Identification of Homer1 as a Potential Prognostic Marker for Intrahepatic Cholangiocarcinoma

  • Background: The aim of the present study was to analyze whether Homer1 is a potential prognostic marker for intrahepatic cholangiocarcinoma (ICC). Materials and Methods: The expression of Homer1 in ICC tissue was detected with immunohistochemistry and levels of protein in ICC and paratumor tissues were evaluated by Western blotting. Survival analysis by the Kaplan-Meier method was performed to assess prognostic significance. Results: Homer1 expression was high in 67.4% (58/86) of ICC samples, and there was significant difference between ICC and adjacent noncancerous tissues (p<0.001); high expression was associated with poor histologic differentiation (p=0.019), TNM stage (p=0.014), lymph node metastasis (p=0.040), and lymphatic invasion (p=0.025). On Kaplan-Meier analysis, a comparison of survival curves of low versus high expressors of Homer1 revealed a highly significant difference in OS (p=0.001) and DFS (p=0.006), indicating that high expression of Homer1 was linked with a worse prognosis. Multivariate analyses showed that Homer1 expression was an independent risk factor predicting overall survival[Hazard ratio(HR), 7.52; 95% confidence interval (CI), 2.63-21.47; p=0.002] and disease-free survival (HR, 11.56; 95%CI, 5.17-25.96; p<0.001) in ICC. Conclusions: Homer1 promotes lymphatic invasion and associates with lymph node metastasis and poor prognosis of ICC. The current study shows that Homer1 may be an independent prognostic factor for ICC patients after curative resection, and it provides an important basis for screening/treating high-risk patients.

직장암의 수술 후 방사선 치료 시 국소 재발의 임상 병리적 예후 인자 (The Clinicopathological Factors That Determine a Local Recurrence of Rectal Cancers That Have Been Treated with Surgery and Chemoradiotherapy)

  • 목 적: 진행된 직장암의 수술 후 보조적 방사선 치료를 시행한 환자에서 국소 재발한 군의 병리학적 예후 인자를 규명함으로써 향후 치료 방침의 결정에 도움이 되고자 하였다. 대상 및 방법: 1993년 2월부터 2001년 12월까지 원자력의학원에서 수술을 시행한 후 병기 3기 이상으로 방사선치료 및 항암 요법을 시행 받은 직장암 환자 110명을 대상으로 검체를 모두 조사하여 면역조직화학검사가 가능한 총 54명을 대상으로 하였다. 이중 국소전이가 발견된 군이 14명, 발견되지 않은 군이 40명이었다. 이들의 조직 검체를 대상으로 종양의 침윤 깊이, 종양의 조직학적 등급, 임파절 침윤 여부, 혈관 침윤 여부, 신경 침윤 여부 등의 병리적인 특징 및 p53, Ki-67, c-erb, ezrin, c-met, phospho-S6K, S100A4, HIF-1 alpha의 다양한 암 유전자의 발현양상을 단변량 분석 및 다변량 분석, hierarchical clustering 분석 기법을 사용하여 치료 후 예후와 관련된 인자를 찾았다. 결 과: 병리학적 예후 인자 중 단변량 분석상 종양 침윤 깊이, 종양의 등급, 혈관 침범이 의미 있었고 다변량 분석상 침윤의 깊이가 5.5 mm 이하, 혈관 침윤이 없는 경우가 국소 재발이 낮은 군이었다. 면역조직화학검사 결과의 단변량 분석상 c-met 양성, HIF-1 alpha 양성이 국소 재발률이 높은 예후 인자였고 다변량 분석상 c-met이 의미 있는 예후 인자였다. Hierarchical clustering을 통해서 조사한 결과 HIF-1 alpha, c-met 및 종양 침윤 깊이가 국소재발과 관련된 인자로써 국소 재발을 한 군의 71.4%가 3가지 인자 중 2개 이상을 가지고 있는 반면 국소 재발을 하지 않은 군에서는 27.5%가 2개 이상을 가지고 있었다. 결 론: 국소적으로 진행되어 방사선 치료를 시행해야 하는 직장암 환자군 중에서 HIF-1 alpha 양성, c-met 양성, 종양 침윤 깊이 5.5 mm 이상의 병리학적 예후 인자를 두 개 이상 가지는 환자는 국소 재발의 가능성이 높다. 이러한 인자가 방사선치료 저항군의 지표로써 유용한지에 대한 전향적 연구가 향후 필요할 것이다.

