서울지역 대학생들의 요천추 만곡과 목-허리둘레의 관계에 대한 연구 (A Study on Relationship between Lumbosacral Curvature and Neck-Waist Circumference on College Students in Seoul)

  • Objectives To investigate correlation between anthropometric data (neck circumference (NC), waist circumference (WC), body mass index (BMI), and body shape indexes) and radiological parameters of lumbosacrum. Methods The data of college students living in Seoul (n=24) were analyzed retrospectively. Anthropometric data of NC, WC, and BMI were measured. Lumbar spine X-ray film was taken to measure lumbar lordotic angle, Ferguson's angle. To evaluate body shape of participants, three indexes of neck-to-waist ratio (NWR), neck-to-height ratio (NHR), and waist-to-height ratio (WHR) were used. Anthropometric data's correlations with radiological parameters of lumbosacrum were investigated. Results Anthropometric data of NC, WC, and BMI had no significant correlation with radiological parameters of lumbosacrum. NWR had significant positive correlation with lumbar lordotic angle and Ferguson's angle. NHR and WHR had no significant correlation with radiological parameters of lumbosacrum. Conclusions The results suggest that NWR-related fat distribution in neck has significant correlation with radiological parameters of lumbosacrum regardless of obesity.

요추간판탈출증 환자의 요천각, 요추중력중심선 및 요추전만각의 통계적 관찰 (Statistical Study of the Ferguson's Angle, Lumbar Gravity Line and Lumbar Lordotic Angle in HIVD Patients.)

  • Objectives : The lumbosacral joint is unstable area from an anatomical viewpoint, while it is also a very mobile area in ordinary life, so that clinically major causes of low back pain originate in this joint. The purpose of this study is to assess the difference of the Ferguson's angles, Lumbar gravity lines, Lumbar lordotic angles among Herniated of Intervertebral Disc(HIVD) patients. Methods : We analyzed the lateral view of lumbar spine checked at erect position on 88 patients who had been diagnosed as HIVD by Magnetic Resonance Imaging(MRI). We investigated the Ferguson's angle, Lumbar gravity line, Lumbar lordotic angle on X-ray film. Results and Conclusions : In the acute lumbago group the Ferguson's angle had a tendency to decrease, while in the chronic group it had a tendency to increase. In the acute lumbago group the Lumbar gravity line fell in front of the normal range(sacrum), while in the chronic group it fell behind the normal range(sacrum). In the acute lumbago group the Lumbar lordotic angle usually decreased, while in the chronic group it increased. The Ferguson's angle and the Lumbar gravity line, the Ferguson's angle and the Lumbar lordotic angle, the Lumbar gravity line and Lumbar lordotic angle each had a positive realtionship. The Ferguson's angle, the Lumbar gravity line and the Lumbar lordotic angle was less influenced by the level of HIVD and was more influenced by how long the patient had the pain. The correlationship between each factor was less in the chronic lumbago group than the acute group. In the chronic lumbago group the instability of the lumbosacral joint increased, while in the acute group the compression of the weight on the sacrum increased.

  • The purpose of this study were to measured peak torque of lumbar extensor at various degrees and to compare with the choronic low back pain patients and healthy subjects back extensor peak torque. Research subject are fifty choronic low back pain patients and fifty healthy subjects are Dong-a university hospital visited to 2000, January since 1999, august none lumbosacral traumal past history and neurologic disorder that is twenty generation, thirty generation, forty generation, fifty generation, sixty generation in healthy subjects and twenty generation, thirty generation, forty generation, fifty generation, sixty generation in cause choronic low back pain patients. The result were as follows. 1. There were each generation choronic low back pain patients and healthy subjects back extensor peak torque are consideration (p<.05). 2. Twenty generation was all degree of angle in lumbar extention peak torque in choronic low back pain patients back extensor are lower than healthy subjects(p<.05). 3. Thirty generation was all degree of angle in lumbar extention peak torque in choronic low back pain patients back extensor are lower than healthy subjects(p<.05). 4. Forty generation was all degree of angle in lumbar extention peak torque in choronic low back pain patients back extensor are lower than healthy subjects(p<;.05). 5. Fifty generation was all degree of angle in lumbar extention peak torque in choronic low back pain patients back extensor are lower than healthy subjects(p<.05). 6. Sixty generation was all degree of angle in lumbar extention peak torque in choronic low back pain patients back extensor are lower than healthy subjects(p<.05).

