만성 불응성 외 상과염에서 시행한 관절경적 단 요 수근 신건 유리술 (Arthroscopic Release of the Extensor Carpi Radialis Brevis Tendon for Chronic Recalcitrant Lateral Epicondylitis)

  • 목적: 보존적 치료에 반응하지 않는 만성 불응성 주관절 외 상과염에 대한 관절경적 단 요 수근 신건 유리술의 임상적 결과를 알아보고 예후에 영향을 주는 인자를 분석하고자 한다. 대상 및 방법: 주관절 외 상과염으로 관절경적 단 요 수근 신건 유리술을 시행 받은 환자 24명을 후향적으로 분석하여 술 후 환자 자신의 평가, visual analog scale (VAS) 점수, Quick-disabilities of the arm, shoulder and hand (DASH) 점수와 결과에 영향을 줄 수 있는 술 전 인자를 평가하였다. 결과: 21예(88%)에서 술 후 결과에 만족 하였고 평균 VAS 점수와 Quick-DASH 점수는 최종 추시 상 의미 있는 호전을 보였다. 나이, 성별, 우세수 여부, 증상 이환 기간, 관절낭 파열이나 석회 침착유무는 임상적 결과와 상당한 인과 관계가 없었다. 술 전 자기 공명 영상 T2강조 영상 상 단 요 수근 신건 기시부에 고강도 신호부가 없는 4예 중 3예에서 보다 높은 Quick-DASH 점수를 보였다. 결론: 보존적 치료에 반응하지 않는 만성 불응성 주관절 외 상과염에 대한 관절경적 단 요 수근 신건 유리술은 유용한 치료법 중 하나이며 술 전 자기공명영상 T2강조 영상에서 고강도 신호부가 없는 경우는 술 후 불량한 결과를 유발할 수 있으므로 수술 적응 시 고려해야 할 요소로 사료된다.

Microvascular Decompression for Hemifacial Spasm Associated with Vertebrobasilar Artery

  • Objective : Hemifacial spasm (HFS) is considered as a reversible pathophysiological condition mainly induced by continuous vascular compression of the facial nerve root exit zone (REZ) at the cerebellopontine angle. As an offending vessel, vertebrobasilar artery tends to compress much more heavily than others. The authors analyzed HFS caused by vertebrobasilar artery and described the relationships between microsurgical findings and clinical courses. Methods : Out of 1,798 cases treated with microvascular decompression (MVD) from Jan. 1980 to Dec. 2004. the causative vessels were either vertebral artery or basilar artery in 87 patients. Seventy-nine patients were enrolled in this study. Preoperatively, computed tomography (CT) or brain magnetic resonance (MR) imaging with 3-dimentional short range MR technique was performed and CT was checked immediately or 2-3 days after anesthetic recovery. The authors retrospectively analyzed the clinical features. the compression patterns of the vessels at the time of surgery and treatment outcomes. Results : There were 47 were male and 32 female patients. HFS developed on the left side in 52 cases and on the right side in 27. The mean age of onset was 52.3 years (range 19-60) and the mean duration of symptoms was 10.7 years. Many patients (39 cases; 49.1%) had past history of hypertension. HFS caused only by the vertebral artery was 8 cases although most of the other cases were caused by vertebral artery (VA) in combination with its branching arteries. Most frequently, the VA and the posterior inferior cerebellar artery (PICA) were the simultaneous causative blood vessels comprising 32 cases (40.5%). and in 27 cases (34.2 %) the VA and the anterior inferior cerebellar artery (AICA) were the offenders. Facial symptoms disappeared in 61 cases (77.2%) immediately after the operation and 68 cases (86.1%) showed good outcome after 6 months. Surgical outcome just after the operation was poor in whom the perforators arose from the offending vessels concurrently (p<0.05). Conclusion : In case where the vertebral artery is a cause of HFS, commonly branching arteries associated with main arterial compression on facial REZ requires more definite treatment for proper decompression because of its relatively poor results compared to the condition caused by other vascular compressive origins.

