Comparison of Abbreviated MRI and Full Diagnostic MRI in Distinguishing between Benign and Malignant Lesions Detected by Breast MRI: A Multireader Study

  • Objective: To compare the performance of simulated abbreviated breast MRI (AB-MRI) and full diagnostic (FD)-MRI in distinguishing between benign and malignant lesions detected by MRI and investigate the features of discrepant lesions of the two protocols. Materials and Methods: An AB-MRI set with single first postcontrast images was retrospectively obtained from an FD-MRI cohort of 111 lesions (34 malignant, 77 benign) detected by contralateral breast MRI in 111 women (mean age, 49.8. ± 9.8; range, 28-75 years) with recently diagnosed breast cancer. Five blinded readers independently classified the likelihood of malignancy using Breast Imaging Reporting and Data System assessments. McNemar tests and area under the receiver operating characteristic curve (AUC) analyses were performed. The imaging and pathologic features of the discrepant lesions of the two protocols were analyzed. Results: The sensitivity of AB-MRI for lesion characterization tended to be lower than that of FD-MRI for all readers (58.8-82.4% vs. 79.4-100%), although the findings of only two readers were significantly different (p < 0.05). The specificity of AB-MRI for lesion characterization was higher than that of FD-MRI for 80% of readers (39.0-74.0% vs. 19.5-45.5%, p ≤ 0.001). The AUC of AB-MRI was comparable to that of FD-MRI for all readers (p > 0.05). Fifteen percent (5/34) of the cancers were false-negatives on AB-MRI. More suspicious margins or internal enhancement on the delayed phase images were related to the discrepancies. Conclusion: The overall performance of AB-MRI was similar to that of FD-MRI in distinguishing between benign and malignant lesions. AB-MRI showed lower sensitivity and higher specificity than FD-MRI, as 15% of the cancers were misclassified compared to FD-MRI.

MRI Image Super Resolution through Filter Learning Based on Surrounding Gradient Information in 3D Space (3D 공간상에서의 주변 기울기 정보를 기반에 둔 필터 학습을 통한 MRI 영상 초해상화)

  • Three-dimensional high-resolution magnetic resonance imaging (MRI) provides fine-level anatomical information for disease diagnosis. However, there is a limitation in obtaining high resolution due to the long scan time for wide spatial coverage. Therefore, in order to obtain a clear high-resolution(HR) image in a wide spatial coverage, a super-resolution technology that converts a low-resolution(LR) MRI image into a high-resolution is required. In this paper, we propose a super-resolution technique through filter learning based on information on the surrounding gradient information in 3D space from 3D MRI images. In the learning step, the gradient features of each voxel are computed through eigen-decomposition from 3D patch. Based on these features, we get the learned filters that minimize the difference of intensity between pairs of LR and HR images for similar features. In test step, the gradient feature of the patch is obtained for each voxel, and the filter is applied by selecting a filter corresponding to the feature closest to it. As a result of learning 100 T1 brain MRI images of HCP which is publicly opened, we showed that the performance improved by up to about 11% compared to the traditional interpolation method.

  • Many radiological modalities has been applied to medicine as a basic fundamental diagnosis and therapy recently. The prevalence of computer systems affect most images to be digitized. However conventional X-ray film images are not digital images eventhough they covers 70% of all radiologica images. This is the hinderacne of building PACS. In this paper all radiological digital imaging parts such as DSA. CR. MRI. SPECT. PET and ultrasonography were briefly introduced and the applications were described. In brief digital radiography contribute to enhance the medical service quality. And the digital substituition of conventional X-ray film image is inevitable.

Diagnostic Value of MRI in Schwannoma (신경초종 진단에 있어 자기공명영상촬영(MRI)의 진단적 가치)

  • Purpose: To diagnose soft tissue tumor, such as lipoma and Schwannoma, magnetic resonance imaging (MRI) is sufficient in most cases. However, various characteristics are found in MRI images of Schwannoma, thus other type of tumors are often misdiagnosed as Schwannoma with MRI images. In this study, we evaluate the diagnostic value of specific MRI findings of Schwannoma. Materials and Methods: From January 2002 to May 2013, 104 patients who are suspected as Schwannoma rith MRI images are included in data, and the final diagnosis is confirmed with biopsy. Patients are divided into group 1 and group 2 who are confirmed as Schwannoma and other disease with biopsy, respectively. Results: 92 patients were diagnosed as Schwannoma (group 1) and 12 patients were diagnosed as other disease (group 2). We investigate the diagnostic value of specific MRI findings of Schwannoma. 41 patients of group 1 (45%) and 0 patients of group 2 (0%) showed target sign, 47 patients of group 1 (51%) and 2 patients of group 2 (17%) showed fascicular sign, 44 patients of group 1 (48%) and 5 patients of group 2 (42%) showed fat split sign, 28 patients of group 1 (30%) and 1 patients of group 2(9%) showed nerve entering and exiting sign, and 8 patients of group 1 (9%) and 6 patients of group 2 (50%) showed none of four specific findings on their MRI images. 52 patients of group 1 (57%) and 5 patients of group 2 (42%) have tumors on the pathway of nerve. Target sign could be considered as the best diagnostic value of the sign we investigate (p<0.05). Conclusion: Although specific MRI findings have powerful diagnostic value, patients are often misdiagnosed as Schwannoma with MRI findings. Therefore, if patients who are suspected as Schwannoma based on MRI findings have no target sign on their MRI images, we should consider the possibility of other disease.

