Background: Well-validated risk prediction models help to identify individuals at high risk of diseases and suggest preventive measures. A recent systematic review reported lack of validated prediction models for low back pain (LBP). We aimed to develop prediction models to estimate the 8-year risk of developing LBP and its recurrence. Methods: A population based prospective cohort study using data from 435,968 participants in the National Health Insurance Service-National Sample Cohort enrolled from 2002 to 2010. We used Cox proportional hazards models. Results: During median follow-up period of 8.4 years, there were 143,396 (32.9%) first onset LBP cases. The prediction model of first onset consisted of age, sex, income grade, alcohol consumption, physical exercise, body mass index (BMI), total cholesterol, blood pressure, and medical history of diseases. The model of 5-year recurrence risk was comprised of age, sex, income grade, BMI, length of prescription, and medical history of diseases. The Harrell's C-statistic was 0.812 (95% confidence interval [CI], 0.804-0.820) and 0.916 (95% CI, 0.907-0.924) in validation cohorts of LBP onset and recurrence models, respectively. Age, disc degeneration, and sex conferred the highest risk points for onset, whereas age, spondylolisthesis, and disc degeneration conferred the highest risk for recurrence. Conclusions: LBP risk prediction models and simplified risk scores have been developed and validated using data from general medical practice. This study also offers an opportunity for external validation and updating of the models by incorporating other risk predictors in other settings, especially in this era of precision medicine.
Purpose: This study investigated the prevalence of hypertension, explored sleep duration, and examined the related factors to hypertension in Korean middle-aged adults. Methods: Using raw data from the Korean National Health and Nutritional Examination Survey (KNHANES) conducted from 2008 to 2010, a secondary analysis was performed with data from 13,230 adults aged 30~64 years. ${\chi}^2$-test and multiple logistic regressions was used for the data analysis. Results: The prevalence of hypertension among Korean middle-aged adults was 21.8% and 40.3% among participants reported that their average sleep duration was below six hours a night. Risk factors for hypertension include: female (the odds ratio (OR) 1.66 for male), 50~64 years age group (OR 3.66 for 30~49 years age group), education level of elementary school (OR 1.84 for university level), low household income (OR 1.27 for upper), obesity (OR 2.41), high risk drinking (OR 1.64), and sleep duration${\leq}6$ hrs (OR 1.16 for 8 hrs of sleep). Conclusion: High risk population of hypertension could be male, aged, low education, and low income. Obesity, high risk drinking, and short sleep duration should be considered as risk factors for hypertension. Interventions for obesity management, adequate alcohol drinking and sleep duration could be considered for preventing hypertension.
The relationship between socio-economic status and food and nutrient consumption patterns was studied in 7,370 Koreans aged 20 years and older in the 1995 Korean Health and Nutrition Survey. The aim of this study was to investigate the effect of rapid economic growth on food and nutrient consumption for Korean adults in the last 30 years. Monthly household income, and individual's educational level and occupation were chosen as variables of socio-economic status for individuals. A one day 24 hour recall method was used for the dietary survey. One way analysis of varience was adopted to test tole association between socio-economic variables and food and nutrient consumption patterns. Individuals who had a high socio-economic status had significantly higher daily intake of most of the nutrients including calcium, vitamin A, vitamin B$_2$which reached above the recommended dietary allowances(RDA) and a higher percentage energy consumption from fat. In addition, individual who belonged to a low socio-economic status consumed less animal foods, including meat, egg, milk and consumed low proportion of energy from fat. The results suggest that in spite of rapid economic growth during the last 30 years in Korea, individuals who belonged to low sorio-economic status categories are still nutritionally vulnerable. Among the sorio-economic variables, income and education except occupation were the influential factors on the food and nutrient consumption of Koreans. Therefore, nutrition policy should focus on influencing the dietary patterns of lower social class individuals to improve the health status of the population as a whole.
