인체등가형 흉부팬텀과 유리선량계를 이용한 고해상력 및 저선량 CT의 선량측정 (Measurement of Radiation Dose of HR CT and Low Dose CT by using Anthropomorphic Chest Phantom and Glass Dosimetry)

  • MDCT에서 인체등가형 흉부팬텀과 유리선량계를 이용하여 고해상력 및 저선량 CT로 검사하여 영상의 평가 및 흡수선량 및 유효선량을 측정하여 임상 기초자료를 제공하는데 목표를 두고자한다. 인체등가형 흉부팬텀내부에 유리선량계를 삽입하여 조직선량을 측정하였다. 64-slice CT system (SOMATOM Sensation 64, Siemens AG, Forchheim, Germany)과 CARE Dose 4D를 이용하였고, 고해상력 CT에서의 파라메터는 관전압 120 kVp, Eff. mAs 104, scan time 7.93 sec, slice 1.0 mm (Acq. 64×0.6 mm), convolution kernel (B60f sharp)의 스캔 파라메터가 사용되었고, 저선량 CT는 120 kVp, Eff. mAs 15, scan time 7.41 sec, slice 3.0 mm (Acq. 64×0.6 mm), convolution kernel B50f medium sharp의 스캔하였다. 인체등가형 흉부팬텀을 이용하여 스캔에 따른 CTDIvol은 고해상력 CT에서 8.01 mGy, 저선량 CT는 1.18 mGy로 측정되었다. 저선량 CT 검사는 고해상력 CT 검사에 비해 흡수선량이 85.49%가 감소하였고 영상의 차이는 없어 임상에서 유용하게 적용할 수 있다.

  • MDCT의 시간분해능 향상과 등방성 해상능(isotrophic resolution) 영상의 획득, 그리고 지능적인 심전도 동조를 바탕으로 하여 심혈관 질환의 효과적인 진단검사로 인정받고 있는 후향적 심전도 동조화(retrospective ECG gating) 하의 coronary CT angiography는 상대적으로 많은 환자선량을 제공함으로 인해 우수한 진단방법으로서의 장점을 반감시키고 있다. 이에 각 장치 제조사에서는 환자선량을 감소시키는 방법의 연구가 활성화되어 왔으며, 이의 일환으로 지능적인 cardiac dose modulation 기술과 전향적 심전도 동조화(prospective ECG gating)를 사용한 sequential scan이 도입되고 있다. 이에 본 연구에서는 64 채널 MDCT에서 54 kg, 163 cm인 여성 인체모형팬텀을 대상으로 하고 형광유리선량계를 사용하여 후향적 심전도 동조화 하의 coronary CT angiography 프로토콜에서 환자선량의 정량적 평가와 환자선량 감소를 위해 본원에서 선택적으로 적용하고 있는 5가지 검사 프로토콜을 적용하였을 경우의 effective dose와 중요 부위의 organ dose를 측정 비교하여 다음과 같은 결과를 얻었다. 1) Dose modulation없이 120 kVp와 210 mAs의 노출조건으로 retrospectively ECG gated helical scan으로 시행한 conventional coronary CT angiography 프로토콜의 effective dose는 17.8 mSv였으며, 심장의 organ dose는 103.8 mGy였다. 2) 관전압을 120 kVp에서 100 kVp로 낮추었을 경우 effective dose는 11.0 mSv로 conventional coronary CT에 비해 38.2%가 감소하였으며, 심장은 67.3 mGy로 45.2%가 감소하였다. 3) Cardiac dose modulation을 적용한 경우 effective dose는 13.3 mSv로 conventional coronary CT에 비해 25.3%가 감소하였으며, 심장은 80.0 mGy로 22.9%가 감소하였다. 4) 100 kVp의 저관전압과 cardiac dose modulation을 적용한 경우 effective dose는 8.1 mSv로 conventional coronary CT angiography에 비해 54.5%가 감소하였으며, 심장은 49.5 mGy로 52.3%가 감소하였다.

