Lower Extremity Paralysis Developed during Pain Control in Lung Cancer Patient -A case report- (폐암 환자에서 통증치료중 발생한 하지 마비 -증례 보고-)

  • Continuous epidural analgesia has been used widely for chronic pain control, especially in cancer patients. As one of the complications, paraplegia developed during continuous epidural analgesia may be caused by epidural abscess, epidural hematoma, neural damage, chronic adhesive arachnoiditis, anterior spinal artery syndrome, delayed migration of extradural catheter into subdural space or subarachnoid space and preexisting disease. A 55-years-old male with lung cancer was implanted with continuous thoracic epidural catheter for pain control. Twenty days after catheterization, moderate back pain, weakness of lower extremity and urinary difficulty were developed. We suspected epidural abscess at first and made differential diagnosis with MRI which showed metastatic cancer at T2-4 spine, And compressed spinal cord was the main cause of the lower extremity paralysis.

Effects of Trunk Control Rehabilitation Robot Training on Dynamic Balance, Lower Extremity Strength, Gait Ability and Pain in Bipolar Hemiarthroplasty

  • Purpose: This study examined the effects of trunk control rehabilitation robot training (TCRRT) on the dynamic balance, lower extremity strength, gait ability and pain for bipolar hemiarthroplasty. Methods: Hemiarthroplasty (n=28) patients participated in this study. The subjects were randomized into two groups: trunk control rehabilitation robot training group and control group. Results: The TCRRT group showed significantly more improvement in the MFRT, MMT, 10MWT, TUG, and VAS compared to that before intervention (p<0.05). In addition, all tests were significantly greater in the experimental group than in the control group. Conclusion: These results suggest that TCRRT is feasible and effective for improving the dynamic balance, lower extremity strength, gait ability, and pain efficacy after bipolar hemiarthroplasty.

Relationships among Pain, Upper Extremity Function, and Anxiety in the Breast Cancer Survivors (유방암 생존자의 상지기능, 통증 및 불안과의 관계)

  • Purpose: This study was to compare pain, upper extremity function, and anxiety among disease characteristics in the breast cancer survivors and to clarify the relationship among these variables. Methods: One hundred twenty two participants with breast cancer survivors over the age of 30 were recruited from a general hospital. Data were collected from November 1 to December 25, 2006 using a structured questionnaire. Results: The mean age was 51.17 and their mean survival period was 38.08 months. The breast cancer survivors who had received radiation therapy reported lower levels of pain and upper extremity function, and higher levels of anxiety than those who had other treatments. Pain and anxiety were positively related, and upper extremity function was negatively related to pain and anxiety. Conclusion: The breast cancer survivors experienced pain, upper extremity function disorder and anxiety. This study indicates that nursing interventions for the breast cancer survivors may be needed to improve upper extremity function, and to reduce pain and anxiety.

A case report of patient who has spinal cord infarction with monoplegia and pain of lower extremity and dysuria (하지 단마비와 통증 및 배뇨장애를 주소로 하는 척수경색환자의 한방치료 치험 1례)

  • Objective : The purpose of this study is to report the improvement after oriental medical treatment about a patient who has spinal cord infarction with monoplegia and pain of lower extremity and dysuria. Methods : We diagnosed that symptoms as Wejeung, and employed oriental medical treatment, acupuncture, herbal medicine and moxibustion. This study was measured by MMT(Manual Muscle Testing), VAS(Visual Analogue Scale) and the number of Nelaton Catherization. Results : After oriental medical treatment, power and pain of lower extremity and dysuria were improved. Conclusions : There is no specific treatment for spinal cord infarction in western medicine and the prognosis is not so good. But in the case of this patient, power and pain of lower extremity and dysuria were improved remarkably through oriental medical treatment. Further clinical studies are needed to verify the findings.

Effects of McConnell Taping and Kinesio Taping on Pain and Lower Extremity Joint Angles During Stair Ascent in People with Patellofemoral Pain Syndrome (McConnell 테이핑과 Kinesio 테이핑이 무릎넙다리통증증후군 환자의 계단 올라가기 시 통증과 다리관절 각도에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effect of McConnell taping and Kinesio taping on pain and lower extremity joint angles when patients with patellofemoral pain syndrome (PFPS) ascend stairs. Methods: Fifty young adults who were experiencing anterior knee pain due to PFPS were selected as participants. Then, 25 patients were randomly assigned to the McConnell taping group and 25 to the Kinesio taping group. Pain and lower extremity joint angle were measured while ascending stairs before and after the intervention. A paired t-test was performed to evaluate the amount of change in the parameter values after the intervention within the groups, and an independent t-test was used to compare the results of the groups. Results: In the within-group comparisons, a statistically significant difference was found in both groups between the anterior knee pain scale scores recorded before and after the intervention (p < 0.05). A statistically significant difference was also found between the groups (p < 0.05). Comparison of the lower extremity joint angles at initial contact, loading response, terminal stance, and pre-swing within the groups showed that there were statistically significant differences in the hip, knee flexion, abduction, lateral rotation, and dorsiflexion angles in both the McConnell and Kinesio taping groups (p < 0.05). There was also a statistically significant difference in all angles between the groups during the following events (p < 0.05): (1) at initial contact, (2) at loading response (except hip flexion angle), (3) at terminal stance (except hip flexion and lateral rotation angles), and (4) at pre-swing (except hip, knee abduction, and inversion angles). Conclusion: McConnell taping and Kinesio taping both effectively improved the occurrence of knee pain and the lower extremity joint angles during stair ascent in patients with PFPS. However, McConnell taping had a significant impact on pain reduction and lower extremity joint angles compared to Kinesio taping.

