Effects of Robotic Gait Training with Lower Extremity Restraint on Static Balance, Lower Extremity Function, Gait Ability in Subacute Stroke Patients

  • Objective: The purpose of this study is to compare the effect of static balance, lower extremity function, and gait ability between a lower extremity restrain robot gait training and a general robot gait training in subacute stroke subjects. Design: Two-group pretest-posttest design. Methods: A total of 12 subacute stroke patients were randomly divided into an Experimental group (n=6) and a control group (n=6). Both groups were performed for four weeks, three times a week, for 20 minutes. To compare the Static balance function, the center of pressure (COP) path-length and COP velocity were measured. The Fugl-Meyer assessment lower extremity (FMA-LE) were evaluated to compare the Lower Extremity function. 2D Dartfish Program and 10 Meter Walking Test (10 MWT) on Gait ability were evaluated to compare the gait function. Results: In the intra-group comparison, Experimental groups showed significant improvement in COP path-length, velocity, Lower Extremity Function, 10 MWT, Cadence, by comparing the parameters before and after the intervention (p<0.05). Comparison of the amount of change between groups revealed significant improvement for parameters in the COP path-length, velocity, Lower extremity function, 10 MWT by comparing the parameters before and after the intervention (p<0.01). Conclusions: The Experimental group showed enhanced efficacy for variables such as COP path-length, velocity, Lower extremity function, 10 MWT as compared to the control group.

The Effect of Swiss Ball Program on Lower Extremity Function of Elderly with Mild Cognitive Impairment

  • The purpose of this study was to determine the effects of swiss ball program training on lower extremity function of old elderly with mild cognitive impairment. The subjects were 30 mild cognitive impairment eldrly people aged between 65 and 80, who were divided into the swiss ball program training group(n=15) and the control group(n=15). The swiss ball program training group engaged in a 50-minute exercise session using. Lower extremity function scale of the two groups were measured before and after the intervention. The results are as follows. According to the swiss ball program training conducted to examine the effects of the training on lower extremity function, Moreover, the two groups showed significant differences in lower extremity function. These results indicate that virtual reality training is effective in improving lower extremity function. To conclusion, swiss ball program was found to have a positive effect on elderly lower extremity function. Swiss ball program training can be proposed as a form of fall prevention exercise for the mild cognitive impairment. Swiss ball program may be helpful to reduce the incidence of dementia and behavioral complications.

Effects of Mat Pilates Exercise on Lower Extremity Function, Postural Balance, and Walking in the Older Women with Total Knee Arthroplasty

  • Objective: The rehabilitation protocols for functional recovery have been emphasized after total knee arthroplasty, and Pilates is in the spotlight as a safe and easily modified exercise method. The purpose of this study was to investigate the effects of mat Pilates exercise on lower extremity function, postural balance, and walking in the individuals with total knee arthroplasty. Design: One group pretest-posttest design. Methods: Eighteen older women with unilateral total knee arthroplasty was recruited in the study. The subjects were evaluated on lower extremity function, postural balance, and walking before and after mat Pilates exercise. All subject performed one hour mat Pilates exercise, 3 times a week for 8 weeks. Mat Pilates exercises were focused on core stability and lower extremity strengthening and, more dynamic movements were added to increase the difficulty of movements every two weeks. The lower extremity function was measured using the Western Ontario and McMaster Osteoarthritis Index (WOMAC), knee joint position sense, and five times sit-to-stand test. Postural balance was assessed by single leg stance test, functional reach test, and timed up and go test. Walking was measured by OptoGait system to temporospatial parameter. Results: The lower extremity function, postural balance, and walking were significantly improved after mat Pilates exercise, except for five times sit-to-stand test (p<0.05). Conclusions: This study demonstrated that the mat Pilates exercise was a useful method to improve lower extremity function, postural balance, and walking in the older women with unilateral total knee arthroplasty.

  • 본 연구는 뇌졸중 환자에게 하지 근력강화 프로그램이 균형, 보행 및 상지 기능에 미치는 효과에 대해서 알아보았다. 신경계 손상 환자가 있는 병원을 기반으로 해서 하지 근력 강화에 대한 연구 방법을 만들어서 진행하였고, 무작위대조시험을 적용하였다. 24명의 뇌졸중 환자를 두 군으로 분류하였다. 하지 근력 강화 프로그램그룹 (Lower extremity strengthening program group, LESPG) (12명)과 트레드밀 훈련그룹 (Treadmill trainig group, TTG) (12명)으로 실험하였다. LESPG는 마비측에 하지 근력 강화프로그램을 수행하였다. TTG는 하루 30분 동안 트레드밀 운동을 적용하였다. 평가 도구는 일어나서 보행 검사(Timed Up and Go test, TUG), 기능적 보행평가 (Functional Gait Assessment, FGA) 및 뇌졸중 상지 기능검사(Manual Function Test, MFT)를 적용하였다. 균형, 보행 및 상지 기능에 대한 두 군 간의 TUG, FGA 및 MFT 점수는 LESPG가 TTG보다 유의한 차이가 있었다(p<.01). 4주간의 하지 근력강화 프로그램의 효과는 뇌졸중 후 편마비 환자의 균형, 보행 및 상지 기능에 효과가 있었다.

