A Case of Diffuse Alveolar Damage Induced by Cyclophosphamide (Cyclophosphamide에 의해 유발된 미만성 폐포 손상 1예)

  • Approximately 100 drugs have been reported to affect the lungs adversely. Among these, pulmonary toxicity caused by antieneoplastic agent. is being recognized more frequently. Cyclophosphamide is an immunosuppressive alkylating agent used for the treatment of a wide variety of malignant and nonmalignant diseases. The incidence of pulmonary toxicity is probably less than 1 percent The first case was reported in 1967. Since then, more than 20 well-documented cases of pulmonary toxicity associated with cyclophosphamide have been reported in the literature. In Korea, three patients were identified with cyclophosphamide-induced lung disease. The typical features of toxicity include dyspnea, fever, cough, new parenchymal infiltrates, gas exchangs abnormalities on pulmonary function tests, and pleural thickening on chest roentgenogram. The best approach to management is early diagnosis, discontinuation of the offending drug and administration of corticosteroid therapy. Recently, we experienced a case of diffuse alveolar damage induced by cyclophosphamide. The patient presented with early-onset pulmonary toxicity and died of repiratory failure despite early use of corticosteroid.

Relationship of Front Points' Location and Function to Response Zone (모혈(募穴)의 위치(位置) 및 작용(作用)과 반응대(反應帶)의 상관성(相關性) 고찰(考察))

  • Front Points are told to be deeply related with viscera and bowels in oriental medicine. Since the Front Points are treated as response zone, it can be used for the diagnosis and treatment of disease in viscera and bowels. The location of Front Points are very similar to the several response zones in western medicine. Diagnostic aspect of Front Points to several response zones were discussed in here based on many thesis and reported laboratorial experiments. Front Points are located in the same latitude of viscera and bowels. So, diagnosis result in Front Points represent condition of each organs. Palpitation of the Front Points can make diagnosis of body surface and the organ beneath the body surface. Such use of Front Points for diagnosis can used as the treatment points also. Only three Front Points (LU-1, LI-14, GB-24) in lung, liver and gall bladder meridian are located on its meridian. The Front Points of Stomach (CV-12) cross its meridian or closely located to it. Unlike to those four Front Points that are matched to its meridian, other eight Front Points do not located or cross its meridian at all. It seems that the location of Front Points are decided by the location of organs and the conditions at a certain organ do not delivered by the meridians but delivered by main collaterals, tertiary collaterals and superficial collaterals instead. Among visceral response zones, Five Front Points (CV-3, CV-4, CV-12, LI-14, GB-25) are exactly matched to Head's response zone and other Front Points are closely matched to the Head's response zone. There are five Front Points (CV-12, CV-14, CV-17, LI-14, GB-24) that are matched with the location of pressing palpitation point and other Front Points are closely located to the pressing palpitation point. So far, it was clear that the Front Points do have important role as response points. Symptoms expressed to the Front Points were delivered conditions or symptoms occur in corresponding organ and the anatomical location of Front Points were also found near the corresponding organ. Diagnostic and therapeutic application of Front Points for Organ theory and in the Interpromotion-restraint of the five elements in oriental medicine can be made in future to increase its potential.

Effects of five element constitutional acupuncture(Sa-am acupuncture) using needle manipulation to sedate Six Fu-Organ on serum lipid and liver function of hyperlipidemic rats induced by high fat diet (사암침법(舍岩鍼法) 중 육양경(六陽經)의 승격(勝格) 혈위(穴位)에 대한 염전보사(捻轉補瀉) 침자(鍼刺)가 고지방식이(高脂肪食餌)로 유발된 고지혈증(高脂血症) 백서(白鼠)에 미치는 영향(影響))

