Purpose: This study was conducted to examine the effect of visiting nursing of the visiting nursing services based on the district management system on the subjects of stroke patients. Method: A nonequivalent control group pretest-posttest design was applied to 50 stroke patients (31 from the experimental group, 19 from the control group). To evaluate the effects of visiting nursing services. health status(SF-36) and activity daily living(ADL/IADL) were measured from June 2003 to November 2004. Result: Health condition and the activities of daily living (ADL/IADL) of the subjects who received visiting nursing service were shown to be improved. Conclusion: Visiting health service model based on the district management system in public health center is considered to be an effective measure of visiting nursing services in large cities.
Purpose: This study was performed to explore the trend of nursing research in the Korean long-term care system. Methods: Articles published between 2008 and 2021 were searched using the terms "long-term care system" and "nursing." Ninety-one articles were analyzed and classified into five categories according to research methods. Data were extracted through a systematic review process and underwent descriptive statistics and content analyses. Results: The most analyzed variable in the survey studies was job satisfaction. Many variables were classified into input and output factors using Donabedian's model. The content analysis showed that most suggestions were about improving the political regulation system. Conclusion: It is necessary to establish a research foundation to provide research funding and support to cultivate future nursing research in long-term care. Systematic improvement of research in nursing should be continuously pursued to revitalize nursing services, expand nursing service interventions, and improve management competency programs in nursing institutions.
Purpose: This study was performed to explore research trends in the intention to stay of hospital nurses and provide basic data to establish nursing management strategies to increase the intention to stay. Methods: Articles published between 2009 and 2018 were searched. The search terminologies were "intention to stay," "nurse", and "hospital". In the first search, 381 articles were extracted from academic databases. Thirty articles were used in the systematic review, and 29 articles were used in the correlation meta-analysis. Results: Thirty-two variables were explored in relation to the intention to stay. In the correlation analysis, job satisfaction and work environment showed statistically significant positive correlations in many studies. In the correlation meta-analysis, 7 variables including organizational commitment showed statistically significant effect sizes. Conclusion: We suggest that structural equation model analysis to identify causal relations among influencing variables of the intention to stay of hospital nurses may be conducted. This study can be used as a guideline to develop intention-to-stay enhancement programs for hospital nurses.
The home health care industry has grown rapidly and can be expected to continue to grow in the foreseeable future. Home health care refers to the practice of nursing applied to clients with a health condition in the clients place of residence. clients and their designated care givers are the focus at home health nursing practice. The goal of care is to initiate. manage and evaluate the resources needed to promote the clients optimal level of well-being and function. Nursing activities necessary to achieve this goal may warrant preventive maintenance and restorative emphases to prevent potential problems from developing. Home care nurses move through a variety of communities and often care for patients from different cultural back grounds. Therefore Leininger's work has a good that with home care because home care nursing practice is very culturally focused. 4. Dorothea Orem's : Self care deficit theory. Orem's theory views care as something to be performed by both nurses and patients. The role of the nurse is to provide education and support that help patients acquire the necessary activities to perform self-care. Orem's theory is foundational to have care because it begins to truly acknowledge the role of the patient in managing his or her own health. which is referred to as self-care. 5. Margaret Neuman's; Health as expending consciousness theory. Neuman believes that health compasses disease and reflects an underlying pattern of person-environment interaction. This study aims to explore the variable factors by which fathers affect nursing practices. for this study 544 married couples were interviewed, which resulted in the following conclusions. First, wives have higher fairness than husbands in sharing their parental roles. Second, in some couples, there was a difference seen in the expectations of each parents nursing capability, but rarely were differences in real nursing practices actually observed. Third, all couples interviewed showed some disparity between their expectations of parental nursing capability and their real nursing practices, where the expectations for nursing capability were higher than the real nursing practices. Fourth, higher expectations for nursing capabilities from fathers result in them having better parental roles. Fifth, there is no correlation between parental nursing practices and social demographic variables. Lastly, the impacts on the parental nursing practices are ranked by relative importance as parental nursing capability, the wife's expectation for parental nursing capability and so on.
