스티칭 기법을 이용한 대퇴골 방사선검사 시 확대 표식자 위치 및 스티칭 범위 변화가 영상에 미치는 영향 (The Effect of the Position Change of the Magnification Marker and the Stitching Range on the Image in the Examination of the Femur Radiography Using Stitching Technique)

  • In this study, we would like to examine the effect of the location of the magnification marker and the change in the stitching range on the magnification marker and the expansion of the anatomical structure in the examination of femoral stitching radiation using the stitching technique, and to find out the usability of the femoral stitching radiation examination in the preoperative examination for THRA. The measured values measured by 5 researchers were summed and presented through descriptive statistics. When the position of the magnification marker was changed, the average value of the measured value was in the range of 113.54 to 113.90 mm. The anatomical structures of the femur were measured in the range of 57.24 to 57.78 mm, 27.20 to 27.56 mm, and 70.11 to 70.81 mm, respectively. This showed a similar deviation from the expansion rate of previous related papers using markers. Therefore, it is considered that femoral stitching X-ray examination using magnification markers can be used for preoperative examination for THRA.

Nasal eosinophilia and eosinophil peroxidase in children and adolescents with rhinitis

  • Background: Researchers have shown that eosinophil peroxidase (EPO) is a relatively accurate marker of eosinophilia and eosinophil activity. However, its use as a marker of eosinophilic inflammation in nasal secretions is limited because the diagnostic cutoff values of EPO for use as a one-time test for allergic diseases such as allergic rhinitis have not been established. Purpose: To identify the correlation between nasal eosinophil count and EPO in children and adolescents with rhinitis. Methods: We recruited patients <18 years of age with rhinitis for more than 2 weeks or more than 2 episodes a year whose nasal eosinophil and EPO were measured at a single allergy clinic. The eosinophil percentage was calculated by dividing the eosinophil count by the number of total cells under light microscopy at ${\times}1,000$ magnification. EPO and protein were measured from nasal secretions. We retrospectively analyzed the correlation between nasal eosinophils and protein-corrected EPO (EPO/protein) value. Results: Of the 67 patients enrolled, 41 were male (61.2%); the mean age was $8.2{\pm}4.0years$. The median nasal eosinophil count was 1 and percentage was 1%. The median protein-corrected EPO value was $12.5ng/{\mu}g$ (range, $0-31ng/{\mu}g$). There was a statistically significant correlation between eosinophil count and percentage (P<0.001). However, the eosinophil percentage and EPO did not correlate. The eosinophil count and EPO had a statistically significant correlation (P=0.01). The EPO cutoff value examined for nasal eosinophil counts of 2, 5, 10, and 20 was $17.57ng/{\mu}g$ regardless of the reference count. The largest area under the curve value was obtained when the receiver operating characteristic curve was drawn using the eosinophil count of 2. Conclusion: Nasal eosinophil count was significantly associated with protein-corrected EPO.

Improvement of Histopathological Sample Preparation by Employing Microwave Heating Method on Frozen Section Specimens

  • Biological samples can be fixed either by chemical method by using chemical solution or physical methods by using heat treatment. The problem in traditional heat fixation is unsatisfactory quality due to uneven heat conduction in specimen and loss of inner cell contents. Chemical fixation method also bears several intrinsic problems like the limit in specimen size, time consumption in fixative impregnation, and loss of low molecular weight cell components. These factors deteriorate the quality of fixed specimen, thus limit the magnification and contrast of tissue pictures. Microwave heat has been reported to be a good alternative to current chemical methods to overcome these problem. In this study, we tried to introduce the microwave energy method to routine fixation work in hospital. We replaced chemical fixative with saline to provide moderate reaction condition, and used frozen section to reduce time for sample preparation. Temperature was measured at each experiment. The fixation of rat kidney tissue with 2.45 GHz electromagnetic wave and saline showed similar result to the control group fixed with traditional chemical method. Human tumor tissue fixed with 2.45 GHz electromagnetic in frozen section was improved in terms of histochemistry of PAS and immunohistochemistry of tumor marker like cytokeratin. Total turnaround time was reduced from $24\sim38$ h to to $2\sim4$ h. In conclusion, the quality of samples prepared by microwave heating method was at least as good as that of traditional method. If the condition for the fixation of different specimens is standardized, this new method could be applied to routine work in hospital, and could save working time as well.

Chalkley Microvessel but not Lymphatic Vessel Density Correlates with Axillary Lymph Node Metastasis in Primary Breast Cancers

  • This study aimed to investigate tumor microvessel density (MVD) and lymphatic vessel density (LVD) using the Chalkley method as predictive markers for the risk of axillary lymph node metastasis and their relationship to other clinicopathological parameters in primary breast cancer cases. Forty two node-positive and eighty node-negative breast cancers were immunostained for CD34 and D2-40. MVD and LVD were counted by the Chalkley method at x400 magnification. There was a positive significant correlation of the MVD with the tumor size, coexisting ductal carcinoma in situ (DCIS) and lymph node metastases (P<0.05). In multivariate analysis, the MVD (2.86-4: OR 5.87 95%CI 1.05-32; >4: OR 20.03 95%CI 3.47-115.55), lymphovascular invasion (OR 3.46, 95% CI 1.13-10.58), and associated DCIS (OR 3.1, 95%CI 1.04-9.23) independently predicted axillary lymph node metastasis. There was no significant relationship between LVD and axillary lymph node metastasis. However, D2-40 was a good lymphatic vessel marker to enhance the detection of lymphatic invasion compared to H and E staining. In conclusion, MVD by the Chalkley method, lymphovascular invasion and associated DCIS can be additional predictive factors for axillary lymph node metastases in breast cancer. No relationship was identified between LVD and clinicopathological variables, including axillary lymph node metastasis.

