저강도 맥동성 초음파 적용이 관절연골세포에 미치는 영향 (Effect of Low Intensity Pulsed Ultrasound in Rat Chondrocyte)

  • Low intensity pulsed ultrasound(LIPUS) is known to accelerate bone regeneration, but the precise cellular signaling mechanism is still unclear. The purpose if this study was to determine the effect of LIPUS on the signaling mechanism of rat chondrocyte. In the explant culture condition, there was inhibition effect of 1 $W/cm^2$ intensity LIPUS on chondrocytes proliferation but chondrocytes proliferation was increased at 0.25 $W/cm^2$ intensity. In addition, western blot analysis of MAPKs showed that LIPUS increased ERK1/2 activity from the 10 min treatment of LIPUS. Hydrogen peroxide($H_2O_2$), resulted in a time- and dose-dependent cell proliferation, which was largely attributed to apoptosis. $H_2O_2$ treatment caused marked sustained nucleus condensation in Hoechst stain. LIPUS and $H_2O_2$ activates phosphorylation of p-ERK1/2 and PD 98059($10^{-5}M$) blocked the effect of LIPUS and $H_2O_2$. Moreover, the synergistic phosphorylation of p44/42 MAPK by $H_2O_2$, LIPUS was selectively inhibited by PD 98059, ERK1/2 inhibitor. In order to determine whether the increase in cell proliferation caused by $H_2O_2$ and LIPUS could be explained by changes in the level of the prostaglandin $E_2$. Our study demonstrated that LIPUS stimulate the cell proliferation via activated phosphorylation of ERK1/2 in condrocyte. LIPUS has anabolic effects on rat cartilage in explant cultures, indicating a potential important method for the treatment of osteoarthritic cartilarge.

  • 본 연구는 저 출력 초음파 장치를 이용하여 osteoblastlike MG-63 세포에 1 MHz, 3 Mz 강도의 초음파 처리 후 조골세포의 변화를 관찰하였다. 3 MHz 강도의 주파수를 osteoblast-like MG-63 세포에 처리한 결과 7일 후 성장률이 대조군보다 증가하였다. 저 출력 초음파가 골의 치유에 영향을 미치는 인자를 확인하였을 때 ALP, osteocalcin, VEGF, colla 1A1의 증가를 확인하였으며, 1 MHz 주파수 처리시 integrin alpha 2의 발현이 대조군과 유사하게 발현되는 것을 확인하였다. 반면에, osteonectin, osteopontin, fibronectin, MMP 2 등은 변화가 없음을 확인하였다. 그러므로 본 연구는 저 출력 초음파 장치는 뼈 치유 촉진 단백질들의 발현을 증가시켜 구강 내 외과적 수술 시 골 치유 촉진 효과에 대한 가능성을 제시하였으며 임상에서 골 치유 촉진을 위한 장비로서의 사용 가능성을 제시하였다고 생각된다.

Histologic evaluation of low-intensity pulsed ultrasound effects on bone regeneration in sinus lift

  • Purpose: Many techniques have been described for achieving vertical augmentation of the maxillary sinus. The aim of this study is to evaluate the effect of low-intensity pulsed ultrasound (LIPUS) to enhance bone regeneration after sinus floor elevation. Methods: The sinus lifting technique was performed through a lateral approach on 8 different sites of 5 patients (3 males and 2 females) and their mean age was 45.7 years old. The sites were randomly assigned to the control or test groups. The control group had 4 sites that received lateral sinus lifting procedure only, while the test group had 4 sites that received LIPUS application after the lateral sinus lifting procedure. 24-32 weeks (an average of 29 weeks) postoperatively, new bone formation in the augmented sinus sites was evaluated through histologic and histomorphometric analyses of the biopsy specimens obtained during implant placement. Results: In the test group, the mean percentage of newly formed bone was $19.0{\pm}2.8$%. In the control group, the mean percentage of newly formed bone was $15.2{\pm}3.1$%. The percentage of newly formed bone was approximately 4% higher in those cases where the sinus was treated by LIPUS than the percentage in those cases where it was not used. The difference was statistically significant. Conclusions: Within the scope of this study, low-intensity pulsed ultrasound application after sinus lifting appeared to have a significant effect on the development of new bone formation.

