Effects of The Home Physical Therapy on Recovery of Muscle Strength and Gait for Discharged Patients Who Underwent Total Knee Replacement

  • This study aims to identify the effect of home physical therapy on patients who left the hospital after total knee arthroplasty, compared to the Daily living group, when it was applied to them, under the guidance of physical therapists. As research subjects, 20 patients that were scheduled to leave the hospital after unilateral total knee arthroplasty, were chosen, and they were randomly divided into a home physical therapy group(10 patients) and an Daily living group(10 patients) in order to conduct an experiment. During the 4-week research, home physical therapy was offered for 40 mins once for 5 days a week, and muscular strength, gait components were measured. For muscular strength, quadriceps muscle strength, hamstring muscle strength were measured, and as gait components, endurance, speed, step time, single-limb support were analysed. For this experiment, pre- and post-measurement were performed, and collected data were analyzed using SPSS ver. 18.0 statistical program. From the analysis of data, the following study results were obtained. Home physical therapy group and Daily living group both showed significant improvements in quadriceps muscle strength, hamstring muscle strength, and quadriceps muscle strength, hamstring muscle strength of home physical therapy group more significantly improved than Daily living group's. In relation to gait components, gait endurance, gait speed, step time significantly improved in both of home physical therapy group and Daily living group, whereas home physical therapy group only showed significant improvements in single-limp support. According to the comparison between two groups, gait speed, single-limp support and step time improved more significantly in the home physical therapy group than in the Daily living group. In conclusion, positive results were revealed in both home physical therapy group and Daily living group, with regard to muscular recovery of lower limbs of patients discharged from the hospital after total knee arthroplasty and walking, but considering the comparison results between two groups, it seems that applying home physical therapy is more effective than maintaining a daily life.

Assessment of Congruence Angle according to the Central X-ray in the Merchant View of Patellofemoral Joint (슬대퇴관절의 Merchant View에서 중심 X선 위치에 따른 일치각 평가)

  • The Precise measurement of the knee's Congruence angle is required for diagnosis of patella dislocation. The purpose of this study is to consider the distortion diagram and usefulness about the test of the bilateral side and one side through the evaluating congruence angle from Merchant view of patellofemoral joint to central X-ray. We've evaluated the sulcus angle and Congruence angle following central X-ray's changes when we took Merchant view of patellofemoral joint aimed to right lower limb from human whole body phantom. The subject of the evaluation was classified as normal group and varus group, and the varus group has experimented with External rotation of legs as 15° and 30°. When normal groups result as 0, 7.5, 15 cm, it was measured as 17.25 ± 1.34°, 19.60 ± 1.41°, 20.55 ± 1.77° each. The gap between minimal and maximum angle was 3.3°, and the value was shown as getting bigger when it further away from the centeral X-ray. When Congruence angle in 15° varus group was 0, 7.5, 15 cm, it was each 16.45 ± 1.34°, 17.10 ± 0.99°, 17.80 ± 1.13°. And when Congruence angle in 30° varus group was 0, 7.5, 15 cm, it was measured each 18.35 ± 1.63°, 18.95 ± 1.06°, 19.60 ± 1.41°. The difference between minimum and maximum of angle in 15° varus group and 30° varus group was each 1.35° and 1.25°, the angles have shown as increasing the further away from the center. The patellofemoral joint showed that the congruence angle increases as the further away from Central X-ray, and also it is judged that is possible to change the congruence angle by a degree of varus knee. Thus, accurate measurement of the congruence angle is shown to advantage that methods for examining by split each side at two times than methods for examining both sides at a time. Therefore, it is expected to helpful on the diagnostic side of patella dislocation and subluxation.

