Surgical Correction of TO in Adults - 42 Cases Report - (성인 활로씨 사징증의 외과적 교정)

  • This report describes our 10-year experience with intracardiac repair in 42 patients older than 16 year with tetralogy of Fallot. The mean age was 22.0$\pm$5.18 years[range 16~41]. The preoperative clinical manifestations were cyanosis & clubbing[93%], frequent URI [55%], anoxic spell[40%], pulmonary tuberculosis[21%], tuberculous empyema[7%], chronic renal failure[7%], congestive heart failure[7%] and subacute bacterial endocarditis [2%], etc. The previous shunt procedure for palliation had been performed in 7 patients. The type of VSD were typical perimembranous type[67%], total canal defect[28%] and combined type[5%]. The right ventricular outflow tract obstruction were combined type[69%], infundibular type[21%] and valvular type[10%]. Transannular patch was used in 50% of patients. Associated cardiovascular anomalies were ASD[33%], PFO[31%], Rt. aortic arch[10%], Lt. SVC[10%], single Rt. pulmonary artery, single Lt. coronary artery, ASI, proximal stenosis of Rt. pulmonary artery and anomalous systemic venous return, etc. Hospital mortality was 7.1%[3 cases]in overall. The causes of hospital deaths were revealed low cardiac output & acute renal failure[2 cases], postoperative bleeding[1 case]. There were 2 late deaths 3 & 68 months after surgery. Residual intracardiac shunt was detected in 2 patients. one patient was successfully reoperated and another patient had Qp /Qs ratio less than 1.5.

Occlusion and articulation in digital dentistry: A review (디지털 치의학 시대의 교합)

  • With the fourth industrial revolution, digitization is accelerating in all healthcare areas. In the field of dentistry, active discussions on digital dental technologies are ongoing, with increasing interest from clinicians daily. Thus far, accuracy and efficiency have primarily been emphasized in digital dentistry, and interest in occlusion has been relatively low. This is because digital dentistry has been predominantly used to restore small numbers of teeth rather than extensive prosthetic reconstruction. However, in the future, most dental treatments will undergo a digital transformation that will require the application of digital technology to more extensive prosthetic rehabilitation, for which discussion of occlusion is essential. In extensive prosthetic reconstruction, occlusion and articulation involve determining the position of the dental arch in relation to the reference plane of the skull or the long axis of the face and the position of the transverse horizontal axis. It also includes determining an occlusal surface with a shape that allows the mandible to move in an eccentric path and masticate most efficiently without any occlusal interference. To better understand how digitization will impact dentistry, this review article summarizes and discusses occlusion and articulation using digital dental technologies. This discussion is divided into several aspects, including facial scan, virtual articulation, augmented reality, and virtual reality.

A Laterally-Driven Bistable Electromagnetic Microrelay

  • In this letter, a laterally-driven bistable electromagnetic microrelay is designed, fabricated, and tested. The proposed microrelay consists of a pair of arch-shaped leaf springs, a shuttle, and a contact bar made from silicon, low temperature oxide (LTO), and gold composite materials. Silicon-on-insulator wafers are used for electrical isolation and releasing of the moving microstructures. The high-aspect-ratio microstructures are fabricated using a deep reactive ion etching (DRIE) process. The tandem-typed leaf springs with a silicon/gold composite layer enhance the mechanical performances while reducing the electrical resistance. A permanent magnet is attached at the bottom of the silicon substrate, resulting in the generation of an external magnetic field in the direction vertical to the surface of the silicon substrate. The leaf springs show bistable characteristics. The resistance of the pair of leaf springs was $7.5\;{\Omega}$, and the contact resistance was $7.7\;{\Omega}$. The relay was operated at ${\pm}0.12\;V$.