Relationship between needle depth for lumbar transforaminal epidural injection and patients' height and weight using magnetic resonance imaging

  • Background: Optimal needle depth in transforaminal epidural injection (TFEI) is determined by body measurements and is influenced by the needle entry angle. Physician can choose the appropriate needle length and perform the procedure more effectively if depth is predicted in advance. Methods: This retrospective study included patients with lumbosacral pain from a single university hospital. The skin depth from the target point was measured using magnetic resonance imaging transverse images. The depth was measured bilaterally for L4 and L5 TFEIs at 15°, 20°, and 25° oblique angles from the spinous process. Results: A total of 4,632 measurements of 386 patients were included. The lengths of the left and right TFEI at the same level and oblique angle were assessed, and no statistical differences were identified. Therefore, linear regression analysis was performed for bilateral L4 and L5 TFEIs. The R-squared values of height and weight combined were higher than the height, weight, and body mass index (BMI). The following equation was established: Depth (mm) = a - b (height, cm) + c (weight, kg). Based on the equation, maximal BMI capable with a 23G, 3.5-inch, Quincke-type point spinal needle was presented for three different angles (15°, 20°, and 25°) at lumbar levels L4 and L5. Conclusions: The maximal BMI that derived from the formulated equation is listed on the table, which can help in preparations for morbid obesity. If a patient has bigger BMI than the one in the table, the clinician should prepare longer needle than the usual spinal needle.

요통과 경추, 요추전만의 관계에 대한 임상적 연구 (A Clinical Study on Correlation between Cervical, Lumbar Lordosis and Low Back Pain)

  • Objectives: This study was designed to investigate the correlation between cervical, lumbar lordosis and low back pain(LBP), sex, age and duration of LBP. Methods : Cervical, lumbar lordosis(by Cobb's Method) and Ferguson's angle were measured and evaluated in LBP group and control. Radiograph was taken in lateral direction, erect position. Cobb's angle between C1-C7, C2-C7, L1-L5, L1-S1 and Ferguson's angle were measured and investigated with statistical program. Results: 1. Cervical lordosis have no relation to LBP, sex and age. 2. Lumbar lordosis and Ferguson's angle have no relation to LBP and sex. 3. Cobb's angle L1-L5 have no relation to age. Lumbar lordosis from L1 to S1(Cobb's angle L1-S1) increased in old group(Age>40) compared to young group(Age${\leq}$40). 4. In LBP group, Cobb's angle L1-S1 have no relation to duration of LBP. Lumbar lordosis from L1 to L5(Cobb's angle L1-L5) decreased in acute LBP group compared to Chronic group. Conclusions : Cervical, lumbar lordosis and Ferguson's angle have no relation to LBP and sex. As far as age is concerned, old group have larger lumbosacral lordosis than young group. Acute LBP group have smaller lumbar lordosis(Cobb's angle L1-L5) than chronic group.

Factors Affecting Cage Obliquity and the Relationship between Cage Obliquity and Radiological Outcomes in Oblique Lateral Interbody Fusion at the L4-L5 Level

  • Objective : This retrospective study investigated the factors that affect cage obliquity angle despite orthogonal maneuvers performed during oblique lateral interbody fusion (OLIF) and assessed the relationship between cage obliquity angle and radiological outcomes post-surgery. Methods : Twenty-nine males who underwent L4-L5 OLIF for lumbar degenerative disease between 2019 and 2021 with a followup duration greater than 12 months were analyzed. Radiological parameters were measured including psoas muscle volume, total psoas area index (total psoas muscle area [cm2]/height squared [m2]), distance from the iliac artery to the origin of the psoas muscle (DIAPM), angle between the origin of the psoas muscle and the center of the vertebral disc (APCVD), iliac crest height, disc height, lumbar flexibility (lumbar flexion angle minus extension angle), cage location ratio, cage-induced segmental lumbar lordosis (LL) (postoperative index level segmental LL minus used cage angle), foraminal height changes, fusion grade. Results : DIAPM, APCVD, iliac crest height, postoperative index level segmental LL, and cage-induced segmental LL were significantly correlated with OLIF cage obliquity angle. However, other radiological parameters did not correlate with cage obliquity. Based on multiple regression analysis, the predictive equation for the OLIF cage obliquity angle was 13.062-0.318×DIAPM+0.325×1APCVD+0.174×iliac crest height. The greater the cage obliquity, the smaller the segmental LL compared to the cage angle used. Conclusion : At the L4-L5 level, OLIF cage obliquity was affected by DIAPM, APCVD, and iliac crest height, and as the cage obliquity angle increases, LL agnle achievable by the used cage could not be obtained.