First Report of Myiasis Caused by Cochliomyia hominivorax (Diptera: Calliphoridae) in a Diabetic Foot Ulcer Patient in Argentina

  • Myiasis is usually caused by flies of the Calliphoridae family, and Cochliomyia hominivorax is the etiological agent most frequently found in myiasis. The first case of myiasis in a diabetic foot of a 54-year-old male patient in Argentina is reported. The patient attended the hospital of the capital city of Tucum$\acute{a} $n Province for a consultation concerning an ulcer in his right foot, where the larval specimens were found. The identification of the immature larvae was based on their morphological characters, such as the cylindrical, segmented, white yellow-coloured body and tracheas with strong pigmentation. The larvae were removed, and the patient was treated with antibiotics. The larvae were reared until the adults were obtained. The adults were identified by the setose basal vein in the upper surface of the wing, denuded lower surface of the wing, short and reduced palps, and parafrontalia with black hairs outside the front row of setae. The main factor that favoured the development of myiasis is due to diabetes, which caused a loss of sensibility in the limb that resulted in late consultation. Moreover, the poor personal hygiene attracted the flies, and the foul-smelling discharge from the wound favoured the female's oviposition. There is a need to implement a program for prevention of myiasis, in which the population is made aware not only of the importance of good personal hygiene and home sanitation but also of the degree of implication of flies in the occurrence and development of this disease.

Open Source-Based Surgical Navigation for Fracture Reduction of Lower Limb (오픈소스 기반 수술항법장치의 하지 골절수술 응용검토)

  • Minimally invasive intramedullary nail insertion or plate osteosynthesis has shown good results for the treatment of long bone fractures. However, directly seeing the fracture site is impossible; surgeons can only confirm bone fragments through a fluoroscopic imaging system. The narrow field of view of the equipment causes malalignment of the fracture reduction, and radiation exposure to medical staff is inevitable. This paper suggests two methods to solve these problems: surgical navigation using 3D models reconstructed from computed tomography (CT) images to show the real positions of bone fragments and estimating the rotational angle of proximal bone fragments from 2D fluoroscopic images. The suggested methods were implemented using open-source code or software and evaluated using a model bone. The registration error was about 2 mm with surgical navigation, and the rotation estimation software could discern differences of $2.5^{\circ}$ within a range of $15^{\circ}$ through a comparison with the image of a normal bone.

Effects of Controlled Photoperiod on Body Development in Growing Juvenile Rats

  • Melatonin is induced by light information through the retina and leads to growth factor activation. Thus, we investigated the effects of melatonin by controlling the photoperiod of growing young rats. Male Sprague-Dawley rats (n=6; 4 weeks old) were divided into two experimental groups: the L/D group (normal photoperiod; light/dark: 12/12 h; lights on at 9:00 a.m.) and the L/L group (light/light: 24 h). Rat body weight and food consumption were measured daily for 8 weeks. After 8 weeks, the rats were anesthetized with a mixture of ketamine (50 mg/kg) and xylazine (10 mg/kg) and sacrificed. Tissue was then collected for RNA isolation (from brain, heart, liver, kidney, adrenal gland, testis, tibia, hind limb muscles). Also, serum was isolated from blood using a centrifugal separation. The L/L group had significantly lower body weight than the L/D group from 4 to 6 weeks (p<0.05). The L/D group had increased tissue mass, compared with the L/L group, but the difference was not statistically significant. The L/D group had a significantly higher melatonin concentration than the L/L group between the hours of midnight and 2:00 a.m (p<0.01). These results indicate that photoperiod length may affect the secretion of melatonin from the pineal gland. Also, the reduction of nocturnal melatonin secretion may retard the development of growing young rats. In future studies, we plan to compare exogenous melatonin administration with endogenous melatonin concentration induced by photoperiod control. Moreover, we will confirm whether the effects seen in pathological animal models can be reversed by controlling the photoperiod.

