뇌전이 환자의 조영 증강 후 지연 FLAIR 영상의 유용성 (Patients with brain metastases the usefulness of contrast-enhanced FLAIR images after delay)

  • Purpose: FLAIR image is beneficial for the diagnosis of various bran diseases including ischemic CVS, brain tumors and infections. However the border between the legion of brain metastasis and surrounding edema may not be clear. Therefore, this study aims to investigate the practical benefits of delayed imaging by comparing the image from a patient with brain metastasis before a contrast enhancement and the image 10 minutes after a contrast enhancement. Materials and methods: Of the 92 people who underwent MRI brain metastases in suspected patients 13 people in three patients there is no video to target the 37 people confirmed cases, and motion artifacts brain metastases in our hospital June-December 2013, 18 people measurement position except for the three incorrect patient (male: 11 people, female: 7 people, average age: 60 years) in the target, test equipment, 3.0T MR System (ACHIEVA Release, Philips, I was 8ChannelSENSE Head Coil use Best, and the Netherlands). TR 11000 ms, TE 125 ms, TI2800 ms, Slice Thickness 5 mm, gap 5 mm, is a Slice number 21, the parameters of the 3D FFE, T2 FLAIR variable that was used to test, TR 8.1 ms, TE 3.7 ms, Slice number 240 I set to. The experiment was conducted by acquiring the FLAIR prior to contrast enhancement (heretofore referred to as Pre FLAIR), and acquiring the 3D FFE CE five minutes after the contrast enhancement, and recomposing the images in an axial plane of S/T 3mm, G 0mm (heretofore referred to as MPR TRA CE). Using the FLAIR 10 minutes after the contrast enhancement (heretofore referred to as Post FLAIR) and Pi-View, a retrospective study was conducted. Using MRIcro on the image of a patient confirmed for his diagnosis, the images before and after the contrast media, as well as the CNR and SNR of the MPR TRA CE images of the lesion and the site absent of lesion were compared and analyzed using a one-way analysis of variance. Results: CNR for Pre FLAIR and Post FLAIR were 34.35 and 60.13, respectively, with MPR TRA CE at 23.77 showing no significant difference (p<0.050). Post-experiment analysis shows a difference between Pre FLAIR and Post FLAIR in terms of CNR (p<0.050), but no difference in CNR between Post FLAIR and MPR TRA CE (p>0.050), indicating that the contrast media had an effect only on Pre FLAIR and Post FLAIR. The SNR for the normal site Pre FLAIR was 106.43, and for the lesion site 140.79. Post FLAIR for the normal site was 107.79, and for the lesion site 167.91. MPR TRA CE for the normal site was 140.23 and for the lesion site 183.19, showing significant difference (p<0.050), and post-experiment analysis shows that there was a difference in SNR only on the lesion sites for Pre FLAIR and Post FLAIR (p<0.050). There was no difference in SNR between the normal site and lesion site for Post FLAIR and MPR TRA CE, indicating no effect from the contrast media (p>0.050). Conclusions: This experiment shows that Post FLAIR has a higher contrast than Pre FLAIR, and a higher SNR for lesions, It was not not statistically significant and MPR TRA CE but CNR came out high. Inspection of post-contrast which is used in a high magnetic field is frequently used images of 3D T1 but, since the signal of the contrast medium and the blood flow is included, this method can be diagnostic accuracy is reduced, it is believed that when used in combination with Post FLAIR, and that can provide video information added to the diagnosis of brain metastases.

뇌의 고속스핀에코 T2강조영상에서 지방조직 억제를 위한 SPAIR의 반전시간(TI) 결정 및 STIR 영상과의 비교 연구 (Selection of TI for Suppression Fat Tissue of SPAIR and Comparative Study of SPAIR and STIR of Brain Fast SE T2 Weighted Imaging)

