Objective : This is to report the effectiveness of intraoral distraction osteogenesis, iliac bone graft for alveolar augmentation in the extremely atrophied alveolar defects after infected allobone grafted area. Subjects and Methods : Anterior segmental osteotomy was performed and the trans-oral alveolar distractors (Martin, Germany) were applied in patient with the severe acquired anterior mandibular and mandibular defect after ameloblastoma enucleation. Iliac bone grafts were performed in defect sites and distraction osteogenesis were treated. After latent period for 1 week, the osteomized alveolar segments were distracted by 0.75 mm a day (0.25 mm/1 turn) for 10 days The consolidation period was about 12 weeks. Thereafter, 2 titanium threaded implants were simultaneously installed with removal of distractor. For oral rehabilitiation, The implants were installed in maxilla, mandible. It was tested with clinically and radiographically. Results : Amounts of acquired alveolar bone were 10 mm with the increased width of the ridge crests and soft tissue expansion. Dental implants installated on the augmented alveolar ridges in 12 weeks after distraction were confirmed as in good osseointegration and in good function without any complications. Conclusion : Intraoral distraction osteogenesis can be a good option for alveolar ridge augmentation of the severely atrophied ridges and soft-tissue defects.
Objective: This study aimed to assess three-dimensional changes in the temporomandibular joint positions and mandibular dimensions after correction of dental factors restricting mandibular growth in patients with Class II division 1 or division 2 malocclusion in the pubertal growth period. Methods: This prospective clinical study included 14 patients each with Class II division 1 (group I) and Class II division 2 (group II) malocclusions. The quad-helix was used for maxillary expansion, while utility arches were used for intrusion (group I) or protrusion and intrusion (group II) of the maxillary incisors. After approximately 2 months of treatment, an adequate maxillary arch width and acceptable maxillary incisor inclination were obtained. The patients were followed for an average of 6 months. Intraoral and extraoral photographs, plaster models, and cone-beam computed tomography (CBCT) images were obtained before and after treatment. Lateral cephalometric and temporomandibular joint measurements were made from the CBCT images. Results: The mandibular dimensions increased in both groups, although mandibular positional changes were also found in group II. There were no differences in the condylar position within the mandibular fossa or the condylar dimensions. The mandibular fossa depth and condylar positions were symmetrical at treatment initiation and completion. Conclusions: Class II malocclusion can be partially corrected by achieving an ideal maxillary arch form, particularly in patients with Class II division 2 malocclusion. Restrictions of the mandible in the transverse or sagittal plane do not affect the temporomandibular joint positions in these patients because of the high adaptability of this joint.

Anterior open bite is a condition in which maxillary and mandibular incisors do not occlude at central occlusion. It is a vertical discrepancy of the jaws and dental arches that has many etiologic factors making it difficult in diagnosis, treatment and prediction of prognosis. One of the causes of open bite is abnormal size and shape of the tongue. Macroglossia, a condition in which tongue is oversized, is caused by several factors which are not clearly identifiable, and it may be a major factor of anterior and posterior open bite. Macroglossia is subdivided into true, functional and pseudomacroglossia depending on its relative size in the oral cavity. In this case report, a patient was diagnosed as skeletal Class II with pseudomacroglossia, and was treated with SARPE in order to expand the narrowed maxillary arch and Quad helix for the mandibular arch. As a result the transverse deficiency was treated. In the adult patients where no skeletal growth is expected, SARPE has shown to be effective in treating maxillomandibular transverse discrepancies in which macroglossia was accompanied as in this case.
11 years 7 months old female had a reverse object of anterior teeth and class III molar relationship. Maxillary right and left canines were erupted labially and lower left lateral incisor was congenitally missed. She was treated by rapid palatal expansion and full band technique with extraction of maxillary and mandibular second molars. The following results were obtained.
1. Maxilary arch was expanded.
2. Anterior cross-bite and crowding was corrected.
3. Molar relationship was corrected.
4. Profile was somewhat improved.