Development of Closed-loop Control Type FES System for Restoration of Gait in Patients with Foot Drop (족하수 환자의 보행보조를 위한 피드백 제어형 전기자극기 개발)

  • The purpose of this study was to develop a portable and convenient closed-loop contrel type electrical stimulator for patients with foot drop. This system restores walking movement as well as prevents from atrophy or necrosis of lower limb muscles and increases blood circulation in hemiplegic patients caused by traffic accident, industrial disaster or stoke. This system detects the changes of the ankle joint angle during walking, and then controls the stimulus intensity automatically to maintain the programmed level of the ankle joint angle. Also, this automatic system controls the stimulus intensity which is affected by increased electrode impedance resulting from long time use. The system detects the joint angle by an optical sensor and includes modified PID control which adjusts the stimulus intensity if the joint angle deviates from the preset value. Stimulus parameters are 30~80 volt, 40 Hz, and 0.2 ms. The system was applied to five hemiplegic patients for 42 days. Duration of stimulation was 15 min/day for the first week and then the duration was gradually increased to 30, 60, 90 and 120 min/day. The muscle force was increased up to 29.7%, muscle fatigue was decreased compared with the level before stimulation and the pattern of locomotion was improved. These results suggest that the electrical stimulator with closed-loop control type is more convenient and effective in restoration of locomotion of patients with foot drop than open-loop system.

  • Our conclusions for function evaluation tools of Stroke patients are as follows. 1. Evaluating tools of Activities of Daily Living, Katz Index, Barthel Index, Modified Barthel Index have high validity and reliability because of ease of measuring, high accuracy, consistency, sensitivity and sufficient stastistics, but they mainly measure motor function except sense, mentation, language, and social conception. Therefore cerebrovascular disease and brain injury in trauma patients with lacked acknowledgement and sensation, we are not able to apply these tools. 2. PULSES Profile is a useful scale for measuring the patient's over-all status, upper and lower limb functions, sensory components, excretary functions, and intellectual and emotional adaptabilities. It is recognized as a good, useful tool to evaluate patient's whole function. 3. Motor Assessment Scale was designed to measure the progress of stroke patients. The scale was supplemented with upper arm function items. We believe that the Motor Assessment Scale could be a useful evaluation tool with inter-rater reliability ,test-retest reliability. 4. The existing evaluation tools, Katz Index, Barthel Index, Modified Barthel Index, PULSES Profile, Motor Assessment Scale, mainly measured the rehabilitational motor function of sequela of cerebrovascular patients. On the other hand CNS & INH stroke scale can measure cerebrovascular disease patient's neurologic deficits and over-all stautus, which are recognition ability, speech status, motor function, sensory function, activities of daily living. Those scales have been recognized as useful tools to measure function of cerebrovascular disease patients and have increased in use. 5. Every function evaluation tool was recognized to have some validity and inter-rater, test-retest reliability in items of each evaluation tool and total scores of each evaluation tools, but it is thought that none of these scales have been fully validated and proved reliable. Therefore afterward, the development of a highly reliable rating system may best be accomplished by a careful comparison of several tools, using the same patients and the same observers in order to choose the most reliable items from each. 6. Ideal evaluation tools must have the following conditions; (1) It should show the objective functional statues at the same time. (2) It should be repeated consecutively to know changed function status. (3) It should be easy to observe the treatment program. (4) It should have the same result with another rater to help rater exchange information with treatment team members. (5) It should be practical and simple. (6) The patient should not suffer from the observer.

The Comparative Study between Leg Length Analysis and X-ray on Diagnosis of Pelvic Malpositions - according to Positions and Valuation Bases - (기능성 족지장단분석과 X-ray의 골반변위 진단의 비교 연구 - X-ray 촬영 자세 및 평가 기준에 따라 -)

  • Objectives: This study was carried out to investigate the relationship of leg length analysis and X-ray finding according to positions and valuation bases on diagnosis of pelvic malpositions. Methods: Twenty-two people who get $33.09{\pm}10.73$ as average were evaluated by leg length analysis and X-ray findings. After measuring innominate measurement(IM), femur head line. distance between S2 and posterior superior iliac spine(PSIS). ilium shadow measurement(ISM), major axis of obturator foramen(MaF), minor axis of obturator foramen(MiF) and distance between off centering measurement and symphysis pubis, those were analyzed. Results: 1. In assessment of posterior rotation malposition ilium(PI), it was showed the best coincidence between leg length analysis and X-ray analyzed by 1M in supine position(11 cases, 50.5%). 2. In assessment of inflare, coincidence index between leg length analysis and X-ray were not good($4.00{\pm}3.03$ cases, $18.15{\pm}13.82%$). And leg length analysis were not sensitive. 3. On the whole, coincidence index between leg length analysis and X-ray were not good(best: 1 case, 45.5%; worst: 11 cases, 50.0%). Conclusions: Results form this investigation showed the relationship of leg length analysis and X-ray according 10 positions and valuation bases on diagnosis of pelvic malpositions. This results are expected to contribute to establish method of assessment in diagnosis of pelvic malpositions.

