Objectives: We aimed to investigate the acute effects of heat stress on body temperature and blood pressure of elderly individuals living in poor housing conditions. Methods: Repeated measurements of the indoor temperature, relative humidity, body temperature, and blood pressure were conducted for 20 elderly individuals living in low-cost dosshouses in Seoul during hot summer days in 2010. Changes in the body temperature, systolic blood pressure (SBP) and diastolic blood pressure (DBP) according to variations in the indoor and outdoor temperature and humidity were analyzed using a repeated-measures ANOVA controlling for age, sex, alcohol, and smoking. Results: Average indoor and outdoor temperatures were $31.47^{\circ}C$ (standard deviation [SD], $0.97^{\circ}C$) and $28.15^{\circ}C$ (SD, $2.03^{\circ}C$), respectively. Body temperature increased by $0.21^{\circ}C$ (95% confidence interval [CI], 0.16 to $0.26^{\circ}C$) and $0.07^{\circ}C$ (95% CI, 0.04 to $0.10^{\circ}C$) with an increase in the indoor and outdoor temperature of $1^{\circ}C$. DBP decreased by 2.05 mmHg (95% CI, 0.05 to 4.05 mmHg), showing a statistical significance, as the indoor temperature increased by $1^{\circ}C$, while it increased by 0.20 mmHg (95% CI, -0.83 to 1.22 mmHg) as outdoor temperature increased by $1^{\circ}C$. SBP decreased by 1.75 mmHg (95% CI, -1.11 to 4.61 mmHg) and 0.35 mmHg (95% CI, -1.04 to 1.73 mmHg), as the indoor and outdoor temperature increased by $1^{\circ}C$, respectively. The effects of relative humidity on SBP and DBP were not statistically significant for both indoor and outdoor. Conclusions: The poor and elderly are directly exposed to heat waves, while their vital signs respond sensitively to increase in temperature. Careful adaptation strategies to climate change considering socioeconomic status are therefore necessary.
Purpose: The purpose of this study is to identify the effect of low sodium dialysate (LSD) rather than conventional dialysate on interdialytic weight gain (IWG), blood pressure (BP), and intradialytic side-effects (ISEs) in patients on maintenance hemodialysis (HD). Methods: The study was performed in 43 patients, who were treated in the university hospital over 8 weeks. Each participant's serum sodium set point was measured using dialysate sodium 140 mEq/L during the first 4 weeks. For the next 4 weeks dialysate sodium concentrate (134,136,and138 mEq/L) was provided to each participant but only to an experimental group (EG) (n=22) based on the individual set point, although 140 mEq/L dialysate was given to all of the control group (CG) (n=21). Consequently, outcomes including IWG, BP, and ISE were evaluated. Results: In EG, difference of pre-HD SBP (p=.047) and post-HD serum sodium (p=.006) were significantly decreased compared to CG. Also, ISEs was not more frequent in EG. However, the differences were not statistically significant, IWG (kg & %), pre-HD DBP, post-HD SBP & DBP, and serum osmolality in EG. Conclusion: Individual application of LSD in patients on maintenance hemodialysis is beneficial to decrease pre-HD SBP and post-HD serum sodium without increasing ISE. Therefore, LSD can be better treatment than conventional dialysate.
Objective : The purpose of this study was to evaluate the risk factors of hematoma enlargement in patients with spontaneous intracerebral hemorrhage(ICH). Methods : A series of 214 ICH patients diagnosed by brain CT scan in our neurosurgery department from June 1995 to July 1998 were reviewed with clinical status, past medical histories, laboratory findings, CT findings and prognosis. Results : In 27 patients(12.6%), the second CT scan showed an enlarged hematoma. Age, sex, and site of hematoma were not related to hematoma enlargement. A long interval(>6 hours) between the onset and the 1st CT scan strongly reduced the incidence of hematoma enlargement. The incidence of hematoma enlargement significantly increased in patients with previous history of hypertension, cerebral infarction and ICH. This analysis also demonstrated the following independent factors predisposed to hematoma enlargement : initial high systolic blood pressure, high serum total protein, low serum albumin, low serum sodium, prolonged prothrombin time(>14 sec) and activated partial thromboplastin time(>29.5 sec), irregular hematoma shape, and combined intraventricular hemorrhage. Prognosis in the group of hematoma enlargement showed high mortality(48.1%) and poor outcome. Conclusion : Patients with previous history of hypertension, cerebral infarction and ICH, and with high systolic blood pressure, prolonged coagulation time, irregular hematoma shape and intraventricular hemorrhage in CT scan should be observed carefully. And, early surgical therapy of large hematoma and meticulous control of blood pressure may decrease the mortality and morbidity in patients with spontaneous ICH.
