Changes in the Medical Cost and Practice Pattern according to the Implementation of per Diem Payment in Hospice Palliative Care (완화의료 일당정액수가제 시행에 따른 진료비와 진료행태의 변화)

  • Background: As of July 2015, per diem payment was changed from fee for service Therefore, this study aims to analyse changes in medical charges and medical services before and after enforcement of the palliative care, targeting palliative care wards in a general hospital, and provide basic data needed for development of per diem payment. Methods: The subjects of the study were a total of 610 cases consisting of 351 patients of service fee who left hospital (died) from July 2014 to June 2016 and 259 ones of per diem payment at Chosun University Hospital in Gwangju Metropolitan City. Results: The results are summarized as follows. First, after the palliative care system was applied, benefit medical service charges and insurance increased significantly (p<0.001). As benefit medical service charges increased, benefit private insurance payment increased significantly (p<0.001). Second, after the per diem payment was applied, total private insurance payment to medical institutes decreased significantly (p=0.050) and non-benefit also decreased significantly (p=0.001). Conclusion: It is suggested that additional rewards in the obligatory palliative care items should be continuously remedied and monitored to provide good quality hospice palliative care.

Variation in hospital length of stay according to the DRG-based prospective payment system in the voluntarily participating providers (DRG(Diagnosis-Related Group)를 이용한 포괄진료비 지불제도의 선택 참여에 따른 재원일수 변화)

  • This study explored the impact on the DRG(Diagnosis-Related Groups)-based prospective payment system(PPS) operated by voluntarily participation providers. We analyzed whether the provides in the DRG-based PPS and in traditional fee-for-service(FFS) systems showed different the degree of variation in length of stay(LOS), and the providers' behaviors depending on the differences according to the varied participation periods. The study sample included all data 2,061 institutions participated in DRG-PPS in 2007 and all cases 473 FFS institutions which reported fee-for-service claims were reviewed same diagnosized diseases at least 10cases claims during three months We compared the differences of the LOS among health care institutions according to their type, region, and size. For DRGs showing significant differences in LOS, multiple regression analyses were performed to find out factors associated with LOS and interaction effect participation and hospital types or participation periods. The result provide the evidence that the DRG payment system operated by volunteering health care institutions had impact on resources use, which can reduce the institutions' the length of stay. While some DRGs had no correlation between participation periods and LOS, other DRGs, DRG participation period reversely linear relationship with LOS. That is to say, the longer participation year, the less reducing the LOS. These results support the future expansion of the DRG-based PPS plan to all health care services in Korea.

A Comparative Analysis of RBRVS for a Doctor's Consultation in Western and Oriental Medicine (양한방 진찰행위의 상대가치 비교분석)

  • This paper analysed the RBRVS for a doctor's consultation by measuring the time consumed in outpatient consultation, and compared the time among medical doctors, dentist, and oriental medical doctors. The time used in consultation could be a proxy for measuring RBRVS for medical services because it is the only common factor we observe in three different clinical settings. The results show that the optimal RBRVS for consultation is 183.22 for medical doctor, 99.12 for dentist, and 236.17 for oriental medical doctor. This implies the current fee schedule for consultation should be revised as 10,740 Won for a visit to medical doctor, 5,808 Won for dentist, and 13,832 Won for oriental medical doctor.

Determinants of Inpatients Satisfaction and Intent to Revisit Oriental Medical Hospitals

  • Objectives: This study aimed to investigate the determinants of inpatients satisfaction and intent to revisit oriental medical hospitals. Methods: The independent variables of the study contain the level of satisfaction with doctors, nurses, staffs, medical fee, environment, facilities and hospital life. Each variable was measured by three to eight items about the level of satisfaction with various aspects of hospital ward life. The level of overall patient satisfaction was used as an intervening variable and the level of intent to revisit was used as a dependent variable. The sample used in this study consisted of 268 inpatients from 3 oriental medical hospitals located in Chungnam Province. Data were collected with a structured and self-administrated questionnaire and analysed using path analysis. Results: The major findings of the study were as follows : First, it was found that the 3 variables of hospital life satisfaction, ancillary staffs satisfaction, and metropolitan residents has significant positive effect on the level of overall patient satisfaction. Second, the overall satisfaction and the satisfactory level of facilities, medical fee, and quality of ward life were found to have significant effects on the level of intent to revisit of respondents. Conclusions: The results of the study indicate that oriental medical hospitals should make an effort to improve the overall satisfaction of inpatients, especially focusing on the facilities convenience, medical price, and hospital ward life which will lead to high level of intent to revisit of inpatients.