  • Objective : The aim of this research was to investigate the effect on five element constitutional acupuncture(Sa-am acupuncture) at 4 acupoints by needle manipulation to sedate Six Fu-Organ on hyperlipidemia induced by high fat diet in rats. Method : We recorded data of weight gain, food intake, food efficiency, weight of liver, heart, spleen, lung, kidney, levels of total cholesterol, triglyceride, HDL-cholesterol, AST, ALT in serum of rats fed high fat diet for 5 weeks. Spraque Dawley rats were divided into eight groups(normal; normal diet and non-acupuncture group, control; non-acupuncture group, Acu-LI; acupuncture at $SI_5{\cdot}LI_5{\cdot}BL_{66}{\cdot}LI_2$ group, Acu-ST; acupuncture at $GB_{41}{\cdot}ST_{43}{\cdot}LI_1{\cdot}ST_{45}$, Acu-SI; acupuncture at $BL_{66}{\cdot}SI_2{\cdot}ST_{36}{\cdot}SI_8$, Acu-BL; acupuncture therapy at $ST_{36}{\cdot}BL_{40}{\cdot}GB_{41}{\cdot}BL_{65}$, Acu-TE; acupuncture at $BL_{66}{\cdot}TE_2{\cdot}ST_{36}{\cdot}TE_{10}$, Acu-GB; acupuncture at $LI_1{\cdot}GB_{44}{\cdot}SI_5{\cdot}GB_{38}$. After needles inserted, it was rotated clockwise and anticlockwise 12 times on rats fed high fat diet. Results : The body weight was decreased in Acu-SI, Acu-TE, Acu-GB group, /the food intake was decreased in Acu-TE, / the food efficiency was decreased in Acu-SI, /serum triglyceride was decreased in Acu-LI, Acu-ST, Acu-BL, Acu-GB, /serum total cholesterol was decreased in Acu-LI, Acu-BL, serum ALP was decreased in Acu-LI, Acu-ST, Acu-BL, Acu-TE, Acu-GB. While the relative weight of heart was increased in Acu-LI, Acu-SI, the relative weight of kidney was increased in Acu-SI, Acu-GB. Conclusion : These results suggest that five element constitutional acupuncture(Sa-am acupuncture) may be having an therapeutic influence on hyperlipidemia induced by high fat diet. Further researches of acupuncture manipulation are needed in the future based on our study.

Surgical Management of Multidrug Resistant Pulmonary Tuberculosis (다제내성 폐결핵 환자에서의 수술적 치료)

  • Background: Medical treatment of multiple drug resistant(MDR) pulmonary tuberculosis has been quite unsuccessful. We analyzed our experience to identify the benefits and complications of the pulmonary resection in MDR pulmonary tuberculosis. Material and Method: A retrospective review was performed in 27 patients who unerwent pulmonary resection for MDR pulmonary tuberculosis between January 1994 and March 1998. Mean age was 40 years and the average history of diagnosis prior to surgery was 3.1 years. All had resistance to an average of 4.4 drugs, and received second line drugs selected according to the drug sensitivity test. Most patients (93%) had cavitary lesions as the main focus. Bilateral lesions were identified in 19 patients (70%), however, the main focus was recognized in one side of the lung. Eleven patients (41%) were converted to negative sputum smear and/or culture before surgery. Result: Pneumonectomy was performed in 9 patients, lobectomy in 16 and segmentectomy in 2. There was no operative mortality. Morbidity had occurred in 7 patients (26%), prolonged air leak in 3 patients, reoperation due to bleeding in 2, bronchopleural fistula in 1, and reversible neurologic defect in 1. Median follow up period was 15 months (3-45 months). Sputum negative conversion was initially achieved in 22 patients (82%), and with continuous postopertive chemotherapy negative conversion was achieved in other 4 patients (14%). Only one pneumonectized patient (4%) failed due to considerable contralateral cavity. Conclusion: For patients with localized MDR pulmonary tuberculosis and with adequate pulmonary reserve function, surgical pulmonary resection combined with appropriate pre and postoperative anti-tuberculosis chemotherapy can achieve high success rate with acceptable morbidity.

