The purpose of this study was to evaluate the difference and relationship between the slope of articular eminence and the inclination of upper and lower incisor teeth, which are related to the functional stability of occlusion in mandibular protrusion, according to Ricketts' facial types, by the use of lateral cephalogram and SAM2 articulator in 68-adult normal occlusion without tooth missing, orthodontic treatment and occlusal equilibration. The results of this study were as follows : 1 . The angle of articular eminence slope to occlusal plane in brachyfacial type was steeper than that in dolichofacial type, but the angle of articular eminence slope to SN plane and FH plane was not different between facial types. 2. The upper incisor axis in dolichofacial type was steeper than that in brachyfacial type, but lingual surface slope of upper incisor was not different between facial types. 3. In all samples there was a positive correlation between the angle of articular eminence slope and the lingual surface slope of upper incisor to SN plane and FH plane, and in mesofacial type there was a positive correlation between the angle of articular eminence slope and the lingual surface slope of upper incisor to SN plane, FH plane and occlusal plane, and in brachyfacial type there was a positive correlation between the angle of articular eminence slope and the lingual surface slope of upper incisor to occlusal plane. 4. In all samples there was a positive correlation between the angle of articular eminence slope and the angle of Dc-Gn to SN plane, and in mesofacial type there was a positive correlation between the angle of articular eminence slope and the angle of Dc-Gn to occlusal plane, and in brachyfacial type there was a positive correlation between the angle of articular eminence slope and the angle of Dc-Gn to SN plane and FH plane. 5. In all samples there was a positive correlation between the angle of Dc-Gn and the lingual surface slope of upper incisor to SN plane and FH plane, and in mesofacial type there was a positive correlation between the angle of Dc-Gn and the lingual surface slope of upper incisor to SN plane. 6. In all samples and facial types there was a negative correlation between MP to 1 axis and condylar incisal angle.

Objective: To provide reliable prediction models based on dentoskeletal and soft tissue variables for customizing maxillary incisor positions and to optimize digitalized orthodontic treatment planning. Methods: This study included 244 Chinese women (age, 18-40 years old) with esthetic profiles after orthodontic treatment with fixed appliances (133 in group I: 1° ≤ The angle between the nasion [N]-A point [A] plane and the N-B point [B] plane [ANB] ≤ 4°; 111 in group II: 4° < ANB ≤ 7°). Dental, skeletal, and soft tissue measurements were performed on lateral cephalograms of the participants. Correlation and multiple linear regression analyses were used to determine the influence of dentoskeletal and soft tissue variables on maxillary incisor position. Results: The ideal anteroposterior position of the maxillary incisor varied between sagittal skeletal patterns. The position of the maxillary incisor correlated with the sagittal discrepancy between the maxilla and the mandible (ANB), protrusion of the midface, nasal tip projection, development of the chin, and inclination of both the maxillary and mandibular incisors. Distance from the maxillary central incisor to nasion-pogonion plane predicted using multiple linear regression analysis was accurate and could be a practical measurement in orthodontic treatment planning. Conclusions: Instead of using an average value or norm, orthodontists should customize a patient's ideal maxillary incisor position using dentoskeletal and soft tissue evaluations.

The aim of this case report is to describe the treatment of a patient with skeletal Class III malocclusion with maxillary retrognathia using skeletal anchorage devices and intermaxillary elastics. Miniplates were inserted between the mandibular lateral incisor and canine teeth on both sides in a male patient aged 14 years 5 months. Self-drilling mini-implants (1.6 mm diameter, 10 mm length) were installed between the maxillary second premolar and molar teeth, and Class III elastics were used between the miniplates and miniscrews. On treatment completion, an increase in the projection of the maxilla relative to the cranial base (2.7 mm) and significant improvement of the facial profile were observed. Slight maxillary counterclockwise ($1^{\circ}$) and mandibular clockwise ($3.3^{\circ}$) rotations were also observed. Maxillary protraction with skeletal anchorage and intermaxillary elastics was effective in correcting a case of Skeletal Class III malocclusion without dentoalveolar side effects.
