Postcontrast T1-weighted Brain MR Imaging in Children: Comparison of Fat-suppressed Imaging with Conventional or Magnetization Transfer Imaging

  • To assess the merits and demerits of postcontrast fat-suppressed (FS) brain MR imaging in children in the evaluation of various enhancing lesions, compared with postcontrast conventional or Magnetization Transfer (MT) imaging. 대상 및 방법: We reviewed patients with enhancing lesion on brain MR imaging who underwent both FS imaging and one of conventional or MT imaging as a postcontrast T1-weighted brain MR imaging. Inclusion criteria of our study were as follows: MR studies should be peformed within one-year interval and showed no significant interval change of imaging findings. Thirty-four patients (21 male, 13 female; mean age, 8 years) with 43 enhancing lesions (19 intra-axial, 19 extra-axial, and 5 orbital location) were included in this study, Twenty-one pairs of FS and conventional imaging, and 15 pairs of FS and MT imaging were available. Two radiologists visually assessed the lesion conspicuity and the presence of flow or susceptibility artifacts in a total of 36 pairs of MR imaging by consensus. For 21 measurable lesions (19 pairs of FS and conventional imaging, 5 pairs of FS and MR imaging), contrast ratio between the lesion and the normal brain( [SIlesion-SIwater]/[SInormal brain-SIwater]) were calculated and compared.

흉부질환의 자기공명영상 (Magnetic Resonance Imaging in Thoracic Disease)

  • The role of magnetic resonance(MR) imaging in the evaluation of thoracic disease has been limited Nontheless, MR has inherent properties of better contrast resolution than CT allowing tissue-specific diagnosis. MR has capability of direct imaging in sagittal, coronal, and oblique planes which provide better anatomic information than axial images of CT such as lesions in the pulmonary apex, aorticopulmonary window, peridiaphragmatic region, and subcarinal region. MR is sensitive to blood flow making it an ideal imaging modality for the evaluation of cardiovascular system of the thorax without the need for intravenous contrast media. Technical developments and better control of motion artifacts have resulted in improved image quality, and clinical applications of MR imaging in thoracic diseases have been expanded. Although MR imaging is considered as a problem-solving tool in patients with equivocal CT findings, MR should be used as the primary imaging modality in the following situations: 1) Evaluation of the cardiovascular abnormalities of the thorax 2) Evaluation of the superior sulcus tumors 3) Evaluation of the chest wall invasion or mediastinal invasion by tumor 4) Evaluation of the posterior mediastinal mass, especially neurogenic tumor 5) Differentiation of fibrosis and residual or recurrent tumor, especially in lymphoma 6) Evaluation of brachial plexopathy With technical developments and fast scan capabilities, clinical indications for MR imaging in thorax will increase in the area of pulmonary parenchymal and pulmonary vascular imaging.

  • The role of neuroimaging in patients with acute ischemic stroke has been gradually increasing. The ultimate goal of stroke imaging is to make a streamlined imaging workflow for safe and efficient treatment based on optimized patient selection. In the era of multimodal comprehensive imaging in strokes, imaging based on computed tomography (CT) has been preferred for use in acute ischemic stroke, because, despite the unique strengths of magnetic resonance imaging (MRI), MRI has a longer scan duration than does CT-based imaging. However, recent improvements, such as multicoil technology and novel MRI acceleration techniques, including parallel imaging, simultaneous multi-section imaging, and compressed sensing, highlight the potential of comprehensive MR-based imaging for strokes. In this review, we discuss the role of stroke imaging in acute ischemic stroke management, as well as the strengths and limitations of MR-based imaging. Given these concepts, we review the current MR acceleration techniques that could be applied to stroke imaging and provide an overview of the previous research on each essential sequence: diffusion-weighted imaging, gradient-echo, fluid-attenuated inversion recovery, contrast-enhanced MR angiography, and MR perfusion imaging.