구두 굽 높이가 20대 여성의 요천추부 각도에 미치는 영향 (The Effect of Heel-height on the Lumbosacral Region Angle of Young Ladies)

  • Objective : To purpose of this study was the most of the ladies wear high-heeled shoes at lease 4 to 5 day a week but the effect of it's height on the lumbo-sacral legion angle has not been clearly defined. Method : Subject were 20 young ladies, who had majored in physical therapy of the Dae-gu Health College. Method 1. PACS system X-ray was used to measure the lumbo-sacral legion angle under the condition of bare foot, 3cm, 7cm high-heeled at standing position. 2. Spinal Mouse was used to measure the spinal segment motion angle and length under the condition of bare foot, 3cm, 7cm high-heeled at being Flexion-Extension position Result : The result of this study were as follow I. Significant statistical increase in lumbar lordosis was observed as the heel height was increased from bare foot to 7cm high-heeled(p<.05), but there was no significant difference in the lumbo-sacral angle & sacral angle(p>.05). 2. The Height and the weight of the subjects, their preference on the shoes didn't affect the lumbo-sacral lesion angle(p>.05) 3. The variation of the heel height didn't affect the spinal segment motion angle and length(p>.05). Conclusion : There is strong relationship between the high of heel with increasing the lumbar lordosis(p<.05).

The Effect of Proprioceptive Position Sense by Lumbar Flexors and Extensors

  • Purpose: Muscle fatigue affects proprioception, and it causes problems in spinal stability. The purpose of this study was to examine the effect on the accuracy of reproducing the lumbar angles before lumbar exercise and after fatiguing isokinetic lumbar exercise. Methods: Thirty healthy adults participated in this study. Before induction of fatigue by exercise, the proprioception was measured by Biodex. Lumbar positions were passively maintained on stimulation position ($25^{\circ}$ flexion and $25^{\circ}$ extension), and back to the starting position. Subjects actively repositioned the remembered stimulation position, and error degrees between the stimulation position and reposition were measured. Using an isokinetic device at $120^{\circ}$/sec of velocity of angle lumbar flexion/extension exercise resulted in muscle fatigue. The post-fatigue proprioceptive position sense was used in the same way as in pre-fatigue measurement. Results: Means of position sense of pre-fatigue were $2.19{\pm}1.97$ on flexion angle, and $5.04{\pm}2.84$ on extension angle. After exercise induced fatigue, means of position sense were $2.37{\pm}1.83$ on flexion angle, and $4.93{\pm}2.57$ on extension angle. Results of this study showed significant differences of lumbar proprioceptive position sense between pre- and post-fatigue. Conclusion: Lumbar proprioception sense in active repositioning in flexion and extension was affected in the presence of muscle fatigue. Therefore, it should be noted that therapeutic exercise for patients with abnormal proprioceptive sense or elderly people must be performed with care because muscle fatigue can cause secondary damage.

노인 퇴행성디스크 환자의 안정화운동이 척추불안정과 피로도에 미치는 영향 (The Effects of Segmental Instability and Muscle Fatigue after Applying Sabilization Exercise Program In Degenerated Disc Disease Patients of Aged)

  • The purpose of this study was designed to find out the effectiveness of vertebral segment instability, muscle fatigue response on lumbar spine after apply lumbosacral stabilization exercise program to 4 patients with chronic low back pain and for 12 weeks. In this study, the lumbar spine motion with blind by MedX test machine and the difference of instability to lumbar vertebra segments in flexion, extension test of standing position and spinal load test(Matthiass Test) by Spinal Mouse. The stabilization exercise program was applied 2 times a week for 12 weeks in hospital and 2 times a day for 20 minutes at home. The results of the present study were as follows: 1. Instability test of lumbar vertebra segment is 2 type differential angle test between vertebrae segment and loading test of spine(matthiass) by Spinal Mouse. It appeared to improve stability of segments in sagittal plane after applying program. So lumbar spine curve increased lordosis toward anterior and was improved of the lumbar spine flexibility in flexion and extension. Specially, in matthiass test, (-) value was increased between lumbar vertebra segment when was the load on spine. And so applying stability improved after program. 2. Fatigue response test(FRT) results, in male, was raised muscle fatigue rate during increase weight, on the other hand female appeared lower than male. As a results, lumbosacral stabilization exercise was aided to improvement of lumbar spine vertebra segments stabilization. Spine instability patients will have a risk when in lifting a load or working with slight flexion posture during the daily of living life and it is probably to increase recurrence rate. Thus, not only lumbar extension muscle strength but also stability of vertebra segments in lumbar spine may be very important.
