The Changes of Range of Motion of Lumbar Region and Muscle Activities of Lumbar Extensor During Trunk Motions Between Subjects With Low Back Pain and Healthy Subjects (만성요통환자와 정상인의 체간 운동시 요추 신전근의 근활성도와 관절운동범위의 변화)

  • The purpose of this study was to compare the electromyography (EMG) activities of the lumbar extensor muscles during motion of trunk flexion-extension and compare range of motion (ROM) with a 3-dimensional motion analysis system of the lumbar region between subjects with chronic low back pain (CLBP) and healthy subjects during the trunk flexion-extension, trunk rotation and trunk lateral flexion cycle. Thirty CLBP subjects and thirty healthy subjects were included. We measured the root mean square (RMS) value of the lumbar extensor muscles from resting, standing, lumbar flexion and return position. The RMS ratio was normalized from maximal EMG activity of the lumbar extensor muscles during trunk motion. The results of this study showed that the RMS ratio of the lumbar extensor was significantly higher in CLBP subjects than healthy subjects during all of trunk motion (p<.05). The ratio of the highest RMS value during flexion and extension was higher in CLBP subjects than in healthy subjects (p<.05). The ROM of the lumbar region was significantly lower in CLBP subjects than healthy subjects during trunk flexion-extension, trunk rotation and lateral flexion cycle. The relationship between the RMS ratio for full lumbar flexion and the ROM of lumbar flexion was not correlated significantly. CLBP subjects have both decreased ROM of the lumbar region and higher muscle activities of the lumbar extensor muscle than healthy subjects.

The Effects of Yoga Low Back Pain Exercise and Lumbar Extensor Muscle Endurance Exercise on Chronic Low Back Pain Patients (요가 요통체조와 요부 신전근 지구력 운동이 만성요통 환자의 통증에 미치는 영향)

  • Purpose: In the present study, the degree of subjective pain in chronic low back pain patients through the course of six week of yoga low back pain exercise and lumbar extensor muscle strengthening training was analyzed, and a systematic and effective home exercise therapy program was proposed. Methods: For this study, we sampled 21 random patients with chronic low back pain who were receiving outpatient treatment at G Hospital Pusan, and conducted experiment for six weeks by applying yoga low back pain exercise+modalities (7) lumbar extensor muscle endurance exercise+ modalities (7) and modalities (7). To measure the degree of pain, the visual analogue scale (VAS) and Oswestry Low Back Pain Questionnaire were used. Pain was measured before the experiment and after six weeks. Results: Significant differences were observed between yoga, extensor muscle endurance exercise and modalities (VAS=0.00 Oswestry=0.00). The yoga and extensor endurance muscle exercise reduced low back pain by more than the modalities. Conclusion: This study will be used to develop a preventive and reduction method of LBP. Therefore, Yoga and extensor muscle endurance exercise are appropriate for use as a home program exercise to reduce low back pain in patients.

A Study on Effect by Lumbar Extension Exercise (요부신전운동에 의한 효과에 관한 연구)

  • The purpose of this study is to improve the effect of Lumbar extensor exercise program and develop the Lumbar extensor exercise program which will be suitable to the patients characteristics. In this study the experimental group was made up of 38 subjects. They are the patients with low back pain using the Lumbar extension exercise program in C hospital. The lumbar extension exercise program was given two times a week for 8 weeks. The results were estimated by Lumbar extensor strength by diagnosis agent. The results were measured three times, one time pre-treatment and two times post-treatment at 4weeks and 8weeks by lumbar extensor curve angle(0, 12, 24, 36, 48, 60, 72). The results were compared at pre-test 4weeks and 8weeks. The muscle strength measured at both 4weeks and 8weeks with the greater strength was shown at 4weeks. The muscle strength of patients with M.strain, Laminectomy and HNP was increased at all angles except for patients with stenosis. The results of this study indicated that diagnosis influenced the muscle strength in Lumbar extensor exercise program.