ATP-CRA 방법을 이용한 위암조직의 항암제 감수성 검사결과 (The Results of the ATP Based Chemotherapy Response Assay in Gastric Cancer Tissues)

  • 목적: 진행성위암의 경우 근치적 수술 후 보조항암요법에도 불구하고 5년 생존율이 그리 높지 않은 것으로 보고되고 있다. 이에 항암제 감수성 검사를 기초로 한 항암요법은 최소한 환자들에게 효과가 없는 약제 투여를 막을 수 있는 기회를 제공할 수 있다는 측면에서는 고무적이라 할 수 있다. 본 연구에서는 보조항암요법의 효과를 높이기 위한 유효약제 선정을 위하여 항암제 감수성검사를 실시하였으며 그 결과를 분석하여 항암제 선택에 이용하고자 임상병리학적 요인에 따른 항암제 감수성을 비교하였다. 대상 및 방법: 2005년 8월부터 2006년 8월까지 영남대학교 병원 외과에서 위절제술을 받은 위암환자 81명의 위암절제조직을 이용하여 항암제 감수성 검사를 실시하였다. 검사방법은 ATP (Adenosine Triphosphate) based chemotherapy response assay였다. 항암제 감수성과 임상병리학적 요인과의 상관관계를 보기 위하여 성별, 연령, 종양표지자(CEA 치, CA19-9 치), 암의 위치, 진행암 형태, 조직학적 형태, 분화 유무, 침윤도, Lauren 분류, Ming 분류, 림프관 침윤 유무, 맥관 침윤 유무, 신경 침윤 유무, 림프절 전이 유무, 병리학적 병기를 선정 비교하였다. 결과: 위암 환자를 대상으로 한 항암제 감수성 순위를 보면 5-FU, Epirubicin, Irinotecan, Oxaliplatin 순이었으며 통계학적으로 유의했다(P=0.000). 성별, 연령, 종양표지자(CEA 치, CA19-9치), 암의 위치, 진행암 형태, 조직학적 형태, 분화 유무, 침윤도, Lauren 분류, Ming 분류, 림프관 침윤 유무, 맥관 침윤 유무, 신경 침윤 유무, 림프절 전이 유무, 병리학적 병기 모두에서 유의하게 감수성의 차이를 보였다. 결론: 위암절제조직에서 시행한 항암제 감수성 순위는 5-FU, Epirubicin, Irinotecan, Oxaliplatin의 순이었으며 위절제술을 받은 위암환자의 절제조직을 이용 항암제 감수성검사를 시행한 결과 약제별 임상병리학적 인자에 따라 감수성의 차이가 있으므로 획일적인 항암화학요법을 실시하는 것보다 항암제 감수성 검사를 통하여 감수성 높은 약제들을 조합하여 사용하는 것이 바람직할 것으로 보인다.

흉벽침습이 있던 T3 비소세포암환자의 완전절제후의 결과 (Result of Complete Resection of T3 Non-Small Cell Lung Cancer Invading the Chest Wall)

  • 배경 : 흉벽을 침습한 비소세포암의 외과적 절제후의 장기 생존율에는 현재까지 림프절 전이, 완전 절제, 흉벽침습 깊이 등이 관여한다고 알려져 있다. 본 연구에서는 완전절제후의 예후에 관련된 요소들에 대해 알아보고자 한다. 대상 및 방법 : 1988년부터 1998년까지 원발성 비소세포암으로 수술을 받은 680명중, 벽측늑막과 흉벽을 침범한 경우에 완전절제를 실시한 55예(8.0%)에 대해 후향적 방법을 통해 분석하였다. 결과 : 29예(47.3%)에서 총괄절제를 시행하고 26예(52.7%)에서 늑막외절제를 실시하였다. 늑막외절제를 시행한 모든 예에서 침습정도는 벽측늑막에 국한되어 있었고, 총괄절제군에서는 9예(31.0%)에서 벽측늑막에 나머지 20예(69.0%)에서는 흉벽에의 침습이 병리학적으로 판명되었다. 수술사망은 3명에서 발생해 5.4%였으며, 추적은 100%에서 가능하였다. 전체환자군의 5년 생존율은 26%였으며, T3N0M0군의 생존율이 29%로 T3N2M0군의 18% 보다 높기는 하나 통계학적인 유의성은 없었다.(p=0.35) 흉벽침습 깊이정도에 따른 생존율은 림프절 전이의 유무에 관련없이 유의한 차이가 없었다.(p=0.99) 결론 : 흉벽을 침습한 T3 비소세포암에 대해 완전절제를 실시해 만족할 만한 생존율을 보였다. 림프절 전이 여부는 완전절제후의 생존율에 관여하는 바가 크나, 침습 깊이는 관여하는 정도가 덜하다.

