한국 노인의 인구·사회경제적 특성과 미 충족 치과진료와의 관련성 (Relationship between demographic and socioeconomic characteristics of the Korean elderly and unmet dental care needs)

  • Objectives: This study investigated the relationship between demographic and socioeconomic characteristics of the Korean elderly and their unmet dental care needs, by using the 2015 data from the $6^{th}$ Korea National Health and Nutrition Examination Survey (NHANES). Methods: In total, 1,372 elderly persons aged 65 and over, who responded to the $6^{th}$ NHANES, were included in the final analysis. Logistic regression analysis was performed in order to identify any relationship between demographic and socioeconomic characteristics and unmet dental care needs. Results: The rate of unmet dental care needs was shown to be less by 0.799 times in the elderly who reside in dong than those who live in eup and myeon (OR: 0.799, 95% CI: 0.679-0.959). Unmet dental care needs were higher in participants with 'low' and 'below average' than 'above average' income, by 1.645 times (OR: 1.645, 95% CI: 1.087-2.366) and 1.172 times (OR: 1.172, 95% CI: 1.108-1.880), respectively. Elderly individuals living alone had a higher rate of unmet dental care needs than those living with their family by 1.157 times (OR: 1.157, 95% CI: 1.084-1.498). Conclusions: Demographic and socioeconomic factors influenced unmet dental care needs, causing inequality. Proper policy support to the vulnerable should be considered in order to enhance the elderly's access to dental care.

Impact of socio-demographic factors, lifestyle and health status on nutritional status among the elderly in Taiwan

  • BACKGROUND/OBJECTIVES: Aging is an imperative problem for many countries in this century, and presents several challenges for the maintenance of good nutritional status. This study aims to assess the impact of socio-demographic factors, lifestyle and health status on the nutritional status among the elderly in Taiwan. SUBJECTS/METHODS: A cross-sectional study was carried out in Taiwan. Data were obtained from the Mei Jau Health Management Institution, which is a private health evaluation provider with multiple health screening centers in Taiwan and Asia. This study included 7947 adults aged 65 years or above. The data were extracted between 2001 to 2010. Nutritional status was assessed using anthropometric data, biochemical data and dietary intake information. RESULTS: Among the 7947 participants with mean age of 70.1 (SD = 4.5) years, 20.2%, 6.6%, 10.5% and 52.5% experienced underweight, protein malnutrition, anemia and inadequate dietary intake in the past month, respectively. Age was negatively correlated with body weight (r = -0.19, P = 0.02), body mass index (r = -0.41, P < 0.001), albumin level (r = -0.93, P < 0.001) and hemoglobin level (r = -0.30, P = 0.008). Age above 70 years, gender, unmarried status, retirement, lack of education, low family income, smoking, alcohol drinking, sleep duration of 6-8 hours, vegetarian diet, multiple medications, comorbidity and dysphagia were positively associated with malnutrition in older adults. CONCLUSIONS: Underweight and inadequate dietary intake are prevalent among the elderly in Taiwan. Vegetarian diet, multiple medications, comorbidity, dysphagia and lifestyle factors such as smoking, alcohol drinking and sleep duration of 6-8 hours are risk factors for undernutrition in older adults.

  • The relationship between mortality and the Korean Healthy Eating Index ("KHEI") is well established. This study was to investigate the associations between health habits, mental health, and nutritional status and KHEI in older Koreans (aged≥65 years). A total of 4,247 subjects (1,842 men and 2,405 women) that participated in the 2016~2018 Korean National Health and Nutrition Examination Survey were included in the analysis. The lowest scoring KHEI item was milk and milk products. KHEI tertile groups were classified by total KHEI score. Tertile group percentages were related to general characteristics such as gender, residential area, educational level, income level, number of family members, and age. Logistic regression analysis adjusted for general characteristics, showed that ex-smokers (OR: male 1.53 female 2.29), smokers (OR: male 2.90), low hand grip strength (OR: male 1.42 female 1.90), poor self-rated health status (OR: female 1.83), stressful mental status (OR: female 1.51), poor health-related quality of life (OR: female 1.64), poor nutritional status (OR: male 2.88~37.20 female 1.98~16.12), and food insecurity (OR: male 6.87 female 2.03) were significantly related to a lower KHEI. This study suggests that gender-specific associations exist between mental health status and KHEI.