Effects of the Application of Elastic Compression Stockings on Edema and Pain of Lower Extremity in Hospital Nurses (근무 중 탄력압박스타킹 착용이 간호사의 하지부종 및 통증에 미치는 효과)

  • Purpose: This study was to identify the effects of the application of elastic compression stockings during the hours of work on edema and pain of lower extremity in hospital nurses. Method: The participants were categorized into three groups, 15 for an experimental group of the application of Thigh-Length (T-L) elastic compression stocking, 15 for another experimental group of the application of Knee-Length(K-L) elastic compression stocking and 15 for a control group. The data were analyzed by $X^{2}-test$, ANOVA, Repeated Measures ANOVA and Sheffe-test using SPSS Win 12.0 program. Result: As a result of the study "There are significant differences in lower extremity edema among three groups." was partly supported. "There are significant differences in girth of ankle after work among three groups." was supported(Rt.:F=4.56, p=.016; Lt.: F=5.82, p=.006). "There are significant differences in girth of calf after work among three groups." was not supported(Rt. : F=1.76, p=.183; Lt. : F=1.94, p=.155) "There are significant differences in lower extremity pain after work among three groups." was supported(Rt. : F=95.33, p=.000; Lt. : F=81.33, p=.000). Conclusion: The results of this study revealed that the application of elastic compression stockings during the hours of work is effective for the relief of edema and pain of lower extremity in hospital nurses.

The Effects of a Progressive Lower-extremity Exercise Program on Pain, Self-efficacy of Exercise, and Life Satisfaction among Older Women with Total Knee Replacement Arthroplasty (TKRA) (슬관절 전치환술 후 점진적 하지운동요법이 통증, 운동자기효능감, 삶의 만족도에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effect of a progressive lower-extremity exercise program on pain, efficacy of exercise, and the life satisfaction among older women with TKRA. Methods: This study was a nonequivalent control group non synchronized design and was conducted from 12 September 2012 to 05 February 2014 in G-university hospital located in J-city. The sample was composed of an experimental group (n=20) and a control group (n=20). The progressive lower extremity exercise program was applied to the experimental group for 13 days. NRS scale was used to measure the pain, self-efficacy of exercise was measured by Exercise Self Efficacy, and life satisfaction was measured by 8 item questionnaire. Results: The experimental group was significantly lower in the degree of pain (F=73.53, p<.001), higher in the degree of self-efficacy of exercise (F=61.42, p<.001) and life satisfaction (F=80.91, p<.001) than the control group. Conclusion: The progressive lower-extremity exercise program for TKRA patients was useful to reduce pain and improve self-efficacy of exercise and life satisfaction, To reduce the pain and th improve the life satisfaction of older women with TKRA, we need to provide the progressive exercise at bedside during the postoperative recovery phase.

The Effect of Lumbar Sympathectomy Using Radiofrequency Thermocoagulation in Patients with Buerger's Disease -A case report- (Buerger병 환자에서 고주파 열응고술을 이용한 요부 교감신경절 절제술의 효과 -증례 보고-)

  • Buerger's disease is a nonatherosclerotic occlusive inflammatory disease of the small and medium arteries, and veins of the distal leg or arm. Percutaneous lumbar sympathectomy is used to lower extremity occlusive vascular disease as well as Buerger's disease. Lumbar sympathectomy improves blood flow and provides pain relief in the lower extremity. We report two cases of lumbar sympathectomy using radiofrequency thermocoagulation in patients with Buerger's disease. After no paresthesia and muscle contracture at 50 Hz, 1 volt and 2 Hz, 3 volts, respectively, radiofrequency lesioning was performed for 90 sec at $80^{\circ}C$. After the procedure, both patients showed skin temperature increases greater than $2^{\circ}C$ on the affected extremity. Both patients received relief from pain and symptoms without complications. We consider that lumbar sympathectomy using radiofrequency thermocoagulation is a safe and effective procedure that can relieve pain in patients with Buerger's disease.

  • Continuous epidural analgesia has been used widely for treatment of acute abdominal postoperative pain. Neurologic complications related to epidural analgesia occur infrequently but may be caused by various chemical, mechanical, ischemic or idiopathic factors. We report a case of abdominal hysterectomy in which unilateral lower extremity paralysis occurred after continuous epidural analgesia.

Comparison of Hip Muscle Strengthening Exercise and Stretching of Lower Extremity on Pain, Navicular Drop and Foot and Ankle Functional Ability in Plantar Fasciitis (엉덩근육 강화 운동과 하지 스트레칭이 족저근막염 환자의 통증, 주상골 하강 및 발과 발목의 기능적 능력에 미치는 효과 비교 연구)

  • Background: Plantar fasciitis is one of the most common conditions of the lower limbs. The present study aimed to compare the effect of hip muscle strengthening exercise and stretching of lower extremity in plantar fasciitis, with regard to pain, navicular drop and foot and ankle functional ability. Methods: A total of 42 patients diagnosed with plantar fasciitis were randomly assigned to the experimental group (n=21) that applied the hip muscle strengthening exercise and the control group(n=21) that applied the general lower limb stretching. The intervention period was 6 weeks. Results: The results of the experiment showed that the exercises for strengthening the hip muscle had significant effects on pain, navicular drop and foot and ankle functional ability In addition, lower extremity stretching showed improvement effects in pain, navicular drop and foot and ankle functional ability, however, the difference between the groups except navicular drop did not reach statistical significance Conclusion: Both interventions tested in this study were confirmed to be effective treatment options for patients with plantar fasciitis.