추간판 탈출증 환자에서 길잡이철사가 삽입된 경막외카테터를 이용한 미추경막외조영술 (Caudal Epidural Injection with a Guidewire-Reinforced Epidural Catheter in Patients with Herniated Nucleosus Pulpose)

  • Background: The epidural injection technique is a commonly used intervention in the management of chronic spinal pain, which has the advantage of delivering various drugs, such as local anesthetics or steroids, in higher concentrations to the inflamed nerve root. A guidewire-reinforced epidural catheter was introduced through a Tuohy needle during the caudal epidural procedure, with a catheter threaded into the affected nerve roots and the spread-pattern of contrast agents observed under fluoroscopy. Methods: Sixty-seven patients with low back pain, who showed evidence of a herniated nucleus pulposus on magnetic resonance imaging, were included. All patients received fluoroscopically guided caudal epidural injections, with the guidewire-reinforced epidural catheter introduced through a Tuohy needle and threaded either to the right or left side toward the target nerve roots. After confirming the catheter tip position at the affected nerve root, 2 ml increments of contrast agents (up to 6 ml) were injected, and their corresponding AP fluoroscopic views were obtained. Three radiologists reviewed all the radiographic findings and measured the proportion of the area of contrast spread at the side of target nerve roots. Results: Greater proportion of the area of contrast spread was observed at the side of the target nerve roots (P < 0.0001). At each level of contrast injection (2-⁣, 4- ⁣ and 6 ml), more than 70% of the spread of contrast dye was observed at the side of the target nerve roots in 85%, 70%, and 55% of cases, respectively. Conclusions: The combination of a caudal epidural injection and use of a guidewire-reinforced epidural catheter significantly enhances the target specificity, as revealed by the selective spread of contrast dye at the side of target nerves.

관절외 배부 폐쇄 쐐기 절골술을 이용한 Freiberg병의 치료 결과 (Outcome of Extraarticular Dorsal Closing Wedge Osteotomy for Freiberg's Disease)

  • Purpose: The aim of this study was to evaluate the result of extraarticular dorsal closing wedge osteotomy in Freiberg's disease. Materials and Methods: Between February 2012 and July 2014, total 10 patients who underwent dorsal closing wedge osteotomy and followed up more than 1 year were selected for inclusion. Average age was 16.3 years, and average follow-up period was 15.5 months. The diagnosis was made using magnetic resonance imaging of those with a limitation in walking or usual activity due to pain in the metatarsal head. During operation, we removed loose body, and synovectomy was done. Osteotomy at the metatarsal neck and fixation with Kirschner wire were performed. X-ray was taken to check shortening of 2nd metatarsal and bone union. Moreover, we checked the active range of motion of 2nd metatarsophalangeal joint before and after surgery. At the last follow-up, the shortening of metatarsal, American Orthopaedic Foot and Ankle Society (AOFAS) score, visual analogue scale (VAS), and patient's subjective satisfaction were evaluated. Results: According to the Smillie's stage, there were 3 cases of stage II, 4 cases of stage III, and 3 cases of stage IV. Average bone union time on the osteotomy site was 8 weeks. Average shortening of metatarsal was 2.53 mm. Average AOFAS score improved significantly from 56.9 to 82.8 points at final follow-up (p<0.05), and average VAS score also improved significantly from 6.4 to 1.4 points at final follow-up (p<0.05). Average active range of motion at metatarsophalangeal joint improved from $28.0^{\circ}$ preoperatively to $46.5^{\circ}$ at the final follow-up. Other complications, such as metatarsalgia and arthritis, were not found; however, there was 1 case of delayed union with no symptom. Conclusion: In Freiberg's disease, dorsal closing wedge osteotomy is recommended for the improvement of clinical symptoms and range of motion.

후족부 관상면 배열 영상에 대한 고안 (Introduction of Hindfoot Coronal Alignment View)

  • 목 적: 후족부의 병리학적 상태의 치료와 평가를 위해서는 관상면에서 종골과 경골이 연관된 배열상태의 정확한 평가가 필수적이다. 이전의 방사선학적 검사인 발과 발목의 전후방향, 측방향, 사방향 촬영과 종골 축방향 촬영 등의 X-선 촬영상은 관상면에서 종골과 경골이 연관된 배열상태를 증명하지 못했다. 이에 본 연구에서는 후족부 관상면 배열영상(hindfoot coronal alignment view)을 새롭게 소개하고자 한다. 검사방법 : 1) 양쪽 발을 지탱할 수 있는 방사선투과성의 스탠드형 보조기구를 제작한다. 2) 양측 발은 weight-bearing position이 되게 한다. 3) 각각의 발의 위치는 발의 종축이 보조기구 판과 수직이 되도록 자세를 유지한다. 4) silhouette tracing: 발뒷꿈치 outline과 둘째 발가락이 일직선상으로 지나도록 위치시킨다. 5) 중심 X-선: 발바닥 쪽을 향해 약 $15{\sim}20^{\circ}의 각도로 종골의 뒷쪽을 향해 입사한다. 결 과 : 1) 경골 축과 종골의 내측, 외측 결절의 영상이 함께 표출된다. 2) 종골이 회전되지 않아야 한다. 3) 거퇴관절강(talotibial joint space)이 함께 나타나야 한다. 결 론: CT나 MRI 영상에서도 관상면에서 후족부의 배열상태를 증명할 수 있지만, 환자의 체중이 주어지지 않기 때문에 발의 임상적인 증상을 보여주기에는 충분하지 못했다. 하지만 후족부 관상면 배열영상은 후족부의 inversion, eversion의 자세변화를 보여주고, 경골 원위부와 종골의 varus, valgus deformity의 치료를 위한 평가 자료로 좋은 검사방법이며, 비교적 자연스럽고 편안한 자세로 환자에게 큰 도움을 줄 것으로 사료된다.

