Can indirect magnetic resonance arthrography be a good alternative to magnetic resonance imaging in diagnosing glenoid labrum lesions?: a prospective study

  • Background: This study was designed to evaluate and compare the diagnostic value of magnetic resonance imaging (MRI) and indirect magnetic resonance arthrography (I-MRA) imaging with those of arthroscopy and each other. Methods: This descriptive-analytical study was conducted in 2020. All patients who tested positive for labrum lesions during that year were included in the study. The patients underwent conservative treatment for 6 weeks. In the event of no response to conservative treatment, MRI and I-MRA imaging were conducted, and the patients underwent arthroscopy to determine their ultimate diagnosis and treatment plan. Imaging results were assessed at a 1-week interval by an experienced musculoskeletal radiologist. Image interpretation results and arthroscopy were recorded in the data collection form. Results: Overall, 35 patients comprised the study. Based on the kappa coefficient, the results indicate that the results of both imaging methods are in agreement with the arthroscopic findings, but the I-MRA consensus rate is higher than that of MRI (0.612±0.157 and 0.749±0.101 vs. 0.449±0.160 and 0.603±0.113). The sensitivity, specificity, negative predictive value, positive predictive value, and accuracy of MRI in detecting labrum tears were 77.77%, 75.00%, 91.30%, 50.00%, and 77.14%, respectively, and those of I-MRA were 88.88%, 75.00%, 92.30%, 66.66%, and 85.71%. Conclusions: Here, I-MRA showed higher diagnostic value than MRI for labral tears. Therefore, it is recommended that I-MRA be used instead of MRI if there is an indication for potential labrum lesions.

  • It is well known that the electrical impedance of biological tissues is very sensitive to their temperature. In this paper, we have analyzed the effects of temperature change on the phase of magnetic resonance images obtained with external current injection. It has been found that the local phase in the current injected magnetic resonance image can be changed noticeably when local temperature change appears at a part of the tissue. At the experiments with a 0.3 Tesla MRI system, we observed the local phase changes at the phantom images when the phantom temperature was varied between 25 -45$^{\circ}C$. We think that the current injection MRI technique can be used for in-vivo monitoring of the temperature inside biiological tissues if the relation between the local temperature and phase can be quantified.

  • Image findings of hepatic lymphoma have been reported as variable, ranging from single or multiple small nodules to diffuse infiltrative patterns. On MRI, most hepatic lymphomas show T1 low signal intensity, T2 high signal intensity. Dynamic imaging reveals a hypointense appearance in the arterial phase, followed by delayed enhancement in the portal venous and transitional phase. In the hepatobiliary phase using a hepatocyte-specific contrast agent (which have recently aided in increasing the access to the focal liver lesions), hepatic lymphoma is known to exhibit low signal intensity. We report a case of hepatic lymphoma, which shows iso-signal intensity on hepatobiliary phase, using gadoxetic acid (Gd-EOB-DTPA).

  • 국소성 소결절성 활막염은 무릎관절에 발생하는 드문 양성종양으로, 색소 융모결절성 활막염과 감별이 필요하다. 저자들은 자기공명영상에서 경 (pedicle)을 가지며 특징적인 MR 영상소견을 보이고 병리조직학적으로 확진된, 무릎에서 발생한 국소성 소결절성 활막염 1예를 보고한다.

자기공명영상에서 다양한 구조들을 고려한 비선형 이방성 필터링 (Nonlinear Anisotropic Filtering with Considering of Various Structures in Magnetic Resonance Imaging)

  • In this paper, a nonlinear anisotropic filtering method without the loss of important information happened due to the repeated filtering in magnetic resonance images is proposed. First of all original images are divided into four regions, e.g., SPR(Strong Plain Region), EPR(Easy Plain Region), SER(Strong Edge Region), and EER(Easy Edge Region). An optimal template among multiple templates is selected, then the nonlinear anisotropic filtering based on the template is applied in pixel by pixel basis. In the proposed algorithm, filtering strength of EER containing important information is adjusted very weak and filtering strength for remaining regions is also adjusted according to the degree of the importance. In spite of repeated filtering, resulting images by the proposed method could still preserve anatomy information of original images without any degradation. Compared to the existing nonlinear anisotropic filtering, the proposed filtering method with multiple templates provides higher reliability for filtered images.

