Clinical Effect of Left Ventricular Dysfunction in Patients with Mitral Stenosis after Mitral Valve Replacement

  • Background: Mitral stenosis (MS) remains one of the important heart diseases. There are many factors that influence the clinical outcomes, and little is known about how left ventricular (LV) dysfunction clinically affects the prognosis of the patient with MS after mitral valve replacement (MVR). We reviewed our clinical experiences of MVR in patients with MS who had LV dysfunction. Methods: Between January 1991 and January 2013, 110 patients with MS who underwent MVR were analyzed and divided into two groups according to ejection fraction (EF). Group 1 ($EF{\leq}45%$) included 13 patients and group 2 (EF>45%) included 97 patients. Results: Thromboembolism occurred in 8 patients after MVR (group 1: n=3, 23.1%; group 2: n=5, 5.2%) and its incidence was significantly higher in group 1 than in group 2 (p=0.014). There were 3 deaths each in groups 1 and 2 during follow-up. The overall rate of cardiac-related death in group 1 was significantly higher than in group 2 (group 1: n=3, 23.1%; group 2: n=3, 3.1%; p=0.007). The cumulative survival rate at 1 and 15 years was 83.9% and 69.9% in group 1 and 97.9% and 96.3% in group 2 (p=0.004). The Cox regression analysis revealed that survival was significantly associated with postoperative stroke (p=0.011, odds ratio=10.304). Conclusion: This study identified postoperative stroke as an adverse prognostic factor in patients with MS after MVR, and a s more prevalent in patients with LV dysfunction. Postoperative stroke should be reduced to improve clinical outcomes for patients. Preventive care should be made in multiple ways, such as management of LV dysfunction, atrial fibrillation, and anticoagulation.

  • Hemiplegia cruciata (HC) manifests as paralysis of the ipsilateral arm and contralateral leg. Herein, we report a 64-year-old man with weakness of the right leg and of the left arm after multiple sclerosis (MS). His brain and spine magnetic resonance imaging show a lower medulla lesion, which is extended to posterior part of C1 spine through cervicomedullary junction. HC usually results from stroke or trauma, but it is rare as presenting symptom of MS.

항공기 낙뢰 간접영향 인증시험 동향

  • 오늘날 항공기 설계와 제작은 다국적인 산업의 형태로 이루어지고 있다. 즉 항공기 서브시스템은 세계 각국에서 독립적으로 설계, 제작 및 시험한 후 한 곳으로 납품되어 조립 제작되는 데 이 때 중요한 것이 표준화이다. 항공전자장비의 낙뢰 간접영향에 대한 인증의 경우 RTCA DO-160E, Section 22 Lightning Induced Transient Susceptibility(낙뢰 유도 과도현상 적응성)로 표준화 되어 있다. 이 Section 22는 낙뢰 간접영향에 대한 항공전자장비 단위 즉, LRU (Line Replaceable Units)와 같은 부품단위의 시험 요건을 규정하고 있으며 이 규격은 현재 전 세계적으로 통용이 되고 있다. 1980년대 초 상용 수송기에 디지털 "Fly by Wire" 비행 시스템과 엔진제어시스템(EEC, Electronic Engine Control)의 도입 이후, 항공기 시스템이 낙뢰 환경에서 운용시 신뢰성을 보증할 필요성이 대두되었다. 데이터 처리를 통하여 제어되는 각종 항공전자장비에는 다중타격(MS)과 다중파열(MB) 기법에 의한 시험 사항이 최근 추가 되었다. 실제 낙뢰 환경과 유사한 시험실 모사를 위해 계속적인 연구가 진행 중이며 신규 시험 규격서가 새로이 출간되고 있다.

다발성 경화증에 대한 한방치료 치험 1례 (A Clinical Case Report on Multiple Sclerosis Treated by Korean Medicine)

  • Objectives : This study was to report the improvement of the patient with multiple sclerosis treated by Korean Medical treatment. Methods : The patient diagnosed as multiple sclerosis treated by acupuncture, bee venom pharmacupuncture, chuna manual therapy, and herbal medicine. To evaluate the effects of korean medicine, we used manual muscle test(MMT), the standard for assessment of the effect of stroke treatment, functional system(FS), visual analog scale(VAS). Results : 1. The patient's pain and optic dysfunction were reduced by the above therapy. 2. There was no change in manual muscle test(MMT), but there was mild improvement in finger joints function in the standard for assessment of the effect of stroke treatment. 3. In functional system(FS) scale, there was enhancement in optic function within a range. And there was improvement in expanded disability statue scale(EDSS) from 4.0 to 3.0. 4. In visual analog scale(VAS), there was considerable improvement from 9 to 6. Conclusions : The korean medical treatment was reduced pain and optic dysfunction.

Inhibitory Effects of Coptis japonica Alkaloids on the LPS-Induced Activation of BV2 Microglial Cells

  • Coptis japonica (C. japonica) is a perennial medicinal plant that has anti-inflammatory activity. C. japonica contains numerous biologically active alkaloids including berberine, palmatine, epi-berberine, and coptisine. The most well-known anti-inflammatory principal in C. japonica is berberine. For example, berberine has been implicated in the inhibition of iNOS induction by cytokines in microglial cells. However, the efficacies of other alkaloids components on microglial activation were not investigated yet. In this study, we investigated the effects of three alkaloids (palmatine, epi-berberine and coptisine) from C. japonica on lipopolysaccharide (LPS)-induced microglial activation. BV2 microglial cells were immunostimulated with LPS and then the production of several inflammatory mediators such as nitric oxide (NO), reactive oxygen species (ROS) and matrix metalloproteinase-9 (MMP-9) were examined as well as the phosphorylation status of Erk1/2 mitogen activated protein kinase (MAPK). Palmatine and to a lesser extent epi-berberine and coptisine, significantly reduced the release of NO, which was mediated by the inhibition of LPS-stimulated mRNA and protein induction of inducible nitric oxide synthase (iNOS) from BV2 microglia. In addition to NO, palmatine inhibited MMP-9 enzymatic activity and mRNA induction by LPS. Palmatine also inhibited the increase in the LPS-induced MMP-9 promoter activity determined by MMP-9 promoter luciferase reporter assay. LPS stimulation increased Erk1/2 phosphorylation in BV2 cells and these alkaloids inhibited the LPS-induced phosphorylation of Erk1/2. The anti-inflammatory effect of palmatine in LPS-stimulated microglia may suggest the potential use of the alkaloids in the modulation of neuroinflammatory responses, which might be important in the pathophysiological events of several neurological diseases including Alzheimer's disease (AD), multiple sclerosis (MS), Parkinson's disease (PD) and stroke.
