Objective: Idiopathic intracranial hypertension (IIH) is a condition of unknown etiology associated with venous sinus stenosis. This study aimed to develop a magnetic resonance venography (MRV)-based radiomics model for predicting a high trans-stenotic pressure gradient (TPG) in IIH patients diagnosed with venous sinus stenosis. Materials and Methods: This retrospective study included 105 IIH patients (median age [interquartile range], 35 years [27-42 years]; female:male, 82:23) who underwent MRV and catheter venography complemented by venous manometry. Contrast enhanced-MRV was conducted under 1.5 Tesla system, and the images were reconstructed using a standard algorithm. Shape features were derived from MRV images via the PyRadiomics package and selected by utilizing the least absolute shrinkage and selection operator (LASSO) method. A radiomics score for predicting high TPG (≥ 8 mmHg) in IIH patients was formulated using multivariable logistic regression; its discrimination performance was assessed using the area under the receiver operating characteristic curve (AUROC). A nomogram was constructed by incorporating the radiomics scores and clinical features. Results: Data from 105 patients were randomly divided into two distinct datasets for model training (n = 73; 50 and 23 with and without high TPG, respectively) and testing (n = 32; 22 and 10 with and without high TPG, respectively). Three informative shape features were identified in the training datasets: least axis length, sphericity, and maximum three-dimensional diameter. The radiomics score for predicting high TPG in IIH patients demonstrated an AUROC of 0.906 (95% confidence interval, 0.836-0.976) in the training dataset and 0.877 (95% confidence interval, 0.755-0.999) in the test dataset. The nomogram showed good calibration. Conclusion: Our study presents the feasibility of a novel model for predicting high TPG in IIH patients using radiomics analysis of noninvasive MRV-based shape features. This information may aid clinicians in identifying patients who may benefit from stenting.
Cerebral venous sinus thrombosis (CVST) following a closed head injury in pediatric patients is a rare condition, and an early spontaneous recanalization of this condition is extremely rare. A 10-year-old boy was admitted with a mild, intermittent headache and nausea five days after a bicycle accident. The brain computed tomography showed an epidural hematoma at the right occipital area with pneumocephalus due to a fracture of the occipital skull bone. The brain magnetic resonance imaging and the magnetic resonance venography demonstrated a flow signal loss from the right sigmoid sinus to the right jugular vein. The diagnosis was sigmoid sinus thrombosis, so close observations were selected as a treatment for the patient because of his gradually improving symptoms; however, he complained of vomiting 14 days the after conservative treatment. The patient was readmitted for a further examination of his symptoms. The laboratory and the gastroenterological examinations were normal. Due to concern regarding the worsening of the sigmoid sinus thrombosis, the brain magnetic resonance venography was rechecked and it revealed the recanalization of the venous flow in the sigmoid sinus and in the jugular vein.

Nephrotic syndrome (NS) is a hypercoagulable state in which children are at risk of venous thromboembolism. A higher risk has been reported in children with steroid-resistant NS than in those with steroid-sensitive NS. The mortality rate of cerebral venous sinus thrombosis (CVST) is approximately 10% and generally results from cerebral herniation in the acute phase and an underlying disorder in the chronic phase. Our patient initially manifested as a child with massive proteinuria and generalized edema. He was treated with albumin replacement and diuretics, angiotensin-converting enzyme inhibitor, and deflazacort. Non-contrast computed tomography showed areas of hyperattenuation in the superior sagittal sinus when he complained of severe headache and vomiting. Subsequent magnetic resonance imaging revealed empty delta signs in the superior sagittal, lateral transverse, and sigmoid sinuses, suggesting acute CVST. Immediate anticoagulation therapy was started with unfractionated heparin, antithrombin III replacement, and continuous antiproteinuric treatment. The current report describes a life-threatening CVST in a child with steroid-dependent NS, initially diagnosed by contrast non-enhanced computed tomography and subsequently confirmed by contrast-enhanced magnetic resonance imaging, followed by magnetic resonance venography for recanalization, addressing successful treatment.
목적: 뇌내 정맥혈관을 평가하기 위한 조영 증강 자기공명 정맥촬영술의 arterial trigger 와 venous trigger 방법으로 시행한 영상의 차이점을 비교 분석하고자 한다. 대상과 방법: 건강검진을 목적으로 자기공명정맥촬영술을 시행한 41명의 환자들을 대상으로 해면부위의 내경 동맥에서 arterial triggering하여 6초 후에 얻은 영상 (n = 20) 과 상시상 정맥동에서 venous triggering (n = 21) 방법으로 시행한 영상을 후향적으로 분석하였다. 영상은 가돌리늄 조영제 ($Magnevist^{(R)}$, Schering, Germany)를 0.1 mmol/kg 정맥주입하여 시행하였고, 두개강 전반에 대하여 시상영상을 fast spoiled gradiend echo sequence로 시행하였다 (TR/TE 5.2/1.5, matrix $310{\times}310$, 절편수 124 절편, 두께 15 cm). 두 그룹의 영상을 해부학적 정맥 혈관 구조에 따라 17 정맥구역에 대하여 평가하였고, 정맥의 영상품질은 세 단계 (안보임, 일부 보임, 완전히 보임)로 나눠서 평가하였다. 결과: 정맥이 완전히 보인 구역은 arterial triggering 자기공명 정맥 촬영술에서 84% (272/323), venous triggering 자기 공명 정맥촬영술에서 91% (310/340) 이다. Venous triggering 자기공명촬영술과 arterial 자기 공명 정맥촬영술을 비교하였을 때 뇌내 정맥 구조를 평가하는데 있어 venous triggering 방법이 통계적으로 유의하게 높았다 (Fisher exact test, p<0.006). 결론: 조영 증강 자기공명 정맥 촬영술은 정맥 혈관 구조에 대한 고화질의 이미지를 제공하였고 arterial triggering 방법보다 venous triggering 방법이 뇌내 정맥 구조 평가에 우월한 것으로 나타났다.