The effects of lumbar extensors strengthening program on low back muscle power and mass, pain, return to work of patients who took laser operation for herniated lumbar disc (요부신전강화 운동프로그램이 단순추간판탈출증 수술환자의 요부근육 및 통증 그리고 사회복귀에 미치는 영향)

  • OBJECTIVE: The objective of this study was to know the effects of the postoperative lumbar extensor strengthening exercise program on back muscles strength and volume, pain, and the time of return to work. METHODS: A prospective controlled trial of lumbar extensor exercise program in patients who underwent microdiscectomy or percutaneous endoscopic discectomy for prolapsed lumbar intervertebral disc. Seventy-five patients were randomized into exercise group (20 male, 15 female) and non-exercise group (18 male, 22 female). Six weeks after surgery, patients in exercise group undertook a 12-week lumbar extension exercise (MedX) program. Assessment of spinal function was performed in all patients on postoperative 6 weeks, 18 weeks. The assessment included measures of lumbar extensor power, muscle mass of erector spinalis. All patients completed the visual analog scale (VAS) for evaluation of pain, and return to work. RESULTS: In muscle power, there were statistically significant improvements between pre and post test on muscle power in exercise group. But there were not statistically significant difference on muscle power in non-exercise group. In muscle mass, there were statistically significant difference between pre and post test on muscle mass in exercise group. But there were not statistically significant difference on muscle mass in non-exercise group. In the pain, there were statistically significant decrease between pre and post test on both group. But there were not statistically significant difference on fatty tissue and obesity in non-exercise group. The percentages of return to work in postoperative 4 months were significantly greater in the exercise group than in the non -exercise group. CONCLUSIONS: Postoperative lumbar extensor strengthening exercise program appears to be more beneficial to the patients who underwent operation for prolapsed lumbar intervertbral disc.

  • The purpose of this study was to investigate relationship of lumbar extensor muscle power & spinal column curve for old female patients with LDK(Lumbar degenerative kyphosis). Subjects were composed of 37 old female with LDK. The subjects were tested in their spinal segment movement and spinal column curve with Spinal-$Mouse^{\leq}$ in $1^{st}$ loaded test and $2^{nd}$ loaded test and then tested lumbar extensor muscle power with $Medx^{\leq}$ lumbar extension machine. The results of this study, were as follow; There were statistically significant difference $1^{st}$ loaded test and $2^{nd}$ loaded test in upright position increase spinal column forwardly(p<0.01) and decrease lumbar lordosis angle(p<0.01), but no statistically significant difference $1^{st}$ loaded test and $2^{nd}$ loaded test thoracic and hip & sacrum curve angle. Their lumbar extensor muscle poser is very weakness, 61.4% of normal people.

Effect of lumbar extensor strength according to lumbosacral angle change on chronic lumbar back pain patients (만성요통환자의 요천추 각도가 요부신전 근력에 미치는 영향)

  • The Purpose of this study was to investigate the change of lumbar extensor strength according to lumbosacral angle on chronic lumbar back pain patients. For this investigation lumbar extensor strength was administered to 60 patients who were diagnosed chronic lumbar back pain The subjects was to investigate lumbosacral angle in standing position and it were calculated lumbar extensor strength by using Medex. The result of this study summarized are as follows ; 1. Total experimental group exhibited significantly higher difference than control group in lumbar extensor strength among all degree lumbosacral angle. 2. In the relationship between experimental group and control group in lumbar extensor strength among lumbosacral angle, all degree difference was revealed II, I, III order. 3. In the relationship between experimental group and control group in lumbar extensor strength among lumbosacral angle, I group difference was did not. 4. In the relationship between experimental group and control group in lumbar extensor strength among lumbosacral angle, all degree among II group was noted significantly difference except 24, 72 angle. 5. In the relationship between experimental group and control group in lumbar extensor strength among lumbosacral angle, control group was revealed higher muscle strength 48, 60, 72 angle, however no significantly difference was noted 0, 12, 24 angle. The study was objected difference of other group in both of experimental and control group. Because lumbar extensor weakness with bad position was gradually increased back pain, to Maintain normal lumbosacral angle befor exercising lumbar extensor strength was most important.

A Clinical Study of 17 Cases of Patients with Big Toe Extensor Muscles Weakness Caused by Lumbar Disc Herniation (요추 추간판 탈출증으로 유발된 족무지 신전 근력약화 환자 17례의 임상적 고찰)

  • Objectives : To investigate the improvement rate of muscle weakness caused by lumbar disc herniation, 17 patients was treated with oriental medicine. Methods : To evaluate weakness of big toe extensor muscles caused by lumbar disc herniation, we measured the active range of motion(ROM) of big toe extension and muscle grading of big toe extensor muscles. We also measured numerical rating scale(NRS), oswestry disability index(ODI) and compared them with the active ROM of big toe extension. Results : 1. Progressive weakness of big toe extensor muscles was observed in 2 patients. Weakness of big toe extensor muscles were improved in 13 patients(76%). 2. Changes in manual muscle testing(MMT) scores corresponded with improvement rate of big toe extension. 3. Numerical rating scale(NRS) and oswestry disability index(ODI) improvement rate did not correspond with improvement rate of big toe extension. Conclusions : Muscle weakness caused by lumbar disc herniation can be treated with oriental medicine and progressive muscle weakness must be observed constantly.