  • Objectives and Methods : This study was performed to investigate the effect of electro -acupuncture at four gates(bilateral LR3 and LI4) on brain activity in normal subjects using fMRI. Results and Conclusions : 1. fMRI signal increase by electro-acupuncture at Lt. LR3 was observed in Rt. Middle frontal gyrus in group average as well as more than half of the subjects. 2. fMRI signal decreases by electro-acupuncture at Lt. LR3 were observed in Rt. Superior frontal gyrus, Rt. Middle temporal gyrus, Rt. Cingulate gyrus in group average as well as more than half of the subjects. 3. fMRI signal increases by electro-acupuncture at Lt. LI4 were observed in Lt. Superior frontal gyrus, Lt. Middle frontal gyrus, Lt. Inf. Semi-Lunar Lobule(cerebellum), Rt. Middle frontal gyrus, Rt. Cingulate gyrus in group average as well as more than half of the subjects. 4. fMRI signal decreases by electro-acupuncture at Lt. LI4 were observed in Lt. Middle frontal gyrus, Lt. Inferior frontal gyrus, Lt. Precentral gyrus and Rt. Middle frontal gyrus, Rt. Middle temporal gyrus, Rt. Precuneus, Rt. Inferior frontal gyrus, Rt. Postcentral gyrus in group average as well as more than half of the subjects. 5. fMRI signal increase by electro-acupuncture at Lt. LR3 and Lt. LI4 in group average as well as more than half of the subjects was not observed. 6. fMRI signal decreases by electro-acupuncture at Lt. LR3 and Lt. LI4 were observed in Lt. culmen(cerebellum), Lt. Cingulate gyrus와 Rt. Middle frontal gyrus, Rt. Cingulate gyrus, Rt. Inferior frontal gyrus in group average as well as more than half of the subjects. 7. fMRI signal increases by electro-acupuncture at four gates (bilateral LR3 and LI4) were observed in Lt. Middle temporal gyrus and Lt. Postcentral gyrus in group average as well as more than half of the subjects. 8. fMRI signal decrease by electro-acupuncture at four gates (bilateral LR3 and LI4) were observed in Lt. Middle frontal gyrus, Lt. Precentral gyrus, Lt. Inferior frontal gyrus, Lt. Middle temporal gyrus, Lt. Frontal sub-gyral and Rt. Effect of Cervical Interlaminar Epidural Steroid Injection: Analysis According to the Neck Pain Patterns and MRI Findings

  • Background: It is widely accepted that cervical interlaminar steroid injection (CIESI) is more effective in treating radicular pain than axial neck pain, but without direct comparison. And the differences of effect after CIESI according to MRI findings are inconsistent. In this retrospective study, we evaluated the therapeutic response of CIESI according to pain sites, durations, MRI findings, and other predictive factors altogether, unlike previous studies, which evaluated them separately. Methods: The medical records of 128 patients who received fluoroscopy guided CIESI were analyzed. We evaluated the therapeutic response (more than a 50% reduction on the visual analog scale [VAS] by their second visit) after CIESI by (1) pain site; neck pain without radicular pain/radicular pain with or without neck pain, (2) pain duration; acute/chronic (more than 6 month), and (3) findings of MRI; herniated intervertebral disc (HIVD)/spinal stenosis, respectively and altogether. Results: Eighty-eight patients (68%) responded to CIESI, and there were no significant differences in demographic data, initial VAS score, or laboratory findings. And there were no significant differences in the response rate relating to pain site, pain duration, or MRI findings, respectively. In additional analysis, acute radicular pain with HIVD patients showed significantly better response than chronic neck pain with spinal stenosis (P = 0.04). Conclusions: We cannot find any sole predictive factor of therapeutic response to the CIESI. But the patients having acute radicular pain with HIVD showed the best response, and those having other chronic neck pain showed the worst response to CIESI.