편평세포폐암에서 Argyrophilic Nucleolar Organizer Regions(AgNORs)의 임상적의의 (Clinical Significance of Argyrophilic Nucleolar Organizer Regions(AgNORs) In Squamous Cell Carcinoma of the Lung)

  • 연구배경: 세포의 증식능력을 반영한다고 알려진 NORs를 간편한 은염색으로 발현시켜서 AgNORs 수가 정상조직과 종양조직의 비교감별과 종양의 증식능력판단에 유용한지를 알기위하여 파라핀 포매된 편평세포폐암 조직을 이용하여 본 연구를 시행하였다. 방법: 수술로써 절제한 파라핀 포매된 편평세포폐암 조직 36예를 Mourad 등의 방법으로 은염색하였다. 1,000 배 배율하에서 종양조직과 정상조직에서 임의로 100 개씩 세포를 선정해서 핵당 평균 AgNORs수를 구하였다. 결과: 종양조직에서 TNM병기에 따른 핵당 평균 AgNORs수는 유의한 차이가 없었으나 정상조직과 종양조직을 비교허여 정상조직에서는 $1.74{\pm}0.25$, 종양 조직에서는 $4.05{\pm}0.80$으로 종양조직의 핵당 평균 AgNORs수가 정상조직에 비해서 유의하게 높았다(p<0.001). 결론: TNM분류에 따른 각 병기별 종양조직과 인접 정상조직의 핵당 평균 AgNORs수를 비교하여도 각 병기에서 종양조직의 평균 AgNORs수가 정상조직에 비하여 유의하게 높았다(p<0.005).

비소세포성 폐암종의 CD44s 및 CD44v6의 발현에 대한 연구 -CD44의 발현에 대한 연구- (A Study on the Expression of CD44s and CD44v6 in Non-Small Cell Lung Carcinomas)

  • 배경: CD44는 세포상호간 그리고 세포와 기질 사이의 부착을 조절하는 세포표면당단백질로 표준형인 CD44s와 여러 동종변형이 있다. CD44는 림프구와 단핵구를 활성화할 뿐만 아니라, 여러가지 상피성 종양의 진행과정에 참여하여 종양의 침습과 전이를 도와줄 것이라는 연구결과가 나오고 있다. 그러나, 종양의 종류에 따라 CD44의 표준형과 여러 동종변형의 발현양상이 다르고, 종양의 생물학적 특성과의 관련성에 대해서도 아직은 잘 알려져 있지 않다. 폐장에는 많은 종류의 원발성 악성종양과 전이성종양이 발생하기 때문에 폐암조직에서 CD44 당단백의 발현양상에 대해 연구하는 것이 폐암의 생물학적 특성뿐만 아니라 다른 종양의 전이에 관한 이해의 폭을 넓히는데 도움이 될 것으로 생각하여 본 연구를 시행하였다. 대상 및 방법: 1985년부터 1994년까지 비소세포성폐암으로 진단한 후 절제하여 의뢰된 48예의 편평세포암종, 33예의 선암종, 8예의 미분화성대세포암종을 합한 총 89예의 폐암조직을 대상으로 연구하였다. CD44 당단백질은 표준형인 CD44s와 동종변형인 CD44v6에 대해 면역조직화학염색을 시행하여 발현정도를 평가하였다. 종양의 미세혈관분포는 혈관내피세포 표지자인 CD34에 대한 면역조직화학염색을 시행하여 200배 및 400배 현미경 시야에서 혈관의 수를 헤아렸다. CD44s와 CD44v6의 발현정도와 미세혈관의 수 사이에 연관성을 검정하였다. 이 결과를 환자의 나이, 성별, 병기, 종양의 크기, 림프절 전이 여부, 종양의 병리조직학적 유형 및 생존율과 비교하였다. 결과: CD44s와 CD44v6는 89예의 비소세포폐암종 중 각각 71예(79.8$\%$)와 64예(71.9$\%$)에서 발현하였다. 이 두 당단백의 발현은 상호 관련성이 있었다(p < 0.0001). CD44s와 CD44v6모두 편평세포암종에서 각각 95.8$\%$로 가장 높은 발현율을 보였다(p < 0.0001). CD44s의 발현은 편평세포암종의 분화도와 관련이 있었는데 (p=0.008), 불량한 분화를 보이는 암종이 양호한 분화의 암종보다 발현율이 높았으며(p=0.002), 중등도 분화를 보인 암종과는 유의한 차이가 없었다. CD44v6의 발현은 편평세포암종과 선암종의 분화도와 관련이 없었다. CD44s의 발현은 종양의 미세혈관의 수와 상관관계를 보였다(p=0.019 및 p=0.007). 종양의 미세혈관의 수는 종양의 크기와 상관 관계가 있었다 (각각 p=0.043). 그러나, 나이, 성별, 병기, 림프절 전이 및 생존율과는 관련성이 없었다. 결론: 이상의 결과에서, 종양의 미세혈관의 수가 CD44s의 발현과 상관관계가 있고, 종양의 크기와도 상관관계를 보이는 점으로 미루어 CD44s가 종양의 성장과 혈관 형성에 관련되어 있을 가능성을 시사한다. CD44s와 CD44v6의 발현이 편평세포암종에서 가장 높은 발현율을 보이는 것으로 보아 조직형태학적 특성과 관련이 있을 것으로 생각한다.