This report describes osteotomy and iliac bone graft for malunion caused by failed mandibular fracture reduction. A 27-year-old man was referred 3 months after a motor vehicle accident. At another hospital, two operations had been performed for symphyseal fracture using two resorbable plates. Malocclusion was noted, and panoramic radiography and computed tomography revealed a misaligned dental arch, with a 9.37-mm gap between the central and the lateral incisor of the left mandible. A wafer was made from the patient's dental model, and a maxillary arch bar was applied. Through a lower gingivolabial incision, osteotomy was performed between the malunited symphyseal fracture segments. Both segments were reduced to their original position using the wafer and fixed with titanium miniplates via intermaxillary fixation (IMF). The intersegmental gap was filled with cancellous bone from the iliac crest. The gingival defect was covered with a mucosal transposition flap from the gingivolabial sulcus. IMF and the wafer were maintained for 5 and 9 weeks, respectively. At postoperative week 13, the screws were removed from the mandible and satisfactory occlusion was noted. His mouth opening improved from 2.5 to 3 finger breadths (40 mm). This case demonstrates the need for sufficient IMF when using resorbable plates.
일반적이고 포괄적인 Edgewise방법으로 치료하고 유지시킨 II급 부정교합의 골격 및 치성요소의 변화를 치료 전, 후 및 유지시의 측모두부방사선사진을 비교하여 알아봄으로써 성인 II급 부정교합의 치료 양식을 살펴보고자 하였다. 비발치로 치료한 II급 부정교합자 22명과 상하악 제1소구치 발치로 치료한 20명을 대상으로 치료 전, 후의 측모두부방사선사진을 비교하였으며, 발치치료군 4명과 비발치치료군 4명의 유지시 측모두부방사선사진을 채득하여 유지시의 변화를 살펴보았다. 두개저 및 상,하악 기저골에 기준으로 서로 중첩시켜 골격양상의 변화 및 치아의 경사도와 수직적, 수평적 변화양상을 관찰하여 평가한 결과 다음과 같은 결론을 얻었다. 1. 발치군과 비발치군 모두에서 전,후방 안면고경과 하안면고경 등의 수직고경이 유의하게 증가하였으며, 교합평면의 유의한 경사를 보였다. 2. 비발치군에서 상악 중절치는 치료에 따른 후방견인과 정출을 보였으나 유지기간 동안 약간 순측경사 되었다. 상악 구치부는 수직적인 정출을, 하악 구치부는 원심경사를 보였으나, 근심이동은 보이지 않았다. 유지 기간 동안 상하악 구치는 약간의 근심경사를 보였다. 3. 발치군에서 상하악 중절치 모두 치료에 따른 후방견인과 정출을 보였으나 유지기간 동안 약간 순측경사되었다. 상악 구치부는 수직적인 정출과 동시에 유의한 근심이동을 보였으며, 하악 구치부는 원심경사와 수직적인 정출 및 근심이동을 보였으나 유지기간 동안 약간의 근심경사만을 보였다.
This study was designed to assess skeletal stabilily after surgical correction of mandibular prognathism by bilateral saggital split ramus osteotomy(BSSRO) and fixation with absorbable screws. From July 2001 to September 2003, 30 patients with Class III malocclusion were treated with BSSRO and mandibular setback. They underwent fixation with Biosorb$^{TM}$ FX screws. Cephalograms were obtained preoperatibely, 2 or 3 days postoperatively, and about 12 months after the operation. Changes in the position of lower incisor tip, B point, and pogonion were examined on lateral cephalograms. The mean mandibular setback just after surgery was 10.6mm. 12 months after surgery, mean relapse at pogonion represented 17.9% and 15.1% at B point. Our results suggest that fixation of the bony segments with absorbable screws after BSSO may be used effectively in properly selected cases.
Background: This study aimed to determine the skeletal and dental effects in pediatric and adolescent Korean patients with Class II Division 1 malocclusion treated using the Invisalign Mandibular Advancement (MA®) appliance. Methods: The study included patients aged 6 to 18 years who received orthodontic treatment with the MA® appliance for Class II Division 1 malocclusion at the Department of Pediatric Dentistry, Wonkwnag University Daejeon Dental Hospital, between July 1, 2018, and December 31, 2021. The treatment group consisted of 20 patients, 10 boys and 10 girls. The control participants were also 10 boys and 10 girls. Lateral cephalometric radiographs were taken before and after treatment, and 41 measurements of skeletal and dental changes were measured and analyzed using the V-CephTM 8.0 (Osstem Implant). All analyses were performed using SPSS software (IBM SPSS for Windows, ver 26.0; IBM Corp.), and statistical significance was tested using paired and independent samples t-tests for within-group and between-group comparisons, respectively. Results: The patients in the treatment group showed significant decreases in ANB (A point, Nasion, B point), maxillary protrusion, maxillary anterior incisor labial inclination, and maxillary protrusion after treatment. However, when compared with the growth changes observed in the control group, only ANB and maxillary protrusion decreased, with no significant differences in SNA, SNB, and mandibular length. Conclusion: Collectively, the results of this study confirm that the use of MA® appliance in pediatric and adolescent Korean patients with Class II Division 1 malocclusion results in a reduction of anteroposterior skeletal and dental disharmony.