창원시 노인의 영양지수에 영향을 미치는 요인 (Factors Affecting Nutrition Quotient for Elderly (NQ-E) for the Elderly in Changwon City)

  • The purpose of this study was to identify nutrition quotient for elderly (NQ-E) and to investigate factors affecting NQ-E of the elderly using welfare center and senior citizen's center in Changwon city. A self-administered questionnaire was conducted between June 2021 and early August 2021 for 320 elderly (≥65 years, male, n=52, female, n=268). As a result of the survey, the mean NQ-E score was 61.12, which was within the medium-high grade. The scores of balance, moderation, and dietary behavior factors were within the medium-high grade, while diversity factor was within the medium-low grade. According to the results of the Pearson's correlation coefficient analysis, NQ-E showed positive correlations with the 'with family' (p<0.01), 'education level' (p<0.01), 'health functional foods consumption' (p<0.01), 'monthly household income' (p<0.05), 'nutrition education experience' (p<0.05), 'reading nutrition labeling of health functional foods' (p<0.01), 'perception of the efficacy of health functional foods' (p<0.01), and 'the number of times of leisure activities per week' (p<0.01) and negative correlations with the 'gender' (p<0.05), 'age' (p<0.01), 'smoking' (p<0.05), and 'social frailty' (p<0.01). As a result of multiple regression analysis, 'gender' (p<0.05), 'perception of the efficacy of health functional foods' (p<0.05), 'the number of times of leisure activities per week' (p<0.05), and 'social frailty' (p<0.05) were found to be factors affecting NQ-E. Based on the results of this study, customized services by characteristic, nutrition education, and counseling for the elderly should be implemented and the development of various programs and continuous support of the community are necessary so that the elderly can carry out social exchange.

사회공헌 일자리를 통한 베이비붐 세대의 퇴직 후 사회참여 경험에 관한 질적 연구 (A Qualitative Study on the Experience of Social Participation after Retirement of Baby Boomers through Social Contribution Jobs)

  • 본 연구는 베이비붐 세대의 퇴직 후 사회공헌 일자리 통한 사회 참여 경험을 내용분석을 중심으로 알아보았다. 면접을 통한 자료 분석 결과 퇴직 후 사회공헌 일자리에 참여한 베이비붐 세대들은 일하고 싶은 강한 욕구를 나타냈고 일자리 참여로 인해 보람과 자기 계발의 기회가 주어졌으며 이로 인해 가족들도 일자리 참여를 긍정적으로 여겼다. 또한 자조 모임과 커뮤니티를 통해 프로그램을 위한 정보를 공유하고 서로의 관심사를 나누는 사회적 관계를 형성하였다. 그러나 일자리로서는 보수가 너무 적고 매년 면접을 다시 봐야 하는 불안정성과 불연속성에 대한 불만을 드러냈다. 사회공헌 일자리의 미래 방향성에 대해서는 베이비붐 세대의 기술과 전문성을 좀 더 활용할 수 있는 일자리의 필요성과 퇴직 후 소득보장으로서의 가교 일자리 정책과 더불어 제도개선 및 규제개혁을 통한 시장성과 공공성을 지닌 산업의 육성과 지원으로 퇴직 베이비붐 세대의 인적 자원을 적극적으로 활용해야 함이 강조되었다.

영아기 첫자녀를 둔 어머니의 아동발달지식에 관한 연구 (A Study on the Knowledge of Child Development of Mothers with Infants)

  • 본 연구에서는 자녀 양육 초기 영아모의 아동발달지식 수준을 살펴보기 위하여 만 2세 미만의 영아를 자녀로 둔 어머니를 대상으로 아동발달지식 수준과 특성을 살펴보았다. 본 연구를 수행하기 위하여 만2세 미만의 첫 자녀를 둔 어머니 156명을 대상으로 양육관련 설문지와 KCDI(Knowledge of Child Development Inventory, Larsen, & Juhasz, 1986)를 실시하였고, 수집된 자료는 SPSS 11.5 프로그램을 사용하여 t-test, ANOVA 등을 실시하여 결과를 처리하였다. 본 연구결과는 다음과 같다. 영아모의 아동발달지식에 대한 평균 정답률은 70.0%로 나타났고, 하위영역별로 살펴보면 인지발달, 정서발달, 사회성발달, 신체발달 순으로 자녀의 발달에 대한 지식을 지니고 있는 것으로 나타났다. 양육관련 변인에 따른 어머니의 아동발달지식의 차이를 살펴본 결과, 여아를 자녀로 둔 어머니일수록 유아의 정서발달에 관한 지식이 많은 것으로 나타났고, 교육수준이 높을수록, 가족 월수입 정도가 많을수록 지식이 많은 것으로 나타났다. 출생시 체중이 많았던 자녀를 둔 어머니일수록 신체발달에 관한 지식 정도가 높게 나타났고, 수유방법에 따른 아동발달지식의 차이를 살펴본 결과 혼합수유를 하는 어머니가 모유수유, 인공유 수유를 하는 어머니보다 자녀의 인지발달, 사회성발달에 관한 지식이 보다 풍부한 것으로 나타났다. 또한 남편, 시부모가 기대하지 않았던 성의 자녀를 키우는 어머니일수록 아동발달지식을 더 많이 갖고 있는 것으로 나타났다. 본 연구결과는 향후 영아모 대상의 부모교육, 임산부 교실, 특수아동 자녀를 둔 부모대상 교육 프로그램, 입양센터, 미혼모센터 등의 프로그램을 개발하는 데 기초자료로 활용될 수 있을 것이다.

