중년여성의 요통에 따른 골밀도 측정 (Measurement of Bone mineral density According to Middle aged Women with Low Back Pain)

  • 본 연구는 2000년 7월 20일부터 2001년 1월 12일까지 대구광역시에 소재한 가톨릭병원 건강검진센타에서 골다공증 검사를 시행한 여성 50명을 대상으로 실시하였다. 연령은 50~59세가 66.0%로 가장 많았고, 평균연령은 56세이고, 49세 이하는 정상군이 18.7%, 50~59세는 골감소군 83.4%, 60세 이상은 골다공증군이 36.4%로 각각 가장 많았으며, 요추부의 평균골밀도는 연령의 증가에 따라 감소되었지만 유의하지 않았다. 요통유무는 요통이 있다가 76.0%로 많았고, 요통이 있다는 골다공증군 90.9%, 요통이 없다는 정상군이 43.7%로 각각 유의하게 많았으며 (p<0.05), 골밀도는 요통이 있다가 1.00g/$cm^2$로 유의하게 낮았다(p<0.05). 좋아하는 식품은 채소류가 62.0%로 많았고, 생선류는 요통군과 비요통군에서 정상군이 각각 33.3%, 57.1%, 채소류는 요통군과 비요통군에서 골다공증군이 각각 85.0%, 100.0%로 각각 가장 많았고, 골밀도는 요통군에서 채소류가 0.71g/$cm^2$로 각각 유의하게 낮았으며 (p<0.05), 비요통군에서도 채소류가 0.82g/$cm^2$로 낮았지만 유의하지 않았다. 혈액형은 B형은 요통군과 비요통군에서 골다공증군이 각각 50.0%, O형은 요통군과 비요통군에서 정상군이 각각 44.4%, 42.9%로 요통군이 많았고, 골밀도는 요통군과 비요통군에서 O형이 각각 가장 유의하게 높았다(p<0.05). 골밀도(BMD)는 평균골밀도(BMD)는 0.79g/$cm^2$, 요통군에서 0.90~0.99g/$cm^2$는 정상군이 77.8%, 0.60~0.69g/$cm^2$는 골다공증군이 50.0%로 각각 가장 많았고, 골밀도는 요통군과 비요통군에서 골밀도 수치가 증가할수록 유의하게 높았다(p<0.05) 다중회귀분석을 이용한 요추골밀도의 관련성 있는 요인은 연령, 결혼상태, 운동시간, 좋아하는 식품, 칼슘(Calcium), 골밀도의 표준편차인 T값이다(P<0.05). 요통과 골밀도의 관련변수별 상관관계에서 연령과 요통유무, 알카리포스파타제(Alkaline phosphatase), 몸무게와 체질량지수는 유의한 상관관계를 나타냈고 (p<0.01), 몸무게와 신장, 알카리포스파타제(Alkaline phosphatase)와 칼슘(Calcium), 골밀도와 폐경유무는 유의한 상관관계를 나타냈으며(p<0.05), 연령과 폐경유무, 골밀도, 골밀도의 표준편차인 T값, 골밀도와 좋아하는 식품, 알카리포스파타제(Alkaline phosphatase)는 유의한 역상관관계를 나타냈다(p<0.01). 이상의 결과를 요약하면, 요추골밀도의 관련성있는 요인은 연령, 운동시간, 좋아하는 식품, 칼슘(Calcium), 골밀도의 표준편차인 T값이고, 본 연구의 연구대상자의 50대가 66.0%로 가장 많아 폐경유무에서 폐경이다는 골감소군이 83.3%로 가장 많았으므로 폐경 후 기간이 길수록 요추골밀도가 낮아지는 현상을 본 연구에서는 의미 있는 설명 변량이 되지 못했다. 요통과 골밀도의 관련변수별 상관관계에서 연령, 요통유무, 폐경유무, 칼슘(Calcium), 인(Phosphorus), 알카리포스파타제(Alkaline phosphatase)가 중요한 요인으로 생각된다.

우리나라 50세 이상 남성의 골감소증·골다공증 유병률과 관련 요인: 2010~2011 국민건강영양조사 자료 (Prevalence of Osteopenia/Osteoporosis and Related Risk Factors of Men Aged 50 Years and Older: Korea National Health and Nutrition Examination Survey 2010~2011 Data)

