Objectives: This study was conducted to prepare basic data to propose the necessity and utilization of oral welfare products in the welfare services of the long-term care insurance system, focusing on facility workers working in elderly facilities. Methods: The analysis was conducted on 144 workers working at some local elderly facilities. The questionnaire was constructed by classifying the use of oral welfare tools into 6 questions and the necessity and demand for oral welfare devices into 13 questions. Frequency analysis and technical analysis were performed for data analysis, and one-way ANOVA was performed for differences in the necessity and demand for oral welfare equipment. The statistical significance level was p<0.05. Results: As a result of examining the awareness of the necessity and demand for oral welfare equipment among workers in elderly facilities, the awareness of the necessity of including oral welfare equipment in the items of welfare equipment in the current long-term care insurance system was high at 4.15 points. As a result of analyzing the correlation between awareness of care products and the need and demand for oral welfare equipment, it was confirmed that there was a statistically significant positive correlation (p<0.01). Conclusions: In the long-term care insurance system for the elderly, oral welfare products need to be considered for welfare equipment services. The provision of oral welfare products within the long-term care insurance system for the elderly can provide opportunities and services to select various self-care tools. In addition, it is expected that it will be possible to promote changes in the long-term care insurance system for the elderly and to improve the system in a variety of positive ways.
Background: This study aimed to establish a strategy to improve the poor working environment and working conditions among long-term healthcare workers in Korea. Methods: A total of 600 questionnaires with which long-term health care workers participated in the targeted base areas of each city and province nationwide were distributed directly and 525 responses were collected and 506 responses were analyzed. Surveys, on-site field visits, and in-depth interviews were also conducted to understand the working environment as well as conditions and establish a strategy for improving the working environment among long-term healthcare workers to understand the demands of working conditions and working conditions. Results: Korean long-term care workers firstly and mostly enumerated their risk factors for ill-health when lifting or moving elderly recipients directly by hand (69.9%), followed by increased physical workload with old beds, tools, and facilities (42.3%) in the workplaces, shortage of manpower (32%), and source of infection (30%). To improve the working environment as well as conditions, Korean long-term care workers considered improving low-wage structures, ergonomic improvements to solve excessive physical loads, and increasing various bonus payments as well as implementing the salary system, positive social awareness, and increasing resting time. Of 506 responses, 92.3% replied that the long-term care insurance system for the elderly should be developed to expand publicization at the national level. Conclusion: This study proposes to improve the low-wage structure of Korean long-term care workers, automation and improvement of facilities, equipment, and tools to eliminate excessive physical loads (beneficiary elderly lifting), and reduction of night labor.
This study purports to investigate the determinant of job satisfaction among workers working at Long-term care hospitals. The independent variables contain three groups of determinants: organizational characteristics variables(job autonomy, job variety, distributive justice, role conflict, supervisor support, job suitability, job significance, job security, organizational support, job growth, promotional opportunity), environmental variables(job opportunity), and psychological variables (met expectation, job efficacy, positive affectivity, and negative affectivity). The sample used in this study consisted of 250 workers from 4 Long-term care hospitals nationwide. Data were collected with self-administered questionnaires and analyzed using multiple regression analysis. The results of the study are as follows: 1) the following variables, listed in order of size, have significant effects on job satisfaction: negative affectivity(-), job significance(+), job growth(+), age(+), positive affectivity(+), organizational support(+), job opportunity(-). 2) the variance of job satisfaction explained by the variables used in the study are 53.8%. When demographic variables added to Model I, job satisfaction explained by variables are 55.4%. 3) the results of this study indicate that three variables of negative affectivity, job significance, job growth are especially important for improving the level of job satisfaction among workers at Long-term care hospitals.
