Postcontrast T1-weighted Brain MR Imaging in Children: Comparison of Fat-suppressed Imaging with Conventional or Magnetization Transfer Imaging

  • To assess the merits and demerits of postcontrast fat-suppressed (FS) brain MR imaging in children in the evaluation of various enhancing lesions, compared with postcontrast conventional or Magnetization Transfer (MT) imaging. 대상 및 방법: We reviewed patients with enhancing lesion on brain MR imaging who underwent both FS imaging and one of conventional or MT imaging as a postcontrast T1-weighted brain MR imaging. Inclusion criteria of our study were as follows: MR studies should be peformed within one-year interval and showed no significant interval change of imaging findings. Thirty-four patients (21 male, 13 female; mean age, 8 years) with 43 enhancing lesions (19 intra-axial, 19 extra-axial, and 5 orbital location) were included in this study, Twenty-one pairs of FS and conventional imaging, and 15 pairs of FS and MT imaging were available. Two radiologists visually assessed the lesion conspicuity and the presence of flow or susceptibility artifacts in a total of 36 pairs of MR imaging by consensus. For 21 measurable lesions (19 pairs of FS and conventional imaging, 5 pairs of FS and MR imaging), contrast ratio between the lesion and the normal brain( [SIlesion-SIwater]/[SInormal brain-SIwater]) were calculated and compared.

  • Phase-contrast(PC) methods have been used for quantitative measurements of velocity and volume flow rate. In addition, phase contrast cine magnetic resonance imaging (MRI) combines the flow dependent contrast of PC MRI with the ability of cardiac cine imaging to produce images throughout the cardiac cycle. In this method, the through-plane velocity has been encoded generally. However, the accuracy of the flow data can be reduced by the effect of flow direction, finite slice thickness, resolution, pulsatile flow pattern, and so on. In this study we calculated the error caused by misalignment of tomographic plane and flow directon. To reduce this error and encode the velocity for more complex flow, we suggested 3 directional velocity encoding method.

정위방사선수술시 영상공동등록을 이용한 자기공명영상 유용성 평가 (Evaluation of Magnetic Resonance Imaging using Image Co-registration in Stereotactic Radiosurgery)

  • 본 연구는 방사선수술 치료계획 시 영상공동등록을 이용한 자기공명영상의 3차원적 위치에 대하여 구조평가 및 뇌 정위 수술의 해부학적 기준점으로 사용되어지는 전교련(anterior commissure: AC)과 후교련(posterior commissure: PC)의 일치성을 평가하여 방사선수술시 영상공동등록의 임상 적용 안전성을 확인하고자한다. 정위 방사선수술 시행 후 2016년 3월~2017년 3월까지 영상공동등록을 이용한 자기공명영상 추적검사를 시행한 32명의 영상을 이용하여 3차원 좌표 측정을 통한 후향적 연구를 시행하였다. 전교련의 3차원 좌표 오차는 $1.0443{\pm}0.5724mm$(0.10 ~ 1.89), 후교련 3차원 좌표 오차는 $1.0348{\pm}0.5473mm$ (0.36 ~ 2.24)로 약 1 mm의 오차를 나타내었다. 자기공명영상 장비별 전교련과 후교련 좌표의 평균오차는 MR1(3.0 T)의 오차 값이 MR2 (1.5 T)에 비하여 낮게 나타났다. 영상공동등록 기법을 이용한 치료계획 시 자기공명영상의 오차를 최소화하여야 하며, 정확도를 확인하는 과정이 필요한 것으로 생각된다.

유두 종양의 자기공명 영상소견 (MR Findings of Papillary Neoplasms of the Breast)