[ $45^{\circ}$ ] OBLIQUE CEPHALOMETRIC ANALYSIS OF MESIODISTAL AXIAL INCLINATION IN NORMAL OCCLUSION (정상교합자의 치축경사도에 관한 $45^{\circ}$ 측모두부방사선 계측학적 연구)

  • The development of good arch form, the orientation of the dentition with relation to the craniofacial skeleton and the establishment of correct relationship of axial inclination of upper and lower teeth are required in normal occlusion, but different teeth present different degrees of axial inclination. The purpose of this study was to investigate the axial inclination of upper and lower teeth by analyzing $45^{\circ}$ oblique and $90^{\circ}$ cephalometric roentgenograms of 35 Korean males and 34 females with normal occlusion. The obtained results were as follows: 1. Mean and standard deviation of mesiodistal axial inclination of upper and lower teeth related to palatal plane and occlusal plane were obtained. 2. Mesiodistal axial inclination of upper first premolar was nearly perpendicular to palatal plane, and the axis of lower first premolar was nearly perpendicular to occlusal plane. 3. There was no difference in the mesiodistal axial inclination of anteriorly positioned teeth between the three groups: third molar eruption into good alignment (Group 1), third molar impaction or partial eruption (Group 2), third molar agenesis (Group 3). 4. There were low correlationships only between the axial inclination of central incisor and the mesiodistal axial inclination of canine and first premolar, but no correlationships between central incisor and posterior teeth behind first premolar.

Experimental Study on Hydraulic Characteristics of Wave Dissipating Modified- Tribar (Modified- Tribar의 수리특성에 관한 실험적 연구)

  • Specially shaped concrete blocks are used for the armor layer of rubble structure for breakers, seawalls, or other shore protection work. In this study, the hydraulic characteristics of the Modified-Tribar(MTB), which addresses the shortcomings of the Arch-Tribar, and the most widely used Tetrapod(TTP) in Korea are examined through hydraulic model tests. The MTB are much more stable than the TTP, as shown through the stability model tests under non-breaking and non-overtopping condition. The value of the stability coefficient(KD) was obtained at around 30. The model tests show that the TTP random two layers and MTB uniform 1.5 layers have similar effects, but the MTB one layer shows slightly low effects in dissipating wave energy. The TTP random two layer model is the most effective in reducing wave overtopping rate, under overtopping condition, while the MTB uniform one layer and the MTB uniform 1.5 layer models follow respectively.

Experimental and numerical investigations on reinforcement arrangements in RC deep beams

  • Reinforced concrete (RC) deep beams are critical structural elements used in offshore pile caps, rectangular cross-section water tanks, silo structures, transfer beams in high-rise buildings, and bent caps. As a result of the low shear span ratio to effective depth (a/d) in deep beams, arch action occurs, which leads to shear failure. Several studies have been carried out to improve the shear resistance of RC deep beams and avoid brittle fracture behavior in recent years. This study was performed to investigate the behavior of RC deep beams numerically and experimentally with different reinforcement arrangements. Deep beams with four different reinforcement arrangements were produced and tested under monotonic static loading in the study's scope. The horizontal and vertical shear reinforcement members were changed in the test specimens to obtain the effects of different reinforcement arrangements. However, the rebars used for tension and the vertical shear reinforcement ratio were constant. In addition, the behavior of each deep beam was obtained numerically with commercial finite element analysis (FEA) software ABAQUS, and the findings were compared with the experimental results. The results showed that the reinforcements placed diagonally significantly increased the load-carrying and energy absorption capacities of RC deep beams. Moreover, an apparent plastic plateau was seen in the load-displacement curves of these test specimens in question (DE-2 and DE-3). This finding also indicated that diagonally located reinforcements improve displacement ductility. Also, the numerical results showed that the FEM method could be used to accurately predict RC deep beams'behavior with different reinforcement arrangements.

  • We report a case in which routine chest roentgenograms of an 840 g infant led to the belief that the peripherally inserted central catheter (PICC) was appropriately positioned within the superior vena cava when, in actuality, it was within the azygous arch. Although many cases of pleural effusions have been reported to be caused by a central venous catheter, a right-sided hydrothorax caused by azygous vein rupture from the use of a PICC is an extremely rare complication. Sudden changes in the condition of a preterm infant with PICC should raise the suspicion of a catheter-related problem.

