Removal of miniplates following facial trauma and orthognathic surgery: a 3-year study

  • The purpose of this study was to determine the cause and risk factors of removing bone plateby investigating and analyzing 359 patients who underwent reduction of fracture or orthognathic surgery with bone plate insertion over the past 3 years. Patients were evaluated with respect to age, smoking status, reason for insertion of plates, the numbers of inserted plates, sites of insertion, time between insertion and removal, reasons for removal of plates. The removal rate of bone plates was 33.1%. Of these, 17.0% of patients had clinical symptoms which led to remove plates. The removal rate of men was 29.9% and the rate of women was 39.2%. There were high removal rates from less than 20s (45.8%) and 20s (34.4%) those who are relatively young age group. On the other hand people in their 50s had a removal rate of 27.8% which was higher than those in their 60s with a rate of 20.7%. The removal rate of bone plate inserted into the mandible was 33.5%, and the removal rate of bone plate inserted into the maxilla was 34.7%. The mean period between the insertion and removal of bone plate was 10.9 months. The main reason for the removal of bone plate was the patient's requirement (44.5%). The most common cause of clinical symptoms was infection (22.7%). Infection was manifested within about a year and led to the plate being removed. Therefore, lowering the possibility of infection after surgery could decrease the removal rate of bone plate.

  • Maxilla protraction, orthopedic correction of deficient maxilla is necessary for treatment of most Class III growing patients. Protraction method has evolved from tooth-born appliance to miniplates on the maxilla, and from face mask to miniplates on the chin. By placing miniplates on the maxilla and the mandible and running elastics between them, we can minimize dentoalveolar relapse and maximize orthopedic change.

  • The authors have observed a recurrent fibrous dysplasia at the lower left canine region of a 16 years old Korean male admitted the Taegue Presbyterian Hospital on Nov. 28, 1968. Subtotal mandible resection was performed for the radical treatment and metal plate attached to anterior and posterior fragments by means of stainless steel wire and the result was satisfactory.

  • Condylar process of mandible is an important and fuctionally versatile part of the mandible. There were quite large amount of investigations on the functional and anatomical adaptation of the temporomandibular joint(TMJ) to the surrounding tissues. But controversies on the mechanism of functional adaptation of the joint still exist. In this research, we investigated changes in the TMJ by the lateral deviation of the maxillary incisor to shift the mandible right, and bone the undecalcified microscopic sections with fluorescent microscope and von Kossa staining with bright field microscope. Results were as follows: 1. Lateral deviation rendered shifting and tilting of the mandible, There were, compressions in the right joint and opening of the left joint space at early stage. At the same time, both condyles shifted slightly to anterior. 2. After $2{\sim}4$ weeks, left condyle showed anterior displacement and compressions in the joint space. Right condyle showed only slight shift to the anterior. 3. Regardless of the direction of the lateral shift, anterior bite plate compressed both condyle heads until 2 weeks. 4. There are bone resorptions in the anterior aspect of the condyle head and apposition of posterior border. Bone remodeling were observed between 3 and 4 weeks. 5. After 8 weeks of the experiment, there were little differences in condylar morphology between experimental and control group, though slight shifting and compression were still present in the experimental group. Lateral deviation of mandible evoked active remodeling of the TMJ until functional and anatomical reconstruction of TMJ position was achieved.

전치 유도각의 인위적 증가에 의한 저작근과 하악 운동 양상의 변화에 대한 연구 (A STUDY FOR THE CHANGES OF THE MASTICATORY MUSCLES AND THE MANDIBULER MOVEMENT EFFECTED BY INTENTIONAL INCREASE OF ANTERIOR GUIDANCE ANGLE)

  • This study was performed to measure the changes of the mandibular movement and the masticatory muscular activities - anterior temporal and masseter muscle of both side - reflected by intentional increase of anterior guidance angie. For this study, 5 volunteers (3 males and 2 females with average age of 24.0) were selected. Each volunteer had Angle's classification I and did not have any missing tooth except third molar and any extensive restorations. Metallic guide plate was made at volunteer's working model fabricated by improved dental stone and cemented to the palatal surface of maxillary central incisor using resin cement(Panavia $21^{(R)}$) and then adjusted not to give any occlusal interferences at intercuspal position. The activity of masticatory muscles and the changes of mandibular movement were recorded by EMG and Sirognathograph in Biopak analysing system(Bioresearch Inc., Milwaukee, Wisconsin, USA). Measurement was done at before experiment, immediatley after placement, 1 week after placement, immediately after removal, and 1 week after removal. The results were as follows: 1. Moderate phonetic disturbance and mild headache were occured to 3 volunteers for 2 days after setting and 1 volunteer had positive reaction to percussion and slight midline diastema. But all of these clinical signs were diappeared 1 week after removal and the other volunteer did not have any special clinical sign. 2. In the EMG of the mandibular rest position, the mean value of anterior tempotal muscle was increased immediately after placement(p<0.01) and then decreased 1 week after placement(p<0.05) and increased 1 week after removal(p<0.05) but not recovered as before experiment. The mean value of masseter muscle was decreased during the experiment period. 3. In the EMG during mandibular protrusive movement, all muscular activity was decreased during the experiment period. Reduced activity was not recovered 1 week after removal(p<0.03). 4. During the habitual opening, anteroposterior movement of mandible was decreased immediately after placement(p<0.05) and then increased 1 week after placement but not statistically significant(p>0.1). Vertical movement was not shown significant difference during the experiment period(p>0.1). Lateral movement was decreased immediately after placement(p<0.05) and then increased 1 week after placement but not recovered as before experiment. The opening and closing velocity of mandible was shown minor changes but not statistically significant. 5. During the habitual opening, anteroposterior movement of mandible was decreased 1 week after placement(p<0.05) and then increased immediately after removal and recovered 1 week after removal as before experiment. Vertical movement was not shown significant changes. Lateral displacement of mandible was increased continuously and recovered 1 week after removal. Opening velocity was temporarily increased immediately after removal but recovered and closing velocity was not shown significant changes. 6. During the right side chewing, anteroposterior movement of mandible was increased immediately after removal but recovered and vertical movement was not shown statistically significant results. Lateral displacement and velocity of mandible were not shown significant results. 7. During the left side chewing, the changes of mandibular movement pattern were not shown statistically significant results.