경막정맥동 혈전증은 다양한 원인을 가진 비교적 드문 질환으로 알려져왔으며 임상양상 및 발현형태가 매우 다양하여 임상적 진단이 쉽지 않았다. 그러나 최근에는 여러 영상진단 방법, 특히 자기공명영상 혈관 촬영등을 통해 조기 진단이 가능하게되었다. 치료 방법을 결정할 때, 환자의 임상증상과 정맥순환계의 개인간 차이가 매우 중요한 인자중 하나이다. 저자들은 보존적 치료로 좋은 결과를 얻은 2례의 경막정맥동 환자의 특성을 문헌고찰과 함께 보고하는 바이다.

Arterial adventitial cystic disease is an uncommon type of non-atherosclerotic peripheral vessel disease. Most cases of arterial adventitial cystic disease occur in the popliteal arteries; however, fewer cases have been reported in the femoral arteries. A 59-year-old male patient visited the hospital with a complaint of a swelling on the lower extremity that had begun two months earlier. Suspecting deep vein thrombosis based on a physical examination and ultrasonography from another hospital, tests were performed. Magnetic resonance imaging (MRI) was performed for exact diagnosis because venous adventitial cystic disease was suspected by computed tomography venography. The MRI indicated venous adventitial cystic disease as well. Thus, a cystic mass excision was performed. In the end, a cystic mass compressing the common femoral vein that originated from the common femoral artery was diagnosed based on the macroscopic findings. This case is reported because blood circulation in the vein was impeded due to arterial adventitial cystic disease, and the symptoms improved after the cystic mass excision and polytetrafluoroethylene roofing angioplasty.
Nephrotic syndrome (NS) is associated with cerebral venous sinus thrombosis (CVST), which is a rare cerebrovascular disorder in children. Systemic anticoagulation with heparin is the standard therapy for CVST, and mechanical thrombectomy (MT) has been described as a salvage treatment for adult anticoagulant refractory CVST, However, it has never been reported in children. We describe a case of MT for refractory CVST in a child with NS. A 13-year-old boy with newly diagnosed NS presented to an emergency department with acute headache. A head computed tomography showed acute thrombus in the superior sagittal sinus, straight sinus and transverse sinus. The child was started on heparin therapy, but clinically deteriorated and became unresponsive. In view of the rapid deterioration of the condition after anticoagulation treatment, the patient received intravascular treatment. Several endovascular technologies, such as stent retriever and large bore suction catheter have been adopted. After endovascular treatment, the patient's neurological condition was improved within 24 hours, and magnetic resonance venography of the head demonstrated that the CVST was reduced. The child recovered with normal neurological function at discharge. This case highlights the importance of considering MT for refractory CVST, and we suggest that MT may be considered for refractory CVST with NS in children.
4.7T의 고자장 자기공명 영상(Magnetic Resonance imaging : MRI) 장치에서 Tailored RF 경사자계반향(Tailored RF Gradient-Echo : TRGE)기법을 이용하여 phantom 영상과 고양이 뇌의 정맥혈관 영상을 얻었다. 증류수에 MgCl$_2$를 섞어 T1을 짧게한 지름 6cm의 원통형 phantom으로 TRGE기법에 대한 검증을 먼저 하였다. 이후 halothane으로 전신 마취된 몸무게 3.0~4.0kg의 고양이 뇌로부터 얻은 시단면(sagittal slice) 영상에서 Dorsal sagittal sinus, Straight sinus, Vein of corpus callosum과 Internal cerebral vein등의 혈관이 강조되어 나타남을 고양이 뇌의 해부학적 그림과 비교해 보였다. Tailored RF 파형은 ASPECT 3000 computer(스위스 BRUKER사 제품)의 PASCAL 언어를 이용하여 제작하였다. 사각형의 절편모양(slice profile)과 원하는 절편의 두께(slice thickness)에 선택절편내에서 절편방향으로 위상분포가 최대 2$\pi$가 되는 bi-linear ramp 모양을 갖는 절편함수를 공간상에서 Fourier 변환시켜 Tailored RF를 만들었다. TR/TE=205/10 msec, 절편두께 TH=7mm, 화소배열수(matrix size)=256$\times$256, 평면해상도(in-plane resolution)=0.62$\times$0.31mm$^2$, 관심영역 크기(field of view : FOV)=8cm의 영상조건으로 자기공명 TRGE 영상을 얻었다.
