Alternative Therapies with Tacrolimus and Low-Dose Doxycycline for Oral Chronic Graft-versus-Host Disease That Is Resistant to Topical Corticosteroid Medication: Case Report

  • Graft-versus-host disease (GVHD) is frequent complications of hematopoietic stem cell transplantation. In the chronic GVHD (cGVHD), the oral cavity is the most commonly affected region. The clinical manifestations include erythema, ulceration, lichenoid-hyperkeratotic change in oral mucosa, dry mouth, and limitation of mouth opening. The initial treatment strategy of oral cGVHD patients is topical corticosteroid therapy in various formulation. However, corticosteroid resistance appears in some patients. We report a case of a 25-year-old male patient with oral cGVHD, who has resistance to topical corticosteroid medication, treated with 0.03% tacrolimus ointment and low-dose doxycycline. The patient showed subjective and objective improvement without side effect.

  • It was reported that low dose doxycycline(LDD) regimen could inhibit pathologically elevated collagenase activity in the gingival crevicular fluid of petients with adult periodontitis without producing typical antibiotic side effects. The purpose of this study was to evaluate the effects of LDD regimen(20mg) administered during non-surgical therapy on clinical index and gingival crevicular fluid enzyme activity in diabetics who are at high risk for periodontal disease. Forty-nine subjects having at least two sites with probing pocket depths greater than 4mm were selected. In this double-blind, placebo-controlled study, the patients were administered 20 mg doxycycline capsule or placebo capsule b.i.d. for 2 weeks. Clinical parameters of dental plaque, gingival inflammation, probing pocket depth and probing attachment level were assessed at week 0, 2, 4, and 8. Gingival crevicular fluid samples were collected at the same time to evaluate the activities of collagenase and elastase. Clinical parameters and elastase activity were significantly reduced in all groups compared to the baseline value after treatment. Significantly greater reduction in pocket depth and gain in attachment level was shown in the LDD-administered group compared to placebo group in both adult periodontitis and diabetic patients. Total collagenase activity was also reduced significantly in all groups after treatment, but the greater reduction was seen in the LDD-administered diabetics group compared to relevant placebo group(at 4, 8week). Significantly greater reduction in active collagenase activity was also seen in the LDD-administered group compared to placebo group in diabetic patients(at 2, 4, 8week). These results indicated that use of low dose doxycycline could be aueseful adjunct to instrumentation therapy in the management of diabetic patient with periodontitis as well as adult periodontitis patient.

Effects of Six Antibiotics on the Activity of the Photosynthetic Apparatus and Ammonium Uptake of Thallus of Porphyra yezoensis

  • The modern integrated fish-seaweed mariculture has been tested to reduce the environmental impacts of an intensive fed culture. To obtain the best seaweed bioremediation performance, the effects of therapeutants used for fish disease control on the selected seaweed species should be considered. As a selected seaweed, Porphyra yezoensis was tested with six commercial antibiotics including erythromycin thiocyanate_A, erythromycin thiocyanate_B, oxytetracycline, doxycycline, pefloxacin, and amoxicillin trihydrate under the batch incubation at a photon flux density of 10 $\mu$mol ${\cdot}m^{-2}\;{\cdot}\;s^{-1}$ at 15$^{\circ}C$. Among the tested commercial antibiotics, erythromycin thiocyanate_A, erythromycin thiocyanate_B, oxytetracycline, and doxycycline showed decreases in Fv/Fm, the photochemical efficiency of photosystem II, with a dose-dependant and time-dependant manner. From the quenching analysis of chlorophyll fluorescence, three differential patterns were observed in the antibiotics-treated Porphyra: (1) high nonphotochemical quenching (NPQ) and low photochemical quenching (qP) in the cases of Erythromycin thiocyanate_B and amoxicillin trihydrate, (2) high NPQ and high qP in the case of pefloxacin and (3) low NPQ and low qP in the case of oxytetracycline. These results indicated that antibiotics affected in various ways on the photosynthetic apparatus, reflecting differential lesion sites of antibiotics. In addition, the rates of ammonium uptake also decreased with a decrease of Fv/Fm in P. yezoensis thalli treated with erythromycin thiocyanate_B and oxytetracycline. Therefore, the four antibiotics mentioned could affect the bioremediation capacity of the selected seaweed species in the integrated fish-seaweed mariculture system due to the decrease of photosynthetic activity and the simultaneous decrease of ammonium uptake.

TGF-${\beta}1$으로 자극한 사람중피세포주에서 조직플라스미노겐 활성제가 미치는 영향 (The Effect of Tissue Plasminogen Activator on TGF-${\beta}1$ Pre-Treated Human Mesothelial Cell Line)

  • Background: In an effort to find alternative therapeutic agents to prevent excessive fibrosis as a sequela to complicated parapneumonic effusion or empyema, we examined the effect of tissue plasminogen activator (tPA) as a fibrinolytic agent combined with talc or transforming growth factor (TGF)-${\beta}1$ in a human pleural mesothelial cell line, MeT-5A. Methods: MeT-5A cells were stimulated with various doses of talc, doxycycline or TGF-${\beta}1$ for 24 h and then were treated with tPA for an additional 24 h. Cell viability was measured by MTT assay. The production of interleukin (IL)-8 and vascular endothelial growth factor (VEGF) in the culture supernatants was measured by ELISA. Real-time PCR was carried out for measurement of type I collagen mRNA. Results: MeT-5A cells treated with talc showed a dose-dependent increase in production of IL-8. Talc also increased production of type I collagen mRNA at low doses, but talc did not influence the induction of VEGF. Addition of tPA to talc-stimulated cells showed further increases in the production of IL-8, but tPA did not influence the production of VEGF or type I collagen mRNA. TGF-${\beta}1$ increased the production of both VEGF and collagen type I mRNA, both of which were effectively inhibited by additional tPA treatment in MeT-5A cells. Conclusion: TGF-${\beta}1$ is a potent inducer of collagen synthesis without induction of IL-8 in MeT-5A cells. Addition of tPA after TGF-${\beta}1$ stimulation inhibited further fibrosis by direct inhibition of collagen mRNA synthesis as well as by inhibition of VEGF production.
