An application and evaluation of Alzheimer's Garden Audit Tool(AGAT) to assess outdoor space of the long-term care facility in Korea (국내 노인전문시설 옥외공간 평가를 위한 외국도구(AGAT)의 적용 후 평가)

  • Purpose: The aging population is the quickly increasing in Korea. Since 2008, the National Long term Care Insurance, a number of long term care facilities have established during short time in Korea. Especially, the environmental assessment tool is important for managing healing environment in a long term care facility for the elderly. Alzheimer's Garden Audit Tool (AGAT) is used to assess whether a garden incorporates those elements and qualities as healing outdoor space. In this paper, we discuss the benefits, limitations, and future directions of the assessment tool for long-term care facilities. Methods : The AGAT audit was done through content analysis by 5 experts from diverse discipline as post occupancy evaluation of a long term care facility located in suburban area. The expert group was asked to describe their comments for modification and improvement in application of AGAT. Results : The results of this study show that it is necessary to refine the instrument's items to better meet the needs of the criteria and items for Korean culture-friendly tool. Especially, greenhouse elements (various plants, birds etc.), multisensory experiences, users-oriented space, programmed activities in garden, and hazards on pathway has somewhat limitation as measurement to assess outdoor of long-term care facilities in Korea. Implications : Consequently, AGAT could be applicable to evaluate the outdoor space of long term care facility in Korea with culturally sensitive revision.

CNN-LSTM Coupled Model for Prediction of Waterworks Operation Data

  • In this paper, we propose an improved model to provide users with a better long-term prediction of waterworks operation data. The existing prediction models have been studied in various types of models such as multiple linear regression model while considering time, days and seasonal characteristics. But the existing model shows the rate of prediction for demand fluctuation and long-term prediction is insufficient. Particularly in the deep running model, the long-short-term memory (LSTM) model has been applied to predict data of water purification plant because its time series prediction is highly reliable. However, it is necessary to reflect the correlation among various related factors, and a supplementary model is needed to improve the long-term predictability. In this paper, convolutional neural network (CNN) model is introduced to select various input variables that have a necessary correlation and to improve long term prediction rate, thus increasing the prediction rate through the LSTM predictive value and the combined structure. In addition, a multiple linear regression model is applied to compile the predicted data of CNN and LSTM, which then confirms the data as the final predicted outcome.

The Effects of Visiting Nursing Services in Long-term Care Insurance: A Difference-in-Difference Analysis (이중차이분석을 활용한 노인장기요양보험제도의 방문간호서비스 효과)

  • Purpose: The purpose of this study was to demonstrate the effects of visiting nursing services in long-term care (LTC) insurance on changes in health status and physical functions among the elderly. Methods: We analyzed survey data on the living status and welfare needs of the elderly from 2008 and 2011 using the difference-in-difference method with propensity score matching. Results: The subjects were 76 elders including 38 visiting nursing service users(intervention group) and 38 home-based LTC service users (control group). Results from DID analysis confirmed that the subjective health status level of the intervention group was 2.2 points higher (p=.044) and the depression level was 0.3 point lower (p=.039) than the control group. In addition, the intervention group's ADL score was 1.9 (p=.027) and IADL 3.9 (p=.030), showing that their health was deteriorated less. Conclusion: Visiting nursing service in LTC insurance was associated with delayed deterioration of subjective health status, depression, ADL and IADL. These findings suggest the need of rebuilding visiting nursing service programs focusing on prevention services, which will be more contributive to elderly health care and the reduction of social costs.

Analysis of the Change of the Neck Pressure Pain Threshold in Long Term Computer Users (장시간 컴퓨터 사용자의 경부압력통증역치 변화 분석)

  • Poor posture of the neck and head has long been recognized as a factor contributing to the onset and perpetuation of pain in the head and neck region. This study were to evaluate the change of the neck pressure pain threshold in long term computer users. To elucidate change of the neck pressure pain threshold in long term computer users, the effect of computer using time(3, 6, 9, 12 and 15 hours) on neck pressure pain threshold were studied in 20 subjects. Neck pressure pain threshold were recorded 3, 6, 9, 12 and 15 hours group, and evaluated by pressure algometry to Trapezius muscle, Sternocleidomastoidius muscle, Suboccipitalis muscle and Temporalis muscle. Neck pressure pain threshold was significantly larger in 15 hours group(p<.05). But relation between neck pressure pain threshold in male group and female group were not significant differences(p>.05).