폐암의 조직학적 분류, 위치 및 크기와 주위 림프절 전이의 양상에 관한 연구 (Mode of regional and mediastinal lymph node metastasis of bronchogenic carcinoma in accordance with the location, size and histology of primary tumor of the lung)

  • A total of 178 patients with primary lung cancer who had undergone complete resection of the tumor in combination with complete mediastinal lymphadenectomy were reviewed at the Department of Thoracic and Cardiovascular Surgery of Yonsei Medical Center from January 1980 through July 1989. Materials; 1. There were 45 men and 33 women ranging of age from 25 to 78 years with a mean age of 55.4 years. 2. Histological types were squamous carcinoma in 115 cases [64.6%] adenocarcinoma in 42 cases [23.6 %], bronchioloalveolar carcinoma in 9 cases [5.1%], large cell carcinoma in 8 cases [4.5 %] and small cell carcinoma in 4 cases [2.2%] Results were summarized as follows: 1. The size of primary tumor was not directly proportional to the frequency of mediastinal lymph node metastasis. [P =0.0567] 2. The histologic types of the primary tumor did not related to the incidence of mediastinal lymph node metastasis. [P >0.19] 3. The chance of mediastinal lymph node metastasis in the case with lung cancer located in right middle lobe[31.8%, N=22] and left lower lobe [31.4%, N=32] were the highest and the lowest was the one located in right lower lobe, while over all incidence of mediastinal lymph node metastasis in this series was 25.4 % [N=55]. 4. The rate of mediastinal lymph node metastasis without evidence of regional and hilar lymph node metastasis was 13%. [N=23] The chance of mediastinal lymph node involvement without N1 lymph node metastasis was 16.3 % [N=17] in both upper lobes and 8.2 % [N=6] in both lower lobes. It was statistically significant that the tumors in the upper lobes had greater chance of the mediastinal lymph node metastasis without N1 than the tumors in the lower lobes. 5. In this series majority of the patients with lung cancer the mediastinal lymph node metastasis from the tumor in each pulmonary lobes usually occurs via ipsilateral tracheobronchial and paratracheal lymphatic pathway. Especially the lung cancer located in lower lobes can metastasize to subcarinal, paraesophageal and inferior pulmonary ligamental lymph node through the lymphatic pathway of inferior pulmonary ligament. It can be speculated that in some cases of this series otherwise mediastinal lymph node metastasis can also occur with direct invasion to the parietal pleura and to the mediastinal lymph node via direct subpleural lymphatic pathway .

위암 절제조직에서 nm23 단백질 발현의 임상적 의의 (The Clinical Eelevance of nm23 Protein Expression in Resected Gastric Cancer Patient)

  • 위암환자에서 nm23 단백질이 예후인자로서의 영향을 규명하기 위해 위절제술로 얻어진 76명의 원발암 조직에 대해서 nm23 단백질의 단세포군 항체를 이용한 면역조직화학적 염색을 실시한 결과 nm23 단백질의 양성발현율은 53.9% (23/76)였다. 병리조직학적 소견들과의 비교에서 nm23 단백질의 양성발현율은 조기암보다 진행암에서, 림프절 전이 양성군에서, 그리고 암세포의 림프관 침습이 있을때 높게 나타났으며 유의한 차이가 있었다. nm23 단백질의 발현유무가 재발과 관련이 있는지에 때한 분석 결과 재발율에서 차이가 없었다. nm23 단백질 발현 유무가 생존기간에 영향을 끼치는가에 대한 단변량 분석결과 생존과 관련이 없는 것으로 나타났다. 이상의 결과로 nm23 단백질의 양성발현 여부를 예후인자로 사용하기는 어려울 것으로 사료된다.

Expression of MiR200a, miR93, Metastasis-related Gene RECK and MMP2/MMP9 in Human Cervical Carcinoma - Relationship with Prognosis

  • Aim and Background: Cervical cancer remains the third most common cancer in women globally after breast and colorectal cancer. Well-characterized biomarkers are necessary for early diagnosis and to predict metastatic progression and effective therapy. MiRNAs can regulate gene expression, cell growth, differentiation and apoptosis by targeting mRNAs for translational repression or degradation in tumor cells. The present study was conducted to assess expression of miR93, miR200a, RECK, MMP2, MMP9 in invasive cervical carcinoma, and analyze their clinical significance. Method: A total of 116 patients with invasive cervical carcinoma and 100 patients undergoing hysterectomy for benign lesions were retrospectively examined. Quantitative real-time PCR was performed to determine expression of miR93 and miR200a while RECK, MMP2, MMP9 and MVD were assessed by immunohistochemical staining. Results: Cervical carcinoma patients demonstrated up-regulation of miR-93, miR-200a, MMP2 and MMP9, with down-regulation of RECK as compared to benign lesion tissues. RECK was significantly inversely related to invasion and lymphatic metastasis. The 5-year survival rate for patients with strong RECK expression was significantly higher than that with weakly expressing tumors. Conclusion: MiR-93 and miR-200a are associated with metastasis and invasion of cervical carcinoma. Thus together with RECK they are potential prognostic markers for cervical carcinoma. RECK cooperating with MMP2, MMP9 expression is a significant prognostic factor correlated with long-term survival for patients with invasive cervical carcinoma.