저강도 초음파 치료기기의 안전성 및 성능평가 가이드라인 수립을 위한 연구 (A Study on the Safety and Performance Test Guideline of Low Intensity Therapeutic Ultrasound Device)

  • In this study, we suggested the performance and safety testing guideline for low intensity pulsed ultrasound (LIPUS) represented by the ultrasound fracture treatment device and cartilage treatment device and low intensity focused ultrasound (LIFU) represented by ultrasonic face lifting device. For these study, the international standards and management regulations of Korea, Japan and United State were analyzed. And the usefulness and applicability were evaluated by testing with commercial equipment and reflecting the views of the industry and experts. As a result of this study, the safety and performance test guidelines for low intensity therapeutic ultrasound device were proposed by presenting the 10 items for LIPUS and 12 items for LIFU. The suggested guidelines are considered a high utilization in the domestic testing and approval authorities. And they are also thought to be useful to new technology development.

The effects of therapeutic ultrasound stimulation on the inflammation cytokine in rat articular chondrocytes

  • Objective: The aim of this study was to investigate the effect of therapeutic ultrasound (US) of cell viability and inflammatory cytokine in rat articular chondrocyte cultures stimulated with lipopolysaccharide (LPS). Design: One group pretest-posttest design. Methods: Cultured chondrocytes were treated with US and/or LPS and assessed for viability, Tumor necrosis factor $(TNF)-{\alpha}$ and Interleukin (IL)-1 production. Results: Oxidative stress was induced in rat chondrocytes with LPS. The cell viability was decreased in chondrocytes after treatment with LPS. The viability revealed that low-intensity pulsed ultrasound (LIPUS) exerted no significant cytotoxicity in the rat chondrocyte. LIPUS inhibited decreased cell viability in the presence of LPS ($30{\mu}g/ml$) in a intensity dependent pattern at LIPUS (p<0.05). $TNF-{\alpha}$ production in the presence of LPS was also inhibited in a dose dependent manner (p<0.05 from $30mW/cm^2$). IL-1 production in the presence of LPS was inhibited as well (p<0.05 from $7.5mW/cm^2$). Conclusions: Our results demonstrate that US was the anti-inflammatory effect of chondrocytes. LIPUS may exert its anti inflammatory effects through inhibition of $TNF-{\alpha}$ and IL-1 synthesis. These results suggest that US have potential for use as a pain relief and reduce the articular destruction.

Effect of low intensity pulsed ultrasound in activating the mitogen-activated protein kinase signaling pathway and inhibition inflammation cytokine synthesis in chondrocytes

  • Objective: Low intensity pulsed ultrasound (LIPUS) has been shown to accelerate cell proliferation and tissue healing in both animal models and clinical trials. However, details of the clinical effects of LIPUS have not been well characterized. The aim of this study was to investigate the effect of LIPUS on mitogen-activated protein kinase (MAPK) activation in rat articular chondrocytes. Design: Cross-sectional study. Methods: Chondrocyte were cultured in six well cell culture plates for 72 hours at $37^{\circ}C$ with 5% $CO_2$, and then exposed to LIPUS at 1.5 MHz frequency and $30-mW/cm^2$ power. Changes in chondrocyte activities were evaluated in response to oxydative stress in dose-dependent (0 and 300 uM) and time-dependent (0-24 hr) manner. The cell viability were analyzed using MTT [3-(4.5-dimethylthiazol-2-yl)-2.5 diphenyltetrazolium bromide]. The expression of p38 MAPK was measured using western blotting. Results: Oxidative stress was induced in rat chondrocytes using hydrogen peroxide ($H_2O_2$). The cell viability was decreased in chondrocytes after the $H_2O_2$ dose and time-dependent treatment. The p38 MAPK phosphorylation occurred at a significantly increased rate after $H_2O_2$ treated (p<0.05). Expression of p38 MAPK was decreased in the p38 inhibitor groups compared with the oxidative stress-induced chondrocyte damage via the p38 MAPK signaling pathways (p<0.05). Conclusions: It could be concluded that LIPUS can inhibit oxidative stress-induced chondrocyte damage via the p38 MAPK signaling pathways.

