PURPOSE: This study was to investigate of pain and spine shape in elementary school students. The aim of this study was to prevent pain and spinal misalignment is to provide basic data and the need for regular screening for elementary school students. METHODS: This study had a survey research about pain and spine shape by using questionnaires and Formetric 4D. Subjects were 301 Elementary School Students RESULTS: In the presence or absence of pain and the presence of pain was in a high proportion and many of them indicated the back as a pain area, As a result of the spine shape contour, pelvic tilt was normal and trunk inclination was asymmetric as well as thoracic kyphosis and lumbar lordosis were diminished. There was a significant difference in relationship of trunk inclination according to pain CONCLUSION: The asymmetric of trunk inclination influenced the pain, In contrast, the pain did not influence thoracic kyphosis and lumbar lordosis in diminished. but, the diminution in thoracic kyphosis and lumbar lordosis are potential sources of pain. Regular checkups are necessary to prevent elementary school students from a change of spine shape and a pain, Moreover, education of maintaining normal posture should be followed.

Objectives : The objective of this study was to observe the positive effects of acupuncture and bee-venom acupuncture treatment on lumbar hypolordosis. Methods : Acupuncture and bee-venom acupuncture were performed twice a week for 30 days to treat patients with low back pain patients showing a Cobb's angle [L1-S1] of less than $35^{\circ}$(treatment group=20, control group=13). Lumbar lordosis(Cobb's angle [L1-S1], Centroid angle [L1-S1]) was measured using an L-spine Lat. X-ray before and after treatment. Pain scales(VAS scale, ODI) were also was measured before and after treatment. SPSS 12K for Windows was used for statistical analysis of the results. Results : 1. VAS and ODI decreased significantly in the treatment group. 2. Lumbar lordosis, presented by Cobb's angle, increased significantly from $25.00{\pm}7.06^{\circ}$ to $30.95{\pm}10.02^{\circ}$ (p = 0.006). The increase of Cobb's angle in the treatment group $5.95{\pm}8.60^{\circ}$, and that of the control group was $-3.08{\pm}8.41^{\circ}$(p = 0.006). The increase of the Centroid angle of the treatment group was $3.85{\pm}8.83^{\circ}$, and that of the control group was $-2.92{\pm}7.63^{\circ}$(p=0.031). 3. Cobb's angle and Vas scale showed a significant correlation coefficient of -0.250(p=0.043). Increase of Cobb's angle and decrease of Vas scale showed a significant correlation coefficient of 0.420(p=0.015). Conclusions : Acupuncture and bee-venom acupuncture were verified to have a positive effect on pain alleviation of lumbar lordosis decreased patients while also affecting lumbar lordosis to be increased as a result of structural changes. It was also shown that lumbar hypolordosis has significant correlation with pain.

Objectives: The purpose of this study was to investigate the relationship among lumbar lordotic and obesity in Low Back Pain Patient Methods: The subjects of this study were patients with low back pain who visited Jung-Dong Oriental Hospital. X-ray were taken in lateral decubitus. The measurements of the Ferguson angles(FA) and the lumbar lordotic angles(LLA) were performed. We measured BMI and WHR has been accessed bio-impedance analyzer(inbody 3.0). This results were statistically analyzed using SPSS 12.0. Results 1. In female group. FA and LLA were significantly higher than male group. 2. LLA was shown to decrease to rise with increasing WHR and BMI 3. FA had no realtion with WHR and BMI Conclusions: This data shows that obesity related to mechanical structures such as lumbar curvature. Obesity can be a one of the stressor of lumbar spine, and one of the causing factor of low back pain.
Objectives : To study the cervical lordosis and disc degeneration in neck pain patients with or without low back pain. Subjects. The study was composed of 57 neck pain patients with low back pain(LBP group) and 40 neck pain patients without low back pain(Non-LBP group). Methods : Radiographic measures of spinal lordosis(cervical and lumbar) and disc degeneration were collected, and statistically analyzed. Results: LBP group showed a significant increase in cervical lordotic angle as compared with Non-LBP group, whereas no significant change in cervical disc degeneration. A relationship was found between cervical and lumbar disc degeneration in LBP $group({\gamma}-0.3064)$. Conclusions : The findings from this study suggest that the curvature of the cervical spine is related to the subject's low back pain.