  • Osteoporosis is a major health problem that can lead to mortality. This study was conducted to estimate the prevalence of osteopenia and osteoporosis separately and to assess the risk factors associated with osteopenia/osteoporosis in Korean men aged 50 years and over. A total of 1,136 subjects were analyzed among the participants of the Korea National Health and Nutrition Examination Survey (KNHANES) 2010~2011 by using SPSS statistics complex samples (windows ver. 23.0). The prevalence rates of osteopenia and osteoporosis were 46.3% and 7.3%, respectively, and the mean ages of both osteopenia and osteoporosis risk groups were significantly higher than that of the normal group. The mean values for lifetime tallest height and bone mineral density in whole body, total femur, femoral neck, and lumbar spine were significantly lower in the risk group(osteopenia/osteoporosis) than in the normal group, whereas the mean values for height, body weight, BMI, waist circumference, alkaline phosphatase, parathyroid hormone, fasting blood glucose, total cholesterol, HDL-cholesterol, LDL-cholesterol, and triglycerides of the two groups showed no significant differences. The intakes of energy and carbohydrates were significantly higher in the risk group than in the normal group. The results of multiple logistic regression showed that being underweight and having hypercholesterolemia were significantly related with the prevalence of osteopenia/osteoporosis, whereas health habits such as smoking and exercise, chronic diseases such as obesity and hypertension, and nutrient intakes were not. These findings suggest the need for further studies to examine osteopenia/osteoporosis risk factors and outcomes specificly focused on Korean men.

서울지역 20~30대 성인여성의 직업유무에 따른 골밀도와 영향요인: 제5기 국민건강영양조사자료 분석, 2010~2011 (Various Factors Affecting the Bone Mineral Density in Korean Young Adult Women: Data from the Fifth Korea National Health and Nutrition Examination Survey (KNHANES V), 2010~2011)

  • This study investigated factors affecting the bone mineral density (BMD) of Korean young adult women (20~30 yrs) living in Seoul, according to the state of employment. Data on BMD and anthropometric and biochemical measurements were obtained from the Fifth Korea National Health and Nutrition Examination Survey (KNHANES V) (2010~2011). BMD of the working group (WG) was generally higher than that of the non-working group (NWG). The prevalence of osteopenia, and osteoporosis between the groups was 29.0/31.5%, and 12.9/18.5% from 20~29 yrs as well as 25.5/24.1%, and 20.2/20.5% from 30~39 yrs, respectively. From 20~29 yrs, prevalence of osteopenia and osteoporosis in the WG was lower than that of the NWG. Alkaline phosphatase activity of the WG was lower than that of the NWG. In the WG, from 20~29 yrs, total femur BMD showed significant positive correlations with waist circumstance and BMI. In the NWG, from 20~29 yrs, total femur, femoral neck, and lumbar spine BMDs showed significant positive correlations with weight and fat mass, whereas they showed significant negative correlations with vitamin D. In the WG, from 30~39 yrs, total femur and femoral neck BMDs showed significant positive correlations with weight and free fat mass. The results of this study show that BMD of young adult women is insufficient regardless of their employment status. Therefore, practical and systematic programs are required to improve the BMD of young adult women as well as to maintain healthy bone levels.

컴퓨터 적외선(赤外線) 전신체열촬영(全身體熱撮影)으로 본 경추추간판(頸椎椎間板) 탈출증(脫出症)의 침구치료효과(鍼灸治療效果) (Clinical evaluation of Acupuncture on Herniated of Cervical Intervertebral Disc by DITI)

  • Object : This study is designed to evaluate the correlation between the data of DITI(Digital Infrared Thermographic Imaging)examination and the changes of clinical symptom after the therapy of acupuncture in the patients with herniated cervical intervertebral disc. Content : The conservative therapy with acupuncture was performed during $4{\sim}5$ weeks, The acupuncture points of SI3, B62, TE3, G34, LI4, S36, GV26, G21, SII5 were used. In the pre- and post therapy, DITI examinations were performed in patients who had HIVD and were treated by acupuncture simultaneously, and then tried to correlate the results of clinical symptoms with the difference of thermographic findings at pre-acupuncture and post-acupuncture. Setting : The standard routine thermoghaphic examinations were performed with thermography(DITI) in the 15 patients with lumbar disc herniations at pre- and post acupuncture. Patient : Thermographic imaging of 15 cases were analyzed. They has disc herniations in X-ray, CT scan and MRI and they were treated by acupuncture theraphy in our hospital from March, 1994 to January, 1995. Result : 1) The causes were trauma(60%), retention of phlegm and fluid(8.7%) and cold & moisture(33.3%) 2) Levels of herniated cervical disc are C2/3(1 case). C3/4(2 cases), C5/6 & C6/7 (2 cases), C6/7(4 cases), Normal(6 cases). 3) Classifications of thermographic pattern are radiculopathy(80%), spot(13.3%), and nonspecific(6.7%) in order. 4) In evaluation Results of clinical symptom are Excellent(80%), Good(6.7%), and Poor(6.7%). Data of DITI are Excellent(80%), Good(6.7%), Fair(6.7%), and Poor(6.7%). Conclusion ; Acupuncture showed good results over 86.7% in clinical evaluation and 86.7% in DITI. Thermographic examination was valuable in the evaluation of therapeutic effect of acupuncture treatment.