  • Purpose: The purpose of this study was to confirm whether the Swiss Ball exercise program is effective to improve lower extremity function and activity of daily living of elderly women with mild cognitive impairment (MCI). Methods: This study was a double blind control study. Subjects participated 34 female elderly women who were assigned to exercise group (n=17) and control group (n=17). The Swiss Ball exercise program was performed 2 times a week during the 12 weeks. Pre-and post-exercise various measurements were made: lower extremity function (OLS, TUG, STS). Results: In exercise group, There showed statically significance improve between pre-and post-exercise in OLS (7.29${\pm}$1.10 and 7.64${\pm}$1.32), TUG (10.47${\pm}$2.03 and 10.05${\pm}$2.04), STS (7.71${\pm}$1.04 and 7.94${\pm}$.82)(p<.05). ADL also showed statically significance improve between pre-and post-exercise (13.76${\pm}$3.54 and 12.82${\pm}$3.39, p<.05). There showed statically significance difference between two groups (p<.05). Conclusion: The Swiss Ball exercise program improved lower extremity function and ADL of elderly women with MCI. Further studies are required to examine the significance of the assessment of motor function of lower extremities.

The Immediate Effect of Static and Dynamic Stretching on Flexibility of Hamstring, Dynamic Balance Ability, Function of Lower Extremity: Randomized Controlled Trial

  • Purpose: This study examined the acute effects of static and dynamic stretching on the flexibility of the hamstring, dynamic balance ability, and function of the lower extremities in healthy adults. Methods: Thirty participants were assigned randomly to three groups: static stretching group (SSG), basic dynamic stretching group (BDSG), and 5 sec dynamic stretching group (5DSG). SSG performed three sets of 30 seconds of static stretching. BDSG performed a single-leg deadlift (SLD) at the same time as SSG, and 5DSG performed SLD held for five seconds. Hamstring flexibility, dynamic balance ability, and lower extremity function were evaluated before and after intervention. Results: The hamstring flexibility significantly improved in all groups (p<0.05). The dynamic balance ability improved significantly after intervention in all groups except BDSG in the anterior direction (p<0.05). There were significant differences in the posterolateral and posteromedial direction in all groups. The function of the lower extremity showed significant improvement over time only in BDSG (p<0.05). Conclusion: This study suggested that basic dynamic stretching and 5 sec dynamic stretching positively affect the hamstring flexibility, dynamic balance ability, and lower extremity function. Therefore, it is recommended to include dynamic stretching in a program for improving the hamstring flexibility, dynamic balance ability, and the lower extremity function. In addition, it is recommended to apply it at different times depending on the purpose.

경직형 뇌성마비 아동의 신체기능과 하지 촉각의 상관관계 연구 (The Correlation between Physical Function and Lower Limb Tactile Sense in Children with Spastic Cerebral Palsy)

  • Purpose: The purpose of this study was to investigate tactile sense perception of the lower extremities according to physical function in children with spastic cerebral palsy. Methods: This study was conducted on 15 children diagnosed with spastic cerebral palsy. Physical function measurement items included dynamic balance ability, gross motor function level, and lower extremity ankle spasticity. The lower extremity tactile sensation uses a monofilament to measure the sole of the first metatarsal head, the sole of the fifth metatarsal head, the heel, the anterior part of the shin midway between the patella and the ankle joint, the kneecap, the upper anterior iliac spine, and the knee. A total of six measurements were taken in the mid-femoral region of the bone. Spearman correlation analysis was performed to determine the degree of body function and lower extremity tactile perception. Results: As the physical function of children with spastic cerebral palsy deteriorated, there was a decrease in tactile sensation in the thigh area corresponding to the proximal lower extremity. (p <.05). Conclusion: Children with spastic cerebral palsy and poor physical function have sensory loss not only in the distal part but also in the proximal part, so a treatment approach that recognizes and improves it is necessary.