Background: Smoking is a primary risk factor for cancer development. While most research has focused on smoking cigarettes, the increasing popularity of shisha or water pipe smoking has received less attention. This study measured the prevalence and risk factors for shisha and cigarette smoking and related knowledge. Materials and Methods: This cross-sectional analytical study was conducted in Shah Alam, Malaysia. Participants aged ${\geq}18years$ were selected from restaurants. Data regarding demographic variables, smoking patterns, and knowledge about shisha smoking were collected in local languages. Logistic regression was performed to assess risk factors. Results: Of 239 participants, 61.9 % were male and 99.2% revealed their smoking status. Some 57.4% were smokers: 50.7% only cigarettes, 5.9% only shisha and 42% both. Mean age of starting cigarette smoking was $17.5{\pm}2.4years$ and for shisha smoking $18.7{\pm}2.0years$. In a univariate model, male gender, age 33-52 years and monthly income > MYR 4,000 increased the risk and unemployment and being a student decreased the risk. In a multivariate model, male gender increased the risk of smoking, while being a student decreased the risk, adjusting for age and income. The perception of shisha being less harmful than cigarettes was present in 14.6% and 7.5% had the opinion that shisha is not harmful at all, while 21.7% said that it is less addictive than cigarettes, 39.7% said that shisha did not contain tar and nicotine, 34.3% said that it did not contain carbon monoxide and 24.3% thought that shisha did not cause health problems. Conclusions: Prevalence of shisha and cigarette smoking is high in the general population in Malaysia and knowledge about shisha smoking is relatively low. The findings of our study might have implications for understanding similarities and differences in incidence of shisha and cigarette smoking in other cultural/geographic regions.
본 논문에서는 선행 연구를 통해 중진국 함정 논의의 주요 근거들을 추출하고 이들 근거를 중국경제 현실에 적용하여 '중국 중진국 함정 논의'의 객관적 근거와 위기 요인을 우선 확인해 보려 하였다. 그런 다음 향후 함정에 빠지지 않기 위해 중국이 어떤 경제개혁을 추진해야 하는지에 대해 살펴보는 것을 중요한 과제로 삼고 있다. 함정 유발 요인에 대한 선행 연구 검토 후 추출한 6가지 핵심 요인을 준거로 삼아 중국 경제에 대응시켜 본 결과 중국경제가 중진국 함정에 빠질 수 있는 가능성이 높아지고 있어 중대한 전환점에 서 있음을 알 수 있었다. '과잉투자'-'과잉설비'에 따른 'TFP 지속감소, '인구보너스 소멸과 고령화', '경제주체별 과다 부채와 기업구조조정 및 금융불안정', '소득불균등 확대', '정보통신 확산과 금융 인프라 접근성 및 투명성 지수 등 사회적 자본 미비' 등의 문제가 큰 우려요인으로 등장했음을 알 수 있었다. 그런데 이들 위기 신호들은 모두 지금까지의 중국이 추진해 온 성장모델, 즉 요소투입형 성장과 정부주도 부채의존형 성장모델의 한계를 보여주고 있다는 점에서 정부실패 교정을 필요로 하는 것들임을 또한 확인할 수 있었다. 함정을 피하기 위한 중국 정부의 정책방향은 대체로 제대로 설정된 상태이지만 이행과정이 쉽지 않다는 증거가 곳곳에서 나타나고 있었다. 결국 지금까지의 각종 '정부 실패'를 수정할 수 있는 정부 차원의 혁신에만 매달려서는 명확한 한계에 봉착할 수 있으므로 어떻게든 시장의 자체적인 상시 구조조정 능력이 배양될 수 있는 틀을 만들어 내는 것이 중국의 경우 더 없이 중요한 요소임을 알 수 있었다. 특히 2015년부터 위안화 SDR 편입이 확정되면서 대외적 측면에서도 큰 제약에 직면할 수밖에 없어 향후 중진국 함정 탈출을 위한 정책지향은 더욱 큰 제약에 직면할 수 있음도 알 수 있었다.