11 years 7 months old female had a reverse object of anterior teeth and class III molar relationship. Maxillary right and left canines were erupted labially and lower left lateral incisor was congenitally missed. She was treated by rapid palatal expansion and full band technique with extraction of maxillary and mandibular second molars. The following results were obtained.
1. Maxilary arch was expanded.
2. Anterior cross-bite and crowding was corrected.
3. Molar relationship was corrected.
4. Profile was somewhat improved.
골격성 III급 부정교합자의 두개안면골격형태가 하악이부형태와 하악전치의 위치에 미치는 영향을 알아보고자 연구를 시행하였다. 전후방적인 악골부조화가 나타나지 않고 교정치료의 경험이 없는 만 20세 이상의 성인남녀(정상군)와, 하악전돌의 악골부조화를 보이며 교정치료의 경험이 없 만 20세 이상의 성인남녀(하악전돌군)를 비교하여, 두개안면골격형태에 따른 하악이부형태와 하악전치의 위치에 관하여 다음과 같은 결론을 얻었다. 1. 두개안면골격형태의 비교에서 하악에 관련된 ${\angle}SN-Mn,\;{\angle}FMA,\;{\angle}Pal-Mn,\;{\angle}LFH$은 하악전돌군이 정상군에 비하여 크게 나타났지만,(p<0.05) 상악에 관련된 항목은 차이를 나타내지 아니하였다(p>0.n). 2. 하악전치를 둘러 싸고 있는 치조돌기는 하악전돌군이 정상군에 비해서 수평적으로 좁고,수직적으로 긴 형태를 나타내었다(p<0.05). 하악이부의 형태에서도 수평적으로는 하악전돌군이 정상군에 비해서 좁게 나타났으나, 수직적으로는 차이를 나타내지 아니하였다(p>0.05). 3. 하악전돌군의 두개안면골격형태와 이부계측항목과의 상관관계에서 두개저와 하악하연에 기인하는 수직적인 악골부조화가 증가할수록 LaABH, LiACBW, LaACBW, LiABBW, SW, ${\angle}LISA$가 감소하는 역상관관계를 나타냈지만 (p<0.05), 전후방적인 악골부조화와는 별다른 상관관계가 나타나지 않았다(p>0.05). 4. 하악전돌군에서 수직적인 두개안면골격형태를 나타내는 ${\angle}SN-Mn,\;{\angle}FMA,\;{\angle}Pal-Mn,\;{\angle}LFH$ 와 하악이부의 계측 항목인 LiACBW, LiABBW, SW, ${\angle}LISA$ 사이에 높은 상관관계가 존재하여, 수직적인 악골부조화로 하악이부의 형태와 하악전치의 경사도를 예측할 수 있었다(p<0.001).
이번 연구의 목적은 골격성 III급 부정교합자의 악교정 수술 전과 수술 후의 하악전치부 치조골의 형태 변화를 평가하는 것이다 : 하악지 시상골분할절단술만을 시행받은 30명의 성인 골격성 III급 부정교합자를 대상으로 하였다. 초진시와 수술 전과 수술 후 3개월이 경과하였을 때의 측모두부규격방사선 사진을 비교하였다 골격과 하악이부의 계측치를 비교하였으며 이들 사이의 상관성 분석을 시행하여서 다음과 같은 결과를 얻었다. 1. 술전교정후군과 악교정수술후군에서 치료전군에 비해 하악전치부의 순측과 설측의 치조골 높이가 감소하였다. 2. 치조골 기저부의 전후방적인 폭경은 두개안면골격의 수직계측항목과 역상관관계를, IMPA와는 순상관관계를 보였다(P<0.01). 3. 하악이부 계측항목중 하악이부의 장경과 폭경은 두개안면골격과 상관관계를 보이지 않았다. 4. 순측의 치조골 높이와 설측의 치조골 높이는 순상관관계를 나타냈지만(p<0.001), 설측의 치조정의 폭경과는 역상관관계를 보였다(p<0.01). 순측과 설측의 치조정의 폭경은 역상관관계를 보였다(p<0.05). 하악이부의 장경과 폭경은 서로 순상관관계를 보였다(p<0.01). 5. IMPA와 LISA는 순, 설측치조골의 장경과 설측의 치조골 폭경과는 역상관관계를 보였지만, 순측치조골폭경과는 순상관관계를 보였다