Synergistic Anticancer Activity of 5-Aminolevulinic Acid Photodynamic Therapy in Combination with Low-dose Cisplatin on Hela Cells

  • Objective: Photodynamic therapy (PDT ) is a promising modality for the treatment of various tumors. In order to assist in optimizing treatment, we applied 5-ALA/PDT in combination with low-dose cisplatin to evaluate cytotoxicity in Hela cells. Methods: Antiproliferative effects of 5-ALA/PDT and cisplatin, alone and in combination, were assessed using MTT assay. To examine levels of apoptosis, Hela cells treated with 5-ALA/PDT, and combination treatment were assessed with Annexin-V/PI by flow cytometry. To investigate the molecular mechanisms underlying alterations in cell proliferation and apoptosis, Western blot analysis was conducted to determine the expression of p53, p21, Bax and Bcl-2 proteins. Results: MTT assays indicated that combination treatment obviously decreased the viability of Hela cells compared to individual drug treatment. In addition, it was confirmed that exposure of Hela cells to 5-ALA/PDT in combination with low-dose cisplatin resulted in more apoptosis in vitro. Synergistic anticancer activity was related to upregulation p53 expression and alteration in expression of p21, Bcl-2 and Bax. Conclusion: Our findings suggest that administration of 5-ALA/PDT in combination with the low-dose cisplatin may be an effective and feasible therapy for cervical cancer.

Antioxidant Effect of Annexin A-1 Induced by Low-dose Ionizing Radiation in Adipose-derived Stem Cells

  • Radiation therapy is one of the primary options for the treatment of malignant tumors. Even though it is an effective anti-cancer treatment, it can cause serious complications owing to radiation-induced damage to the normal tissue around the tumor. It was recently reported that normal stem cell response to the genotoxic stress of ionizing radiation can boost the therapeutic effectiveness of radiation by repairing damaged cells. Therefore, we focused on annexin A-1 (ANXA1), one of the genes induced by low-dose irradiation, and assessed whether it can protect adipose-derived stem cells (ADSCs) against oxidative stress-induced damage caused by low-dose irradiation and improve effectively cell survival. After confirming ANXA1 expression in ADSCs transfected with an ANXA1 expression vector, exposure to hydrogen peroxide (H2O2) was used to mimic cellular damage induced by a chronic oxidative environment to assess cell survival under oxidative conditions. ANXA1-transfected ADSCs demonstrated that increased viability compared with un-transfected cells and exhibited enhanced anti-oxidative properties. Taken together, these results suggest that ANXA1 could be used as a potential therapeutic target to improve the survival of stem cells after low-dose radiation treatment.

  • Pretreatment with low concentration of Bleomycin and Cadmium rendered Chinese Hamster Ovary Cells more resistant to the induction of chromosome aberration by subsequent high concentration of same agent, however Mitomycin C did not function in that way. The cells pre-exposed to low dose of Cadmitim did not show cross-resistance to challenge dose of Mitomycin C for the induction of chromosome aberration, but cells pre-exposed to Bleomycin showed cross resistance. And the cells pre-exposed to low dose of Mitomycin C showed cross resistance to challenge of Bleomycin, but Cadmium did not.

Continuous Low-Dose Temozolomide Chemotherapy and Microvessel Density in Recurrent Glioblastoma

  • Objective : The purpose of this study was to evaluate the clinical efficacy of continuous low-dose temozolomide (TMZ) chemotherapy for recurrent and TMZ-refractory glioblastoma multiforme (GBM) and to study the relationship between its efficacy and microvessel density within the tumor. Methods : Thirty patients who had recurrent GBM following Stupp's regimen received TMZ daily at $50mg/m^2/day$ until tumor progression between 2007 and 2013. The median duration of continuous low-dose TMZ administration was 8 weeks (range, 2-64). Results : The median progression-free survival (PFS) of continuous low-dose TMZ therapy was 2 months (range, 0.5-16). At 6 months, PFS was 20%. The median overall survival (OS) from the start of this therapy to death was 6 months (95% CI : 5.1-6.9). Microvessel density of recurrent tumor tissues obtained by reoperation of 17 patients was $22.7{\pm}24.1/mm^2$ (mean${\pm}$standard deviation), and this was lower than that of the initial tumor ($61.4{\pm}32.7/mm^2$) (p-value=0.001). It suggests that standard TMZ-chemoradiotherapy reduces the microvessel density within GBM and that recurrences develop in tumor cells with low metabolic burden. The efficacy of continuous low-dose TMZ could not be expected in recurrent GBM cells in poor angiogenic environments. Conclusion : The efficacy of continuous low-dose TMZ chemotherapy is marginal. This study suggests the need to develop further treatment strategies for recurrent and TMZ-refractory GBM.