Backgrounds/Aims: Socioeconomic determinants of health are incompletely characterized in cholangiocarcinoma (CCA). We assessed how socioeconomic status influences initial treatment decisions and survival outcomes in patients with CCA, additionally performing multiple sub-analyses based on anatomic location of the primary tumor. Methods: Observational study using the 2018 submission of the Surveillance, Epidemiology, and End Results (SEER)-18 Database. In total, 5,476 patients from 2004-2015 with a CCA were separated based on median household income (MHI) into low income (< 25th percentile of MHI) and high income (> 25th percentile of MHI) groups. Seventy-three percent of patients had complete follow up data, and were included in survival analyses. Survival and treatment outcomes were calculated using R-studio. Results: When all cases of CCA were included, the high-income group was more likely than the low-income to receive surgery, chemotherapy, and local tumor destruction modalities. Initial treatment modality based on income differed significantly between tumor locations. Patients of lower income had higher overall and cancer-specific mortality at 2 and 5 years. Non-cancer mortality was similar between the groups. Survival differences identified in the overall cohort were maintained in the intrahepatic CCA subgroup. No differences between income groups were noted in cancer-specific or overall mortality for perihilar tumors, with variable differences in the distal cohort. Conclusions: Lower income was associated with higher rates of cancer-specific mortality and lower rates of surgical resection in CCA. There were significant differences in treatment selection and outcomes between intrahepatic, perihilar, and distal tumors. Population-based strategies aimed at identifying possible etiologies for these disparities are paramount to improving patient outcomes.
Purpose: The purpose of this study was to identify the quality of life and its predictors in low income Korean aged. Methods: This was a predictive correlational study. An accessible sample from the population of people who were 65 and over and were supported by the basic livelihood security system was 1,040. Quota sampling with strata of state division in the nation was chosen. Quality of life and its predictors in the subjects were measured. Results: The mean quality of life in the subjects was 47.0$\pm$10.7. Predictors of this study significantly explained 54.3% of the total variance of quality of life. Depression was the most significant predictor of quality of life. Health problems, district, social support, leisure activity, and health behavior had effects on quality of life. Conclusion: This finding indicates that quality of life in lower income Korean aged is different from other populations by economic status. Demographics, health status and social status were predictors of quality of life in the aged with a small income.
The U.S. government have concerned about food safety over the last two decades. The concept of the continuum, “from farm to table” was created to explore ways to prevent foodborne illnesses in all stages of food systems. On the continuum, consumers were recognized as the last line of defense to prevent foodborne illnesses, and much efforts were made to educate them safe food handling. This research was conducted to investigate infant formula handling and hand-washing behaviors of low-income families, especially parents and guardians of infants. The subject was selected from participants of the Special Supplemental Nutrition Program for Women, Infants, and Children(WIC), a federal program for low-income families in the U.S. Stratified 200 local WIC offices were randomly selected based on the number of WIC participants in each state, and 20 randomly selected WIC participants from each selected office were asked to complete questionnaires. SPSS for Windows was used for statistical analyses including frequency, cross- tabulation, and chi-square analyses. A total of 87 WIC offices returned completed question-naires (N = 1,598), and 492 were parents/guardians of infants. Most respondents were white (51.3%), high school graduates (41.5%) , and participated in WIC>1 yew. Most respondents (80.9%) teamed about food safety from WIC, and only limited number of respondents (10.2%) used the Internet for food safety information. Most respondents stored prepared formula safely (94.6%) and discarded formula left in the bottle after feeding (84.5%) , but fewer used brushes to wash formula bottles (71.3%) and boiled water(15.2%) Chi-square analyses showed respondents in different race/ethnicity had different food handling behaviors. Respondents showed generally good hand-washing behaviors as 94.2% always washing hands after using restroom, 93.2% after touching meat items, and 87.l% before preparing foods. Fewer respondents, however, washed hands after changing baby diapers (77.0%) and touching pets (67.2%). Researchers concluded that WIC education on food safety was effective, as limited food safety education covered during WIC education were followed well (e.g., storing prepared formula and discarding leftover). However, results also indicated that there were many behaviors needed to be reinforced especially to overcome family tradition and culture on food handling behaviors. The WIC may serve as good food safety resources and education agents utilizing mandatory education sessions because the vast amount of food safety information on the Internet was not readily accessible for this low-Income Population.