  • Tracheobronchial rupture due to blunt chest trauma is an uncommon injury although the incidence is increasing. Early diagnosis and primary repair of tracheobronchial rupture not only restore a normal lung function but also avoid the difficulties and complications associated with delayed diagnosis and repair We present one case of the tracheal transsection caused by traffic accident. Patient suffered from progressive dyspnea, subcutaneous emphysema on the neck and anterior chest wall and tension pneumothorax at both sides were noted. Although both closed thoracostomy were done, massive air leakage through the chest tube continued and subcutaneous emphysema spread to the anterior abdominal wall and scrotum and the degree of dyspnea aggravated. With the impression of tracheobronchial injury, we performed the emergency operation. Preoperative bronchoscopy at the operation room was proceeded, which revealed the trachea was near totally transsected in transverse direction. Operation was performed through collar incisiion on the anterior neck, and the trachea was anastomosed with 4-0 $Vicryl^{(R)}$ interruptedly. Postoperative course were uneventful and patient discharged without any complications.

  • Pseudohypoaldosteronism (PHA) is a condition characterized by renal salt wasting, hyperkalemia, and metabolic acidosis due to renal tubular resistance to aldosterone. Systemic PHA1 is a more severe condition caused by defective transepithelial sodium transport due to mutations in the genes encoding the ${\alpha}$ (SCNN1A), ${\beta}$ (SCNN1B), or ${\gamma}$ (SCNN1G) subunits of the epithelial sodium channel at the collecting duct, and involves the sweat glands, salivary glands, colon, and lung. Although systemic PHA1 is a rare disease, we believe that genetic studies should be performed in patients with normal renal function but with high plasma renin and aldosterone levels, without a history of potassium-sparing diuretic use or obstructive uropathy. In the present report, we describe a case of autosomal recessive PHA1 that was genetically diagnosed in a newborn after severe hyperkalemia was noted.

Influencing Factors of Peripheral Blood Lymphocyte subsets in Workers with Chronic Obstructive Pulmonary Disease Exposed to Inorganic Dust (만성폐쇄성폐질환을 동반한 무기분진 노출 이직근로자의 혈중 림프구 아형분포에 미치는 영향요인)

  • Objectives: Inorganic dust is known to be a risk factor for chronic obstructive pulmonary disease (COPD) regardless of smoking and pneumoconiosis. Adaptive and innate immunity, including lymphocyte infiltrate, are involved in the pathogenesis of COPD. The purpose of this study was to analyze the lymphocyte subsets in the blood of workers exposed to inorganic dust and confirm the influencing factors. Methods: The general characteristics of the subjects (n=107) were analyzed through a personal questionnaire. Diagnosis of COPD was established according to pulmonary function tests with FEV1/FVC post bronchodilator lower than 70%, according to the Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines. For lymphocyte analysis, blood was stained with a fluorescent CD marker and analyzed by flow cytometry. Results: The increase in CD4+ T lymphocytes was associated with a decrease in age (𝛽=-0.273, p=0.008) and an increase in the cumulative smoking amount (𝛽=0.205, p=0.034). The increase in NK cells was associated with an increase in age (𝛽=0.325, p=0.001) and a decrease in cumulative smoking (𝛽=-0.220, p=0.019). The period of exposure to dust, %FVC predicted and %FEV1/FVC, and the relative population of peripheral blood lymphocytes did not show a statistically significant relationship. Conclusions: CD4+ T lymphocytes and CD56+CD16+ NK cells in peripheral blood were more related to age and cumulative smoking than the duration of dust exposure. Age and smoking are major risk factors for the development of COPD, so it can be predicted that peripheral blood CD4+ T lymphocytes and CD56+CD16+ NK cells are related to the development of COPD in workers exposed to inorganic dust.