Usefulness of MR Imaging for Diseases of the Small Intestine: Comparison with CT

  • Objective: To evaluate the usefulness of MR imaging for diseases of the small intestine, emphasizing a comparison with CT. Materials and Methods: Thirty-four patients who underwent both CT and MR imaging using FLASH 2D and HASTE sequences were analyzed. All patients had various small bowel diseases with variable association of peritoneal lesions. We compared the detectabilities of CT and MR imaging using different MR pulse sequences. The capability for analyzing the characteristics of small intestinal disease was also compared. Results: MR imaging was nearly equal to CT for detecting intraluminal or peritoneal masses, lesions in the bowel and mesentery, and small bowel obstruction, but was definitely inferior for detecting omental lesions. The most successful MR imaging sequence was HASTE for demonstrating bowel wall thickening, coronal FLASH 2D for mesenteric lesions, and axial FLASH 2D for omental lesions. MR imaging yielded greater information than CT in six of 12 inflammatory bowel diseases, while it was equal to CT in six of seven neoplasms and inferior in five of seven mesenteric ischemia. In determining the primary causes of 15 intestinal obstructions, MR imaging was correct in 11 (73%) and CT in nine (60%) patients. Conclusion: MR imaging can serve as an alternative diagnostic tool for patients with suspected inflammatory bowel disease, small intestinal neoplasm or obstruction.

Postcontrast Brain MR Imaging in Children: Various Pulse Sequences and Imaging Strategies

  • In brain MR imaging, contrast-enhanced study is important in the detection and characterization of lesions. As a postcontrast brain MR imaging, conventional T1 weighted imaging has been usually used. Magnetization transfer imaging has been used to increase conspicuity of enhancing lesions. In addition, fat-suppression imaging can be used as in other parts of the body. Recently, FLAIR sequence has been reported to be useful in detecting subarachnoid, meningeal, and subdural abnormalities. In this exhibit, we demonstrate basic principles and typical appearances of various pulse sequences that can be used as a postcontrast brain MR imaging in children. Furthermore, we discuss imaging strategies to increase clinical usefulness of postcontrast brain MR imaging for specific abnormalities. The advantages and disadvantages of each pulse sequence are also discussed.

  • The two major classes of magnetic resonance (MR) contrast agents are paramagnetic contrast agents, usually based on chelates of gadolinium generating T1 positive signal enhancement, and super-paramagnetic contrast agents that use mono- or polycrystalline iron oxide to generate strong T2 negative contrast in MR images. These paramagnetic or super-paramagnetic complexes are used to develop new contrast agents that can target the specific molecular marker of the cells or tan be activated to report on the physiological status or metabolic activity of biological systems. In molecular imaging science, MR imaging has emerged as a leading technique because it provides high-resolution three-dimension maps of the living subject. The future of molecular MR imaging is promising as advancements in hardware, contrast agents, and image acquisition methods coalesce to bring high resolution in vivo imaging to the biochemical sciences and to patient care.

DEMO: Deep MR Parametric Mapping with Unsupervised Multi-Tasking Framework

  • Compressed sensing (CS) has been investigated in magnetic resonance (MR) parametric mapping to reduce scan time. However, the relatively long reconstruction time restricts its widespread applications in the clinic. Recently, deep learning-based methods have shown great potential in accelerating reconstruction time and improving imaging quality in fast MR imaging, although their adaptation to parametric mapping is still in an early stage. In this paper, we proposed a novel deep learning-based framework DEMO for fast and robust MR parametric mapping. Different from current deep learning-based methods, DEMO trains the network in an unsupervised way, which is more practical given that it is difficult to acquire large fully sampled training data of parametric-weighted images. Specifically, a CS-based loss function is used in DEMO to avoid the necessity of using fully sampled k-space data as the label, thus making it an unsupervised learning approach. DEMO reconstructs parametric weighted images and generates a parametric map simultaneously by unrolling an interaction approach in conventional fast MR parametric mapping, which enables multi-tasking learning. Experimental results showed promising performance of the proposed DEMO framework in quantitative MR T1ρ mapping.