Prevalence of and Factors Associated with Osteoporosis among Korean Cancer Survivors: A Cross-Sectional Analysis of the Fourth and Fifth Korea National Health and Nutrition Examination Surveys

  • Background: Identifying and managing osteoporosis among cancer survivors is an important issue, yet little is known about the bone health of cancer survivors in Korea. This study was designed to measure the prevalence of osteoporosis and to assess related factors among Korean cancer survivors. Materials and Methods: This study was designed as a cross-sectional analysis. Data were obtained from dual energy X-ray absorptiometry measurement of the lumbar vertebrae and femoral neck, and from standardized questionnaires among 556 cancer survivors and 17,623 non-cancer controls who participated in the Fourth and Fifth Korea National Health and Nutrition Examination Surveys (2008-2011). We calculated adjusted proportions of osteoporosis in non-cancer controls vs. cancer survivors, and we performed multivariate logistic regression analysis. Results: The prevalence of osteoporosis among cancer survivors was significant higher than that of the non-cancer controls after adjusting for related factors. Furthermore, osteoporosis among cancer survivors was higher in elderly subjects (60-69 years : adjusted odds ratio (aOR) 3.04, 95% CI : 1.16-8.00, ${\geq}70$ years : aOR 6.60, 95% CI 2.20-19.79), in female cancer survivors (aOR: 7.03, 95% CI: 1.88-26.28), and in a group with lower monthly income (aOR: 3.38, 95% CI: 1.31-8.71). In male cancer survivors, underweight and lower calcium intake were risk factors. Conclusions: These data suggest that the osteoporosis among cancer survivors varies according to non-oncologic and oncologic factors. Effective screening should be applied, and a sufficient and comprehensive management should be matched to individual cancer survivors early after cancer treatment.

고양이 척수 총상 증례: 임상소견, 수술소견, 컴퓨터단층영상소견 (A Case of Gunshot Injury to the Spinal Cord in a Cat:Clinical, Surgical, and Computed Tomographic Features)

  • 총상을 입은 18개월령 중성화된 암컷 고양이가 응급으로 내원하였다. 신체검사 및 신경검사에서 후지마비 및 심부통각소실을 보였으며 방사선사진 및 컴퓨터단층영상에서 1번 요추 왼쪽 근육에 한 개의 탄환(직경 3 mm)이 존재하고 척수강 안에 고신호 점이 나타났다. 탐색적 추궁절제술에서 요추1번 오른쪽 후방 관절돌기의 불완전 골절 및 척수 괴사를 확인하였다. 보호자의 동의하에 안락사 및 부검해 본바 1번 요추 왼쪽 척추뿌리(pedicle)에 균열이 존재했다.

  • Objectives: The purpose of this study was to determine which factors influence the bone mineral density (BMD) of total femur (TF), femoral neck (FN) and lumbar spine (LS) of the adult men by analyzing nationally representative Korean survey data. Methods: This study was conducted based on the data of 1,770 men aged 19-64 years from the Fifth Korea National Health and Nutrition Examination Survey (KNHANES V), 2010~2011. The BMD was analyzed by various factors (general characteristics, anthropometric data, health habits, chronic diseases, nutrient intake status). SPSS statistics for complex samples was used to analyze the data. Results: We observed that the BMD decreased significantly with aging. The BMD in each of the second lowest quartile of waist circumference (in TF & FN) and body mass index (in TF & LS) was lower than the respective BMD in the highest quartile group. The BMD in FN was higher in the group who reported the weight training. The BMD in LS was lower in hypercholesterolemia group than in the normal group. The BMD in TF, FN and LS was lower in hypertriglyceridemia group and in diabetes group than in the normal group. The BMD in TF, FN and LS was higher in the group with < Estimated Average Requirement iron intake. But there was no evidence to suggest that the BMD was related with educational level, income level, smoking, alcohol intake, anemia and nutrient intake status (except for iron). Conclusions: This study suggested that aging, waist circumference, body mass index, weight training, hypercholesterolemia, hypertriglycemia, diabetes were site-specifically associated with the BMD in TF, FN and LS in the adult men. These bone site-specific factors need to be considered for the prevention of osteoporosis.

