Effects of Community-based Case Management Program for Clients with Hypertension (고혈압 대상자의 지역사회 중심 사례관리 프로그램 효과)

  • Purpose: The purpose of this study was to analyze effects of a community-based case management program for clients with hypertension living in the community. Methods: The research design was a one group pre and post-test design with 30 participants with hypertension who agreed to participate in the 8-12 week case management program provided by case managers from the National Health Insurance Corporation in 2002. Data were collected three times, before and after the case management services, and 6 months later. Outcomes included changes in blood pressure, knowledge of hypertension and daily life practices, including alcohol consumption, smoking, exercise, and medication adherence. Results: Repeated-measures ANOVA and post-hoc tests of means revealed significant differences before and after service for systolic blood pressure, daily life practices (monitoring body weight and BP, low salt and cholesterol and high vegetable diet, and stress-relief practices), and exercise. The goal for medication adherence was attained after service. Significant improvements from baseline to 6 months after service were observed in measures of salt and vegetables in diet. There were no significant differences on hypertension knowledge, alcohol consumption or smoking behavior between before service and after, and at 6 months. Conclusion: The findings provide preliminary evidence that case management intervention can have positive outcomes on BP control, daily life practices, exercise, and medication adherence for clients with hypertension. However, additional interventions are needed to sustain long-term effects.

A study on the health awareness and attitude of obese high school students (비만 고등학생의 보건행태조사연구)

  • Understanding average abnormal obese high school students' health awareness and attitude, We analyzed problems affect on this to provide basic data for the school health education in the near future. From March, 1997, covering a month period, We investigated 300 average obese students from each 3 school, both boys and girls, around Taegu area. Followings are the summary. 1) Grouping obesity, 86.6% of boys were abdominal type and 54.0% of girls were lump types. 2) IN physical check-up and serum lipid lab., there were a lot of abnormal students for blood pressure, serum glucose, total cholesterol, high density lipoprotein(HDL), low density lipoprotein(LDL), neutral lipid, the rate of lipid in the body, atherosclerosis index and electrocardiogram (ECG).; Especially, for the rate of Lipid in the body, 100.0% were in the abnormal range. 3) The main reasons of stress were on the physique and grade. Over 50.0% of students get rid of their stress by singing and dancing. 4) 46.7% of boys and 56.0% of girls were satisfied with their desks and 51.3% of boys and 48.0% of girls were satisfied with their chairs.; 70.0% of boys and 78.7% of girls usually felt uncomfortable for their desks and chairs. 5) 100.0% of boys and 98.0% of girls have been educated for obesity.; 53.3% of boys and 50.7% of girls were not interested in whether joining in or not, if ‘Obese school’ is open. 60.7% of boys get information about weight loss from television, while 41.3% of girls get it from all sorts of publications.; Both sexes said eating convenience food is the main reason to be obese. Most frequent disease is high blood pressure.; 30.0% of boys and 31.3% of girls were hypertensives. 6) 84.7% of boys and 78.0% of girls are awake to the seriousness of obesity from their families. 58.7% of boys and 66.0% of girls sometimes have meals with their families. For the boys, 30.0% of them eat fruit and 29.3% eat cookies while 37.3% of girls eat cookies for their snacks. Both 57.3% of boys and 40.0% of girls have a snack once a day and they drink soft drinks frequently. Both 32.0% of boys and 46.7% of girls have midnight meals 1-2times a week. 7) Both 45.3% of boys and 50.0% of girls suddenly gained their weight in the elementary school times. 30.0% of boys exercise 1 hour for a week, while 48.0% of girls don't exercise. For the exercise hour, both 53.3% of boys and 42.0% of girls exercise for 10-30 minutes a time. 59.7% of boys and 82.0% of girls strongly don't trust in the informations on weight loss. Both 74.0% of boys and 75.3% of girls think physical therapy is the most suitable way for obesity care.