Brief Retrospect on the Use of Photobiomodulation (PBM) Therapy for Augmented Bone Regeneration (ABR)

  • As technology advances at a rapid rate, innovations in regenerative medicine will eventually include the use of energy-based therapeutics, such as low intensity-pulsed ultrasound stimulation (LIPUs), pulsed electromagnetic field stimulation (PMFs), and low-level laser/light therapy (LLLt) or photobiomodulation therapy (PBMt). Among these treatments, LLLt/PBMt attracted significant attention by the turn of the century, as evidenced by the numerous publications compared to LIPUs and PMFs, particularly for augmented bone regeneration (ABR). This is a testament of how the maturation of technology and scientific knowledge leads to latent compounded applications, even when the value of a technique is reliant on empirical data. This article reviews some of the notable investigations using LLLt/PBMt for bone regeneration published in the past decade, focusing on how this type of therapy has been utilized together with the existing regenerative medicine landscape.

  • Purpose: Osteoporosis, is a major health problem for the elderly and post-menopausal women and shown to alter the properties of bone as well as impair bone healing around titanium implants in both human and animals. The objective of this study was to examine the effect of LIPUS with adipose-derived stem cells on the healing process around a titanium implant in rats with osteoporosis. Methods: Sixteen osteoporosis-induced rats were divided into two groups: an adipose-derived stem cell injected with Low Intensity Pulsed Ultrasound (LIPUS) application group and a control group. Titanium screw implants (diameter, 2.0 mm: length, 3.5 mm, Cowell Medi, Korea) were placed into both tibia of 16 rats, on 8 rats as the control group and the other 8 rats as the experimental group. Rats were sacrificed at different intervals from 1, 2, 4 and 8 weeks after implantation for histopathologic and immunohistochemical analyses. Results: Histopathological analysis revealed newly formed bone in experimental group earlier than that in control group. Especially at 1 week after implantation, more amounts of new bone matrix and collagen around the implant in the experimental group were seen compared with the control group. Immunohistochemical analysis showed that the levels of osteoprotegerin (OPG) expression in the experimental group were increased at early stages compared with that of control group until 2 weeks after implantation. But after 2 weeks, the expression level of OPG similar in both groups. The expression levels of receptor activator of nuclear factor kB ligand (RANKL) were stronger in the experimental group than the control group until 2 weeks after implantation. After 4 weeks, expression of RANKL in experimental group was similar to the control group. Conclusion: The results of this study suggest that LIPUS with Adipose-Derived Stem Cells in implantation could promote bone healing around titanium implants in rats with osteoporosis.

The rats were sacrificed at 1, 2, 4, and 8 weeks after implantation to examine the histopathology and immunochemistry. Results: The histopathology examination revealed earlier new bone formation in the experimental group than the control group. In particular, at 1 week after implantation, more new bone matrix and collagen were observed around the implant of the experimental group compared to the control group. Immunochemistry analysis showed that the level of OPG expression of the experimental group was higher in the early stages than in the control group. After 8 weeks, the levels of OPG expression were similar in both groups. The expression level of receptor activator of nuclear factor kB ligand (RANKL) was stronger in the experimental group than the control group. After 4 weeks, the level of RANKL expression was similar in both groups. Conclusion: These results suggest that the application of LIPUS to implantation can promote bone healing around titanium in osteoporosis animals.

  • Purpose: Diabetes mellitus (DM) has been shown to alter the properties of the bone and impair bone healing around a titanium implant. The aim of this study is to investigate whether the low-intensity pulsed ultrasound (LIPUS), which has been known to stimulate the bone healing, improve the osseointegration of the titanium implant in tibia of DM-induced rats. Methods: 16 rats were received streptozotocin (60 mg/kg) for inducing diabetes. A total number of 32 titanium implants were placed bilaterally into both tibiae of these rats. The right tibia of each rat received LIPUS application (10 min/day) during 7 days post-operation, while the left side received no treatment. The study was carried on for six weeks and the rats were sacrificed at 1, 2, 4, and 6 weaks postoperatively (4 rats for each week) for histomorphometric and histologic analysis. Bone-implant contact and bone area were measured. Comparisons between the groups were made using statistical analysis on histomorphometric analysis. Results: The histomorphometry parameters showed that the bone-implant contact and the bone area values have decreased in the late osseointegration periods (4, 6 weeks) compared to the early osseointegration periods (1, 2 weeks) in both two groups. The bone-implant contact values of the LIPUS group were somewhat higher than those of controls at 1, 2 weeks, but the difference was not statistically significant. The bone area values of the LIPUS group were also higher than those of controls at 1, 2 weeks, but the difference was not statistically significant as well. Conclusion: Results of this study indicate that LIPUS may have positive effects on early osseointegration but could not improve the long term stability of dental implants.