The purpose of this study was to obtain some informations for the radiographic differential diagnosis between dentigerous cysts and unicystic ameloblastomas in the mandible. The authors observed and compared the clinico-radiographic features of 38 cases of dentigerous cyst and 32 cases of unicystic ameloblastoma associated with impacted mandibular molar. The obtained results were as follows: Dentigerous cysts occurred the most frequently in the 3rd decade, but unicystic ameloblastomas in the 2nd decade, and both lesions occurred with slight predilection in males. Average of lesional size of unicystic ameloblastomas was larger than that of dentigerous cysts, and lesions of over 25㎠ were only in unicystic ameloblastomas. Cortical thinning and expansion were more frequently observed in unicystic ameloblastomas at 72.9% than in dentigerous cysts at 15.8%. Dentigerous cysts showed smooth border at 89.5%, but unicystic ameloblastomas showed smooth border at 53.1% and scalloped border at 46.9%. Dentigerous cysts showed well-defined outline at 81.6%, but unicystic ameloblastomas showed well-defined outline at 53.1% and moderate-defined outline at 46.9%. In both lesions, the mandibular 3rd molar was the most frequent causative tooth. Average of distance between the cemento-enamel junction and lesional wall attachment of the causative tooth was longer in unicystic ameloblastomas than in dentigerous cysts. Severe displacement of causative tooth was more frequent in unicystic ameloblastomas at 62.5% than in dentigerous cysts at 23.7%. Dentigerous cysts showed homogeneous lesional radiolucency at 89.5%, but unicystic ameloblastomas showed inhomogeneous lesional radiolucency at 53.1%. Root resorption of adjacent tooth and displacement of mandibular canal were more frequent in unicystic ameloblastomas at 65.2% and 61.5% than in dentigerous cysts at 15.8% and 38.1% respectively.
Objectives: Understanding the reason for an unsuccessful non-surgical endodontic treatment outcome, as well as the complex anatomy of the root canal system, is very important. This study examined the cross-sectional root canal structure of mandibular first molars confirmed to have failed non-surgical root canal treatment using digital images obtained during intentional replantation surgery, as well as the causative factors of the failed conventional endodontic treatments. Materials and Methods: This study evaluated 115 mandibular first molars. Digital photographic images of the resected surface were taken at the apical 3 mm level and examined. The discolored dentin area around the root canal was investigated by measuring the total surface area, the treated areas as determined by the endodontic filling material, and the discolored dentin area. Results: Forty 2-rooted teeth showed discolored root dentin in both the mesial and distal roots. Compared to the original filled area, significant expansion of root dentin discoloration was observed. Moreover, the mesial roots were significantly more discolored than the distal roots. Of the 115 molars, 92 had 2 roots. Among the mesial roots of the 2-rooted teeth, 95.7% of the roots had 2 canals and 79.4% had partial/complete isthmuses and/or accessory canals. Conclusions: Dentin discoloration that was not visible on periapical radiographs and cone-beam computed tomography was frequently found in mandibular first molars that failed endodontic treatment. The complex anatomy of the mesial roots of the mandibular first molars is another reason for the failure of conventional endodontic treatment.
Alveolar distraction osteogenesis (ADO) has been regarded as an acceptable treatment for the alveolar bone deficiency. For ADO at anterior maxillary area, the vector should be oriented to forward and down-ward direction to get an adequate occlusion with mandibular teeth and to increase bone length and width for implant placement. However, the conventional commercial distraction devices for ADO are designed to allow mainly downward movement of alveolar segment, even though a forward movement can be obtained a little by controlling of inclination of device. To make ADO with controllable bidirectional vector possible, we used customized devices using self-manufactured ABDUL (Alveolar Bone Distractor Using Lag screw principle) and commercial orthodontic palatal expansion device ($Hyrex^{(R)}$). In all cases (n = 4), ADO could be performed successfully and dental implants were able to placed with adequate occlusion. We report the procedures, advantages and disadvantages of these methods.

The author observed a 35-year-old male patient who came to the Dental Infirmary of Kyungpook National University Hospital who had complained of gradual swelling on both side of the mandible for 15 years. As a result of careful analysis of clinical, radiological and histopathological findings, the authors diagnosed it as cementifying fibroma and obtained the results as follows: 1. In clinical examination, main clinical symptom was facial deformity due to gradual swelling on both side of the mandible. 2. In radiographic examination, radiolucent lesions with central radiopaque foci were seen on both mandibular body areas bilaterally, and cortical thinning and expansion of the mandibular body were seen buccolingually. And loss of lamina dura and root resortption of adjacent teeth were also seen. In histopathological examination, this lesion was composed of delicate interlacing collagen fibers interspersed by the fibroblasts and cementoblasts. And round or oval shaped basophilic masses of cementum-like tissue were observed in the connective tissue.

The author compared and analysed the clinico-radiographic features of 48 cases of odontogenic keratocyst and 32 cases of unicystic ameloblastoma. The obtained results were as follows : 1. Odontogenic keratocyst and unicystic ameloblastoma occurred the most frequently in the 2nd and 3rd decades, and both lesions occurred with slight predilection in males. The most frequent lesional site was molar area in odontogenic keratocyst(50.0%) and mandibular angle and ramus area in unicystic amelobla-stoma(71.9%). 2. Cortical thinning and expansion were observed with similar occurrences in odontogenic keratocyst(77.l%) and in unicystic ameloblastoma(72.9%). 3. Typical undulating lesional border was observed more frequently in odontogenic keratocyst(79.2%) than in unicystic ameloblastoma(46.9%). 4. Well-defined lesional outline occurred more frequently in odontogenic keratocyst(97.9%) than in unicystic ameloblastoma(53.1%). 5. Root resorption of adjacent teeth occurred more frequently in unicystic ameloblastoma(65.2%) than in odontogenic keratocyst(18.8%) respectively, but loss of lamina dura was frequently observed in odontogenic keratocyst(79.2%). And tooth displacement occurred more frequently in odontogenic keratocyst(50.0%) than in unicystic ameloblastoma(17.4%). 6. Displacement of mandibular canal occurred more frequently in odontogenic keratocyst(75.0%) than in unicystic ameloblastoma(61.5%). 7. Inhomogeneous lesional radiolucency occurred more frequently in unicystic ameloblastoma(53.l%) than in odontogenic keratocyst(39.6%).

The occurrence of PVNS in the TMJ is very rare. We report a case of PVNS which was misdiagnosed as a parotid tumor at first. CT and conventional radiograph revealed a well defined mass demonstrating higher attenuation than adjacent soft tissue. Erosion and expansion of the cortical plate of the mandibular condyle and sclerotic change beneath the margin of the lesion were also shown. MRI demonstrated well defined mass of very low signal intensity on both Tl and T2-weighted images due to ferromagnetic effect of the high concentrated hemosiderin and clearly delineated the extent of the lesion. Histopathologic findings and characteristic appearance of the various imaging methods were described and the usefulness of these images for diagnosis and pretreatment evaluation of PVNS were also discussed.