Comparison of Nurses' Job Satisfaction, Patients' Satisfaction and Direct Nursing Time according to the Change in Grade of the Nursing Management Fee (입원환자 간호관리료 차등제 변화에 따른 간호사 직무만족, 환자만족도 및 직접간호시간 비교)

  • Purpose : This study aimed to identify nurses' job satisfaction, patients' satisfaction, and direct nursing time according to the change in grade of nursing management fee. Methods : Descriptive design was used in this study. Nurses (n = 200) and patients (n = 200) were recruited from one university hospital in Busan. Four aspects were measured: direct nursing time, overtime, nurses' job satisfaction, and patients' satisfaction. Data were analyzed using descriptive statistics, ${\chi}^2-tests$, and t-tests. Results : There was a significant difference in patient satisfaction (t = -2.09, p = .038) and direct nursing time (t = -4.77, p < .001) when the nurse staffing grade was changed from the level two to the level one. Conclusion : The findings from this study showed that a higher nurse-to-patient ratio can provide a greater amount of direct nursing time for individual patients and increase patient satisfaction.

A Study on The Determinants of the Medical Expenses in the Health Insurance System in Korea (의료보험 진료비의 결정요인에 대한 연구)

  • Since the inauguration of the medical insurance system in 1977, the increasing medical expenses which can be menace to tile finance of the medical insurance system, have become major concern in the medical insurance field In Korea. This study focuses on the determinants of the medical expenses in the health insurance in Korea and analyzes the impact of these factors on the increase in the medical expenses. The empirical work is done using the pooled cross-section and time-series data of the medical insurance for the self-employeds and the industrial workers from the year 1995 to 1997. The result of this study shows that the main determinants of the medical expenses in the health insurance are the ratio of the population of the aged to the total population, the frequency of the utilization, number of doctors per capita and the regime changes. Although the increasing trend in the medical expenses seems to be unavoidable, we probably need to add some efficiency to the medical expenses by suppressing the supply and the utilization of the unnecessary medical services. The fee-for-service reimbursement system of today can't suppress the supply of the unnecessary medical services effectively. So we need to convert the present fee-for-service system into DRG's which is known to reduce the medical costs. The increase in the medical expenses comes from a lot of factors. Therefore, we should develop more systematic and comprehensive measures to control the soaring medical expenses in consideration of the various factors such as demand, supply, and the organizational side of the medical system.

  • The competitive bidding model with the reserve price has been studied by Riley and Samuelson. We extend their studies to the competitive model with the reserve price and the entry fee. First we present the bidder's optimal strategy, the winner's expected profit, the auctioner's expected revenue in the first-price sealed bidding model, and next those in the second-price sealed bidding model.

  • Recently, technology using a PC and the personal server efficiently has been highlighted in accordance with the historical background. Solutions for large enterprises and public institutions and PC and personal server management are being commercialized for fee. Paying a fee for PC and personal server management being a burden and commercialized solutions include a function that does not require. In this paper, proposed the PC and personal server management system include monitoring that cpu, hdd, memory, respond speed. It expects to extend the PC and service life is available without charge through the free distribution of the burden. The Development program and application is available for free from the android market and on-line. Server agent program of the proposed system operates based on a single-threaded system. That is low load and the low functional burden on the server.

Assessing the Willingness-to-Pay of a Scuba Diving Management in a Soft Coral Marine Protected Area (다이버를 대상으로 한 연산호(軟珊瑚) 서식 해양보호구역 다이빙 관리제도의 지불의사액 분석)

  • A marine protected area (MPA) is a spatially designated section of the ocean where the biodiversity is high. Munseom MPA has a unique underwater landscape including soft coral, subtropical fish, and benthos, attracting many scuba divers. This paper intends to evaluate the willingness-to-pay (WTP) for the scuba diving management policy in the Munseom MPA. As a stated preference valuation method, a choice experiment was applied to assess divers' preferences with economic values for the policy. The attributes used in the study included zoning (temporal closure), conservation education, daily permit, soft coral area, and diving fee. We collected 333 questionnaires through online and field surveys and used 296 after excluding incomplete responses for the final data analysis. Study results show that a six-month closure, a decrease in soft coral habitat, and the diving fee were statistically significant. A positive coefficient with regard to the six-month closure variable indicates that divers preferred the policy, and the marginal willingness to pay (MWTP) was estimated to be KRW 8,637 per person per dive. The aggregate WTP of KRW 233 million was obtained by multiplying the MWTP by the number of visiting divers per year. When comparing the levels of recreation specialization, more skilled divers preferred the management option. The results of this study will help predict the priority of management measures and the levels of acceptability among divers when diving management is implemented in Korea in the future. This study suggests that marine environmental policies need to consider divers as one of the important stakeholders for marine conservation.