본 연구의 목적은 서비스 공급자로서 재가요양서비스를 제공하고 있는 요양보호사의 서비스에 대한 평가를 통해 서비스 품질 개선 방안을 찾는 것이다. 이를 위해 부산에서 활동 중인 요양보호사 223명을 대상으로 직무 환경, 서비스 기관에 대한 평가, 재교육 등에 대한 내용을 중심으로 설문조사를 실시하였다. 조사결과 요양보호사는 학력은 낮지 않으나 연령대가 높고 불안정한 고용형태를 가진 이가 대다수였다. 더 나은 요양서비스를 위해 희망하는 슈퍼비전 내용은 서비스 이용자와의 관계성, 이용자 욕구 변화에 따른 대처 등이었다. 우선적으로 필요한 재교육 내용은 노년기 및 노인 가족에 대한 이해, 노인성 질환에 대한 보건 의료적 지식, 이용자 및 가족과의 상담기법 등으로 나타났다. 그리고 건강보험공단 등에 바라는 것은 이용자와 보호자의 인식 개선, 급여 인상, 그리고 전문성 강화 위한 교육 기회 확대 등이었다. 이상의 연구결과를 통해 요양보호서비스 향상을 위한 방안으로 서비스 공급구조적 측면에서의 개선과 요양보호사와 업무에 대한 인식개선, 적절한 슈퍼비전과 재교육 기회 제공 등을 제안하였다.
본 연구는 한국의 장기요양서비스 제도적 맥락 속에서 발생한 시장화와 공존하고 있는 공공성의 관점에서 요양보호사의 근로조건이 서비스 질에 미치는 효과를 실증분석 하였다. 기존 연구들은 사회서비스의 낮은 생산성과 시장화로 가중된 저임금노동시장의 문제가 서비스 질 저하를 가져온다고 추론한 것이지, 근로조건과 서비스 질의 두 변수간의 인과관계를 규명한 것이 아니다. 본 연구의 노인장기요양서비스 제도적 맥락에서 본 요양보호사의 근로조건과 서비스 질과의 이론적 관련성은 Daly와 Lewis(2000)의 돌봄의 통합적 접근을 통해 조망하였다. 자료는 전국에 있는 노인장기요양시설을 소유권을 중심으로 비비례층화표집하여 248개 조직에 대한 설문조사를 통해 수집하였고, 이를 다시 인터넷 자료와 병합하였다. 분석결과, 요양보호사의 근로조건 가운데 3개 영역에서 임금수준이 높을수록, 부가급여가 존재할수록, 교육 및 훈련을 지원할수록 서비스 질이 높은 것으로 나타났다. 이는 정부가 규제하는 노동시장정책과 평가제도상의 최소기준을 넘어서는 조직의 규범적 차원에서의 근로조건과 보상체계가 장기요양서비스 질에 긍정적인 영향을 미치고 있다는 것을 보여준다.
본 연구는 노인 장기요양기관 종사자인 요양보호사들의 업무 스트레스와 대처전략을 파악하고 이 요인들이 생활만족도에 미치는 영향을 확인하고자 시도되었다. 연구 대상은 경기도에 소재한 노인장기요양기관(재가센터, 요양원, 요양병원)에 근무하는 요양보호사 111명을 대상으로 하였다. 자료수집을 위한 설문조사는 2015년 2월 1일부터 3월 30일까지 연구자가 노인장기요양기관에 방문하여 요양보호사들에게 연구에 대해 설명 후 설문지를 회수하는 방법으로 실시하였다. 연구결과 생활만족도에 유의한 영향을 주는 인구학적 변수는 시설형태(F=4.11, p=.020)였으며, 생활만족도와 업무스트레스는 음의 관계(r=-.46, p<.001), 대처전략과는 양의 관계(r=.21, p=.040)로 나타났다. 회귀분석 결과 생활만족도에 유의한 영향을 미친 변수는 업무스트레스, 대처전략, 시설형태였으며 24.0%의 설명력을 가졌다. 본 연구 결과를 통해 요양보호사의 생활만족도에서 업무스트레스와 대처의 중요성을 확인할 수 있을 뿐 아니라 근무시설 에 따른 차이를 확인할 수 있었고 차별화된 직무 환경 개선이 필요할 것으로 사료된다.