This study was done to explore the effect of Korean women's salt usage behavior on their sodium intake and excretion according to aging. Dietary sodium intake and salt usage behavior were analyzed to compare the difference between young and middle-aged women. One hundred fifty six young women and 77 middle-aged women without hypertension or any current medication were recruited. Body mass index, waist hip ratio, blood pressure were measured from each subject. Salt usage behaviors were surveyed with questionnaire, sodium intake with 24-hr recall method, and sodium excretion with spot urine. Middle-aged women were more obese than young women according to body mass index and waist hip ratio. Blood pressure was significantly higher for the middle-aged. Young women consumed more fats and middle-aged women more carbohydrates. Middle-aged women consumed more sodium and potassium, and excreted more sodium. Among questionnaire items, kimchi, soup or pot stew, or salted vegetables were found to be related with high sodium diet. Salted vegetables and salted nuts and potato chips were significantly correlated with young women's high sodium diet, while soy sauce on fried food, kimchi, salted vegetables accounted for middle-aged women's high sodium diet. With these results, we concluded that middle-aged women consumed more carbohydrates, less fat, and more sodium and potassium than young women. Middle-aged women frequently choose kimchi, soup or pot stew, or salted vegetables, and they contribute to high sodium intake. We recommend to choose low-salt kimchi, less soup or pot stew, and more fresh vegetables for lower sodium diet.

Objectives: Excess sodium intake has been linked to obesity and obesity-related indices. However, the scientific evidence for this association is inadequate. The purpose of this study was to investigate the association between urinary sodium excretion and obesity-related indices among Korean adults. Methods: A convenience sample of 120 subjects (60 obese and 60 non-obese subjects) were recruited applying frequency matching for sex and age between two groups. Sodium intake level was assessed through 24-hour urine collection. Obesity-related metabolic risk factors, including fasting blood lipid indices, subcutaneous and visceral fat through computed tomography (CT), insulin resistance indices, blood pressure and liver enzymes were measured in all subjects. These obesity-related metabolic risk factors were compared between obese and non-obese group according to sodium excretion levels (<110 mEq/day, 110~180 mEq/day, >180 mEq/day). Results: After adjusting for age, gender, health behaviors (smoking, exercise, drinking), and energy intake, several obesity-related metabolic risk factors, including abdominal circumference, body fat percentage, subcutaneous and visceral fat, triglyceride, and systolic blood pressure were found to be significantly deteriorated as the sodium excretion level increases. In addition, multivariate adjusted-odds ratios of abdominal obesity, high blood triglyceride, and high blood pressure were found significantly higher in the highest sodium excretion group compared to the lowest group. The mean number of metabolic syndrome risk factors was also significantly greater in the highest sodium excretion group than in the lowest group. Conclusions: The current study findings suggested that high sodium intake can affect obesity and metabolic syndrome risk negatively, implying the necessity of future research on low-sodium diet intervention in relation to obesity and related health problems.

Objective : Cerebral hyperperfusion syndrome (CHS) is a serious complication after carotid endarterectomy (CEA). However, the prevalence of CHS has decreased as techniques have improved. This study evaluates the role of strict blood pressure (BP) control for the prevention of CHS. Methods : All 18 patients who received CEA from February 2009 through November 2012 were retrospectively reviewed. All patients were routinely managed in an intensive care unit by a same protocol. The cerebral perfusion state was evaluated on the basis of the regional cerebral blood flow (rCBF) study by perfusion computed tomography (pCT) and mean velocity by transcranial doppler (TCD). BP was strictly controlled (<140/90 mm Hg) for 7 days. When either post-CEA hyperperfusion (>100% increase in the rCBF by pCT or in the mean velocity by TCD compared with preoperative values) or CHS was detected, BP was maintained below 120/80 mm Hg. Results : TCD and pCT data on the patients were analyzed. Ipsilateral rCBF was significantly increased after CEA in the pCT (p=0.049). Post-CEA hyperperfusion was observed in 3 patients (18.7%) in the pCT and 2 patients (12.5%) in the TCD study. No patients developed clinical CHS for one month after CEA. Furthermore, no patients developed additional neurological deficits related to postoperative cerebrovascular complications. Conclusion : Intensive care with strict BP control (<140/90 mm Hg) achieved a low prevalence of post-CEA hyperperfusion and prevented CHS. This study suggests that intensive care with strict BP control can prevent the prevalence of post-CEA CHS.