  • In this study, the muscle strength and endurance of the lumbar at flexion and extension were determined using an isokinetic muscle strength meter (Biodex) in patients with chronic lumbar go (10 male and 10 females), and the changes in muscle strength and endurance of the lumbar at flexion and extension after application of Y-shaped sacrospinalis muscle taping, typically used for patient with lumbar go, were studied. In addition, the sacrospinalis muscle taping of a different shape (I-shaped taping) was applied bilaterally centering on the spine, and the muscle strength and endurance of the lumbar at flexion and extension were determined and compared with those before taping. In addition, the results after application of Y-shaped taping and I-shaped taping were also compared. 1. The extensor muscle strength of the lumbar at loading of $60^{\circ}$/sec before application of kinesio Taping and after application of Y-shaped taping showed the level of significance (p<.05). The flexor muscle strength of the lumbar at loading of $60^{\circ}$/sec before application of kinesio taping and after application of Y-shaped taping did not show the level of significance. 2. The flexor muscle endurance of the lumbar at loading of $90^{\circ}$/sec before application of kinesio Taping and application of Y-shaped taping did not show the level of significance. The extensor muscle endurance of the lumbar at loading of $90^{\circ}$/sec before application of kinesio Taping and after application of Y-shaped taping did not show the level of significance. 3. The extensor muscle strength of the lumbar at loading of $60^{\circ}$/sec before application of kinesio Taping and after application of I-shaped taping showed the level of significance (p<.05). Comparison of Hip Joint Strength between Young Woman Patient with Chronic Low Back Pain with Lumbar Instability and Normal Subjects

  • Objective: The purpose of this study was to compare the hip joint muscle strength of patients with chronic back pain with lumbar instability and normal subjects. Design: A case control study. Methods: Five types of lumbar instability test were conducted on forty young women with chronic low back pain for more than six months, and those who had 3 or more positive tests were selected as subjects. To select chronic back pain patients with lumbar instability group, aberrant movement patterns during lumbar flexion test (FMT), prone instability test (PIT), posterior-anterior mobility test (PAT), passive lumbar extension test (PLE), and pressure bio-feedback (PBF) were applied. In addition, a digital muscle strength meter was used to measure the hip flexor, extensor, adductor, and abductor muscles of chronic low back pain patients with lumbar instability group (n=20) and normal subject group (n=20). Results: As a result of comparing the hip joint muscle strength between the chronic back pain patient group with lumbar instability and the normal group, there were significant differences in the hip extensor, abductor, and adductor muscles (p<0.05). Conclusions: Patients of chronic back pain with lumbar instability were found to have weak hip joint muscle strength. Therefore, this study suggest that include hip joint strength exercise for functional recovery of chronic back pain patients.

The Effect of Transverse Abdominal Exercise for 3Weeks for Lumbar Muscle Strength and Pain Relief on Chronic Low Back Pain Patients (3주간의 복횡근 강화운동이 만성요통 환자의 요부근력과 통증완화에 미치는 영향)

  • Purpose : The purpose of this study was to analyze the effects of transverse abdominal exercise on the change of chronic low back pain and lumbar muscle strength. Method : 18 chronic lumbago patients were transverse abdominal exercise for 3weeks. Result : 1. The strength of the lumbar extensor and flexor of the male subjects was increased significantly after abdominal exercise(p<0.05). 2. The strength of the lumbar extensor and flexor of the female subjects was increased significantly after abdominal exercise(p<0.05). 3. The study can confirm significant relationship between the lumbar muscle strength and lumbar pain before and after the exercise Conclusion : The study of could find the increase of the ability of the lumbar extensor and flexor of both male and female subjects suffering from chronic low back and pain using three-week transverse abdominal exercise. The study confirmed the general decrease of pain after the experiment.