  • The method of simultaneous data acquisition of arteries and veins(SAAV) was suggested to obtain MR angiography of arteries and veins at 0.3T low filed MRI system (Magfinder, AlLab. Korea). Two separated artery- and vein-images were put together using AVCM(Artery-Vein Color Mapping) algorithm and presented in the same image. In this study, artery- and vein-separated angiograms of volunteer's neck were obtained. Two dimensioal blood-enhanced images wre sequentially obtained using SAAV pulse sequence based on time-of-flight(TOF) method with flow compensation. Imaging parameters were TR/TE=70/12msec. FOV=230mm, slice thickness = 3mm, flip angle=90$^{\circ}$, matrix size=256${\times}$256${\times}$64mm. TSat TH/SPA=15/20mm, Ts_v=10msec and Ts_a=40ms. 3D MRA images were reconstructed using the maximum intensity projection(MIP) and the artery-vein color mapping(AVCM) algorithm. This study showed good possibility of clinical applications of MRA in 0.3T which provides valuable diagnostic information of clinical vascular diseases.

3D Reconstruction of Tissue from a few of MRI Images using Radial Basis Function (BBF를 이용한 적은 수의 MRI 이미지로부터 3차원 조직 재구성)

  • Recent the advanced technologies in medical imaging such as magnetic resonance imaging (MRI) and computed tomography (CT) make doctors improve the diagnostic skill with detailed anatomical information. In general, it is necessary to get a number of MRI images in order to obtain more detail information. However, the performance of MRI machines of privately run hospitals is not good and thus we may obtain only a few of MRI images. If 3D surface reconstruction is accomplished with a few slices, then it generates 3D surface of poor qualify. This paper propose a way to Set a 3D surface of high quality from a few of number of slices. First of all, our algorithm detects the boundary of tissues which we want to reconstruct as a 3D object and find out the set of vortices on the boundary. And then we generate a 3D implicit surface to interpolate the boundary points by using radial basis function. Lastly, we render the 3D implicit surface by using Marching cube algorithms.

Comparison of static MRI and pseudo-dynamic MRI in tempromandibular joint disorder patients (측두하악관절장애 환자에서의 static MRI와 pseudo-dynamic MRI의 비교연구)

  • Purpose: The purpose of this study was to evaluate comparison of static MRI and pseudo-dynamic (cine) MRI in temporomandibular joint (TMJ) disorder patients. Materials and Methods: In this investigation, 33 patients with TMJ disorders were examined using both conventional static MRI and pseudo-dynamic MRI. Multiple spoiled gradient recalled acquisition in the steady state (SPGR) images were obtained when mouth opened and closed. Proton density weighted images were obtained at the closed and open mouth position in static MRI. Two oral and maxillofacial radiologists evaluated location of the articular disk, movement of condyle and bony change respectively and the posterior boundary of articular disk was obtained. Results: No statistically significant difference was found in the observation of articular disk position, mandibular condylar movement and posterior boundary of articular disk using static MRI and pseudo-dynamic MRI (P<0.05). Statistically significant difference was noted in bony changes of condyle using static MRI and pseudo-dynamic MRI (P<0.05). Conclusion: This study showed that pseudo-dynamic MRI didn't make a difference in diagnosing internal derangement of TMJ in comparison with static MRI. But it was considered as an additional method to be supplemented in observing bony change.

  • This paper proposed a curve progress control function of the area base instead of the existing edge indication function, in order to detect the brain ventricle area by utilizing a geodesic active contour model. The proposed curve progress control function is very effective in detecting the brain ventricle area and this function is based on the average brightness of the brain ventricle area which appears brighter in MRI images. Compared numerically by using various measures, the proposed method in this paper can detect brain ventricle areas better than the existing method. By examining images of normal and diseased brain's images by brain tumor, we compared the several brain ventricle detection algorithms with proposed method visually and verified the effectiveness of the proposed method.

Computed Tomographic and Magnetic Resonance Imaging Features of Oral Melanoma in a Dog

  • Oral melanoma is the most common type of oral tumor in dogs. In this report, computed tomography (CT) and magnetic resonance imaging (MRI) were performed to diagnose a right oral pigmented mass in an 8-year-old dog. The oral mass appeared as a homogeneous soft tissue density parenchyma on pre-contrast CT images, and with heterogeneous enhancement on post-contrast images. Bone destruction of the right mandibular body around the mass and mild enlargement of the right mandibular lymph node were also found. On MRI, the bulky oral mass showed mixed hyperintensity and isointensity compared to the adjacent muscle, where irregular hyperintensity on T1-weighted images corresponded to hypointensity on the T2-weighted images. Based on the physical examinations and imaging results, melanoma was suspected and confirmed via fine-needle aspiration. These unique MRI signals were due to the high paramagnetic melanin content in the tumor, therefore MRI examination could be useful for diagnosis of melanoma.

High Signal Intensities on T1-Weighted MRI as a Biomarker of Manganese

  • Increased signal in T1-weighted images was observed in the experimental manganese (Mn) poisoning of the non-human primate and a patient with Mn neurointoxication. However, our study showed that the increased signals in magnetic resonance images (MRI) were highly prevalent (41.6%) in Mn-exposed workers. Blood Mn concentration correlated with pallidal index. These changes in MRI tend to disappear following the withdrawal from the source of Mn accumulation, despite permanent neurological damage. Thus increased signal intensities on a T1-weighted image reflect exposure to Mn, but not necessarily manganism. Our study also showed that the concentration of Mn required to produce increased signal intensities on MRI is much lower than the threshold necessary to result in overt clinical signs of manganism. Increased signal intensities in the globus pallidus were determined by manganese accumulation in the animal experiment. Reanalysis of the previous data with the structural equation model revealed that pallidal index (Pl) on MRI reflects target organ dose of occupational Mn exposure