In oder to study the correlation between daily urinary output of sodium chloride and blood pressure, twenty four hour urine samples were collected from 224 cases (70 male and 154 female) of healthy Koreans whose age varied from 18 to 70 years old. The volume and concentration of sodium, chloride and potassium and total nitrogen were measured, along with the resting blood pressure. Results obtained are summarized as follows; 1. Daily urinary output was increased as a function of age. However, daily urinary output per unit sulface area was maintained at approximately 800 to 900 $ml/m^2$ in all age groups of male and it increased as a function of age in female groups. There was no significant difference between male and female. 2. The daily urinary sodium concentration was decreased gradually acceding to age in both sexes. Daily excretion of sodium was constant regardless of age in both sexes but especially high in 25-39 year female age group, which was slightly greater in males than in females. 3. The daily urinary chloride concentration was at approximately 250 meq/L in all age groups of male and which decreased as a function of age in females. 4. Hence the daily urinary output of sodium chloride was constant in all age groups of males which increased as a function of age in female groups. However, daily excretion of sodium chloride per unit sulface area was maintained at approximately 11 $gm/m^2$ in males and which increased as a function of age in females. 5. The daily urinary excretion of potassium was constant regardless of age in both sexes. 6. Urinary K/Na ratio was maintained at approximately 0.27 in males and 0.33 in females. 7. The daily urinary output of total nitrogen amount was approximately $8{\sim}10$ gm in males and $7{\sim}9$ gm in females. However, daily urinary output of nitrogen per unit sulface area was constant regardless of age in both sexes. 8. The systolic blood pressure was increased gradually according to the age in both sexes and was higher for males than females under 40 years of age. However, there was no significant difference between both sexes in ages over 40 years. 9. Quantitative comparisons indicated that daily urinary output and sodium chloride excretion are higher while daily potassium output, nitrogen excretion and urinary K/Na ratio are significantly lower among Koreans than a among Occidentals. These findings suggest that average Koreans live on low-protein and high-salt diet throughout their livers. Statistical result obtained may he summarized as follows; 10. The relation between blood pressure and sodium concentration of urine. The correlation between systolic blood pressure and sodium concentration was negatively associated for both sexes and the correlation coefficient was significant for females $({\gamma}_1=-.19<-{\gamma}_{152},\;_{0.05}=-0.159)$ and it was not significant for males $({\gamma}_1=-.19>-{\gamma}_{68},\;_{0.05}=-0.232)$ tut may be due to the sample size for males. The correlation between diastolic blood Pressure and sodium concentration was negatively associated for both sexes and the correlation coefficient was significant for males $({\gamma}_1=-.37<-{\gamma}_{68},\;0.05=-0.232)$ and the relation was not significant for females $({\gamma}_1=-.11>-{\gamma}_{152},\;_{0.05}=-0.159)$. 11. The relation between blood pressure and daily urinary sodium chloride excretion. The association between systolic blood pressure and sodium chloride excretion was positively correlated for both sexes and the relation was significant for females $({\gamma}_1=.20>{\gamma}_{152},\;_{0.05}= 0.159)$ and it was insignificant for males $({\gamma}_1=.09<{\gamma}_{68},\;_{0.05}=0.232)$, The relation between diastolic blood pressure and sodium chloride excretion was positively associated and insignificant for both sexes males $({\gamma}_1=.17<{\gamma}_{68},\;_{0.05}=0.232)$ and females $({\gamma}_1=.09<{\gamma}_{152},\;_{0.05}=0. 159)$. 12. The relation between daily urinary nitrogen excretion and sodium chloride excretion. The association between daily nitrogen excretion and sodium chloride excretion was positively significant for both sexes, males $({\gamma}_1=.31>{\gamma}\;_{68},\;_{0.05}=0.232)$ and females $({\gamma}_1=.36>{\gamma}_{-152},\;_{0.05}=0.159)$.