The traditonal inpatient acute hospital setting is organized primarily for the intensive management of disease, but not well-suited for continuity of care for the chronically ill patients after being discharged from hospital. For the planning of the continuity of care, firstly, it is necessary to assess the home care needs of the discharged pateints in the context of the nursing diagnosis. Therefore, this study is designed to identify the home nursing care need trajectory of the patients with chronic illness after discharged from one of the the General Hospitals in Seoul, Korea. The subjects are the patients with chronic illness such as stroke, musculoskeletal disease, hypertension, cancer etc., in average age of 52. 2 years old. The findings of this study are as follows : 1) The limitaion of ADL has been constantly facing to the subjects and has not been changed 4 weeks after being discharged. And the sense of with-drawal was getting worse at 4th weeks than the 1st week after being discharged. 2) The lists of the patient's problems are the impairment of mobility, elimination pattern, inactivity, impairment of skin integrity, ineffective airway clearance, and potential anxiety, self concept deficit, ineffective family coing, etc. Those problems were diminished in quantity at the first week after discharged, but at the 4th week, those problems were getting worse. 3) The need of specialized nursing care such as tube feeding, ostomy care, $O_2$ inhalation, IV therapy, teaching and exercise are considered as the most consisting problems facing to the subjects. 4) In general, the chronically ill patients and their caregivers have not been adapted well even at the 4th weeks after being discharged. 5) Considering those findings, the basic care for patients should be given and the trainging for process of the adaptation after discharged should be encouraged prior being discharged from hostital. For this suggestion, the systematic discharge planning should be carried and the hospital based home nursing model should be implemented at the general hospital for the chronically ill patients.
The purposesof the study are to analyze the community nursing center in U.S.A and to develop the model of nursing care system based on nurse-midwifery clinic in community for women's health in Korea. 1. In America nursing center is defined as nurse-anchored system of primary care delivery or neighborhood health center. Nursing centers are identified the following four types: (1) community outreach centers, which are similar to traditional public health clinics: (2) institutional-based centers following the mission of a large institution, such as a hospital or university: (3) wellness/health promotion centers, which offer screening, education, counseling, triage, and health maintenance services: and (4) independent practice. Nursing centers are a concept of services provided by nurses in practice arrangements in a community. Nursing centers offer a variety of services, ranging from primary care provided by advanced practice nurses with medical acute management and nursing care to the more traditional education, health promotion, screening wellness and coordination services. Some services, such as the care provided by advanced practice nurses are reimbursed under various insurance plan in some instances and states, where as others, such as preventive and educational services, are not. Thus, lack of reimbursement has threatened the survival of some centers. Licensing of nursing centers varies by state and program and accreditation of nursing centers is also limited. 52% of centers are affiliated with another facility and 48% are freestanding centers. The number of registered nurse at the nursing centers ranges from just one to 115, with a mean of eight RNs peragency and a median of three. Nursing centers avail ability varies: 14% are open 24 hours, 27% have variable short hours, 23% are open 6-7 days per week, and 36% are open Monday- Friday. As the result of my visiting three health centers in Seattle and San Francisco, the women's primary care nurse practitioners focus on a systematic and comprehensive assessment of the health status of women and diagnosis and management of common physical and psychosocial health concerns of women in ambulatory settings. Therapeutic nursing strategies are directed toward self-care, risk reoduction, health surveillance, stress reduction, healthy nutrition, social support, healthy coping, psychological well-being, and pharmacological therapy. They function as primary care providers for the well ness and illness care of women from adolescence through the older adult years and pregnant families. 2. In Korea a nurse-midwife practices independently for pregnant women's health including childbearing family at her own clinic in community. Her services are reimbursed under national health insurance but they are not paid on a fee-for-service schedule covering items. Analyzing the nursing centers in America, I suggest that nurse-midwifery clinics offer primary care for women and home care for chronic ill patients. The health law and health insurance policy should be reovised in order to expand nurse-midwife's and home care nurse's roles at nurse-midwifery clinic.
