원발성 시신경교차 주위 종양으로부터 전이한 비장의 생식세포종 - 1예 보고 - (Metastatic Germinoma of Spleen from Perichiasmal Area - A Case Report -)

  • A 20-year-old young man who had undergone treatment for a suprasellar/perichiasmal tumor 2 years before, was presented with a huge palpable splenic mass. A fine needle aspiration cytology from the splenic mass showed dissociated large pleomorphic tumor cells having irregular nuclear outline, coarse chromatin, and one or two macronucleoli, and scattered small lymphocytes in fine granular background. Above cytologic findings were regarded as the characteristics of germinoma. Differential diagnosis from the large cell lymphoma of spleen was emphasized.

  • This study was performed to demonstrate the presence of large granular lymphocyte(LGL) in Sprague-Dawley(SD) rats and morphologically observe NK cell and also establish the method of isolation of natural killer cell in SD rats. By percoll discontinuous density gradients centrifugation, highly enriched LGL population were shown to fraction 2(border line between 44.2% and 50.8%). LGL were shown to bind selectively to YAC1 mouse lymphoma cell. This fraction expressed very high NK cell cytolysis. Therefore, we thought that LGL have NK activity in SD rats. The Morphology of rat LGL is very similar to that of human LGL. These cells have an eccentric kidney-shaped nucleus. Their most distinctive feature was their cytoplasmic azurophilic granules. Another distinguishing feature of rat LGL was their high cytoplasmic : nuclear ratio. It was concluded that LGL played a role part in mediating natural killer activity in this species.

  • The tumour lysis syndrome (TLS) is a group of metabolic abnormalities caused by rapid and unexpected release of cellular components into the circulation as a result of massive destruction of rapidly proliferating malignant cells. It usually develops in patients with hematologic malignancies like acute lymphoid leukemia, non-Hodgkin and Burkitt's lymphoma after initiation of chemotherapy or may, rarely, occur spontaneously. Though TLS is seldom observed in relation to solid tumours, there have been reports of connections with examples such as lung, liver, breast, gastric carcinomas. The clinical manifestations of TLS include hyperuricemia, hyperkalemia, hyperphosphatemia and hypocalcemia. These indications if untreated lead to life-threatening complications such as acute renal failure, cardiac arrhythmias, seizures, and eventually death due to multiorgan failure. Therefore early detection of TLS is of vital importance. This can be accomplished by identification of high risk patients, implementation of suitable prophylactic measures andmonitoring of the electrolyte levels in patients undergoing chemotherapy.

  • The potential application of toxicogenomics to predictive toxicology has been discussed widely, but the utility of the approach remains largely unproven. Using cDNA microarrays, we have compared the gene expression profiles produced in mouse lymphoma cells by three genotoxic compounds, hydroxyurea (a carcino- gen), p-anisidine (a noncarcinogen) and paclitaxel (carcinogenicity unknown). 흉요추 신경근병증으로 발현된 수막암종증 1예 (A Case of Leptomeningeal Metastasis Presented with Thoracolumbar Radiculopathy)

  • We report a case of thoracolumbar radiculopathy presented with first symptom of leptomeningeal metastasis. A 65-year old man with non-Hodgkins lymphoma with complete remission was referred for further investigation of dull pain and numbness of right lower quadrant of abdomen. Electromyogram revealed right thoracolumbar radiculopathy. After two weeks, he complained severe continuous bilateral frontal dull headache. An examination of the cerebrospinal fluid revealed malignant lymphoid cells. We would like to emphasize that radiculopathy sometimes presents with first symptom of leptomeningeal metastasis.

  • Mikulicz's disease is an autoimmune disorder characterized by asymptomatic chronic swelling of the salivary gland or lacrimal gland. It is also called as a benign lymphoepithelial lesion. The clinical manifestations are usually bilateral and symmetric. The diagnosis is confirmed by only histopathologic finding. Microscopically, the lymphoid infiltration and epimyoepithelial island appear as the solid nests surrounded and infiltrated by lymphoid cells. The treatment is symptomatic, but the patients are at an increased risk for the development of malignant lymphoma. So it need a careful observation.

Inflammatory Myofibroblastic Tumor Treated with Laparoscopic Proximal Gastrectomy and Double-Tract Anastomosis

  • Inflammatory myofibroblastic tumors (IMTs) of the stomach are extremely rare in adults, and their oncologic prognosis is not well understood. We present a 28-year-old man with a proximal gastric IMT. The patient visited the emergency department of Yeouido St. Mary's Hospital with syncope and hematemesis. Hemoglobin levels were <5.5 g/dl. Gastric fibroscopy showed a protruding mass $4{\times}4cm$ in size, with central ulceration on the posterior wall of the fundus and diffuse wall thickening throughout the cardia and anterior wall of the upper body. Endoscopic biopsy revealed indeterminate spindle cells, along with inflammation. Given the risk of rebleeding, an operation was performed despite the uncertain diagnosis. Because the mass was circumferential, laparoscopic proximal gastrectomy and double-tract anastomosis were performed to ensure a safe resection margin. The pathological diagnosis was consistent with an IMT originating from the stomach, although the tumor was negative for anaplastic lymphoma kinase gene mutation.

  • Sophoricoside was isolated as the inhibitor of IL -5 bioactivity from Sophora japonica (Leguminosae). It has been reported to have an anti-inflammatory effect on rat paw edema model. To develope as an anti-allergic drug, genotoxicity of sophoricoside was investigated in bacterial and mammalian cell system such as Ames bacterial test and mouse lymphoma tk gene assay (MOLY).
