Purpose : This study was to analyze serious bacterial infections in infants younger than three months of age and to review the direction of treatments for these patients. Methods : 378 febrile infants with a rectal temperature ${\geq}38.0^{\circ}C$ visited from Jan. 2001 through Dec. 2002 were retrospectively studied. Infants with the following criteria belonged to the low risk group. WBC $5,000{\sim}15,000/mm^3$, WBC negative in urine stick test and negative for nitirite test, CSF WBC < $10/mm^3$ and negative in CSF gram stain, negative chest X-ray, stool WBC <5/HFP(high power field), and focal infection. If any of the above criteria were not met, they belonged to the high risk group. SBI was defined as a positive culture of urine, blood or CSF. SI was defined as aseptic meningitis or pneumonia including above laboratory tests of SBI. SBI patients were separately compared with two groups, high risk and low risk. Results : Of the 378 infants that were tested 216(57.1%) were in the high risk group and 162(42.9%) in the low risk group. Among 105 SBI(27.8%) and 172 SI(45.5%), there were 98 urinary tract infection(25.2%), 10 bacteremia(2.6%), 2 bacterial meningitis(0.6%), and 77 aseptic meningitis(22.8%). There were 76 SBI(35.2%) from the high risk group and 29 SBI(17.9%) from the low risk group identified. The results of the sensitivity(72.4%), the specificity(48.7%), the negative predictive value(82.1%) and the positive predictive value (35.2%) were calculated. Conclusion : Even though the probability of SBI in the low risk group is insignificant, it should still be considered in febrile infants younger than 3 months of age. I believe the CSF study is necessary because of the moderate high incidence of abnormal finding in our study.

Purpose: The purpose of the present study was to investigate the prevalence, subtypes and risk factors of irritable bowel syndrome by ROME-III among Korean university students. Methods: This study was descriptive survey research. The sampls were 796 and variables were measured by structured questionaire. Rome-III criteria was used for diagnosis of IBS. The gathered data were analyzed with %, $x^2$-test, t-test, logistic regression by SPSS win 17.0. Results: The students with IBS were 61 (7.7%) and the most of the subtype was IBS-M (42.6%). Meal (times/day), breakfast, stress, quality of sleep, neuroticism, bodily pain, general health, social function, role emotional restriction, mental health, somatization, obcessive-compulsive state, depression, anxiety, hostility, global severity index, positive symptom distress index, positive symptom were significantly different between IBS group and non-IBS group. The prevalence of IBS was low in the higher score of role emotional in general health state. There were more 2 times students who had score of the obsessive-compulsive in psychological health over the 50 than below the 50 in IBS group. Conclusion: 7.7% of students were diagnosed by Rome-III criteria and the most of the sybtype was IBS-M. The risk factors of IBS were role emotional restriction, obsessive-compulsive state.
High sensitive C-reactive protein (hs-CRP) has been associated with metabolic syndrome (MetS) and its risk factors. This study aimed to evaluate the association between hs-CRP and the risk factors of MetS in elderly women with abdominal obesity. The diagnosis of MetS followed the AHA/NHLBI criteria, and abdominal obesity was defined using the WHO Asian-Pacific criteria. We used the data from 174 elderly women, with an average age of 74 years. They were classified into two groups: The absent group (N=97) and the MetS group (N=77). Hs-CRP was significantly higher in the MetS group (p=0.007). Hs-CRP had a positive correlation with abdominal obesity (r=0.190, p=0.014) and fasting blood glucose (r=0.240, p=0.002), while having a negative correlation with HDL cholesterol (r=-0.164, p=0.035). Moreover, hs-CRP was higher in the group with risk of high fasting blood glucose (p=0.006) and low HDL-cholesterol (p=0.010), even in elderly women with abdominal obesity.
Purpose: Incidence, risk factors, and clinical consequences of pancreatic fistula (POPF) after D1+/D2 radical gastrectomy have not been well investigated in Western patients, particularly those from Eastern Europe. Materials and Methods: A total of 358 D1+/D2 radical gastrectomies were performed by surgeons with high caseloads in a single surgical center from 2002 to 2017. A retrospective analysis of data that were prospectively gathered in an electronic database was performed. POPF was defined and graded according to the International Study Group for Pancreatic Surgery (ISGPS) criteria. Uni- and multivariate analyses were performed to identify potential predictors of POPF. Additionally, the impact of POPF on early complications and long-term outcomes were investigated. Results: POPF was observed in 20 patients (5.6%), according to the updated ISGPS grading system. Cardiovascular comorbidities emerged as the single independent predictor of POPF formation (risk ratio, 3.051; 95% confidence interval, 1.161-8.019; P=0.024). POPF occurrence was associated with statistically significant increased rates of postoperative hemorrhage requiring re-laparotomy (P=0.029), anastomotic leak (P=0.002), 90-day mortality (P=0.036), and prolonged hospital stay (P<0.001). The long-term survival of patients with gastric adenocarcinoma was not affected by POPF (P=0.661). Conclusions: In this large series of Eastern European patients, the clinically relevant rate of POPF after D1+/D2 radical gastrectomy was low. The presence of co-existing cardiovascular disease favored the occurrence of POPF and was associated with an increased risk of postoperative bleeding, anastomotic leak, 90-day mortality, and prolonged hospital stay. POPF was not found to affect the long-term survival of patients with gastric adenocarcinoma.