소닉 브랜딩에 대한 신경 반응 : fMRI 연구 (Neural responses to sonic branding : an fMRI study)

  • 소닉 브랜딩은 소리를 활용하여 브랜드 가치를 창조하고 관리하는 것을 의미한다. 소닉 브랜딩의 다양한 방법 중에서도, 브랜드 정체성의 청각적 요소인 소닉 로고는 보통 비주얼 로고와 함께 쓰이며, 가장 널리 사용되는 소닉 브랜딩의 형태이다. 소닉 브랜딩이 중요한 마케팅 도구로 활용되고 있음에도 불구하고, 이에 대한 학문적 연구는 거의 수행된 바 없다. 본 연구에서는 기능적 자기 공명영상 기법을 사용하여 소닉 브랜딩에 대한 신경 반응을 알아보고자 한다. 15명의 오른손잡이 참가자의 뇌 반응이 3T MRI 장비를 통해 스캔되었고, 청각 자극 없이 여러 장의 브랜드 로고 사진(광고에서 소닉 로고와 함께 제시되었던 비주얼 로고 사진 20장과 광고에서 소닉 요소가 없었던 비주얼 로고 사진 20장)이 제시되었다. 연구 결과, 소닉 요소가 없었던 비주얼 로고에 비해 소닉-비주얼 로고가 청각 심상과 관련된 뇌 영역(예를 들어, 상측두이랑)의 활성화 증가와 연합되는 것으로 나타났다. 또한, 소닉-비주얼 로고에 대한 브랜드 친숙도, 광고 친숙도가 높을수록, 이러한 영역의 활성화 정도가 증가한다는 것을 알 수 있었다.

A Novel Method to Measure Superior Migration of the Humeral Head: Step-off of the C-line

  • Background: Superior migration of humeral head has been conventionally determined by measuring the acromiohumeral distance (AHD), We sought to devise a novel measurement system more reliably and accurately than AHD. We described a structural landmark called 'C-line'. In this study, we investigated the clinical usefulness of 'step-off of the C-line (SOC)' compared to that of AHD. Methods: The C-line formed from the medial margin of the proximal humeral head continuing up to the inferior margin of the articular glenoid and then to the lateral border of the scapula. The superior migration of the humeral head triggered by a rotator cuff tear introduces a discontinuity in this C-line. We measured the distance of this discontinuity. We enrolled 144 patients who underwent a rotator cuff repair. We selected 58 controls who didn't have any cuff lesions apparent on magnetic resonance imaging. Using radiographs derived from standardized true anteroposterior views of the shoulder, we measured the SOC and the AHD. We used t-tests for statistical analyses. Results: A rotator cuff tear was associated with an increase in SOC and a decrease in AHD. In control group, the mean SOC was $1.29{\pm}1.71mm$ and AHD was $9.71{\pm}2.65mm$. In cuff tear group, the mean SOC was $3.15{\pm}3.41mm$ and AHD was $8.28{\pm}1.76mm$. The mean SOCs of the patient group in relation to the mean SOC of the control group according to tear size, the SOCs of medium tear and lager groups showed statistically significant increase (p<0.05). Conclusions: The SOC may be a similarly effective to diagnose cuff tears of medium size and larger compared with AHD.

요추 추간판 탈출증 환자 78명의 한방치료 후 디스크 재흡수 증례 보고 (Lumbar Herniated Disc Resorption of 78 Patients after Korean Medicine Treatment)

  • Objectives To analyze the resorption of herniated lumbar intervertebral disc on MRI in patients who were treated with Korean Medicine. Methods 78 patients with lumbar disc herniation were included, 47 males and 32 females. Patients' diagnosis was based on magnetic resonance imaging (MRI). All of the patients were only treated with Korean Medicine for 6 months treatment, and underwent MRI examination twice, pre-treatment and post-treatment. MR images were assessed by reduction in diameter of T2-weighted image between pre-treatment and post-treatment, and analyzed by differences in sex, age and disc type. Results 91.2% (71 people) of total patients had extrusion type. Korean medicine treatment reduced lumbar disc herniation diameter of all the patients from 7.11 mm to 3.67 mm on average (p<0.001). The reduction of diameter in the male group was 3.52 mm on average. The reduction of diameter in the female group was 3.32 mm on average. The reduction of diameter was 3.28 mm in the group below age 29, 3.88 mm in the 30~39 age group, 3.39 mm in the 40~49 age group, and 2.71 mm in the above 50 age group on average. Conclusions The MRI results suggest that lumbar herniated disc can resorb with Korean Medicine treatment. The likelihood of lumbar disc resorption was higher at extrusion type. but there was not a significant difference between the sexes and between ages.