  • 목적 : 유방암을 진단 받은 환자의 자기공명영상에서 추가적으로 발견된 유두상 병변의 자기공명영상 소견과 수술적 절제 후 상향화 비율을 알아보고자 한다. 대상 및 방법 : 본원에서 유방암 진단된 환자의 수술 전 자기공명영상 1729건에서 지표 종양 외에 악성 의심 조영증강을 보인 병변 중 초음파 유도하 핵생검에서 유두 종양으로 진단된 21명의 환자의 22개 병변을 대상으로 하였다. 이 중 수술적 절제를 시행한 병변은 핵생검 결과와 절제 생검 결과를 비교하였다. 또한 이 병변들의 자기공명영상 소견을 BI-RADS MRI lexicon에 따라 후향적으로 분석하여 특징을 알아보았다. 결과 : 자기공명영상에서는 종괴 8예, 비종괴 7예, 초점 7예로 관찰되었고 초점으로 나타난 병변은 병리 조직에서 모두 (100%) 양성 병변으로 진단되었고, 역동적 조영증강 검사에서 고원형과 세척형 양상을 보였다. 양성 유두종 1예 (11.1%), 비정형요소를 가진 유두 종양 3예 (100%), 유두 종양 3예 (60%)가 수술적 절제 후 관상피내암, 침윤성 관상피암, 침윤성 소엽암 등으로 상향화되었다. 결론 : 초음파 유도하 핵생검에서 유두상 병변으로 진단된 병변들의 자기공명영상 소견은 양성과 악성을 예측하기에 특징적인 차이점을 보이지 않아 영상소견만으로 양성과 악성을 구분할 수 없으며 다수에서 수술적 절제 후 조직학적 상향화를 보여 (41.2%, 7/17) 반드시 수술적 절제를 통한 확진이 필요할 것이다.

Comparison of Three, Motion-Resistant MR Sequences on Hepatobiliary Phase for Gadoxetic Acid (Gd-EOB-DTPA)-Enhanced MR Imaging of the Liver

  • Purpose: To compare three, motion-resistant, T1-weighted MR sequences on the hepatobiliary phase for gadoxetic acid-enhanced MR imaging of the liver. Materials and Methods: In this retrospective study, 79 patients underwent gadoxetic acid-enhanced, 3T liver MR imaging. Fifty-nine were examined using a standard protocol, and 20 were examined using a motion-resistant protocol. During the hepatocyte-specific phase, three MR sequences were acquired: 1) gradient recalled echo (GRE) with controlled aliasing in parallel imaging results in higher acceleration (CAIPIRINHA); 2) radial GRE with the interleaved angle-bisection scheme (ILAB); and 3) radial GRE with golden-angle scheme (GA). Two readers independently assessed images with motion artifacts, streaking artifacts, liver-edge sharpness, hepatic vessel clarity, lesion conspicuity, and overall image quality, using a 5-point scale. The images were assessed by measurement of liver signal-to-noise ratio (SNR), and tumor-to-liver contrast-to-noise ratio (CNR). The results were compared, using repeated post-hoc, paired t-tests with Bonferroni correction and the Wilcoxon signed rank test with Bonferroni correction. Results: In the qualitative analysis of cooperative patients, the results for CAIPIRINHA had significantly higher ratings for streak artifacts, liver-edge sharpness, hepatic vessel clarity, and overall image quality as compared to, radial GRE, (P < 0.016). In the imaging of uncooperative patients, higher scores were recorded for ILAB and GA with respect to all of the qualitative assessments, except for streak artifact, compared with CAIPIRINHA (P < 0.016). However, no significant differences were found between ILAB and GA. For quantitative analysis in uncooperative patients, the mean liver SNR and lesion-to-liver CNR with radial GRE were significantly higher than those of CAIPIRINHA (P < 0.016). Conclusion: In uncooperative patients, the use of the radial GRE sequence can improve the image quality compared to GRE imaging with CAIPIRINHA, despite the data acquisition methods used. The GRE imaging with CAIPIRINHA is applicable for patients without breath-holding difficulties.

Magnetic resonance imaging and spectroscopic analysis in 5 cases of Pelizaeus-Merzbacher disease: metabolic abnormalities as diagnostic tools

  • Pelizaeus-Merzbacher disease (PMD) is a rare, X-linked recessive disorder characterized by dysmyelination in the central nervous system. PMD results from deletion, mutation, or duplication of the proteolipid protein gene (PLP1) located at Xq22, leading to the failure of axon myelination by oligodendrocytes in the central nervous system. PMD may be suspected when there are clinical manifestations such as nystagmus, developmental delays, and spasticity, and genetic analysis can confirm the diagnosis. Further diagnostic manifestations of the disease include a lack of myelination on brain magnetic resonance (MR) imaging and aberrant N-acetyl aspartate (NAA) and choline concentrations that reflect axonal and myelination abnormalities on phroton MR spectroscopy. We report 5 cases of PMD (in 1 girl and 4 boys). PLP1 duplication was detected in 2 patients. Brain MR analyses and MR spectroscopy were performed for all the patients. The brain MR images showed white matter abnormalities typical of PMD, and the MR spectroscopic images showed diverse patterns of NAA, creatinine, and choline concentrations. We propose that MR spectroscopic analysis of metabolic alterations can aid the PMD diagnosis and can contribute to a better understanding of the pathogenesis of the disease.
