하악골에 발생한 황문근육종의 일례보고

  • A case of 35-year-man with a rhabdomyosarcoma in the mandible has been presented. Chief complaints are dull sensation and growing mass at the area of the right mandible. In order to make diagnosis, roentgenograms were taken which revealed some sort of radiopaque and punched appearane at the area from right body to angle of the mandible, and biopsy was made, by which rhabdomyosarcoma was observed. The author treated this patient in terms of hemimandiblectomy, neck dissection and resin-made mandible implantation, and has observed this patient for two and half years after operation. The author satisfy that this patient has been recovered.

Three-dimensional finite element analysis of the splinted implant prosthesis in a reconstructed mandible

  • PURPOSE. The purpose of this study was to analyze the effects of the splinted implant prosthesis in a reconstructed mandible using three-dimensional finite element analysis. MATERIALS AND METHODS. Three-dimensional finite element models were generated from a patient's computed tomography data. The patient had undergone partial resection of the mandible that covered the area from the left canine to the right condyle. The mandible was reconstructed using a fibula bone graft and dental implants. The left mandibular premolars and molars remained intact. Three types of models were created. The implant-supported prosthesis was splinted and segmented into two or three pieces. Each of these models was further subcategorized into two situations to compare the stress distribution around normal teeth and implants. Oblique loading of 300 N was applied on both sides of the mandible unilaterally. The maximum von Mises stress and displacement of the models were analyzed. RESULTS. The stress distribution of the natural mandible was more uniform than that of the reconstructed fibula. When the loading was applied to the implant prosthesis of reconstructed fibula, stress was concentrated at the cortical bone around the neck of the implants. The three-piece prosthesis model showed less uniform stress distribution compared to the others. Displacement of the components was positively correlated with the distance from areas of muscle attachment. The three-piece prosthesis model showed the greatest displacement. CONCLUSION. The splinted implant prosthesis showed a more favorable stress distribution and less displacement than the separated models in the reconstructed mandible.

FINITE ELEMENT ANALYSIS OF RECONSTRUCTION OF MANDIBULAR SYMPHYSIS DEFECTS USING RECONSTRUCTION PLATES (유한 요소법을 이용한 하악골 이부 결손수복에 사용된 재건용 금속판의 응력분포에 관한 연구)

  • Purpose: This study aimed to evaluate distribution and maximal value of mechanical stresses on the reconstruction plate, bridging mandibular symphysis defect, and to optimize the most appropriate locations of the plate to distribute the stress causing the fracture of the plate. Materials and methods: Four types of reconstruction were constructed by different number and location of the reconstruction plates on the 3 D finite element model (FEM) of a human edentulous mandible; Type I: one plate on the inferior border of the anterior mandible, Type II: one plate on the middle of the anterior mandible, Type III: one plate on the superior border of the anterior mandible, and Type IV: two plates on the inferior and superior border of the anterior mandible. Results: The results showed that the maximal stress of type I (234.29 Mpa) was lower than that of type II (260.91 Mpa) and type III (247.37 Mpa), but higher than that of type IV (186.64 Mpa). We could also observe that the stresses are tending to focus on the inner side and inferior part of the plate which connected proximal segment from the vertical load. Conclusions: On the basis of the findings, it was concluded that using a plate on the inferior border of mandible or two plates on the inferior and superior border of mandible are more favorable to distribute mechanical stresses, which could reduce the fracture of the plate.

DISTRACTION OSTEOGENESIS ON MANDIBLE SYMPHYSIS WIDENING WITH A BONE-BORNE TYPE DISTRACTOR (Bone-borne type distractor를 사용한 하악골 정중부 골신장술)

  • Transverse mandibular deficiency has a many problem in growing patients. Therefore, Mandible symphysis widening is recommended. A new surgical technique has been developed to widen the mandible. The purpose of this study is to report the effect of mandibluar symphyseal distraction osteogenesis by use of bone-borne type distractor. The surgical procedure was accomplished under general anaesthesia with step-line osteotomy. Bone-borne type distractor was used to generally widen the mandible. The expansion achieved in the mandible was 8.31mm at the device, 6.32mm at the canines, 4.06mm at the first molars. The symphyseal distraction gaps were bridged by new bony regeneration. Mandibular symphseal distraction osteogenesis increased mandibular arch width, stabilized occlusion, and corrected dental crowding.