Conclusion: We experienced a 40 years old woman who had love pack pain with Lumbar hyperlordosis. Purpose : The aim of this study is to consider degenerative spine disease theoretically and compare plain radiography which is a basic study for low back pain with MRI in cases of degenerative lumbar spine disease to find out whether the abnormalities agree with each other. Methods : In 4 cases of lumbar degenerative disease, we studied the relation of the abnormalities such as disc space narrowing, spinal space narrowing, loss of lordosis and osteophytes on plain radiography with those on MRI of HIVD, spinal stenosis and spondylolisthesis. Results : Many abnormalities such as disc space narrowing, spinal space narrowing, loss of lordosis, osteophytes and change of cortex & bone marrow on plain radiography suggest HIVD, spinal stenosis, spondylolysis or spondylolisthesis on MRI. Conclusion : For low back pain patients, plain radiography is a basic study in diagnosis of HIVD, spinal stenosis, spondylolysis or spondylolisthesis but MRI or CT scan is necessary to develop(build) a treatment plan like an operation.
Objectives: The purpose of this study was to compare the difference of Lumbar Lordosis Angle (LLA)between patients with Low Back Pain (LBP) and control groups. Methods: Questionnaires were completed by 40 adults LBP patients seeking physical therapy services and by 40 controls at the department of Physical Therapy, SaeJong Neurosurgical Clinic in Taegu city, South Korea from October 1999 to March 2000. LLA was measured on lateral x-ray films with standing position. The angle between a line parallel to tile top of the first Lumbar (Ll) and the top of the fifth Lumbar (L5) was defined LLA. Results: LLA of 29.88$^{\circ}$ for LBP patients was statistically significant decrease from that of 35.31$^{\circ}$ for controls in the difference of lumbar lordosis (p<0.01). There were statistically significant differences between genders in patients groups. Females(32.22$^{\circ}$) had significantly greater angles than males (27.32$^{\circ}$) (p<0.05), while 36.63$^{\circ}$ for female was also greater than 34.12$^{\circ}$ for male in the controls. No significant difference was found between age. In patient groups, 27.95$^{\circ}$ for below age 40 was a smaller than 32.32$^{\circ}$ for above, however, 35.82$^{\circ}$ for below age 40 was a little greater than 34.27$^{\circ}$ for above in controls. Patients in sitting posture had greater LLA (31.35$^{\circ}$). than those standing (28.93$^{\circ}$), however. values for controls were similar to each other. Conclusions: Results from this study indicate that distinct difference exist among patients and controls and gender, whereas little difference exists in age and working posture.
Objective : To evaluate clinical and radiological results of two different fusion techniques in adult low-grade isthmic spondylolisthesis. Methods : Between November 2003 and December 2004, 46 consecutive patients underwent instrumented mini-transforaminal lumbar interbody fusion (mini-TLIF) (group I) at Wooridul Spine Hospital, Seoul, Korea. Between February 2003 and October 2006, 32 consecutive patients underwent instrumented circumferential fusion (group II) at Leon Wiltse Memorial Hospital, Suwon, Korea. The mean follow-up periods were 29.7 and 26.1 months, respectively. Results : Mean visual analog scale (VAS) scores for back and leg pain decreased, respectively, from 6.98 and 6.33 to 2.3 and 2.2 in group I and from 7.38 and 6.00 to 1.7 and 1.0 in group II. Mean Oswestry disability index (ODI) improved from 51.85% to 14.4% in group I and from 60% to 9.1% in group II. In both groups, VAS and ODI scores significantly changed from pre- to postoperatively (p<0.001), but postoperative outcome between groups was statistically not significant. Radiologic evidence of fusion was noted in 95.7% and 100% of the patients in group I and II, respectively. In both groups, changes in disc height, segmental lordosis, degree of listhesis, and whole lumbar lordosis between the pre- and postoperative periods were significant except whole lumbar lordosis in both groups. Conclusion : Clinical and functional outcomes demonstrate no significant differences between groups in treating back and leg pain of adult patients with low-grade isthmic spondylolisthesis. However, in terms of operative data (i.e. operation time and hospital stay), instrumented mini-TLIF demonstrated better results.