Association of vitamin E levels with metabolic syndrome, and metabolic syndrome components among adults (성인의 비타민E 수준에 따른 대사증후군 및 대사증후군 요인과의 연관성)

  • This study identified vitamin E associated with metabolic syndrome and metabolic syndrome components among Korean adults aged over 19 years. Secondary data from the 2016-2018 Korean National Health and Nutrition Examination Survey were used for this study. Data from 6,425 were analyzed by logistic regression analysis using a complex sample procedure. As a result of logistic regression analysis, the odds ratio was increased in the group with high vitamin E levels compared to the group with normal vitamin E levels. There are metabolic syndrome (Adjusted Odds Ratio [AOR]: 1.889, 95% Confidence Interval [CI]: 1.550-2.303, p<.001), abdominal obesity (AOR: 1.444, CI: 1.205-1.730, p<.001), hyperglyceridemia(AOR: 3.182, CI: 2.641-3.835, p<.001), systolic blood pressure (AOR: 1.711, CI: 1.446-2.026, p<.001), diastolic blood pressure (AOR: 1.806, CI: 1.521-2.144, p<.001), low high density lipoprotein cholesterol (AOR: 1.558, CI: 1.060-2.290, p=.024). Therefore vitamin E was associated with metaboic syndrome and metabolic syndrome components. So when providing nursing intervention for people with metabolic syndrome, education on vitamin E should be actively included.

  • Cardiovascular disease is the leading cause of death worldwide and is caused by a variety of causes. The highest risk factor for cardiovascular disease is high blood pressure, which has no obvious symptoms, but if left untreated, it causes several complications. In order to treat hypertension, medication and regular exercise are required. In people with high blood pressure, excessive physical activity can put a great strain on the heart and lead to cardiovascular disease. Therefore, there is a need for an exercise intensity monitoring system through PTT measurement that can perform exercise at an appropriate intensity. In this study, we implemented a PTT change monitoring system according to exercise intensity by calculating PTT through ECG and PPG measurement. The implemented system differentiates the R-peak of the ECG and P-peak of the PPG, and calculates the PTT using the time difference between R-peak and P-peak. A running experiment was conducted to monitoring PTT change according to exercise intensity. As a result of the experiment, low intensity PTT is 0.313s, moderate is 0.220s, high is 0.188s, it was confirmed that the PTT decreased as the exercise increase increased.

The Effects of 12-Week Training for the Physical Fitness and Cardiovascular Factors to Examine Physical Fitness on Firefighters Test-Taker (소방공무원 수험생의 체력검정을 위한 12주간 훈련이 체력요인, 심혈관계요인에 미치는 영향)

  • The purpose of this study was to investigate the effects of 12-week training on changes in physical fitness and cardiovascular factors for firefighters. For this purpose, 40 men in their 20s and 30s who agreed to participate voluntarily were recruited. They were divided into four groups: the firefighters' physical fitness test training group (hereinafter referred to as PT group), firefighters' physical fitness test and aerobic training group (hereinafter referred to as PT+AR group), firefighters' physical fitness test and both aerobic and anaerobic training group (hereinafter referred to as PT+CO group). Physical fitness factors (grip strength, back muscle strength, seated forward bend, standing long jump, sit-ups, 20-meter shuttle run), cardiovascular factors (total cholesterol, triglycerides, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, glucose, waist circumference, systolic blood pressure, diastolic blood pressure) and the relationship between Framingham Heart Risk Score and physical/cardiovascular factors were compared and analyzed, and the following conclusions were obtained. Aerobic training, anaerobic training, and combined training, including 12 weeks of firefighter physical examinations, all had positive effects on fitness and cardiovascular factors, which would be an appropriate way for firefighter examinees to improve physical strength and reduce the risk of cardiovascular disease.

Beneficial effect of Combination with Korean Red Ginseng and Morus alba in metabolic syndrome (고과당식이 랫드모델에서 홍삼과 상엽 혼합투여에 의한 대사증후군 개선효과)

  • Objectives : Korean red ginseng and Morus alba L. are used as a traditional treatment for diabetes. This study was designed to elucidate whether combination with Korean red ginseng and Morus alba L. (MPM) ameliorates metabolic syndrome in fructose-fed rats. Methods : Animals were divided into four groups; Control receiving tap water, fructose-fed, rosiglitazone-treated fructose-fed rats, and MPM-treated fructose-fed rats both receiving supplemented with 60% fructose (n=10). The MPM or rosiglitazone groups initially received a high-fructose (HF) diet alone for 8 weeks, with supplementation with MPM or rosiglitazone occurring during the final 6 weeks. Results : MPM and rosiglitazone, synthetic $PPAR{\gamma}$ agonist, treatment significantly prevented the increase in fasting serum glucose, leptin, triglyceride, and low density lipoprotein in the HF group when comparing with the control group. MPM and rosiglitazone also led to an increase in high density lipoprotein level in the HF group. The administration of MPM and rosiglitazone prevented the development of the metabolic disturbances such as impaired glucose tolerance, and blood pressure. MPM suppressed increased expressions of endothelin-1 (ET-1) in HF rat aorta. In addition, MPM significantly increased IR-${\beta}$ and PPAR-${\gamma}$ expression in muscle. Conclusions : Based on these results, we suggest that the administration of MPM improves metabolic syndrome through the alteration in lipid profiles and suppression of insulin resistant and blood pressure.