  • 본 연구는 뇌의 T2고속스핀에코 3.0T 자기공명영상에서 지방조직의 억제능력을 유지하면서 동시에 지방조직의 불균일성을 최소화할 수 있는 SPAIR의 적정 반전시간(TI)을 찾아보고, 지방억제기법인 STIR기법과도 비교하고자 한다. SPAIR 프로토콜의 반전시간(TI)은 SPAIR TR(420 msec)의 1/2, 1/3, 1/6, 1/12, 즉 210 msec(8명), 140 msec(26명), 70 msec(26명), 35 msec(18명)로 설정하고, STIR의 TI는 250 msec(26명)로 설정하여 뇌의 축방향 영상(104개)을 획득하였다. 출력영상의 지방조직, 근육조직, 백그라운드의 ROI($50\;mm^2$)를 설정하여 신호강도(SI)를 측정하였고, 지방과 근육조직과의 CNR을 산출하고, 지방조직에서 불균일치는 측정된 지방 신호의 평균치(mean)에 대한 표준편차(SD)로서 그 산출은 SD/mean으로 계산하였다. 측정결과는 첫째, SPAIR TI가 70 ms일 때, 지방조직의 신호강도가 TI가 140 ms 보다 낮게 측정되어 지방억제능력이 우수하였다. 둘째, SPAIR TI가 140 ms일 때, 영상의 질 측면에서 지방조직의 불균일치는 70 ms 보다 낮아서 영상의 질은 우수하게 나타났다. 따라서 SPAIR TI의 선택은 TR(420 ms)의 1/3인 140 ms와 1/6인 70 ms에서 결정되어야 하는데 지방조직의 억제능력과 지방조직의 불균일치에서 각각 통계적으로 유의한 차이(p < .001)를 보여 화질 측면에서 TI : 140 ms를 선택하는 것이 바람직하다고 생각한다. 한편, SPAIR(TI : 140 ms)와 STIR의 비교는 지방억제는 통계적으로 유의한 차이가 없으나(p < .252), 화질 측면에서 SPAIR가 STIR보다 우수한 결과를 보였고, 통계적으로 유의한 차이를 보였다(p < .01).

3테슬러 자기공명영상기기에서 유방의 유선조직과 지방조직의 $T_2{^*}$이완시간 측정 (Estimation of $T_2{^*}$ Relaxation Times for the Glandular Tissue and Fat of Breast at 3T MRI System)

  • 목적: $T_2{^*}$이완시간은 조직의 고유한 특징을 반영하는 자화율 정보를 내재하고 있다. 본 연구는 3테슬러 자기공명영상 기기에서 유방의 정상 유선조직과 지방조직의 $T_2{^*}$이완시간을 알아보고자 하였다. 대상과 방법: 52명의 여자환자에서 (나이, $49{\pm}12 $세; 범위, 25세~75세) 삼차원 경사에코연쇄를 이용하여 각 일곱 개 에코 자기공명영상들을 얻었다. 에코시간은 3.91 ms단계에서 2.28 ms에서 25.72 ms사이 범위였다. 화적소를 기초로 $T_2{^*}$이완시간과 $R_2{^*}$이완율 지도는 각 개체에서 선형곡선맞춤을 이용하여 계산되었다. $T_2{^*}$값과 $R_2{^*}$값을 얻기 위하여 유방의 정상 유선조직과 지방조직에 삼차원 관심영역을 표시하였다. 모든 개체에서 이러한 변수들의 평균치를 계산하였다. 결과: 유방의 정상 유선조직과 지방조직에 삼차원 관심영역 크기는 각각 $4818{\pm}4679$ 화적소와 $1455{\pm}785$ 화적소 였다. 평균 $T_2{^*}$ 값은 유방의 정상 유선조직과 지방조직에서 각각, $22.40{\pm}5.61ms$$36.36{\pm}8.77ms$였다. 평균 $R_2{^*}$값은 유방의 정상 유선조직과 지방조직에서 각각, $0.0524{\pm}0.0134/ms$$0.0297{\pm}0.0069/ms$ 였다. 결론: 유방의 정상 유선조직과 지방조직에서 $T_2{^*}$값과 $R_2{^*}$값을 측정하였다. 유방의 정상 유선조직의 $T_2{^*}$값은 지방조직의 $T_2{^*}$값보다 짧게 나타났다. $T_2{^*}$이완시간의 측정은 유방암과 정상 유방조직에서의 자화율 효과를 이해하는데 도움을 줄 것이다.