An Anatomical Study using CT Images for the Implantation of Micro-implants (CT 이미지를 사용한 Micro-implants 식립을 위한 매부학적 연구)

  • Surgical microscrews were introduced and used as one method to provide absolute anchorage. Some clinicians implanted microscrews or miniscrews into the basal bone below the roots of the teeth to evade damage to the roots. Because the implanted microscrews were positioned too low the applied force was insufficient to retract the anterior teeth or protract the posterior teeth, and the use of microscrews or miniscrews seemed limited in applying vertical force. However Park implanted microscrews(micro-implants (1.2mm in diameter)) into the alveolar bone between the roots of the posterior teeth to change the direction of the applied force toward increasing horizontal component of the force. Moreover, these microscrew implants were positioned in the alveolar bone between the roots without causing discernable damage to the roots. This study was performed to provide guidelines and anatomic data to assist in the determination of the safe location for micro-implants. By measuring the CT images from 21 patients, anatomical data were obtained which were then used as a guide to determine the location for the implantation of micro-implants. The thickness of the cortical bones at the alveaolar bone region increased from the anterior to the posterior teeth area. The mandibular posterior teeth area showed thicker cortical bone. A greater distance was observed in distance between the second premolar root and first molar root in the upper arch, between the first molar root and the second molar root in the lower arch. The alveolar bone of the posterior teeth area is considered the best site for the implantation of micro-implants.

Information Flows, Differences of Opinion, and Trading Volumes : An Empirical Study (정보흐름, 의견차이, 거래량에 관한 실증연구)

  • In this study, we empirically investigate the relations between trading volumes and our proxies for information flows and differences of opnion. Econometric methods to analyze the relations in the equity and KOSPI 200 futures markets include Generalized Method of Moment(GMM) and Generalized Autoregressive Conditional Heteroscedasticity(GARCH) models. Major findings from our empirical analyses are summarized as follows; (i) Trading volume in both the equity and KOSPI 200 futures markets varies positively with proxies for information flows. We find that trading volumes in both markets are closely related to firm-specific information rather than market-wide information. (ii) Trading volumes in the equity and KOSPI 200 futures market have positive relations with our proxies for differences of opinion. (iii) Day-of-the-week effect is clear in both markets. Trading volumes in both the equity and KOSPI 200 futures markets tend to be relatively low early and late in the week. (IV) Futures contract life-cycle effect is clear. In other words, futures trading volume increses in the period around contract expiration. (V) In addition, ARCH effect on trading volumes is reported significant enough to take into account. The disturbance of trading volumes in both markets seem to be conditional heteroscedastic.

Evaluation of the Structural Performance of Tetragonal Lattice Girders (사각 격자지보의 구조 성능 평가)

  • In general, the H-shaped steel ribs or triangular lattice girders have been mostly used in constructing tunnels through the NATM construction method. The H-shaped steel rib has higher flexural and axial strength than the triangular lattice girder, but many unexpected gaps can occur in the concrete lining system after shotcreting if the H-shaped steel rib is used as the support system. To achieve better shotcreting quality, the triangular lattice girder was developed. However, in general, the triangle lattice girder has low flexural and axial strength. Likewise, the triangular lattice girder, which has circular sectional members, has so many fractures from welded points at the joints between the members. Finally, the new type of tetragonal lattice girder was developed to overcome those problems. In this study, the structural performance of the tetragonal lattice girders was evaluated through analytical and experimental studies. In the analytical studies, the four-point bending analysis, the traditional evaluation method to determine the flexural strength of the lattice girder, was performed. Moreover, the linear-elastic analysis and stability analysis of the arch structure made by the lattice girders were performed to measure structural performance. Experiments were likewise performed to compare the structural performances of the tetragonal girder with traditional triangular lattice girders.
