A clinical study of the mediastinal tumors and cysts. [50 Cases Analysis] (종격동 종양 및 낭종 50례에 대한 임상적 고찰)

  • We have experienced 50 cases of mediastinal tumors and cysts from March, 1979 to August, 1985 at Kyung Hee University Hospital. The results of this cases analysis were as followings; 1. Of all 50 mediastinal tumors and cysts, 26 patients were male and 24 patients were female. There was no sex preference. The age distribution was from 27 months to 64 years, and mean age was 33.5 years old, and also no age preference. 2. The most common mediastinal tumor was benign cysts [12 cases], which comprise 24% of all mediastinal tumors and cysts. The second common mediastinal tumor was teratoma [9 cases-18%], and followed by thymic tumors and tuberculous granuloma [7 cases-14% each], neurogenic tumors [5 cases-10%], and other tumors [10 cases-20%]. 3. The anterior mediastinum was most common tumor location, and followed by middle, superior, and posterior. 4. All 9 teratomas were developed at anterior mediastinum, and 4 of 5 neurogenic tumors were developed at posterior mediastinum. Thymomas were developed at anterior and superior mediastinum. The bronchogenic cysts had no predilection of location. 5. The most common chief complaint at admission was chest pain or discomfort [23 cases-46%], and followed by cough with or without sputum, and exertional dyspnea. Asymptomatic patients were only 7 patients [24%]. 6. Of all 50 cases, 38 cases [76%] received radical tumor resection, 7 mediastinoscopic biopsy, 3 explo thoracotomy and biopsy, and 1 neck mass biopsy. 7. There were 2 hospital deaths, one of which was a patient who suffered malignant thymoma and Myasthenia Gravis. The patient received radical tumor excision, but died at 7th POD. The other patient was a patient with malignant transformation of the benign cystic teratoma. The operative mortality was 4%.

  • For the purpose of analysis of clinical and pathological characteristics in mediastinal tumors and cysts, 82 patients with mediastinal tumors and cysts treated in the department of thoracic and cardiovascular surgery in Chosun University Hospital during the period from January 1978 to December 1994 were reviewed. There were 49 male and 33 female patients in the study. Age ranges from 10 months to 84 years, with the mean 37.2 years. Frequently encountered symptoms and signs were dyspnea[40.2% , abnormal breathing sound[37.8% , chest pain[35.7% , cough[26.8% , and 18.2% of patients were asymptomatic. The most frequent tumor was anterosuperior mediastinum [59.8% followed by middle[24.4% and posterior mediastinum[15.8% . The malignant tumors were found in 35 cases[42.7% . Successful removal of the mass was possible in all the benign mediastinal masses[57.3% . But in the malignant cases, the surgical removal was possible in 18 cases and other inoperable cases were treated by radiation and chemotherapy. The postoperative complications occurred in 9 cases. Usual complications were bleeding[4 cases , wound infection[3 cases , pneumothorax[1 case and vocal cord paralysis[1 case . In the pathologic viewpoint, teratodermoid tumors[22.0% were the most frequent tumor followed by thymomas[19.5 , benign cysts[15.8% , lymphomas[13.4% and neurogenic tumors[8.5% .

Report of 25 Cases of Mediastinal Tumors (종격동 종양 25례 에 대한 임상적 고찰)

  • Clinical observation were performed on 25 cases of Mediastinal tumors or Cyst, those were admitted and treated at the Department of Thoracic and Cardiovascular Surgery, Korea University Hospital, during the 6 years period from March 1973 to March 1979. The following results are obtained. Of 25 cases, 19 patients were males and 6 patients were females. Range of age varied widely from 2 years to 72 years. Approximately 28% were younger than 15 years of age at the time of diagnosis. The common subjective symptoms of the patients were anterior chest pain [36%], coughing [27%], dyspnea and a few incidence of hemoptysis. Diagnostic procedures were posteroanterior and lateral chest roentgenorgrams, Chest tomograms, Brochograms, Esophagograms, Mediastinoscopy, Scalene and Axillary Lymph node biopsy, and Needle aspiration biopsy. In the histological distribution on Mediastinal tumors in order of frequency, Neurogenic tumor 6 cases [25%], Lymphoma 5 cases [21%], Bronchogenic cyst 4 cases [17%], Pericardial cyst 2 cases [8.3%], Teratodermoid tumor 2 cases [8.3%], and each one case of Rhabdomyosarcoma, Seminoma, Cavernous hemangioma, Anthracosis, Tuberculoma were noted respectively. Of 24 cases of the histologically confirmed Mediastinal tumors, 6 cases [24%] were malignant. Thoracotomy for removal of tumor or cyst was performed on 17 cases and offered cure of all benign tumors. In 6 cases of malignant tumors, Chemotherapy with Vincristine, Cyclophosphamide and Prednisolone was given to 1 case Lymphoma. There was no case of postoperative mortality.