국민영양조사를 이용한 지방에너지 섭취비율에 따른 식품섭취패턴과 식이관련인자의 비교연구 (Food Consumption Patterns and Other Diet Related Factors Among Three Groups of Households with Different Fat Energy Intakes)

  • With changing socio-economic environments, there have been some changes in the Korean diet. One of major changes is the increasing fat intake. Based on the current trend of increase, it is important to understand factors related to fat intake levels. Objective of this study was to find out the differences in food consumption patterns, demographic characteristics, and other socio-economic factors among three groups of households. The households were devided into three groups by percentage of fat energy intake to total energy intake such as less than 15%(Group 1), from 15% to 25%(Group 2), and more than 25% (Group 3). Among 1, 987 households, the numbers in Group 1, Group 2 and Group 3 were 792(39.6%), 902(45.4%), and 293(15.1%), respectively. The mean size of family was 3.44 person per households for total survey household, 3.42 for Group 1, 3.54 for Group 2, and 3.21 for Group 3. More than 60% of families living in the rural area belonged to Group 1(p<0.05). Group 3 had most educated food preparers and more income than other groups(p<0.05). Group 3 showed higher intake of all nutrient analyzed except carbohydrate (p<0.05). Other than calcium and vitamin A for Group 1, and vitamin A for Group 1, and vitamin A for Group 2, all nutrient intake analyzed for three groups were more than 75% of Korean recommended dietary alowances. Mean fat intakes for Group 1, Group 2, and group 3 had highest amount of intake for all food groups. According to food group analysis, Group 3 had highest amount of intake for all food groups except vegetables. Group 3 consumed more than 10 times of meats compared to Group 1. Group 3 had more dishes prepared by baking, deep-fryng, stir-frying, and pan-frying other two groups. Processed foods were used most frequently in Group 3. Other diet and related factors were analyzed also. These results showed that the amount of fat consumed by Korean households varies widely. Households with high fat energy intake had tendency to reside in the largy city, and to have heigher education level and more income than other groups. They eat less vegetables, use more cooking methods using oils, and use more processed foods. To ensure a proper intake of fat, there should be two separate ways of approach for high fat energy group and low fat energy group, since they are different in many ways.

Women and Tobacco Use: Discrepancy in the Knowledge, Belief and Behavior towards Tobacco Consumption among Urban and Rural Women in Chhattisgarh, Central India

  • Background: Tobacco consumption has become pandemic, and is estimated to have killed 100 million people in the 20th century worldwide. Some 700,000 out of 5.4 million deaths due to tobacco use were from India. The era of global modernization has led to an increase in the involvement of women in tobacco consumption in the low income and middle-income countries. Tobacco consumption by females is known to have grave consequences. Objectives: To assess: (1) the tobacco use among urban and rural women; (2) the discrepancy in the knowledge, belief and behavior towards tobacco consumption among urban and rural women in Durg-Bhilai Metropolitan, Chhattisgarh, Central India. Materials and Methods: The study population consisted of 2,000 18-25 year old young women from Durg-Bhilai Metropolitan, Chhattisgarh, Central India, from both urban and rural areas. Data were collected using a pretested, anonymous, extensive face to face interview by a female investigator to assess the tobacco use among women and the discrepancy in the knowledge, belief and behavior towards tobacco consumption among urban and rural individuals. Results: The prevalence of tobacco use was found to be 47.2%. Tobacco consumption among rural women was 54.4% and in urban women was 40%. The majority of the women from urban areas (62.8%) were smokers whilst rural women (77.4%) showed preponderance toward smokeless tobacco use. Urban women had a better knowledge and attitude towards harms from tobacco and its use than the rural women. Women in rural areas had higher odds (1.335) of developing tobacco habit than the urban women. Conclusions: Increased tobacco use by women poses very severe hazards to their health, maternal and child health, and their family health and economic well-being. Due to the remarkably complex Indian picture of female tobacco use, an immediate and compulsory implementation of tobacco control policies laid down by t he WHO FCTC is the need of the hour.