  • Background: Interferon lambda receptor 1 (IFNLR1) is a type II cytokine receptor that clings to interleukins IL-28A, IL29B, and IL-29 referred to as type III IFNs (IFN-λs). IFN-λs act through the JAK-STAT signaling pathway to exert antiviral effects related to preventing and curing an infection. Although the immune function of IFN-λs in virus invasion has been described, the molecular mechanism of IFNLR1 in that process is unclear. Objectives: The purpose of this study was to elucidate the role of IFNLR1 in the pathogenesis and treatment of porcine reproductive and respiratory syndrome virus (PRRSV). Methods: The effects of IFNLR1 on the proliferation of porcine alveolar macrophages (PAMs) during PRRSV infection were investigated using interference and overexpression methods. Results: In this study, the expressions of the IFNLR1 gene in the liver, large intestine, small intestine, kidney, and lung tissues of Dapulian pigs were significantly higher than those in Landrace pigs. It was determined that porcine IFNLR1 overexpression suppresses PRRSV replication. The qRT-PCR results revealed that overexpression of IFNLR1 upregulated antiviral and IFN-stimulated genes. IFNLR1 overexpression inhibits the proliferation of PAMs and upregulation of p-STAT1. By contrast, knockdown of IFNLR1 expression promotes PAMs proliferation. The G0/G1 phase proportion in IFNLR1-overexpressing cells increased, and the opposite change was observed in IFNLR1-underexpressing cells. After inhibition of the JAK/STAT signaling pathway, the G2/M phase proportion in the IFNLR1-overexpressing cells showed a significant increasing trend. In conclusion, overexpression of IFNLR1 induces activation of the JAK/STAT pathway, thereby inhibiting the proliferation of PAMs infected with PRRSV. Conclusion: Expression of the IFNLR1 gene has an important regulatory role in PRRSV-infected PAMs, indicating it has potential as a molecular target in developing a new strategy for the treatment of PRRSV.

Extended Use of Extracorporeal Membrane Oxygenation for Acute Respiratory Distress Syndrome: A Retrospective Multicenter Study

  • Background: Beyond its current function as a rescue therapy in acute respiratory distress syndrome (ARDS), extracorporeal membrane oxygenation (ECMO) may be applied in ARDS patients with less severe hypoxemia to facilitate lung protective ventilation. The purpose of this study was to evaluate the efficacy of extended ECMO use in ARDS patients. Methods: This study reviewed 223 adult patients who had been admitted to the intensive care units of 11 hospitals in Korea and subsequently treated using ECMO. Among them, the 62 who required ECMO for ARDS were analyzed. The patients were divided into two groups according to pre-ECMO arterial blood gas: an extended group (n=14) and a conventional group (n=48). Results: Baseline characteristics were not different between the groups. The median arterial carbon dioxide tension/fraction of inspired oxygen ($FiO_2$) ratio was higher (97 vs. 61, p<0.001) while the median $FiO_2$ was lower (0.8 vs. 1.0, p<0.001) in the extended compared to the conventional group. The 60-day mortality was 21% in the extended group and 54% in the conventional group (p=0.03). Multivariate analysis indicated that the extended use of ECMO was independently associated with reduced 60-day mortality (odds ratio, 0.10; 95% confidence interval, 0.02-0.64; p=0.02). Lower median peak inspiratory pressure and median dynamic driving pressure were observed in the extended group 24 hours after ECMO support. Conclusion: Extended indications of ECMO implementation coupled with protective ventilator settings may improve the clinical outcome of patients with ARDS.

Immobilization stress increased cytochrome P450 1A2 (CYP1A2) expression in the ovary of rat

  • Under the stressed condition, a complex feedback mechanism for stress is activated to maintain homeostasis of the body and secretes several stress hormones. But these stress hormones impair synthesis and secretion of the reproductive hormones, followed by suppression of ovarian function. Cytochrome P450 1A2 (CYP1A2) plays a major role in metabolizing exogenous substances and endogenous hormones, and its expression is recently identified at not only the liver but also several organs with respect to the pancreas, lung and ovary. Although the expression of CYP1A2 can be also affected by several factors, understanding for the changed pattern of the ovarian CYP1A2 expression upon stress induction is still limited. Therefore, CYP1A2 expression in the ovaries from immobilization stress-induced rats were assessed in the present study. The stress-induced rats in the present study exhibited the physiological changes in terms of increased stress hormone level and decreased body weight gains. Under immunohistological observation, the ovarian CYP1A2 expression in both control and the stressed ovary was localized in the antral to pre-ovulatory follicles. However, its expression level was significantly (p < 0.01) higher in the stress-induced group than control group. In addition, stress-induced group presented more abundant CYP1A2-positive follicles (%) than control group. Since expression of the ovarian CYP1A2 was highly related with follicle atresia, increased expression of CYP1A2 in the stressed ovary might be associated with changes of the ovarian follicular dynamics due to stress induction. We hope that these findings have important implications in the fields of the reproductive biology.