Effects of Remote Ischemic Pre-Conditioning to Prevent Contrast-Induced Nephropathy after Intravenous Contrast Medium Injection: A Randomized Controlled Trial

  • Objective: We aimed to assess the effects of remote ischemic pre-conditioning (RIPC) on the incidence of contrast-induced nephropathy (CIN) after an intravenous (IV) or intra-arterial injection of contrast medium (CM) in patient and control groups. Materials and Methods: This prospective, randomized, single-blinded, controlled trial included 26 patients who were hospitalized for the evaluation of the feasibility of transcatheter aortic valve implantation and underwent investigations including contrast-enhanced computed tomography (CT), with Mehran risk scores greater than or equal to six. All the patients underwent four cycles of five minute-blood pressure cuff inflation followed by five minutes of total deflation. In the RIPC group (n = 13), the cuff was inflated to 50 mm Hg above the patient's systolic blood pressure (SBP); in the control group (n = 13), it was inflated to 10 mm Hg below the patient's SBP. The primary endpoint was the occurrence of CIN. Additionally, variation in the serum levels of cystatin C was assessed. Results: One case of CIN was observed in the control group, whereas no cases were detected in the RIPC group (p = 0.48, analysis of 25 patients). Mean creatinine values at the baseline, 24 hours after injection of CM, and 48 hours after injection of CM were 88 ± 32 μmol/L, 91 ± 28 μmol/L and 82 ± 29 μmol/L, respectively (p = 0.73) in the RIPC group, whereas in the control group, they were 100 ± 36 μmol/L, 110 ± 36 μmol/L, and 105 ± 34 μmol/L, respectively (p = 0.78). Cystatin C values (median [Q1, Q3]) at the baseline, 24 hours after injection of CM, and 48 hours after injection of CM were 1.10 [1.08, 1.18] mg/L, 1.17 [0.97, 1.35] mg/L, and 1.12 [0.99, 1.24] mg/L, respectively (p = 0.88) in the RIPC group, whereas they were 1.11 [0.97, 1.28] mg/L, 1.13 [1.08, 1.25] mg/L, and 1.16 [1.03, 1.31] mg/L, respectively (p = 0.93), in the control group. Conclusion: The risk of CIN after an IV injection of CM is very low in patients with Mehran risk score greater than or equal to six and even in the patients who are unable to receive preventive hyperhydration. Hence, the Mehran risk score may not be an appropriate method for the estimation of the risk of CIN after IV CM injection.

Relation between Health Status and Intake of Soy Isoflavone among Adult Women in Seoul (서울 거주 성인 여성의 대두 이소플라본 섭취와 건강과의 관련성)

  • This study was conducted to survey isoflavone intake among adult women in menopause with diseases such as metabolic syndrome and osteoporosis and to analyze the relationship between each of these chronic diseases followed by isoflavone intake and the related health risk index. The average age of the subjects was 49.97 years old, while that of the pre-menopausal subjects was 45.14 years, and the post-menopausal subjects was 55.99 years. The average body mass index (BMI), waist-hip circumference, body fat percentage, blood pressure, blood sugar and blood lipid content of the post-menopausal subjects were higher in significant difference than those of the pre-menopausal subjects. The bone density of the hip and spine in post-menopausal subjects was lower in significant difference than that of the pre-menopausal subjects. After menopause, the subjects had a lower ratio of individuals at risk of anemia when compared with the subjects before menopause, but had higher health risk ratio related to each type of chronic disease, including obesity, hypertension, high cholesterol and osteoporosis than the subjects before menopause. The intake frequency of each soybean food was similar among subjects before/after menopause. The most common soybean based foods consumed by the subjects were soybean, soybean curd and soybean paste. The average daily intake level of isoflavone among subjects before menopause was 25.48 mg, while that of subjects after menopause was 32.25 mg. Evaluation of the distribution of the isoflavone level revealed that the pre-menopausal subjects consumed 3.29~78.36 mg and the post-menopausal subjects consumed 3.18~116.59 mg. The intake level by each individual varied greatly. The pre-menopausal subjects had a low BMI index and systolic blood pressure as much as their isoflavone intake level was high. Additionally, the post-menopausal subjects had a low menarche age and high menopause age when their isoflavone intake level was high, the BMI index and waist-hip circumference ratio was highest among individuals with lowest isoflavone intake level. This study showed that there was a possible relationship between soybean isoflavone intake and health problems such as obesity, high cholesterol, and osteoporosis in women after menopause with diseases such as metabolic syndrome and osteoporosis, even if this relationship was not great.