The Immediately Effect of Narrow Squats on the Knee Joint Biomechanics During a Gait and Distance Between the Knees of Person With Genu-varum (내로우 스쿼트 운동이 내반슬 성인의 무릎 사이 거리와 보행 시 무릎 관절의 생체역학에 미치는 즉각적인 영향)

  • Background: Genu varum is also known as bow leg. It is a deformity wherein there is lateral bowing of the legs at the knee. it does give rise to pain, and persistent bowing can often give rise to discomfort in knees, hips and ankles. Objects: This study investigated the effect of narrow squats on the knee joint during a gait and distance between the knees of person with genu varum. Methods: This study analyzed 23 patient with genu varum that grade III, 12 narrow squat group and 11 genenal squat group in motion analysis laboratory. The subjects of experiment took gait before and after intervention, the range of joint motion, moment of knee joint adduction, power, distance of the knees were measured. And in order to make an analysis between groups, an paiered t-test and independent t-test was carried out. For statistical significance testing, it was decided that significance level ${\alpha}$ be .05. Results: It was shown that the group of narrow squat exercise significantly decreased in distance of knees (p<.05),In moment of adduction of knee joint, it was shown to significantly decrease in two groups (p<.05), was significantly decreased in adduction, abduction, and rotation (p<.05). In relation of peak-knee adduction moment and valgus angle, there was significant decrease in narrow squat group (p<.05). Conclusion: When the above result of study were examined, a narrow squat exercise given to the genu varum patients significantly decreased the distance between the knees, range of knee adduction and abduction, knee adduction moment, knee power. And stability gains through the decrease of excursion of knee medial part be effective for the correction of genu varum deformation.

Effect of Electro-acupuncture on Regional Fat by Measuring Thigh Circumference (저주파 전침자극이 허벅지 둘레에 미치는 영향에 대한 연구)

  • Objectives: Although electro-acupuncture is being utilized to reduce weight in Korea, its usefulness in obesity management has not been fully evaluated yet. The objective of this study is to evaluate the effect of Electro-acupuncture on thigh circumference. Methods: 12 healthy volunteers participated in this study. We measured the circumference of both thighs, and treated the thicker one with transcutaneous electro-acupuncture at 8 acupoints, 10 times in total. The electrical parameters were : 1st step frequency set was at 15Hz and the treatment proceeded for 50 minutes, 2nd step at 50Hz for 10 minutes and at 3rd step the acupunctures were removed and electrodes were attached on the lower limb at 166Hz for 15 minutes. In order to evaluate the effect of the treatment, total body weight, percentage of body fat and thigh circumference were measured 4 times. Before the treatment, after 5th and 10th treatment and 1 month after completion of the whole treating procedure. Statistical significance(P<0.05) was determined using the Wilcoxon signed rank test. We did not control diet, exercise and lifestyle of the volunteers. Results: No significant change was shown in body weight after 10 times of treatment. Yet the thigh circumference of effect group significantly decreased after 5th treatment and 10th treatment. The circumference of control group that was not treated by electro-acupuncture significantly decreased too. At first, difference between the two thighs in circumference was obvious, but no difference was left after 10 times of treatments on the thicker thigh. Percentage of body fat was significantly decreased. 1 month after having completed the treatment, there was no significant difference of body weight, thigh circumferences and percentage of body fat. Conclusion: These results suggest that electro-acupuncture effectively regulates regional fat, body fat percentage and body shape.