Clinicopathological Characteristics of Alpha-Fetoprotein-Producing Gastric Cancer

  • Purpose: ${\alpha}$-fetoprotein (AFP)-producing gastric cancer is a rare tumor with high rates of liver metastasis and a poor prognosis. Many studies have been performed but there have been no comprehensive investigations of the clinicopathological and prognosis. Materials and Methods: Six hundred ninety four patients with gastric cancer who underwent a curative gastric resection in Hanyang University Hospital from February 2001 to December 2008 were evaluated retrospectively after excluding active or chronic hepatits, liver cirrhosis and preoperative distant metastasis. Among them, thirty five patients had an elevated serum level of AFP (>7 ng/ml) preoperatively. The clinicopathological features of AFP-producing gastric cancer were analyzed. Results: There was poorer differentiation, a higher incidence of lymph node metastasis, more marked lymphatic and vascular invasion in the AFP-positive group than in the AFP-negative group. The 5-year survival rate of the AFP-positive group was significantly poorer than that in the AFP-negative group (66% vs. 80%, P=0.002). A significantly higher incidence of liver metastasis was observed in the AFP-positive group than in the AFP-negative group (14.3% vs. 3.6%, P=0.002) with a shorter median time period from the operation to the metachronous liver metastasis (3.7 months vs. 14.1 months, P=0.043). Multivariate survival analysis revealed the depth of invasion, degree of lymph node metastasis and AFP-positivity to be the independent prognostic factors. Conclusions: AFP-producing gastric cancers have an aggressive behavior with a high metastatic potential to the liver. In addition, their clinicopathological features are quite different from the more common AFP-negative gastric cancer.

Cyclooxygenase-2 Expression in Urinary Bladder Transitional Cell Carcinoma and its Association with Clinicopathological Characteristics

  • Background: Transitional cell carcinoma (TCC) is the most predominant type of urinary bladder tumor. As cyclooxygenase (COX)-2 is recently introduced as an attractive target molecule in bladder TCC, we evaluated the immunohistochemical expression of this marker and its association with several clinicopathological characteristics. Materials and Methods: This cross-sectional study was performed in the Pathology department of Sina Hospital in Tehran, Iran during 2006-2011. Ninety-two paraffin embedded blocks were selected from patients with urinary bladder TCC who underwent cystectomy or transurethral resection (TUR). Then, we assessed COX-2 expression by immunohistochemical staining using antibody against COX-2. Staining in more than 5% of tumor cells was considered as positive expression. Results: COX-2 was expressed in 50 % of our patients. This marker was markedly expressed in high grade bladder TCC (62.1%) versus other grades and there was statistically a significant difference in COX-2 expression between various grades (p=0.008). In addition, patients' age, lymphatic and perineurial invasion were associated with the expression of COX-2 (p=0.001, 0.015 and 0.039, respectively). However, other parameters such as stage, tumor size, venous invasion and lymph node metastasis did not show any significant relationship with this marker (all, p>0.05). Conclusions: COX-2 was expressed in urinary bladder TCC especially in high grade forms, advocating its probable role in the differentiation of this tumor. Accordingly, COX-2 could be a valuable biological target molecule in the evaluation and treatment of patients with bladder TCC.

Scabraside D Derived from Sea Cucumber Induces Apoptosis and Inhibits Metastasis via iNOS and STAT-3 Expression in Human Cholangiocarcinoma Xenografts

  • Scabraside D, a sulfated triterpene glycoside, was extracted from the sea cucumber Holothuria scabra. It shows anti-proliferation in many of cancer cell lines, but the function and mechanisms of action of scabraside D in human cholangiocarcinoma (HuCCA) have not previously determined. In this study, we investigated the activity of scabraside D on HuCCA cell apoptosis, lymphangiogenesis and metastasis in a nude mouse model. Scabraside D induced signs of apoptosis, such as cell shrinkage, nuclear condensation, nuclear fragmentation and DNA fragmentation on TUNEL assays, while effectively decreasing expression of BCl-2 but increasing caspase-3 gene level expression. Immunohistochemistry revealed that scabraside D significantly reduced lymphatic vessel density (LVD). Moreover, scabraside D treatment significantly decreased VEGF-C, MMP-9 and uPA gene expression, which play important roles in the lymphangiogenesis and invasion of cancer cells in metastasis processes. Quantitative real-time PCR showed that scabraside D significantly decreased iNOS and STAT-3 gene expression. This study demonstrated that scabraside D plays a role in activation of HuCCA tumor apoptosis and inhibition of lymphangiogenesis, invasion and metastasis through decreasing BCl-2, MMP-9, uPA and VEGF-C and increasing caspase-3 expression by suppression of iNOS and STAT-3 expression. Therefore, scabraside D could be a promising candidate for cholangiocarcinoma treatment.