성인자녀의 부모 동거 현황 및 분석틀의 탐색 (A Study on the Current Situation of Adult Children Cohabiting with Their Parents and an Exploration of the Frame of Analysis)

  • The purpose of this study is to explore whether the phenomenon of both married and unmarried adults' cohabiting with their parents in Korean society is "unilaterally parasitic" on the child's side, or is "interdependent", characterized by expectation and dependence from the parent's side. As a result of this study possessing the characteristics of theoretic research, the following propensities of parent-dependent adults have been discussed. First of all, it was discovered that the ratio of adult children dependent on and cohabiting with their parents is considerable. Second, parents cohabiting with their adult children have unfavorable sociological features, such as high age, low level of education and income, and lower standards of education and income, compared to parents in normal households. Third, it was found that parent-dependent adults in Korean society maintain a relatively high rate of financial activity and stable employment-based occupation status. Fourth, it was shown that the level of satisfaction on the relationship between parent-dependent adults and their parents was discovered to be high, which is contrastive to the prediction of negative results based on some previous researches. Single adult children's age, their level of education and financial activity status, and their parents' age and level of education were deducted as variables related to the level of satisfaction of the relationship between parents and their children. It seems that the issue of married and unmarried adults' cohabiting with their parents in Korean society should be approached from various perspectives such as political, economic, socio-cultural and developmental aspects. On the basis of this fundamental awareness and several of the materials, it is pertinent that approaches to both married and unmarried adults' cohabiting with parents in Korean society should be distinguished from approaches to those in Japanese or Western society because it reflects the uniqueness of Korean society. In the phenomenon of married and unmarried adults' cohabiting with their parents in Korean society, there are several factors besides the economic factor, especially the socio-cultural factor that have the characteristics of mutual dependence between parents and their children rather than those of unilateral parasitism, in contrast with the phenomenon in Japanese or Western society. This research was aimed to contribute by establishing basic data for policy making by providing necessary information to treat the issues of instability and anxiety related to families and reflection on the matters of generations and parent-child relationships in current Korean society.

입원환자의 의료보장형태에 따른 가정간호 이용의사에 대한 연구 (A Study on Hospitalized Patients' Intent to Use Home Care Nursing According to the Types of Medical Security)

  • Purpose: This study is a descriptive research which is designed to investigate hospitalized patients' intent to use home care nursing according to the types of medical security. Method: This researcher surveyed 236 patients who were hospitalized at B medical center located in Busan,. Data were collected from Sep. 1 to Nov. 30, 2005 using a questionnaire survey, medical records, face-to-face interviews and observations. Collected data were analyzed in terms of frequency, percentage, mean and standard deviation through $x^2$-test and t-test under SPSS WIN 10.0 Program. Result: Out of the total subjects, 59.3% were medical aid clients and the remaining 40.7%, health insurance ones. The hospitalized period and frequency of the former group were 38.0 days and 4.0 times, respectively, while those of the latter, 37.7 and 3.4. When home care nursing clients were examined using a given classification device, it was found that out of the total 236 subjects, 205(86.9%) were needed to receive home care nursing, 121, medical aid and the other 84, health insurance. 24.0% of medical aid clients heard about home care nursing ever before, lower than 39.3% of health insurance clients. 43.8% of the former clients said cost for home care nursing was high while, 47.6% of the latter group responded expense for the nursing intervention was low. 30.6% of medical aid clients had intent to use home care nursing, lower than 47.6% of health insurance clients. 71.7% of those patients whose monthly income was 99 million won or below had no intent to use home care nursing, higher than 62.5% of those who were 100 million or over in monthly income(p<.05). 76.4% of those clients who had no nursing provider intented to use home care nursing, higher than those who had nursing provider(p<.05). Concerning contents of home care nursing, 85.1% of medical aid clients needed education, training and counseling while, 77.4% of health insurance aids wanted medication and injection. Conclusion: In conclusion, the use of home care nursing by medical aid clients should be promoted through improving conditions for home care nursing in terms of expense, family and residence and making public relations about activities and contents of the home care nursing.