뇌전이암 진단을 위한 제한적 뇌 자기공명영상의 유용성에 관한 연구 (Detection of Brain Metastatses Using Limited Brain MR Imaging : Usefulness of Limited Contrast-Enhanced MR Imaging in Brain Metastasis)

  • 연구배경 : 본 논문에서는 뇌 전이 병소의 발견에 있어서 고식적 조영증강 자기공명영상 기법과 비교하여 제한적 조영증강 자기공명영상의 유용성을 알아보고자 하였다. 방 법 : 1998년 4월부터 2002년 9월까지 뇌 전이의 여부를 알아보기 위해 고식적 뇌 자기공명영상올 시행한 폐암 및 기타 암으로 진단을 받은 47명의 환자를 대상으로 하였다. 47명의 환자에서 축상면 T1 강조영상, 축상면 조영증강 T1강조영상, 관상면 조영증강 Tl 강조영상을 포함하는 제한적 뇌 자기공명영상을 선정하여 뇌 전이 결절의 영상판독을 시도하고 이를 고식적 뇌 자기공명영상의 영상소견과 비교하여 뇌 전이 발견의 민감도, 특이도와 일치율을 알아보았다. 결 과 : 47명의 환자 중 고식적 조영증강 자기공명영상에서 43명이 뇌 전이가 있었고, 제한적 자기공명영상에서는 42명에서 뇌 전이를 발견하였다.(민감도=97.67%). 고식적 뇌 자기공명영상에서 뇌 전이가 없었던 4명의 환자는 제한적 뇌 자기공명영상에서도 모두 뇌 전이가 없었다.(특이도=100%) 제한적 뇌 자기공명영상과 고식적 뇌 자기공명영상은 Pearson correlation이 0.884(Confidence Interval: 99%)로 높은 일치율올 보였다. 결 론 : 제한적 뇌 자기공명영상은 적은 비용으로 뇌 전이 여부를 판정할 수 있는 방법으로 제한적 자기공명 영상은 고식적 자기공명영상에 비해 손색없는 진단율을 보이므로 증상이 있는 환자에서만 시행되어 왔던 뇌영상 조영을 무증상 환자에서도 뇌 전이의 여부를 알아보기 위해 시행할 수 있을 것이다.

  • 지난 10년간 신경계의 MR영상은 컴퓨터를 비롯한 하드웨어적인 발달에 힘입어 functional imaging, perfusion imaging, diffusion imaging, MR spectroscopy, MR angiography 등의 분야에서 괄목한 만한 성장이 있었다. 지난 2001년 4월 국제자기공명의과학회 (ISMRM) 학술대회에 약 2300개의 논문이 발표되었으며 이 중 신경계와 관련된 논문의 수는 저자가 파악하기에 약 500개 정도이었다. 매년 수많은 논문이 발표되고 있으나 이 중 기초 또는 임상의학적으로 유용한 논문만이 후속 연구로 연결되어 궁극적으로 많은 사람들이 활용할 수 있게 되고 있다. 따라서 지난 수 년 동안의 연구 동향을 모두 follow-up하는 것은 무의미하며, 이 중 functional, perfusion, diffusion imaging을 중심으로 이들 영상 기법의 기술적 측면과 적용 분야를 알아보고자 한다.

유방에 발생한 일차성 비호지킨림프종의 자기공명 분광법 및 확산강조 영상: 2예 (MR Spectroscopy and Diffusion Weighted Imaging Findings of Primary Non-Hodgkin Lymphoma of the Breast: Two Case Reports)

  • 유방의 일차성 비호지킨림프종은 드문 종양으로 유방촬영술과 초음파에서 다양한 영상소견을 보인다. 하지만 자기공명영상소견은 여러개의 증례로만 보고되어 있고, 자기공명분광법과 확산강조영상에 대한 보고는 거의 없다. 저자들은 유방의 일차성 비호지킨림프종을 진단받은 두명의 여자환자의 증례를 기본 자기공명영상에 더하여 자기공명 분광법 및 확산강조 영상소견과 함께 보고하고자 한다. 종양은 유방촬영술과 초음파에서 경계가 좋은 종괴로 나타났으며, 조영증강자기공명영상에서 강한 조영증강을 보였다. 또한 다른 악성종양에서 나타나는 것과 같이 확산강조영상에서는 확산제한을 보이고, 자기공명분광법에서는 콜린값이 증가하는 소견을 보였다.