Magnitude and its effected factors of non-covered services expenditures among long-term care facilities benefits user in Long-term Care Insurance (노인장기요양보험 시설서비스 이용자의 비급여 본인부담 크기 및 영향요인)

  • The purpose of this study is to understand magnitude and its related factors of user's cost-sharing for non-covered services in long-term care facilities. We corrected data for 1,016 subjects, based on the long-term care benefits cost specification. Eighteen subjects were excluded from the data analysis due to missing data on family care-givers characteristics. Finally, 998 subjects were included in the study. The average cost of non-covered services per month was 209,093 won and distributed from 0 to 1,011,490 won. There was a significant difference by the characteristics of family care-givers and long-term care facilities. The monthly average cost for meal materials per person was 199,181 won(0~558,000), average cost of additional charge caused by using private bed was 232,992 won (50,000~600,000), and costs for haircut and cosmetics were 8,599 won. For the rest, there were various programs costs(93,328 won), diaper and its disposal cost(109,628 won), purchase cost for daily necessaries(24,435 won) and etc. The related factors for the magnitude of non-covered services expenditures were education level of family care-givers, occupancy rate and location of LTC facilities, and the costs of using private bed, haircut and cosmetics, and various programs among non-covered services. These findings suggest that present level range of LTC facilities users' cost-sharing is wide and it is urgent to prepare the standard guideline for cost and level in non-covered services.

  • As malicious apps vary significantly across Android malware, it is challenging to prevent that the end-users download apps from unsecured app markets. In this paper, we propose an approach to classify the malicious methods based on permissions using Long Short Term Memory (LSTM) that is used to embed the semantics among Intent and permissions. Then the malicious method that is an unsecured method will be removed and re-uploaded to official market. This approach may induce that the end-users download apps from official market in order to reduce the risk of attacks.

  • Enhanced radio access technologies (RAT) are deployed in Next Generation Convergence Networks by the service providers so as to satisfy the basic requirements of end-users for e.g. QoS. Whenever the available resources are being shared simultaneously and dynamically by multiple users or distribution of allocated channels randomly, the deficiency of spectral resources and dynamic behavior of Network traffic in real time Networking, we may have problem. In order to evaluate the performance of our proposed algorithm, computer simulation has been performed on NS-2 simulator and a comparison with the existing algorithms has been made.

Comparative Analysis on the Characteristics of High Cost Medical Users between the Health Insurance and Medical Assistance Program (고액진료비 환자의 특성 비교분석 - 의료보험과 의료보호환자를 중심으로 -)

  • Background : A small number of high cost patients usually spend a larger proportion of scarce health resources. Aged, long-term care and readmitted patients usually belong to these high cost patient group. Among others, long length of stay and readmission can be reduced by checking its cause, and these are the areas needed most of quality improvement activity. Characteristics of high cost medical users between health insurance program and medical assistance program were reviewed. Methods : The inpatient claims of health insurance and medical assistance program were analyzed. Patients were divided by 6 groups; long-term, mid-term, short-term, readmitted, cancer and aged. We defined high cost patients as those who had spent one and half million won and over per 6 months. Characteristics of high cost patients for each group were reviewed. Results : medical assistance patients used much more resources than the insured members in the average hospital cost per case but less in daily hospital cost. The former had a longer length of stay and had much heavier diseases. Major diseases of both group were cancer, diseases of circulatory system and chronic degenerative diseases. Gallstone and schizophrenia were more in the insured program. However, pulmonary tuberculosis, asthma were more common among the medical assistance patients. Early readmission before 2 weeks were 28-30% of the total readmission. Readmission rate in the malignat neoplasm and renal failure were 80% and more. Q.A program should be installed to prevent unnecessary readmissions. Conclusion : Almost 30% of early readmissions and admissions due to complications and long length of stay should be reviewed carefully to keep cost down and to enhance the quality of hospital care.

  • The most of the goals of pump designers and users are efficient operation and productivity. But the safety-grade pumps in nuclear power plants are needed to operate continuously for an essential condition of system operation. Also, most of the rules and regulatory standards that have been prepared for nuclear pumps are dedicated to achieve public safety. The study examined pump vibration in a pump outlet flow and distinguished the regions of pump vibration frequency cause by cavitation and recirculation. The study made a counterproposal in determination of pump outlet flow so that the discharge flow will be able to minimize the long-term damage of the pump.

  • Meteorological Joint Frequency Function required indispensably in long-term air quality prediction models were discussed for practical application in Korea. The algorithm, proposed by Turner(l964), is processed with daily solar insolation and cloudiness and height basically using Pasquill's atmospheric stability classification method. In spite of its necessity and applicability, the computer program, called STAR(STability ARray), had some significant difficulties caused from the difference in meteorological data format between that of original U.S. version and Korean's. To cope with the problems, revised STAR program for Korean users were composed of followings; applicability in any site of Korea with regard to local solar angle modification; feasibility with both of data which observed by two classes of weather service centers; and examination on output format associated with prediction models which should be used.
