사하여 만나는 점의 좌표값을 구하고, 이를 바탕으로 술후의 변화를 측정하였다. 하악골 후퇴술시 안모의 변화는 하악골 부분에서만 발생한 것이 아니라, 구각부에서도 관찰되었다. 하악골 부위의 연조직 변화는 대응되는 골조직 이동량에 따른 상대적인 값으로 계산하였다. 정중시상면에서의 변화율은 $77\~102\%$로 나타났다. (p<0.05). 정중시상면 이외의 부분의 변화양상도 이와 유사하였다. 구각부에서의 변화는 하악골의 이동을 대표할 수 있는 점의 이동량에 대한 상대적인 값으로 계산하였다. 정중시상면에서의 변화는 B점을 기준으로 $14\~29\%$이고, Pog점을 기준으로 $17\~37\%$, grid상 83번째 점을 기준으로 $11\~22\%$로 관찰되었다.(p<0.05)

과도한 하안면 고경을 동반하는 하악전돌증환자의 하악골 후퇴술과 이부 감소 성형술식에 관한 연구 (A study on the mandibular setback osteotomy and reduction genioplasty in mandibular prognathism with long anterior facial height)

  • 본 연구는 과도한 하안면 고경을 동반하는 하악전돌증 환자의 악교정수술시 하악골후퇴 술과 이부 감소 성형술을 동시에 시행한 경우의 경조직과 연조직의 변화에 대해 살펴보고, 그 상관성을 평가하여 향후 진단시 보조자료로서 사용하고자 시행되었다. 골격성 III급 부정 교합으로 진단되어 술전 교정치료를 받고 하악골 후퇴술만을 시행한 성인 환자 20명을 대조군(A군)으로, 안모의 수평적 및 수직적 부조화가 심한 골격성 III급 부정교합으로 진단되어 술전 교정치료를 받고 하악골 후퇴술과 이부 감소 성형술을 함께 시행한 성인 환자 20명을 실험군(B군)으로 하였다. 두 군의 경조직 변화에 대한 연조직 변화 양상과, 경조직 변화에 따른 연조직의 이동 비율을 비교 분석하여 다음과 같은 결론을 얻었다. 1. A, B군 모두에서 경조직의 후방 이동과 하안면 하부 2/3의 연조직(Ils, Pog', Me')의 수평적 변화는 높은 상관성을 보였으며, B군에서는 A군에서보다 Ls, Stm, Li의 수평적 변화에도 중등도 이상의 상관성을 보였다. 2. B군의 경조직의 수직 이동에 따른 연조직의 수직적 변화의 상관성은 Point B-Ils, Me-Me'을 제외하고는 A군보다 적은 것으로 나타났다. 3. 경조직의 변화에 대한 연조직의 변화율을 비교해 보면, B군의 경우Pog'의 후방이동 비율이 A군에서보다 적게 나타났고, B군의 Ils, Pog', Me'의 상방이동 비율은 각각 32%, 54%, 60%로 나타났다. 4. 안면의 수직 고경에 대해서는 A, B군 모두에서 하안면 고경의 유의한 감소로 전안면 고경에 대한 하안면 고경의 비율이 감소하였으나, B군에서는 하안면 상부 1/3(Sn-Stm)의 비율은 변화 양상이 유의성있게 나타나지 않았다. 5. 이부 감소 성형술은 하악후퇴술과 함께 시행시 하안면 상부 1/3(Sn-Stm)의 안모비율에 대한 변화가 적고, 하안면 하부 2/3(St-Me')의 고경을 감소시킬 수 있는 술식이다.
