Managing Complications in Abdominoplasty: A Literature Review

  • Background Abdominoplasty, with or without liposuction, is among the most frequently performed aesthetic procedures. Its main objective is to improve the body contour by means of excising redundant skin and fat tissue. Although abdominoplasty is considered a safe procedure with high satisfaction rates, intraoperative and postoperative complications can become a challenge for the surgical team. The aim of this article is to offer a synopsis of the most common complications arising after abdominoplasty, along with evidence-based guidelines about how to prevent and treat them. Methods A systematic MEDLINE search strategy was designed using appropriate Medical Subject Headings (MeSH) terms, and references were scanned for further relevant articles. Results According to the published case series, local complications are considerably more common than complications with systemic repercussions. Approximately 10% to 20% of patients suffer a local complication following abdominoplasty, while fewer than 1% suffer a systemic complication. Prevention and management strategies are critically discussed for complications including seroma, haematoma, infection, skin necrosis, suture extrusions, hypertrophic scars, neurological symptoms, umbilical anomalies, deep venous thrombosis and pulmonary thromboembolism, respiratory distress, and death. Conclusions The complications of abdominoplasty vary in severity and in the impact they have on the aesthetic outcomes. Recommendations for prevention and management are based on various levels of evidence, with a risk of observer bias. However, most complications can be treated appropriately following the current standards, with satisfactory results.

Improvement of Active Shape Model for Detecting Face Features in iOS Platform (iOS 플랫폼에서 Active Shape Model 개선을 통한 얼굴 특징 검출)

  • Facial feature detection is a fundamental function in the field of computer vision such as security, bio-metrics, 3D modeling, and face recognition. There are many algorithms for the function, active shape model is one of the most popular local texture models. This paper addresses issues related to face detection, and implements an efficient extraction algorithm for extracting the facial feature points to use on iOS platform. In this paper, we extend the original ASM algorithm to improve its performance by four modifications. First, to detect a face and to initialize the shape model, we apply a face detection API provided from iOS CoreImage framework. Second, we construct a weighted local structure model for landmarks to utilize the edge points of the face contour. Third, we build a modified model definition and fitting more landmarks than the classical ASM. And last, we extend and build two-dimensional profile model for detecting faces within input images. The proposed algorithm is evaluated on experimental test set containing over 500 face images, and found to successfully extract facial feature points, clearly outperforming the original ASM.

THE USE OF A VARIETY OF INTRAORAL FLAPS IN RECONSTRUCTION OF INTRAORAL SOFT TISSUE DEFECTS (구강내 연조직 결손 재건을 위한 다양한 구내피판의 이용)

  • The purpose of this study is to describe the clinical availability of a variety of intraoral local flaps in reconstruction of oral soft tissue defects, Forty patients with oral soft tissue defects were treated by tongue, buccinator, palatal, labial, facial artery musculomucosal, buccal fat pad, and masseter muscle crossover flap. Total 43 intraoral flaps were used to reconstruct a variety of intraoral soft tissue defects, such as oronasal fistula, oroantral fistula, traumatic deformities and other. The age of patients ranged from 7 to 72 years, with mean age of 39.6 years. Follow up period ranged from 2 to 66 months, mean follow up period of 21.6 months. There were 9 complications, of which four were partial necrosis, three infections, one total necrosis, and 1 speech problem. Except for total necrosis, most of the recipient sited healed uneventually without severe morbidity. We consider that a variety of intraoral local flaps can be available for reconstruction of small of moderate large intraoral soft tissue defects.

  • Mobile devices like smartphones and tablets are becoming increasingly capable in terms of processing power. Although they are already used in computer vision, no comparable measurement experiments of the popular feature extraction algorithm have been made yet. That is, local feature descriptors are widely used in many computer vision applications, and recently various methods have been proposed. While there are many evaluations have focused on various aspects of local features, matching accuracy, however there are no comparisons considering on speed trade-offs of recent descriptors such as ORB, FAST and BRISK. In this paper, we try to provide a performance evaluation of feature descriptors, and compare their matching precision and speed in KD-Tree setup with efficient computation of Hamming distance. The experimental results show that the recently proposed real valued descriptors such as ORB and FAST outperform state-of-the-art descriptors such SIFT and SURF in both, speed-up efficiency and precision/recall.

Contents and Fatty Acid Compositions in Fats Extracted from Ice Creams and Ice Cream-Related Products (아이스크림 제품류에 함유되어 있는 지방함량 및 지방산조성)

  • The objective of the study was to analyze the contents and fatty acid compositions in the extracted fats from selected commercial ice creams and ice cream-related products. Seventy four ice creams and ice cream-related products were collected from local stores: 22 regular 'ice creams', 10 premium 'ice creams', 22 'ice milks', 4 'sherbets', 11 'non-milk-fat ice creams' and 5 'non-milk product ice creams'. Contents and fatty acid compositions of the fats in the ice creams and ice cream-related products were analyzed. Fat contents in regular 'ice creams', premium 'ice creams' and 'ice milks' were $5{\sim}11%,\;13{\sim}17%\;and\;2{\sim}10%$, respectively. 'Sherbets', 'non-milk-fat ice creams' and 'non-milk product ice creams' contained $2{\sim}7%,\;4{\sim}11%\;and\;1{\sim}2%$ fats, respectively. Fats extracted from 14 regular 'ice creams', all of the premium 'ice creams' and 11 'ice milks' contained $63{\sim}75%$ saturated fatty acids and $2{\sim}5%$ trans fatty acids. Their fatty acid compositions were similar to those in milks and butter. However, fats from 8 regular 'ice creams' and 11 'ice milks' contained $11{\sim}28%\;and\;11{\sim}34%$ lauric acid, respectively. Since these levels of lauric acid were 3 times more than in milk or butter, other fats along with milk fat might be used for manufacturing these' ice creams' and 'ice milks'. Out of these 19 products, only 5 products were labelled as 'coconut oil' or 'refined oil' as well as milk fat being used. Fats extracted from 'sherbets', 'non-milk-fat ice creams' and 'non-milk product ice creams' contained $81{\sim}92%,\;76{\sim}99%\;and\;84{\sim}99%$ saturated fatty acids, respectively. Lauric acid was the most abundant fatty acid in the fats of these products, being $33{\sim}34%,\;17{\sim}45%\;and\;27{\sim}46%$ of the total fatty acids, respectively.