EXPERIMENTAL STUDY ON THE EFFECT OF X-RAY IRRADIATION ON THE RAT MANDIBLE DEVELOPMENT (X-선 조사가 악골발육에 미치는 영향에 관한 실험적 연구)

  • The author observed the effect of x-ray irradiation on the development of the mandible in the fetuses and growing albino rats. The fetuses were irradiated on the 7½th day of gestation, 100, 200, 300, and 400 rads of x-ray respectively. The experimental animals were sacrificed on the 18½th day of gestation, and first week, second week and third week after parturition. The results were as follows; 1. The mandible of the 18½th day fetuses showed irregular bone trabeoulae, osteoclasts and osteocyte degeneration on the 300 and 400 rads x-ray irradiated fetuses. 2. In the mandible of the first week rats, there was marked osteocyte degeneration and a lot of osteoclast. 3. In the mandible of the 2nd and 3rd week rats bone remodeling was evident. The 3rd week rats also showed alteration of blood vessel wall.

Post traumatic malocclusion and its prosthetic treatment

  • Mandible fractures belong to the most common fractures encountered in maxillofacial trauma. Because mandible is such a unique structure with hinge joint and masticatory muscles attached to the body of mandible, attention must be paid to avoid displacement during treatment. Displacement during fracture reduction leads to malocclusion. Many TMJs function with complete comfort and apparent normalcy in adapted centric posture, even though they have undergone deformation caused by trauma. This clinical report describes the patient with post traumatic malocclusion and its prosthetic treatment. His fractured mandible was openly reduced in changed position, as a result his occlusion has been changed. He was treated by prosthetic method in so-called adapted centric posture.

Conservative neural symmetry of the caprine mandible

  • Fifteen dry dentate and complete mandible samples from the White Rasquera goat breed were studied for symmetry. Thirty-one landmarks were digitally located on the images of the lateral and medial aspects of each hemimandible. Distances between these landmarks allowed the evaluation of the whole hemimandible and also the neural mandible. In the studied samples, the mandible was rather symmetrical, especially in the medial neural part, and in general, there was no side dominance. Only the diastema differed significantly between the sides, and this was related to the rostral part (incisive arch). The incisive region was the least symmetrical region of the caprine mandible, indicating a modular structure more conservative for the neural part. If unsigned asymmetry is interpreted as a measure of developmental stability, then the studied breed presented a marked ability to develop in good fitness despite the harsh environment. The measurements presented here can also be used as a reference for researchers designing experimental studies, especially on mandibular catch-up growth, and as an aid for zooarchaeologists comparing results from dead animals with those from living goat populations.

A huge glandular odontogenic cyst occurring at posterior mandible

  • The glandular odontogenic cyst is a rare lesion described in 1987. It generally occurs at anterior region of mandible in adults over the age of 40 and has a slight tendency to recur. Histopathologically, a cystic cavity lined by a nonkeratinized, stratified squamous, or cuboidal epithelium varying in thickness is found including a superficial layer with glandular or pseudoglandular structures. A 21-year-old male visited Dankook University Dental Hospital with a chief complaint of swelling of the left posterior mandible. Radiographically, a huge multilocular radiolucent lesion involving impacted 3rd molar at the posterior mandible was observed. Buccolingual cortical expansion with partial perforation of buccal cortical bone was also shown. Histopathologically, this lesion was lined by stratified squamous epithelium with glandular structures in areas of plaque-like thickening. The final diagnosis was made as a glandular odontogenic cyst.

Mandibular Reconstruction with Free Fibular Flap and Dental Implant after Ablative Oral Cancer Surgery Using 3D RP Model: A Case Report

  • Reconstruction of the mandible after ablative oral cancer surgery requires esthetic and functional rehabilitation. Restoring facial symmetry and dentition need accurate preoperative surgical planning and meticulous surgical technique. Free fibular flap is most useful tools to reconstruct mandible because of its adequate length and height, simultaneous harvest of soft and hard tissues and placing dental implants. In this case report, recurred squamous cell carcinoma in the right mandible had been resected and free fibular flap was utilized for mandible reconstruction using 3D rapid prototype. Simulation surgery before dental implant placement has been performed for esthetic and functional prosthodontics.