본 연구는 가정간호기반 호흡관리프로그램이 근위축성 측상경화증으로 가정에서 인공호흡기를 적용하고 있는 환자에게 미충족의료와 의료자원이용에 효과가 있는지를 확인하는 연구이다. 연구대상자는 실험군 19명, 대조군 21명으로 40명이다. Cox의 상호작용모델에 기반을 둔 가정간호기반 호흡관리프로그램은 인지적 동기로 교육, 전문간호, 사례관리, 내적 동기로 기도청결, 흉곽운동, 공기누적운동, 심리적 반응으로 명상과 적극적 경청이다. 이를 실험군은 12주 동안 제공하였고, 대조군은 통상적인 가정간호를 제공하였다. 중재관련변수는 사전, 사후 12주, 24주에 측정하였고, 의료자원이용은 24주에 측정하였다. 연구결과 호흡관리프로그램은 미충족의료정도에는 차이가 없었으나, 의료자원이용정도 중 입원횟수 감소에는 효과가 있었다. 이에 근위축성 측상경화증 환자 호흡관리프로그램은 호흡문제로 인한 의료기관 이용횟수를 감소시켜 환자의 호흡기능 향상에 기여할 수 있을 것이다.
There has been a dramatic increase in public awareness regarding dementia during recent years. However, dementia remains a family affair and patients do not receive adequate care in Korea. This study aims to assist patients and their caregivers by establishing Home and Community based Long-Term Care in a city. The data collected for analysis include five main categories: dementia prevalence, limitations of daily activities of patients, burden of caregivers, the services that patient's family want to utilize, the resources that handle dementia in the community. Major findings can be summarized as follows: 1)The prevalence rate of dementia for elderly people is 13.1 per 100 persons. Alzheimer's disease amount to 38.9% of dementia patients and vascular dementia account for 36.7% of them 2)Eight out of ten patients have mild dementia. Almost all patients have normal ADL. IADL, however, shows different picture. In every items of IADL, about 60% of patients reveals some limitations. 3)The proportion of patients who had medical diagnosis is as low as 20%. Families of patients think dementia as normal aging process and medical doctors in the community do not give special concern to dementia patients. 4)Caregivers does not have proper social support. They suffer from long care time, experience large obstacles in respect of health, daily living, and social activity. 5)Health center and Community welfare center have launched some programs-consultation, home-visiting nursing, day care center, voluntary force mobilization and so on-for dementia patients. But they do not perform expected roles and functions because of lack of skilled personnels and inadequate coordination of relevant organizations for dementia care. 6)Families of dementia patients prefer home helper and home-visiting nurse to hospitalization. For the future, however, demand for institution-based long-term services will increase. We develope community dementia care model based on above findings as follows: 1)Health center execute community cardiovascular control program for the prevention of vascular dementia. 2)Refer to epidemiologic characteristics of patients and preference of family, the most urgent task for dementia care in this city is to expand and organize Home and Community based Long-Term Care. 3)For the continuous and comprehensive care, care plan for a patient must be prepared. Case management team should be builded to prepare this plan and coordinate relevant resources. 4)Special long-term care unit for dementia will be needed in a near future. This unit should have multiple functions, such as day-care center, short stay facility, training center for relevant personnels, besides long-term nursing home considering effective care of dementia and efficient operation of the facility. 5)Voluntary workers deserve their due efforts. Incentive mechanisms must be developed to activate voluntary activities.
본 연구는 방문건강관리사업에서 동일한 문제를 가진 대상자의 자기관리 증진을 위한 자조집단 현황을 파악하기 위한 조사연구이다. 전국 253개 보건소를 대상으로 설문조사하였으며, 2010년 9월 3일부터 9월 20일까지 회수된 109개소의 자료를 SPSS/WIN 18.0프로그램을 이용하여 분석하였다. 연구결과 보건소에서 방문간호사업을 담당하는 간호사는 평균 9.27명, 대상 보건소 중 자조집단 프로그램을 실시한 보건소는 48개(44.0%), 자조집단 중 암 관련 프로그램이 65건(28.8%)으로 가장 많았으며, 실행예산은 500만원 이상은 19.6%로 재정적 지원이 열악한 현실이었다. 반면에 전문인력, 자조집단 관련 정보 및 예산 부족 등의 이유로 자조집단을 운영하지 못하는 보건소들도 자조집단 프로그램의 필요성은 인식하고 있었다. 결론적으로 자조집단 형성과 운영을 위한 매뉴얼 개발 및 효과성 검증을 위한 객관적 척도 개발 연구에 기초자료를 제공할 수 있다고 본다.