  • The clinical acceptance of magnetic resonance imaging(MRI) system has been more rapid than that of the other medical image diagnosis system(X-ray, CT, etc) with the advantage of nonhazardous nature, high resolution capability, potential for chemically specified imaging. MRI system is composed of super conducting magnet, gradient fields, rf transceiver, system controller and imaging software technology. In this paper, introducing the principle of magnetic resonance imaging, it proposes the implementation of PC-based MRI system.

  • Anti-myelin oligodendrocyte glycoprotein (anti-MOG) syndrome is an immune-mediated inflammatory condition of the central nervous system, which usually involves spinal cord and optic nerves. Herein, we studied the case of a 57-year-old female patient who presented with acute/subacute symptoms of sphincter dysfunction, paraparesis, and ocular pain. The patient was diagnosed with anti-MOG syndrome with findings resembling snake-eye appearance (SEA), characterized by nearly symmetrical round high signal intensity lesions located at anterior horns (gray matter) on T2-weighted image.

  • Upon review, it is noted that recurrent painful ophthalmoplegic neuropathy (RPON) is a rare neurological syndrome characterized by recurrent unilateral headaches and painful ophthalmoplegia of the ipsilateral oculomotor nerve. As seen on brain MRI, thickening and enhancement of the oculomotor cranial nerve can be observed in these cases. We experienced a case of RPON in an adult patient who showed thickening and enhancement of the oculomotor nerve on gadolinium-enhanced 3D-FLAIR image. The authors report a case of RPON with a review of the literature.

  • 질병을 진단하기 위하여 환부를 가시화하는 것은 매우 중요하다. 많은 경우 환부의 형태학적 변화를 동반하기 때문이다. 혈관에서의 이러한 형태학적 특성을 가시화하는 기법을 혈관 조영기법 이라 한다. 혈관조영기법은 TOF 기법과 위상대조도 기법, 조영제 증강 기법이 있다. 본 논문에서는 혈관 조영기법에 대한 각각의 원리와 관련된 시퀸스 및 특성에 대하여 소개하고 촬영을 통한 데이터 획득후의 데이터 후처리의 과정을 기술하였으며 인체에 응용의 최근 사례들을 소개하였다.

Hypointensity on Susceptibility-Weighted Images Prior to Signal Change on Diffusion-Weighted Images in a Hyperacute Ischemic Infarction: a Case Study

  • Susceptibility-weighted imaging (SWI) is well known for detecting the presence of hemorrhagic transformation, microbleeds and the susceptibility of vessel signs in acute ischemic stroke. But in some cases, it can provide the tissue perfusion state as well. We describe a case of a patient with hyperacute ischemic infarction that had a slightly hypodense, patchy lesion at the left thalamus on the initial SWI, with a left proximal posterior cerebral artery occlusion on a magnetic resonance (MR) angiography and delayed time-to-peak on an MR perfusion performed two hours after symptom onset. No obvious abnormal signals at any intensity were found on the initial diffusion-weighted imaging (DWI). On a follow-up MR image (MRI), an acute ischemic infarction was seen on DWI, which is the same location as the lesion on SWI. The hypointensity on the initial SWI reflects the susceptibility artifact caused by an increased deoxyhemoglobin in the affected tissue and vessels, which reflects the hypoperfusion state due to decreasing arterial flow. It precedes the signal change on DWI that reflects a cytotoxic edema. This case highlights that, in some hyperacute stages of ischemic stroke, hypointensity on an SWI may be a finding before the hyperintensity is seen on a DWI.