하악골 분쇄골절의 치료 (TREATMENTS OF COMMINUTED MANDIBULAR FRACTURES)

  • This study evaluated retrospectively the treatment method and postoperative complications of communited mandibular fractures. We analyzed the clinical and radiologic data of 14 patients with the comminuted mandibular fractures who were admitted to Chosun University Dental Hospital from January 1998 to December 2003. We reviewed the cause of trauma, fracture sites, treatment methods, and postoperative complications. Thirteen patients (93%) had a successful treatment outcome without complications. Only one patient developed postoperative osteomyelitis requiring early plate removal and sequestrectomy. For the comminuted fractures of mandible, internal fixation using micro- or mini-plate was an effective treatment method with a low incidence of major complications.

Use of a Y-Shaped Plate for Intermaxillary Fixation

  • Maxillomandibular fractures usually require intermaxillary fixation as a means to immobilize and stabilize the fracture and to re-establish proper occlusion. Arch bars or intermaxillary fixation screws cannot be used for edentulous patients or for patients who have poor dental health. Here, we present a case of repeated intermaxillary fixation failure in a patient weak alveolar rigidity secondary to multiple dental implants. Because single-point fixation screws were not strong enough to maintain proper occlusion, we have used Y-shaped plates to provide more rigid anchoring points for the intermaxillary wires. We suggest that this method should be considered for patients in whom conventional fixation methods are inappropriate or have failed.

Functional evaluation of orthopedic and orthodontic treatment in a patient with unilateral posterior crossbite and facial asymmetry

  • An 8-years old boy with facial asymmetry and unilateral posterior crossbite on the left side received orthopedic and orthodontic treatment. During the first phase of treatment, the narrow maxillary arch was expanded using an acrylic plate. Then, the acrylic plate was used as a bite block with occlusal indentations from the construction bite that was obtained with the incisors in a coincident dental midline. After the position of the mandible was stabilized, the second phase of orthodontic treatment was initiated using fixed appliances for detailing of the occlusion. Skeletal symmetry, ideal occlusion, and coincident dental midlines were thus achieved. Functionally, occlusal force balance and masticatory muscle activity were improved, and the chewing patterns were normalized.

  • This study was designed to determine the location of the mandibular canal on lower molar areas. Thirty-three patients were examined with multi-planar reformatted CT scan($Dentascan^{(R)}$). Three kinds of measurements were performed. The first was the distances between the upper border of the mandibular canal and the root apices of the first and second molars, the second was the distance between the cortical plate of the mandible and mandibular canal, and the last was the location of the mandibular canal in the buccolingual plane. The obtained results are as follows 1. The distance between the root apices of lower molars and the superior border of mandibular canal was largest at the mesial root of the first molar, and shortest at the distal root of the second molar(p<0.05). 2. The longest distance between the outer surface of the buccal cortical plate of the mandible and mandibular canal was measured from the distal root of the second molar, and this distance decrease gradually mesially(p<0.05). 3. The distance between the mandibular base and inferior border of mandibular canal was longest at the distal root of the second molar, and shortest at the mesial root of the first molar(p<0.05). 4. The location of mandibular canal was lingually positioned in relation to the axis of teeth and alveolar ridge in molar areas.

THE CLINICAL STUDY OF FACIAL BONE FRACTURE

  • This is a retrospective study on 452 patients with maxillofacial bone fracture. The patients were treated at the Dept. of Oral and Maxillofacial Surgery, WON KWANG UNIV. HOSPITAL from Aug. 1, 1984 to Sept. 30, 1988. The results were as follows. 1. The facial bone fractures occured most frequently in the twenties (35.3%)and male were predominant(75%) than female. 2. The most frequent etiologic factor was traffic accident. 3. The most common location of facial bone fracture was mandible(57%). Zygomatic bone & arch(39.9%) were most common on middle 1/3 of the face and symphysis(37.1%) were most common on the mandible. 4. The time from injury to treatment was variable from under the 1 week (65%) to over the 1 week(35%). In most case of the patients were treated beyond the 3 days(51.5%). 5. The relationship of intermaxillary fixation period and treatment method was an intimate relationship. The IMF period was reduced by the use of plate and screw osteosynthesis. 6. The weight loss was proportioned to intermaxillary fixation period.