본 연구의 목적은 도시 저소득층 밀집지역 중학생의 음주실태와 음주경험에 영향을 주는 요인을 알아보고 음주예방 프로그램 개발의 기초자료를 제공하고자 시도되었다. 본 연구 대상자는 도시 저소득층 밀집지역에 위치한 학교에 다니고 있는 중학생 1192명이다. 수집된 자료는 SPSS 19.0 Program으로 기술통계, logistic regression으로 통계 처리하였다. 대상자의 음주 경험률은 44.1%로, 이 중 57.2%는 초등학교 혹은 그 이전에 첫 음주를 경험한 것으로 나타났다. 음주동기는 부모님 권유가 48.9%로 가장 높게 나타났고, 부모님의 12.0%는 자녀의 음주에 대해 무관심하고, 21.5%는 허용적이기까지 한 것으로 나타났다. 대상자의 외로움과 부 모 양육태도 점수는 각각 36.71점, 40.07점, 50.87점 이었다. 음주에 영향을 주는 예측요인은 학교생활만족도, 자녀의 음주에 대한 부모님의 태도, 부의 양육태도인 것으로 나타났다. 본 연구는 도시 저소득층 밀집지역 중학생의 음주영향요인들을 고려한 음주관련 프로그램 개발과 향후 일반청소년의 음주에 영향을 미치는 변인과의 차이를 비교분석하는 후속연구의 필요성을 제안한다.
본 연구는 관절염 여성노인을 대상으로 건강다이어리 프로그램을 개발하고 그 효과를 분석하여 낙상위험이 있는 관절염 여성노인을 위한 간호중재 서비스의 기초자료로 활용하고자 수행되었다. 2010년 9월 27일부터 11월 26일까지 대상자의 집을 방문하여 낙상예방을 위한 교육과 자가능력 향상을 위한 교육을 실시하였다. 프로그램 시작전 사전 평가를 하였고 종료 후 실험 후 평가를 실시하였다. 연구대상은 전라남도의 지역사회에 거주하며 의사로부터 관절염 진단을 받은 국민기초생활수급자 혹은 건강보험료 부과 하위 20%에 해당되는 65세 이상의 여성 노인으로서 연구자가 연령대별로 짝지어 실험군에 24명 대조군에 24명을 배정하였다. 중재는 8주 프로그램 이었으며 낙상관련 심리적 변수(낙상두려움, 낙상 효능감, 낙상지식), 일상 활동수행 정도, 기분상태를 측정하였다. 프로그램 1회 소요 시간은 약 50분 정도이며 관절염 질환이해 및 재활, 낙상관련 예방교육 등 2영역 교육내용을 16회로 나누어 제공하였으며, 매회 건강증진을 위한 자가 간호 수행정도점검과 개인별 상담으로 구성하였다. 이해력과 학습력을 높이기 위해 다이어리 전 내용을 그림으로 제작하였으며 매일의 건강증진 활동 실천을 확인하기 위해 자가 간호 수행표를 만들어 해당항목에 스티커를 부치도록 하였다. 프로그램 참여에 따른 낙상관련 심리적 변수, 일상 활동수행정도, 기분상태 변수에 대한 변화를 측정하여 분석한 결과 낙상예방 다이어리 프로그램을 시행한 실험군은 대조군에 비해 낙상 두려움, 낙상 지식, 기분상태는 유의한 차이가 있었으나 낙상효능감, 일상 활동수행 정도는 유의한 차이가 나타나지 않았다. 이상의 결과로 볼 때 본 연구의 낙상예방 프로그램은 저소득층 관절염 여성노인의 낙상예방 지식, 낙상 두려움, 기분상태에도 긍정적인 변화를 나타내어 프로그램 적용의 합리성을 보여주었다. 그러나, 본 연구에서는 낙상위험 정도에 따른 대상자 비교를 하지 않았으므로 낙상예방 프로그램의 구성 타당도를 높이기 위해서는 대상자의 낙상위험요인을 중등도로 분류하고 이를 전기노인과 후기노인 등 연령별로 교육효과를 비교하는 추후연구가 필요하다고 생각된다.