  • Author made a clinical study of 48 cases of primary mediastinal tumors experienced in the dept. of the thoracic and cardiovascular surgery of Pusan National University Hospital during the 12 years period from march 1978 to march 1989. There were 34 males and 14 females. Their age distribution was from 4 months to 70 years, with the mean age of 34.4 years. 8.3 % of the patients were younger than 15 years old. There were teratoma 14 cases[29%], thymoma 11 cases[23%], neurogenic tumor 10 cases[21 %], lymphoma 6 cases[13 %], benign cyst 6 cases[13 %], and one case of fibrous histiocytoma in the histological distribution. The malignant tumors were 12 cases[25 %]. The common symptoms were chest pain and discomfort[35.4], coughing[18.8], general weakness and dyspnea. 16.7% of the patients were asymptomatic at admission. The successful removal was done in all cases of benign mediastinal tumors. In malignant cases, the surgical removal could be done in 5 cases. There was not postop. mortality. The frequent complications were atelectasis, infection, bleeding.

  • Fifty-five patients with primary mediastinal tumors and cysts which were seen at T.S. Dept., HYUH, were analyzed clinically, histologically, in an incidence of anatomic location and therapeutic results. The results were summarized as follows; 1] The ages of the patients in this series ranged from 15 months to 79 years with the highest incidence in the age group of third decade, and there were no sex distribution[M:F=0.96]. 2] The most frequently encountered tumors were teratodermoid tumors[29%] followed by neurogenic tumors[22%], thymomas[15%] and benign cysts[11%] in decreasing order of frequency. 3] Based on the subdivision of the mediastinum, 62% of the tumors were in the antero-superior mediastinum, 7% in the middle mediastinum and 31% in the posterior mediastinum. 4] The most frequent symptom was chest pain and others were dyspnea, cough, chest tightness and dysphagia. Asymptomatic patients were 29%. 5] Benign tumors and cysts were 71% and malignant tumors were 29%. 6] The successful removal was possible in all the benign mediastinal tumors and cysts[39 cases] and partial removal or biopsy was performed in the 12 cases among 16 cases of malignant mediastinal tumors. 7] Postoperative complications were bleeding, chylothorax, vocal cord paralysis, wound infection and hypothyroidism. 8] The most frequent mediastinal tumor in the West is neurogenic tumor but is teratoma in Korea.

Recurrent Malignant Thymoma Detected by Tc-99m MIBI, Tc-99m Tetrofosmin and Tc-99m (V) DMSA Scan (Tc-99m MIBI, Tc-99m Tetrofosmin, 그리고 Tc-99m (V) DMSA 스캔에 의해 발견된 재발성 악성 흉선종)

  • Thymoma is the most common primary tumor of anterior mediastinum, accounting for 20% to 30% of all mediastinal tumors. The recurrence rate after total resection of the thymoma ranges 8% to 18%. We reported one patient of recurrent malignant thymoma imaged with Tc-99m MIBI, Tc-99m Tetrofosmin and Tc-99m (V) DMSA. Early and delayed Tc-99m MIBI and Tc-99m Tetrofosmin scintigraphies showed an increased uptake in the mediastinal area. Also, Tc-99m (V) DMSA scintigraphy revealed an increased uptake tn the corresponding area. Coronal SPECT images of Tc-99m MIBI, Tc-99m Tetrofosmin and Tc-99m (V) DMSA revealed increased uptake of each radiopharmaceutical in the tumor lesion corresponding to the mediastinal lesion on the chest CT. However, the normal blood pool activities of the heart and great vessels of Tc-99m (V) DMSA obscured the recurrent malignant thymoma. Although Tc-99m (V) DMSA is a useful tumor seeking agent, we recommend Tc-99m MIBI and Tc-99m Tetrofosmin SPECT rather than Tc-99m (V) DMSA to detect primary and recurrent malignant thymoma.