Objectives: To determine the effectiveness of Tai-chi in treatinglow back pain. Methods: We searched 9 electronic databases(Pubmed, CAJ, EMBASE, Cochrane Library, Web of Science, KISS, KISTI, NDSL, and RISS) till April 2018 and included randomized controlled trials(RCTs) that used Tai-chi for lower back pain treatment. The methodological quality of each RCT was assessed usingthe Cochrane risk of bias tool. Results: As per the inclusion criteria, 5 RCTswere found to be eligible for our review. These studies were divided into 4 groups and meta-analysis was performed. The meta-analysis of 1 study showed favorable results regardingthe use of Tai-chi. Twostudies showed favorable results regardingthe use of Tai-chi with Tuina rather than Tuina alone. All studies were observed to have a high risk of bias. Conclusions: There is limited evidence available to support the effectiveness of Tai-chi in relieving low back pain;hence, additionalwell-designed RCTs should be encouraged for better evidence.

Objectives : Metabolic syndrome (MetS) is associated with increasing risk of cardiovascular morbidity and mortality but the risk of stroke associated with the MetS is less well established. The objective of this study was to evaluate the prevalence of MetS in stroke patients. Methods : We analyzed the data of 806 patients with acute ischemic stroke from Hanbang Stroke Registry supported by the Ministry of Science and Technology of Korea. Subjects' enrollment was done in the oriental medical hospitals of 4 universities located in the metropolitan region from April 2007 to August 2008. We analyzed all registered data and evaluated the prevalence of MetS and specific components of MetS. Results : The prevalence of MetS in stroke patients according to American Heart Association/National Heart Lung and Blood Institute (AHA/NHLBI) criteria was 59.6%. The prevalence according to the International Diabetes Federation (IDF) criteria was 41.6%. The prevalence of female patients was higher than that of male patients (71.8% vs 51% by AHA/NHLBI criteria; 61.7% vs 27.7% by IDF criteria). The prevalence of central obesity, low HDL cholesterol and hypertension was higher in female patients. There was no significant relationship between MetS and stroke subtype. Conclusions : This work gives us the basic data on the association between MetS and risk of stroke. There should be the prospective cohort studies to evaluate MetS and the risk of stroke.

Objective : Post-traumatic stress disorder (PTSD) is a group of diseases that are observed in patients who had experienced a serious trauma or accident. However, some experienced it even after only a mild traumatic brain injury (TBI), and they are easily ignored due to the relatively favorable course of mild TBI. Herein, the authors investigated the incidence of PTSD in mild TBI using brief neuropsychological screening test (PTSD checklist, PCL). Methods : This study was conducted on patients with mild TBI (Glasgow coma scale ${\geq}13$) who were admitted from January 2012 to December 2012. As for PCL, it was done on patients who showed no difficulties in communication upon admission and agreed to participate in this study. By using sum of PCL, the patients were divided into high-risk group and low-risk group. PTSD was diagnosed as the three major symptoms of PTSD according to the Diagnostic and Statistical Manual of Mental Disorders, fourth-edifion. Results : A total of 314 TBI patients were admitted and 71 of them met the criteria and were included in this study. The mean age was 52.9 years-old (range : 15--94). The mean PCL score was 28.8 (range : 17--68), and 10 patients were classified as high-risk group. During follow-up, 2 patients (2.7%) of high risk group, were confirmed as PTSD and there was no patient who was suspected of PTSD in the low-risk group (p=0.017). Conclusion : PTSD is observed 2.8% in mild TBI. Although PTSD after mild TBI is rare, PCL could be considered as a useful tool for screening of PTSD after mild TBI.