Objective : This study examined the change of range of motion (ROM) at the segments within the dynamic posterior stabilization, segments above and below the system, the clinical course and analyzed the factors influencing them. Methods : This study included a consecutive 27 patients who underwent one-level to three-level dynamic stabilization with Bioflex system at our institute. All of these patients with degenerative disc disease underwent decompressive laminectomy with/without discectomy and dynamic stabilization with Bioflex system at the laminectomy level without fusion. Visual analogue scale (VAS) scores for back and leg pain, whole lumbar lordosis (from L1 to S1), ROMs from preoperative, immediate postoperative, 1.5, 3, 6, 12 months at whole lumbar (from L1 to S1), each instrumented levels, and one segment above and below this instrumentation were evaluated. Results : VAS scores for leg and back pain decreased significantly throughout the whole study period. Whole lumbar lordosis remained within preoperative range, ROM of whole lumbar and instrumented levels showed a significant decrease. ROM of one level upper and lower to the instrumentation increased, but statistically invalid. There were also 5 cases of complications related with the fixation system. Conclusion : Bioflex posterior dynamic stabilization system supports operation-induced unstable, destroyed segments and assists in physiological motion and stabilization at the instrumented level, decrease back and leg pain, maintain preoperative lumbar lordotic angle and reduce ROM of whole lumbar and instrumented segments. Prevention of adjacent segment degeneration and complication rates are something to be reconsidered through longer follow up period.

Objective : This multi-center clinical study was designed to determine the long-term results of patients who received a one-level posterior lumbar interbody fusion with expandable cage ($Tyche^{(R)}$ cage) for degenerative spinal diseases during the same period in each hospital. Methods : Fifty-seven patients with low back pain who had a one-level posterior lumbar interbody fusion using a newly designed expandable cage were enrolled in this study at five centers from June 2003 to December 2004 and followed up for 24 months. Pain improvement was checked with a Visual Analogue Scale (VAS) and their disability was evaluated with the Oswestry Disability Index. Radiographs were obtained before and after surgery. At the final follow-up, dynamic stability, quality of bone fusion, interveretebral disc height, and lumbar lordosis were assessed. In some cases, a lumbar computed tomography scan was also obtained. Results : The mean VAS score of back pain was improved from 6.44 points preoperatively to 0.44 at the final visit and the score of sciatica was reduced from 4.84 to 0.26. Also, the Oswestry Disability Index was improved from 32.62 points preoperatively to 18.25 at the final visit. The fusion rate was 92.5%. Intervertebral disc height, recorded as $9.94{\pm}2.69\;mm$ before surgery was increased to $12.23{\pm}3.31\;mm$ at postoperative 1 month and was stabilized at $11.43{\pm}2.23\;mm$ on final visit. The segmental angle of lordosis was changed significantly from $3.54{\pm}3.70^{\circ}$ before surgery to $6.37{\pm}3.97^{\circ}$ by 24 months postoperative, and total lumbar lordosis was $20.37{\pm}11.30^{\circ}$ preoperatively and $24.71{\pm}11.70^{\circ}$ at 24 months postoperative. Conclusion : There have been no special complications regarding the expandable cage during the follow-up period and the results of this study demonstrates a high fusion rate and clinical success.