The objectives of this study are to estimate the prevalence of diabetes mellitus (DM) and to identify risk factors associated with fasting blood sugar in a rural adult population. We carried out a cross - sectional study on the residents over 30-year old. Among the 1077 eligible subjects, 725 persons responded to the study;the study consisted of interview on family and past history of DM, anthropometry, blood pressure, and blood sugar level in each subjects. Only 707 subjects of 725 participants had fasting blood sugar(FBS) examined and the rest, 18 subjects were had casual blood sugar examined due to failure of fasting over 8 - hour. When subjects had $FBS\geq140mg/dl$, 2 hours postprandial blood sugar level was checked after 75g oral glucose loading. The prevalence of DM was 8.9%, and age standarized prevalence rate adjusted to Korean population of 1995 was 5.8%. Mean and SD of fasting blood sugar in men was $99.8{\pm}22.6$, and in women was $111.5{\pm}29.9$. Mean levels of fasting blood sugar were significantly higher in women than in man and as their ages advanced the prevalences increased in both sexes. PP2 blood sugar levels were significantly higher in elder age and in persons with higher FBS levels than others. The risk factors associated with FBS were past history of diabetes, sex, socioeconomic status and waist - to - hip circumference ratio;the risk of diabetes was increased in female, people with past DM history, central obesity and low socioeconomic state. The low socioeconomic status associated with DM in this study, which is different from other study results, should be pursued in further studies.

Journal of the Korean Society of Clothing and Textiles Vol. 18, No. 3 (1994) p. 387∼394 In this study, the restriction of slacks (blue jean) versus box pleats skirt was investigated, from the view point of peripheral blood flow at the toe according to motion variation and E.M.G. analysis of leg muscles after walking on the flat or going up and down stairways. Besides, the pressure of slacks on the lower extrimity was measured statically and dynamically. The main results were summerized as follows; 1. The clothing pressure applied by slacks was; each one of thigh and lower leg was 18.2 g/ cm2 and 22.1 g/cm2 in upright, 63.4 g/cm2 and 26.6 g/cm2 in sitting on the chair, on both sides of thigh and lower leg 272.0 g/cm2 over in squatting. 2. When the motion starts from upright, the dynamic clothing pressure reach their peak before the motions end. When the motion ends and the body comes to a still condition, the clothing pressure grow lower and indicate a constant value. but when the body starts moving again to return to upright, the pressures once grow higher and go to zero value after reaching the peak. The pressure on the knee show much greater than those on the hip. This can be because these pressures depend on the degree of skin stretching motion and of its curvature. 3. The surface E.M.G. in leg muscles M. rectus femoris and M. gastrocnemius were recoreded. In the case of wearing slacks, two muscles were activated much more than wearing skirt. 4. The peripheral blood flow at the toe by wearing slacks was lower than wearing skirt. Also the case when squatting, the peripheral blood flow at the toe was low.
This study was attempted to observe leisure time physical activity pattern and its relationship to coronary risk factors (BMI, systolic blood pressure, diastolic blood pressure, serum total cholesterol, triglycerides, fasting blood sugar). Subjects participated in this study were 277 adult male workers of an industry in Kyungpook province, Korea. Energy expenditure were measured using modified Physical Activity History questionnaire of CARDIA study by interviewing. Serum total cholesterol, triglycerides and blood sugar level were tested with 5 ml fasting blood and height, weight and blood pressure were masured. Data on smoking and drinking habits and others were obtained. Geometric mean of leisure time physical energy expenditure were estimated as 212.80 kcal per week for study subjects and only 22.4% of them expended 2,000 kcal or more per week in leisure time physical activity. Taking walks or hikes and jogging or running were more frequent leisure time physical activities in study subjects. Statistically significant mean differences in total weekly leisure time physical activity for all coronary risk factors were not found among three groups. Because energy expenditure of leisure time was generally low in this subjects and most of them were healthy men, we did not found that leisure time physical activity was significantly associated with coronary risk factors.
