발화속도 및 강도 분석에 기반한 폐질환의 음성적 특징 추출 (Voice Features Extraction of Lung Diseases Based on the Analysis of Speech Rates and Intensity)

  • 현대인의 6대 난치병으로 분류되고 있는 폐질환은 대부분 흡연과 대기 오염으로 발병한다. 이와 같은 이유로 폐기능이 손상되어 폐포내에서 이산화탄소와 산소의 교환이 정상적으로 이루어지지 않아 생명 연장의 위험 질환으로 관심이 증대되고 있다. 이를 위해 본 논문에서는 폐질환 에 대한 음성적 특징 추출을 목적으로 음성 분석 요소를 적용한 폐질환 진단 방법을 제안하였다. 우선 폐질환을 앓고 있는 환자들과 동일한 연령, 성별대의 정상인들로 피실험자 집단을 구성하고 이들의 음성을 수집하였다. 또한 수집된 음성을 통해 다양한 음성 분석 요소를 적용하여 분석을 수행하였으며 발화속도 및 강도 분석 요소 부분에서 폐질환자 집단과 정상인 집단간의 유의성이 있음을 알아 낼 수 있었다. 결론적으로 폐질환자 집단이 정상인 집단보다 발화속도가 느리며 강도가 크게 나타나는 결과를 도출해 내었으며 이를 통해 폐질환의 음성적 특징 추출 방법을 제시하였다.

Radical Oncological Surgery and Adjuvan Therapy in Non-Small Cell Lung Cancer Patients over 70 years of Age

  • Background: The incidence of lung cancer increases with age. Approximately 50% of non-small cell lung cancer (NSCLC) patients are over 70 years old. Because of the increasing elderly population, treatment approaches in this age group continue to be studied similar to groups of young people. Materials and Methods: In the current study, 26 patients who underwent radical surgery and adjuvan chemoradiation at Ataturk Chest Diseases and Chest Surgery Training and Research Hospital were evaluated retrospectively. Results: Of 21 patients (81%) were male and the average age was 74.4. Lobectomy was performed in 18 cases, pneumonectomy in 3, sleeve lobectomy in 3 and bilobectomy in 2. There was no perioperative or early period mortality. Overall survival was 24.5 months. Conclusions: From our study, lung cancer surgery and adjuvant therapy can be performed safely with low morbidity in the elderly.

The Serum Concentrations of YKL-40, IL-6, and TNF-${\alpha}$ in Retired Workers Exposed to Inorganic Dusts

  • Occupational long-term exposure to inorganic dusts may cause a variety of lung diseases such as pneumoconiosis and chronic obstructive pulmonary disease (COPD). Diagnosis of pneumoconiosis and COPD, however, is currently dependent on radiological findings and pulmonary test, which are both late diagnostic tools. Therefore, there is a need to identify novel biomarkers in pneumoconiosis and COPD. Hence, in this current study we investigated the serum concentrations of YKL-40, interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-${\alpha}$) as biomarkers for pneumoconiosis and COPD in 161 retired male workers exposed to inorganic dusts. The serum concentration of YKL-40 was significantly increased with age, pneumoconiosis, and airflow limitation. The serum concentration of IL-6 was significantly higher in airflow limitation. These results suggest that serum concentration of YKL-40 is associated with age, pneumoconiosis, and airflow limitation. Also, serum concentration of IL-6 is associated with airflow limitation.

  • The introduction of high-resolution CT (HRCT) in recent years has improved the ability of radiologists to detect and characterize the diffuse infiltrative lung disease (DILD). The detection and diagnosis of diffuse lung disease using HRCT are based on the recognition of specific abnormal findings. In this article, pattern recognition of HRCT findings is reviewed in the differential diagnosis of diffuse infiltrative lung disease. In general, HRCT findings of lung disease can be classified into four categories based on their appearances. These categories consist of (1) nodules and nodular opacities, (2) linear and reticular opacities, (3) increased lung opacity, and (4) decreased lung opacity, including cystic lesions.

Clinicopathological Characteristics of Iranian Patients with Lung Cancer: a Single Institute Experience

  • Background: Lung cancer has long been a leading cause of cancer related death in both women and men worldwide. The focus of this study was to review clinicopathological features of Iranian patients diagnosed with lung cancer. Materials and Methods: Clinicopathological data of 1353 primary lung cancer patients diagnosed during 17 years (1997-2014) in the "National Institute of Tuberculosis and Lung Disease" (NRITLD), Tehran, Iran, were retrospectively reviewed. Results: The median age of patients was 60 (mean: 58.95 years, range: 16-99) and adenocarcinoma was the most prevalent pathology (45.2%). Male/female ratio was 3.22 and 57.2% of patients were smokers (men 70.3%, women 15%). The majority (85.3%) were referred in advanced stages (stage IIIB and IV). Conclusions: Although some of our findings are in concordance with other studies in lung cancer but there are some discrepancies particularly in terms of smoking status and median age of Iranian patients. Further clinical and epidemiological studies are warranted to elucidate etiologic and factors other than smoking contributing to development of lung cancer such as environmental exposures or genetic predisposition.