고유수용성신경근촉진법이 만성 뇌졸중환자의 하지기능에 미치는 효과 (The Effects of Proprioceptive Neuromuscular Facilitation on Lower Extremity Functions of Chronic Stroke Patients)

  • Purpose: The purpose of this study is to determine the effects of proprioceptive neuromuscular facilitation on the lower extremity function of chronic stroke patients. Methods: The participants consist of 26 chronic stroke patients. They were randomly assigned to either an experimental group (n=13) or a control group (n=13) and engaged in exercise three times per week for eight weeks. The experimental group engaged in proprioceptive neuromuscular facilitation exercises and the control group engaged in general mat exercises. To measure lower extremity strength, the sit to stand test and the static balance test (standing on one leg) were used. To measure the stability index (SI) and weight distribution index (WDI), a Tetrax Portable Multiple System (Tetrax Ltd, Israel) was used in addition to a static balance test (standing on one leg). Results: : Lower extremity strength and static balance showed a significant improvement in the experimental group (p<.05). The SI and WDI were significantly improved in the experimental group (p<.05) for tests of standing with their eyes open, standing with their eyes closed, and standing on a sponge with their eyes open. Conclusion: Proprioceptive neuromuscular facilitation exercises have confirmed that lower extremity function improves after exercise interventions in chronic stroke patients.

뇌졸중 환자의 보행 비대칭성과 속도, 하지 운동 기능과의 상관관계 (The Relationships among Gait Asymmetry, the Gait Velocity and Motor Function of Lower Extremity in Stroke Patients)

  • Purpose : The present study was to examine the difference and severity of asymmetry in independently ambulating stroke survivors and to establish the association between gait asymmetry, velocity, and the motor function of lower extremity. Methods : The subjects used in this study were 43 subjects with hemiparesis being able to walk independently. Motor function of lower extremity was measured clinically with the Fugl Meyer-Lower /Extremity Assessment. Overground gait velocity and spatia-temporal parameters were collected by the GAITRite system. Results : Thirty(69.77%) patients showed statistically significant temporal asymmetry while 28(65.1%) exhibited statistically significant spatial asymmetry. One-way ANOVA results showed a main effect of temporal asymmetry group(normative, mild, severe) for gait velocity(F=74.129), FM-L/E(F=17.270), swing-stance symmetry(F=66.869, F=13.485, respectively), spatio-temporal asymmetry(F=13.166, F=31.800, respectively) 66, F=31.800, respectively). Gait velocity was negatively associated with temporal asymmetry(r=-.83), spatial asymmetry(r=-.60). Motor function of lower extremity was also associated with temporal asymmetry(r=-.58), and spatial asymmetry(r=-.50). Conclusion : The study attempted to establish the standard assessment of hemiparesis gait symmetry in light of the complex relationship with motor impairment and gait velocity. More future work will need to link the degree of gait asymmetry to clinically relevant outcomes to better establish the clinical significance of such observations.

슬개대퇴통증 증후군 환자에게 적용한 고관절 견인과 근력 강화 운동이 무릎 통증, 관절가동범위, 하지기능에 미치는 영향-사례 연구 (A Case Study of a Female Patient with Patellofemoral Pain Syndrome for Effect of Hip Joint Traction and Hip Posterolateral Muscles Strengthening on Knee Pain, Range of Motion, and Lower Extremity Function Scale)

  • Background: The case study examined the effect of a hip joint traction and hip posterolateral muscles strengthening on knee pain, range of motion, and lower extremity function scale of patients with patellofemoral pain syndrome (PFPS). Although PFPS has previously been attributed to quadriceps dysfunction, more recent research has linked this condition to impairment of the hip musculature and kinematic. Methods: Subject is a 27-years-old female with PFPS. Performed hip joint traction with belt and posterolateral muscles(hip abductors, external rotators) strengthening for 4 weeks, 3 times a week, once a day. Before and after the therapy, measurements were made on the visual analog scale (VAS) and of the ROM, and a lower extremity functional scale (LEFS) was conducted. Results: The results showed positive changes in VAS and range of motion and lower extremity functional scale. First VAS of knee changed from 6 to 2. Second hip joint range of motion showed that internal rotation recored from $53^{\circ}$ to $58^{\circ}$ and external rotation recorded from $32^{\circ}$ to $37^{\circ}$. Third The lower extremity functional scale showed before therapy of 44; after therapy, 63. Conclusion: The hip joint traction and hip posterolateral muscles strengthening was effective in alleviating knee pain, increasing ROM and Lower extremity functional scale of the PFPS patients.