면류 섭취 여부에 따른 영양소 섭취와 건강지표의 관련성 - 2010 국민건강영양조사 자료를 바탕으로 - (Comparison of Nutrient Intake and Health Indices by the Intake of Noodles - Using the Korea National Health and Nutrition Examination Survey (KNHANES, 2010) -)

  • The purpose of this study was to evaluate the nutrient intake, diversity and prevalence of diseases according to noodle consumption using data from the Korean National Health and Nutrition Examination Survey (KNHANES) of 2010. Korean adults (>20) were classified into two groups, noodle group (NG) and non-noodle group (NNG). NG was found to have higher intakes of protein, calcium, iron, sodium, potassium, and niacin. The DVS and DDS according to noodle consumption were found to be 21.76 and 3.51 for NG, and 18.56 and 3.40 for NNG, this result was thought to show that the diet of NG was more satisfactory than that of NNG in terms of dietary diversity. In the bone mass of entire subjects, NG showed higher figures in the femoral neck and lumbar spine than NNG. For bone mineral density, NG exhibited higher figures in the femoral neck, and for T-scores, the values of NG was significantly higher in the femoral neck than those of NNG. However, the prevalence of osteoporosis of NG and NNG was 11.6% and 9.5%. The prevalence of osteopenia of NG and NNG was 43.0% and 51.7%. Thus, these results showed that overall bone health of NG was better than that of NNG. Dietary balance and quality were also more satisfactory in NG. These results showed that noodle consumption may have an impact on the prevalence of chronic diseases.

Spinal Epidural Lipomatosis Secondary to Hypothyroidism in a Siberian Husky Dog

  • A 10-year-old neutered male Siberian Husky presented with paraparesis and severe lethargy. On physical examination, the patient was unable to weight-bear and walk and exhibited significant muscle mass loss in both hindlimbs and generalized truncal alopecia with a dull coat of hair. On neurological examination, cranial lumbar vertebral pain, hind limb cross-extensor reflex, delayed hindlimb postural reaction, upper motor neuron bladder dysfunction, and total absence of cutaneous trunci reflex were identified. Computed tomography revealed diffuse idiopathic skeletal hyperostosis and spondylosis deformans of the cervical and thoracolumbar vertebrae. In addition, a generalized decrease in bone mineral density of the vertebrae was identified. Magnetic resonance imaging showed hyperplasia of the epidural fat compressing the spinal cord in the thoracolumbar region and concurrent mild multiple intervertebral disc herniations. No specific findings were observed in cerebrospinal fluid analysis. Blood analysis of thyroid function revealed decreased total T4 and free T4 levels, and increased TSH levels. The patient was tentatively diagnosed with spinal epidural lipomatosis (SEL) secondary to hypothyroidism. The patient was treated with levothyroxine, firocoxib, and gabapentin. Clinical signs gradually improved, and the patient showed normal ambulation 40 days after treatment initiation. SEL is extremely rare in dogs. To the best of our knowledge, this is the first case report of SEL secondary to hypothyroidism that was treated conservatively. Secondary SEL can be sufficiently managed by treating the underlying cause, if possible.

성인의 골밀도와 잔존치아수와의 관련성 (Relationship between bone mineral density and the remaining teeth in Korean adults)

  • Objectives: The purpose of the study is to investigate the relationship between bone mineral density and remaining teeth in Korean adults. Methods: Using National Health and Nutrition Examination Survey(KNHANES) 5th Year 1(2010), 1,985 adults over 50 years old having bone mineral density test were selected. Male adults were 883, and female adults were 1,102. The raw data consisted of general characteristics of the subjects, bone density data, and oral examination data. The questionnaire consisted of general health survey, laboratory examination study, and nutrition study. Data were analyzed using SPSS 18.0 program for multiple regression analysis, ANOVA, t-test. The variables included general characteristics of the subjects, health behavior, diet habit, and oral health behavior. General characteristics consisted of gender, age, residence area, education level, marital status, monthly income, and vocation. Health behavior consisted of smoking, alcohol drinking, body mass index(BMI), physical activity, and mental health. Diet habit consisted of frequency of meal, milk, coffee, and calcium ingestion. Oral health behavior consisted of frequency of tooth brushing, use of oral health care devices, and oral examination. Systemic diseases were measured by the questionnaire for hypertension, diabetes mellitus, and hyperlipidemia. Bone density was measured in T-score of femoral bone, neck of femur, and lumbar vertebrae. Bone density was classified into normal($T-score{\geq}-1.0$), osteopenia(-2.5 < T-score < -1.0), and osteoporosis ($T-score{\leq}-2.5$). The remaining teeth were measured by the differences between normal teeth and missing teeth. Results: Smoking and age are the most influencing factors on the number of remaining teeth and bone density(p<0.01)(p<0.05). Those who had higher bone density, younger age, and nonsmoker significantly had more remaining teeth. Those who were men, diabetic, lowly educated women, and low birth weighted persons tended to have less number of teeth(p<0.01). Conclusions: The study suggested that there were significant differences between men and women according to the oral health management behavior and systemic disease control. Proper management of the oral health and good quality of bone mineral density can reserve the remaining teeth through the whole life.