폐질환의 선별검사를 위한 저선량 전산화 단층촬영의 적용 (Application of Low-Dose CT for Screening of Lung Disease)

  • 전산화 단층촬영은 폐질환의 정밀진단 방법으로 사용이 증가되고 있다. 통상적으로 시행되는 CT촬영은 조기 진단을 위한 선별검사로써는 높은 방사선 노출과 고비용, 조영제 사용으로 인한 부작용 등으로 적당하지 않다. 영상의학분야에서 방사선을 이용한 CT촬영은 진단 능력이 우수한 반면 피폭선량 또한 매우 높아 선량 감축을 위한 노력이 절실하다. 그동안 많은 연구들에서 무증상기의 폐암 및 폐질환을 조기 발견하기 위한 선별검사로써 저선량 CT의 유용성에 대해 이루어져 왔고, 의미있는 결과를 보고하였다. 선량 감소에 많은 인자가 관여하고 있지만 조사선량(mAs)의 감소가 가장 많이 사용되고 있다. 이미 보고 된 많은 연구에서 저선량 CT는 폐질환을 조기 발견하기 위한 선별검사로써 임상에서 사용하기에 유용하지만, 일부 연구에서 조기 폐암을 예측할 수 있는 작은 결절을 발견하는데 낮은 민감도와 특이도를 보였고, 실제 생존율 증가에는 기여하지 못한 보고도 있었다. 따라서, 지금까지의 저선량 CT에 대한 연구결과를 토대로 다양한 위험에 노출된 집단을 대상으로 선량을 감소시키기 위한 연구가 계속적으로 이루어진다면, 향후 저선량 CT촬영은 폐암을 비롯한 폐질환의 조기진단 방법으로써 통상적으로 사용되고 있는 표준 CT촬영을 충분히 대체할 수 있을 것으로 전망한다.

Non-linear Responses of Hordeum vulgare Germs to Low Doses of Ionizing Radiation

  • The induction of chromosome aberrations in Hordeum vulgare germs after irradiation is studied for the dose range of 10 to 1,000 mGy. The relationship between the frequency of aberrant cells and the absorbed dose is shown to be non -linear and has a dose-independent plateau within the range of 56-467 mGy where the level of cytogenetic damage is statistically significantly distinguished from the spontaneous level. The comparison of the goodness of the experimental data fitting with mathematical models of different complexities, using the most common quantitative criteria, demonstrates the benefit of the piecewise linear model over the linear and polynomial ones in approximating the cytogenetical disturbance frequency. The results of our study support the conclusion about indirect mechanism of chromosome aberrations induced by low doses or dose rates mutagenesis.

Evaluation of the Antidiabetic, Islet Protective and Beta-Cell Regenerative Effects of Tinospora crispa (L.)

  • Tinospora crispa (L.) is a medicinal plant traditionally used to treat various ailments including diabetes. The stem has been widely studied for its antidiabetic properties, however the antidiabetic potential of its leaves has not been explored. This study investigates the antidiabetic properties of methanolic T. crispa stem and leaves extracts on alloxan-induced diabetic rats. The stems and leaves were extracted using Soxhlet extraction with methanol as solvent. Crude extracts were administered at 500 mg/kg body weight (BW) (high dose) and 250 mg/kg BW (low dose) via oral route to alloxan induced diabetic rats. T. crispa stem and leaves extracts was found to significantly reduce blood glucose following a twelve-week treatment period. The highest mean difference in blood glucose level was exhibited by animals in the high dose treated stem and low dose leaf extracts. Both extracts showed approximately 75% percentage recovery from hyperglycaemia. The highest regenerative capacity was observed in animals treated with the low dose leaf extract.

저선량 방사선이 고추의 초기생육과 후속고선량 내성에 미치는 영향 (Effects of Low Dose Gamma Radiation on the Early Growth of Pepper and the Resistance to Subsequent High Dose of Radiation)

  • 저선량 방사선 조사한 고추 2품종, 조광고추와 조흥고추의 종자 발아율과 초기생육 및 고추 유식물체의 후속 고선량에 대한 내성 변화를 알아보고자 저선량 0, 4, 8, 20, 50 Gy를 조사하여 재배한 고추 유식물체에 ${\gamma}$선 50 Gy와 100 Gy를 조사한 후 생육상황을 관찰하였다. 저선량 조사한 고추 유식물체의 초기생육 촉진과 후속고선량 내성은 품종에 따라 달랐는데, 조광고추보다 조홍고추 품종에서 뚜렷한 증가효과를 나타냈고 발아율과 초기생육촉진은 4, 8, 20 Gy 조사구에서 효과적이었다. 조홍고추 유식물체의 후속고선량에 대한 내성은 사전 저선량 조사에 의해 크게 증가되었는데 특히 4Gy조사구에서 효과가 가장 뚜렷하였으며 후속고선량에 대해 내성이 증진된 고추 유식물체중의 carotenoid와 단백질 함량 및 CAT활성은 4 Gy와 8 Gy 조사구에서 높았다.