This review aimed to identify risk factors for occupational accidents and illnesses among young workers in the Nordic countries and to attain knowledge on specific vulnerable groups within the young working force that may need special attention. We conducted a systematic review from 1994 to 2014 using five online databases. Of the 12,528 retrieved articles, 54 met the review criteria and were quality assessed, in which data were extracted focusing on identifying occupational safety, health risk factors, and vulnerable groups among the young workers. The review shows that mechanical factors such as heavy lifting, psychosocial factors such as low control over work pace, and organizational factors such as safety climate are all associated with increased injury risk for young Nordic workers. Results show that exposures to chemical substances were associated with skin reactions, e.g., hand eczema. Heavy lifting and awkward postures were risk factors for low back pain, and high job demands were risk factors for mental health outcomes. The review identified young unskilled workers including school drop-out workers as particularly vulnerable groups when it comes to occupational accidents. In addition, apprentices and young skilled workers were found to be vulnerable to work-related illnesses. It is essential to avoid stereotyping young Nordic workers into one group using only age as a factor, as young workers are a heterogeneous group and their vulnerabilities to occupational safety and health risks are contextual. Politicians, researchers, and practitioners should account for this complexity in the education, training and organization of work, and workplace health and safety culture.
The purpose of this study was to investigate the effect of nutrition counseling on improving metabolic syndrome (MS) risk factors. Sixty-eight subjects were grouped according to their numbers of MS risk factors. Subjects who have three or more risk factors of MS were defined as "High risk", subjects who have two risk factors of MS were defined as "low risk", and subjects who have below two risk factors of MS were defined as "no risk" group. All groups finished nutrition counseling every three weeks for 12 weeks. Anthropometric, dietary assessments (24 hr-recall) and blood samples were measured at 0 and 12 weeks nutrition counseling. After 12 weeks of intervention, anthropometric data (weight, BMl, body fat (%), and waist/hip ratio) were significantly decreased (p < 0.05) in all groups. Daily consumption of calorie was decreased significantly (p < 0.05) in the group of low risk and high risk. Blood level of fasting plasma glucose was significantly decreased (p < 0.001) in all groups after 12 weeks of intervention. Significantly, the fasting plasma glucose level was returned to normal range in the high risk group. The number of people who have three or more risk factors of MS (high risk group) was decreased from 25 to 12. Sum of MS Criteria decreased from 85 to 52 in the group of MS and decreased from 143 to 99 in all groups. These results indicate that nutrition counseling for male workers at the worksite proved to be helpful by reducing the risk factors of MS and thereby reducing the risk of cardiovascular disease. Continuing and systematic nutritional management programs should be developed and implemented for male workers at the worksites.
Purpose: This study was designed to identify the incidence risk of cardicerebrovascular disease (CVD) among male bus drivers, and to examine and compare the predictors of their health behavior according to the level of CVD incidence risk. Methods: The convenience sample of 222 male bus drivers were recruited from a bus company located in Jeonnam province. Data were collected from self-reported questionnaires and annual medical examination records from 2010. The CVD incidence risk was calculated based on the risk criteria for industrial workers. Results: The 26.6% and 26.1% of the participants were in the moderate and high risk group, respectively. The 72% of the participants were in the precontemplation stage and reported no intention to change their unhealthy lifestyles. Stepwise multiple regression analyses showed that current smoking, excessive alcohol drinking, physical inactivity and lack of knowledge were negative predictors of good health behavior in the normal/low risk group (Adj $R^2$=.443). Heavy alcohol drinking, current smoking, physical inactivity and dyslipidemia were reported by the moderate/high risk group (Adj $R^2$=.427). Conclusion: This study suggested that targeted education and counseling are needed to modify unhealthy lifestyles such as alcohol consumption, smoking and exercise among middle aged male drivers. Especially, dyslipidemia should be managed among those who are at risk for CVD.