  • 폐질환은 대부분 흡연과 대기 오염으로 발병하며 한 번 손상된 폐는 기능 회복이 어려워 생명 연장의 위험 질환이며 현대인의 6대 질병 중 하나로 사회적 관심이 증대되고 있다. 따라서 평소 흡연을 하지 말며 폐 기능 강화를 위한 행위를 행해야 호흡기 질환으로 고통 받는 경우의 수를 줄일 수 있을 것으로 여겨진다. 본 논문에서는 귀의 혈 자리가 인체 장기와 연계되어 있다는 이혈요법을 적용하여 폐와 상응하는 귀의 혈 자리를 자극했을 때 실제 폐 기능 강화에 도움이 되는지를 음성 분석 기술에 의해 규명하는 방법을 제안하고자 한다. 이를 위해 우선 인체가 폐에 미치는 영향을 분석하기 위한 선행 연구로 흡연 전/후의 음성 변화를 고찰하고자 한다. 이를 기반으로 폐와 상응하는 이혈점을 자극하기 전과 후의 음성 특징 차이를 분석하여 폐의 이혈점 자극이 실제 폐 기능 향상에 도움이 되는지를 수치적, 정량적으로 규명해 내고자 한다.

Clinical Aspect of MicroRNA in Lung Cancer

  • MicroRNAs (miRNAs) are a class of small noncoding RNAs that modulate target gene activity, and are aberrantly expressed in most types of cancer as well in lung cancer. A miRNA can potentially target a diverse set of mRNAs; further, it plays a critical role in lung tumorigenesis as well as affects patient outcome. Previous studies focused mainly on abnormal miRNAs expressions in lung cancer tissues. Interestingly, circulating miRNAs were identified in human plasma and serum in 2008. Since then, considerable effort has been directed to the study of circulating miRNAs as one of the biomarkers of lung cancer. miRNAs expression of tissues and blood in lung cancer patients is being analyzed by more researchers. Recently, to overcome the high false-positivity of low-dose chest computed tomography scan, miRNAs in lung cancer screening are being investigated. This article summarizes the recent researches regarding clinical applications of miRNAs in the diagnosis and management of lung cancer.

A Case of Capecitabine-Induced Sarcoidosis

  • Sarcoidosis is an inflammatory disease involving multiple-organs with an unknown cause. The new onset of sarcoidosis associated with therapeutic agents has been observed in 3 clinical settings; tumor necrosis factor antagonists in autoimmune rheumatologic diseases, interferon alpha with or without ribavirin in patients with chronic hepatitis C or melanoma, and antineoplastic agent-associated sarcoidosis in patients with hematologic malignancies. Here, we report a female patient who developed sarcoidosis after capecitabine treatment as an adjuvant chemotherapy for sigmoid colon cancer. To our knowledge, this is the first report of a capecitabine-induced sarcoidosis.

  • Accurate lymph node staging of lung cancer is crucial in determining optimal treatment plans and predicting patient outcome. Currently used lymph node maps have been reconciled to the internationally accepted International Association for the Study of Lung Cancer (IASLC) map published in the seventh edition of TNM classification system of malignant tumours. This article provides computed tomographic illustrations of the IASLC nodal map, to facilitate its application in day-to-day clinical practice in order to increase the appropriate classification in lung cancer staging.

Recent Advances in Molecular Basis of Lung Aging and Its Associated Diseases

  • Aging is often viewed as a progressive decline in fitness due to cumulative deleterious alterations of biological functions in the living system. Recently, our understanding of the molecular mechanisms underlying aging biology has significantly advanced. Interestingly, many of the pivotal molecular features of aging biology are also found to contribute to the pathogenesis of chronic lung disorders such as chronic obstructive pulmonary disease and idiopathic pulmonary fibrosis, for which advanced age is the most crucial risk factor. Thus, an enhanced understanding of how molecular features of aging biology are intertwined with the pathobiology of these aging-related lung disorders has paramount significance and may provide an opportunity for the development of novel therapeutics for these major unmet medical needs. To serve the purpose of integrating molecular understanding of aging biology with pulmonary medicine, in this review, recent findings obtained from the studies of aging-associated lung disorders are summarized and interpreted through the perspective of molecular biology of aging.