Purpose: This study was conducted to examine whether the level of classification for long-term care service under longterm care insurance reflects resource utilization level for residents in nursing homes. Methods: From 2 long-term care facilities, the researchers selected 95 participants and identified description and time of care services provided by nurses, certified caregivers, physical therapists and social workers during a 24-hr-period. Results: Resource utilization level was: 281.04 for level 1, 301.05 for level 2 and 270.87 for level 3. Resource utilization was not correlated with level. Differences in resource utilization within the same level were similar with the coefficient of variance, 22.7-27.1%. Physical function was the most influential factor on long-term care scores (r=.88, p<.001). The level for long-term care service did not reflect differences in resource utilization level of residents on long-term care insurance. Conclusion: The results of this study indicate that present grading for long-term care service needs to be reconsidered. Further study is needed to adjust the long-term care classification system to reflect the level of resource utilization for care recipients on the long-term care insurance.
본 연구에서는 요양보호사의 인간중심케어와 직무만족, 감정노동, 서비스 질 간의 관계를 분석하였다. 연구대상 및 조사방법은 전국의 장기요양시설에 근무하는 요양보호사 460명을 대상으로 자기 기입식 설문지를 활용하였다. 연구모형에서 독립변수는 인간중심케어, 매개변수는 직무관련 감정노동(표면행위, 내면행위)과 직무만족, 종속변수는 서비스질로 구성하였다. 모형의 적합성과 서비스 질에 영향을 미치는 각 요인들의 직접효과 및 간접효과를 검증한 결과 모형의 적합도는 적합한 수준이었고, 감정노동 중 내면행위가 서비스 질에 영향을 미치는 중요한 변수임을 알 수 있었다. 특히, 감정노동은 인간중심케어의 가치와 서비스 질에 매우 큰 영향을 미치고 있어 중요한 매개변인 역할을 한다는 것을 확인할 수 있었다. 또한, 인간중심케어가 감정노동을 매개로 하여 서비스 질에 영향을 미치는 총 효과는 통계적으로 유의미한 것으로 나타났고, 내면행위와 서비스질의 관계는 정(+)적 상관관계를 보인 반면, 표면행위와 서비스질의 관계는 부(-)적 상관관계를 갖고 있음을 확인할 수 있었다. 위의 연구 결과를 토대로 인간중심케어에 있어서 요양보호사들의 감정노동과 서비스 질에 대한 임상적, 학문적 관심을 환기시킬 필요가 있음을 제안하였다.
Purpose: The purpose of this study was to investigate factors influencing health workers' infection management behavior in long-term care facilities. Methods: A descriptive cross-sectional survey was conducted with 180 health workers who are employed in long-term care facilities. The data were collected from April, 25 until July, in 2016. Results: Infection management behavior positively correlated with the perceived importance of infection management (r=.77, p<.001), but role conflict negatively correlated with infection management behavior (r=.28, p<.001). The hierarchical regression model with general characteristics (first step) and perceived importance of infection management, work environment, and role conflict (second step) against infection management behavior was statistically significant (F=31.93, p<.001). This model could explain 62.8% of infection management behavior ($R^2=.62$, ${\Delta}R^2=.39$). Particularly, perceived importance of infection management was identified as factors influencing infection management behavior(${\beta}=.70$, p<.001). Conclusion: The findings of this study imply that systemic education about infection control and monitoring should be considered, so as to encourage proper infection management behaviors among health workers in long-term care facilities.
Long-term care Insurance system for the Elderly was started to respond to the aged society on July 2008 in Korea. So responsibility of the care for the old is being changed from the family to the society. The introduction of that insurance system has brought the growth of demand for care service. To improve the level of quality for care service providers in long-term care, the role of care worker in providers is recognized as important. But welfare system for the care worker seems to be poor. So the Seoul Metropolitan Council is preparing an ordinance for the care worker and the purpose of this study is for the ordinance as well. Questionnaires, interviews, documentary surveys were used to carry out this study. The results of this study are summarized as follows. The job stability is the most important thing for the care worker, so local government has to make efforts to promote the job stability of care worker. A welfare center for care workers is needed for the health for care workers in the local government. And the local government has to supervise service providers more strictly and evaluate them in treatments of care worker.