Animal Experiment of the Pneumatic Ventrivular Assist Device (공압식 심실 보조기의 동물실험)

  • Background : Ventricular assist devices(VADs) are being used for patients in postcvardiotomy cardiogenic shock status bridge to cardiac transplant settings and in post-myocardial infarction cardiogenic shock. The VAD which was developed at the Deparment of medical engineering in Dankook University College of Medicine was a pneumatically driven device and can maintain pulsatile flow. The goal of this study is to develop animal experimental models using the VAD and to clarify the reliability and hemodynamic property adequacy of end organ perfusion durability and severity of thrombotic-hemolytic tendency of the device. Material and Method : The pneumatic VAD was applied to 8 adult female lambs, We examined some hemodynamic parameters such as arterial blood pressure pulmonary capillary wedge pressure(pcwp) pulmonary artery pressure(PAP) left atrial pressure hour urine output cardiac index VAD flow EKG to determine the reliability of the VAD and hemodynamic compatibility of the experimental animals within 24 hours of experiment. We also observed the end organ perfusion durability of the VAD and thrombotic-hemolytic property of the VAD after 24 hours of VAD insertion. Result: We could monitor all hemodynamic parameters including pcwp PAP cardiac index EKG, adn hour urine as true clinical settings. We observed that the reliability of the VAD was excellent and the hemodynamic property of the experimental animal and end organ perfusion were adequate within 24 hours of experiment. In four lambs surviving 24 hours after insertion the reliability of the VAD and end organ perfusion were excellent and no thrombotic-hemolytic tendency was noted. However after 15 days of experiment the diaphragm of the VAD was torn and it was recommende that the durability of the VAD should be extended. Conclusion : e conclude that the pneumatic VAD developed at Dankook University Biomedical Engineering has good hemodynamic property and low thromboembolic tendency and presents adequate end organ perfusion but we noted that the durability of the device should be expanded further. It will be possible to do more reliable experiment in the future according to the animal experimental method developed in this study especially with the heart failure models.

Anthropometric, Biochemical Characteristics, Nutrient Intakes and Bone Density by Smoking Period in Elderly Male Smokers: Analysis of Data from Korea National Health and Nutrition Examination Survey (KNHANES), 2008~2011 (흡연 노인 남성의 흡연기간에 따른 신체적, 생화학적 특성, 영양소섭취 및 골밀도 차이 : 2008~2011년 국민건강영양조사 자료 분석)

  • This study investigated anthropometric and biochemical characteristics, nutrient intakes, and bone density of Korean elderly men (over 65 yrs). Data on bone density and anthropometric (height, weight, waist circumference, body mass index and blood pressure) and biochemical (total cholesterol, high-density lipoprotein (HDL)-cholesterol, low-density lipoprotein (LDL)-cholesterol, triglyceride and hemoglobin) characteristics, nutrient intakes, and nutrient density were obtained from the Fifth Korea National Health and Nutrition Examination Survey (KNHANES, 2008~2011). Subjects were categorized into smoking and non-smoking groups, and smoking groups were divided into three groups by smoking period (under 20 yrs, 21~40 yrs and over 41 yrs). Serum triglyceride concentrations of the smoking group aged 21~40 yrs were higher than those of other groups (P<0.01), whereas other biochemical factors were not different. Intake ratios of energy, protein, phosphorus and sodium in subjects were over 100% of Dietary Reference Intake for Koreans (KDRI). Nutrient densities according to intakes of thiamin, riboflavin, and niacin per 1,000 kcal were significantly different among the groups (P<0.05). Bone density of subjects decreased according to smoking period (T-score of total femur in non-smoking group -0.3108, and -0.2918, -0.4941, -0.6847 in smoking group, respectively). Ratio of osteoporosis was 38.1% in the non-smoking group and 44.4%, 51.1%, and 64.0% in the smoking group, respectively. The findings of the present study show that smoking may be associated with bone health, higher ratio of osteoporosis, and low nutrient density in elderly men. Therefore, practical and systematic non-smoking programs are required to improve the bone density of elderly men as well as maintain healthy bone levels and desirable lifestyle.