Regional Amyloid Burden Differences Evaluated Using Quantitative Cardiac MRI in Patients with Cardiac Amyloidosis

  • Objective: This study aimed to investigate the regional amyloid burden and myocardial deformation using T1 mapping and strain values in patients with cardiac amyloidosis (CA) according to late gadolinium enhancement (LGE) patterns. Materials and Methods: Forty patients with CA were divided into 2 groups per LGE pattern, and 15 healthy subjects were enrolled. Global and regional native T1 and T2 mapping, extracellular volume (ECV), and cardiac magnetic resonance (CMR)-feature tracking strain values were compared in an intergroup and interregional manner. Results: Of the patients with CA, 32 had diffuse global LGE (group 2), and 8 had focal patchy or no LGE (group 1). Global native T1, T2, and ECV were significantly higher in groups 1 and 2 than in the control group (native T1: 1384.4 ms vs. 1466.8 ms vs. 1230.5 ms; T2: 53.8 ms vs. 54.2 ms vs. 48.9 ms; and ECV: 36.9% vs. 51.4% vs. 26.0%, respectively; all, p < 0.001). Basal ECV (53.7%) was significantly higher than the mid and apical ECVs (50.1% and 50.0%, respectively; p < 0.001) in group 2. Basal and mid peak radial strains (PRSs) and peak circumferential strains (PCSs) were significantly lower than the apical PRS and PCS, respectively (PRS, 15.6% vs. 16.7% vs. 26.9%; and PCS, -9.7% vs. -10.9% vs. -15.0%; all, p < 0.001). Basal ECV and basal strain (2-dimensional PRS) in group 2 showed a significant negative correlation (r = -0.623, p < 0.001). Group 1 showed no regional ECV differences (basal, 37.0%; mid, 35.9%; and apical, 38.3%; p = 0.184). Conclusion: Quantitative T1 mapping parameters such as native T1 and ECV may help diagnose early CA. ECV, in particular, can reflect regional differences in the amyloid deposition in patients with advanced CA, and increased basal ECV is related to decreased basal strain. Therefore, quantitative CMR parameters may help diagnose CA and determine its severity in patients with or without LGE.

THE HST COSMOS PROJECT: CONTRIBUTION FROM THE SUBARU TELESCOPE

  • The Cosmic Evolution Survey (COSMOS) is a Hubble Space Telescope (HST) treasury project. The COSMOS aims to perform a 2 square degree imaging survey of an equatorial field in I(F814W) band, using the Advanced Camera for Surveys (ACS). Such a wide field survey, combined with ground-based photometric and spectroscopic data, is essential to understand the interplay between large scale structure, evolution and formation of galaxies and dark matter. In 2004, we have obtained high-quality, broad band images of the COSMOS field (B, V, r', i', and z') using Suprime-Cam on the Subaru Telescope, and we have started our new optical multi-band program, COSMOS-21 in 2005. Here, we present a brief summary of the current status of the COSMOS project together with contributions from the Subaru Telescope. Our future Subaru program, COSMOS-21, is also discussed briefly.

Proton MR Spectroscopic Changes in Parkinson's Disease

  • Purpose: To investigate whether there are significant changes in regional brain metabolism in patients with Parkinson's disease after thalamotomy using proton magnetic resonance spectroscopy (lH MRS). Methods: Fifteen patients with Parkinson's disease of mean age 56.5 years (7 males and 8 females; mean age, 56.5 years) that have treated with levodopa were included. All patients with tremor experienced amelioration of their symptoms on the side contralateral to the thalamotomy. As a single-voxel technique, 1H MR spectra were obtained from the volume of interested regions in thalamus and primary motor cortex. Spectral parameters were: 20 ms TE, 2000 ms TR, 128 averages, 2500 Hz spectral width, and 2048 data points. Results: We found that NAA/Cho ratios showed generally low levels in thalamus in Parkinson's disease patients with clinical improvement following thalamotomy. Conclusions: 1H MRS may be a useful utility for the aid in better understanding the pathophy-siologic process in Parkinson's disease patients on the basis of the variation of NAA/Cho ratio. Motor Cortex in Hemiparetic Patients due to Deep Intracerebal Hematoma