  • Malignant fibrous histiocytoma (MFH) is a tumor which most often develops in the soft tissues of the extremities and retroperitoneum, but very rarely originates in the mediastinum. We report a 71-year-old man who admitted with anterior mediastinal tumor and underwent surgical resection of tumor in our hospital. The mass was histologically confirmed as MFH.

Diagnostic Significance of the Mediastino-& Thoracoscopy in the Mediastinal Tumors (종격동 종양에 있어서 종격동경 및 흉강경 검사의 진단적 의의)

  • Thoracoscopy, mediastinoscopy and/or mediastinotomy and explo-thoracotomy were performed and confirmed diagnosis of the 26 cases of the mediastinal tumors, those were admitted and treated at the Department of Thoracic and Cardiovascular surgery, Chonbuk National University Hospital, from June,1976 to September, 1986. We experienced 7 thoracoscopies, 7 mediastinoscopies &/or mediastinotomies, and 12 explo-thoracotomies. The most common histologic type was teratoma and lymphoma [6 cases]. Of the 26 cases of the histologically confirmed mediastinal tumor, 19 were benign tumors and 7 were malignants. 19 cases of benign tumors were surgically resected with good result but one case of the surgically resected malignant thymoma was recurred 4 months later. 6 cases of malignant lymphoma and one case of recurred malignant thymoma were treated with chemo-and radiation therapy. Thoracoscopy and mediastinoscopy with tissue biopsy were simple in procedure and had a tissue diagnosis with obtaining the pathologic tissue in a acceptable complication rate and reduced exploratory thoracotomy.

  • Mediastinal tumors have long fascinated the thoracic surgeon of their variety and unpredictability of iagnosis prior to exploration.We report the analysis of the 34 cases of mediastinal tumors,experenced in the dept.of the Thoracic and Cardiovascular Surgery of the National Medical Center.Between the 1993.9-1992.12. The age distribution was relatively even and the mean age was 35 years old.The thymomas were 16 cases [ 47% ].the lipoma were 4 cases [12%].the germ cell tumors were 7 cases [20%].the neurogenic tumors were 3 cases [8%].Histologically analysised .The malignant tumors were 16 cases [17.6%] in classified by hisotlogical types.the tumor size,location,and the clinical manifestation are presented.The successful removal was done in 28 cases [ 100%] among 28 cases of benign mediastinal tumors. Among the 6 cases of malignancy mediastinal tumors, the surgical intervention had done in 5 cases [ 83%] and inoperatable cases were 1 case [16.6%].There was no operative death.

The Usefulness of FNAB for the Diagnosis of Thoracic Surgical Disease (흉부외과 질환의 진단시 FNAB 의 유용성과 객담세포검사 및 기관지 내시경 검사와의 비교 고찰)

  • From March 1986 to June 1990, the percutaneous Fine Needle Aspiration Biopsy[FNAB] of 102 thoracic lesions were performed with Westcott needle [slotted 20G or 22G thin needle], for the purpose of identifying and diagnosing thoracic lesions. There were 94 lung lesions [67 malignant tumors, 27 benign lesions] and 8 mediastinal and chest wall lesions. The results of FNAB were compared with sputum cytology and bronchoscopic examinations. The sputum cytologic examinations were performed in 54 cases of malignant lung tumor and the malignant cell was found at the 18 cases [33%]. We bronchoscopic examinations were performed in 24 cases and the malignant cell was found at the 12 cases [55%]. The positive diagnostic rate of malignancy was 100% by FNAB. Among them, 55 cases [82%] were diagnosed by cytologic examinations and 43 cases[64%] by both. The specific diagnoses for benign lung lesions in 15 cases[55%] and for mediastinal and chest well lesions in 5 cases[62%]. The 7 patients[6.9%] developed the pneumothorax and 5 of them required the treatment. Therefore, the FNAB of thoracic lesions may be a preferred diagnostic method because of its safety, simplicity and accuracy.