  • Purpose: To determine the motor cortex dysfunction in hemiparetic patients due to deep intracerebral hematoma, authors performed proton magnetic resonance spectroscopy (lH MRS) for the evaluation of biochemical changes in the cortex on affected hemisphere according to axonal injury at the level of internal capsule. Methods: Ten control subjects and 14 patients with documentable hemiparesis of varying severity hemiparesis were included. All the hemiparesis was caused by deep intracerebral hematoma (putaminal and thalamic hemorrhage). In vivo 1H MRS study was performed on a 3T MRI/MRS system using STEAM sequence. As a single-voxel technique, Spectral parameters were: 20 ms TE, 2000 ms TR, 128 averages, 2500 Hz spectral width, and 2048 data points. Results: We found that the mean N-acetylaspartate (NAA)/phosphocreatine (Cr) and NAA/choline (Cho) ratios were significantly decreased in the motor cortex of the hemiparesis patients compared with control subjects. Conclusions: 1H MRS examinations of the motor cortex might help to differentiate distinct clinical entities of hemiparesis and to monitor pharmacological effects in therapeutic trials, providing a quantitative biological marker for motor neuron dysfunction. Experience of Meningovascular Syphilis in Human Immunodeficiency Virus Infected Patient

  • Since the start of the antibiotic era, syphilis has become rare. However, in recent times, it has tended to be prevalent concomitantly with human immunodeficiency virus (HIV) infection and coinfection in North America and Europe. Now, such cases are expected to increase in elsewhere including Korea. A 40-year-old male patient visited hospital complaining of a headache for about one month. Brain computed tomography and magnetic resonance imaging, showed leptomeninged enhancing mass with edema an right porisylvian region, which was suspected to be glioma. Patient underwent a blood test and was diagnosed with syphilis and acquired immune deficiency syndrome. Partial cortical and subcortical resection were performed after small craniotomy. The dura was thick, adhered to the brain cortex, and was accompanied by hyperemic change of the cortex. The pathologic diagnosis was meningovascular syphilis (MS) in HIV infection. After the operation, the patient was treated with aqueous penicillin G. Thereafter, he had no neurological deficit except intermittent headache. At first, this case was suspected to be glioma, but it was eventually diagnosed as MS in HIV coinfection. At this point the case was judged to be worth reporting.

High Speed SD-OCT System Using GPU Accelerated Mode for in vivo Human Eye Imaging

  • We developed an SD-OCT (Spectral Domain-Optical Coherence Tomography) system which uses a GPU (Graphics Processing Unit) for processing. The image size from the SD-OCT system is $1024{\times}512$ and the speed is 110 frame/sec in real-time. K-domain linearization, FFT (Fast Fourier Transform), and log scaling were included in the GPU processing. The signal processing speed was about 62 ms using a CPU (Central Processing Unit) and 1.6 ms using a GPU, which is 39 times faster. We performed an in-vivo retinal scan, and reconstructed a 3D visualization based on C-scan images. As a result, there were minimal motion artifacts and we confirmed that tomograms of blood vessels, the optic nerve, and the optic disk are clearly identified. According to the results of this study, this SD-OCT can be applied to real-time 3D display technology, particularly auxiliary instruments for eye operations in ophthalmology.

Synthesis and Characterization of Y2O3:Eu Fine Particle

  • [ $Y_2O_3:Eu$ ] powder was synthesized using a solution-combustion method by dissolving $(CH_3CO_2)_3Y$ and $(CH_3CO_2)_3$ Eu in methyl-alcohol solution. Results from X-ray diffractometery (XRD), thermogravimetry (TG)-differential thermal analysis (DTA) show that $Y_2O_3:Eu$ crystallizes completely when the dry powder is sintered at $500^{\circ}C$. The investigated optical properties were the photoluminescence emission spectra, the excitation spectra and luminescence decay curve. Europium (Eu) concentration had no observable effect on the optical spectrum which depended on the emission intensity. The mean lifetime of synthesized phosphors was $2.3\~2.6 ms$.