Probabilistic fatigue assessment of rib-to-deck joints using thickened edge U-ribs

  • Fatigue cracks of rib-to-deck (RD) joints have been frequently observed in the orthotropic steel decks (OSD) using conventional U-ribs (CU). Thickened edge U-rib (TEU) is proposed to enhance the fatigue strength of RD joints, and its effectiveness has been proved through fatigue tests. In-depth full-scale tests are further carried out to investigate both the fatigue strength and fractography of RD joints. Based on the test result, the mean fatigue strength of TEU specimens is 21% and 17% higher than that of CU specimens in terms of nominal and hot spot stress, respectively. Meanwhile, the development of fatigue cracks has been measured using the strain gauges installed along the welded joint. It is found that such the crack remains almost in semi-elliptical shape during the initiation and propagation. For the further application of TEUs, the design curve under the specific survival rate is required for the RD joints using TEUs. Since the fatigue strength of welded joints is highly scattered, the design curves derived by using the limited test data only are not reliable enough to be used as the reference. On this ground, an experiment-numerical hybrid approach is employed. Basing on the fatigue test, a probabilistic assessment model has been established to predict the fatigue strength of RD joints. In the model, the randomness in material properties, initial flaws and local geometries has been taken into consideration. The multiple-site initiation and coalescence of fatigue cracks are also considered to improve the accuracy. Validation of the model has been rigorously conducted using the test data. By extending the validated model, large-scale databases of fatigue life could be generated in a short period. Through the regression analysis on the generated database, design curves of the RD joint have been derived under the 95% survival rate. As the result, FAT 85 and FAT 110 curves with the power index m of 2.89 are recommended in the fatigue evaluation on the RD joint using TEUs in terms of nominal stress and hot spot stress respectively. Meanwhile, FAT 70 and FAT 90 curves with m of 2.92 are suggested in the evaluation on the RD joint using CUs in terms of nominal stress and hot spot stress, respectively.

A Study on Weight Loss Effect and Safety of Integrated Korean Medicine Treatment on Obese Patients in Local Clinics: A Restrospective Chart Review (한의원 진료환경에서 비만환자 대상 한의통합치료의 체중 감량 효과 및 안전성에 관한 연구: 후향적 차트 리뷰)

  • Objectives: A restrospective chart review were conducted to investigate the overall weight loss effect of the integrated Korean medicine treatment on obese patients (body mass index [BMI] ≥25 kg/m2) in Korean medicine clinic and analyze the difference in the effect according to lifestyle behaviors. Methods: The medical records of 43 obese patients were retrospectively analyzed including body weight, BMI, waist circumference, hip circumference, and body composition who received integrated Korean medicine treatment for 4 weeks at 24 Korean medicine clinics in Daejeon metropolitan city. All outcome measures were evaluated again 8 weeks after the end of treatment at week 12 for follow-up. EuroQol-5D (EQ-5D), the Korean version of the obesity-related quality of life scale (KOQOL) and patient's satisfaction were also evaluated. The analysis was divided according to the treatment period and observation period, and subgroup analysis was performed according to drinking and exercise habits. Results: Body weight, body fat, waist circumference, hip circumference, body fat were significantly reduced at week 4 and week 12. Theses weight reduction effects were significantly greater within treatment period (0 to 4 week) than observation period (4 to 12 week). Especially in the non-exercise group, the changes in body fat mass and body fat percent showed a significant difference between the treatment period and the observation period. KOQOL were also significantly improved at 12 week, but not in EQ-5D. No severe adverse events were observed. Conclusions: The integrated Korean medicine treatment could be effective to treat obesity including weight loss. It is necessary to prevent additional weight regain through regular exercise even after Korean medicine treatment.

The Result and Failure after Adjuvant Postoperative Irradiation in Carcinoma of Recum (직장암의 방사선 치료결과 및 실패양상)

  • From Jan.1982 to Dec.1990, 77 patients with rectal cancer were treated with curative surgical resection followed by postoperative adjuvant irradiation alone or combined with chemotherapy at the Department of Radiation Oncology, Korea University Hospital (KUH). Fifty-four (54/77, $70.1{\%}$) patients underwent abdominoperineal resection , 20 (20/77, $26{\%}$) patients underwent low anterior resection, and 3 (3/77, $3.9{\%}$,) patients had wide excision only. Thirty-nine (39/77, $50.5{\%}$) received sequential chemotherapy (2 cycles to 12 cycles). The 5-year survival rate for the entire group was $43{\%}:\;78.2{\%}$ in B2+B3, $39.4{\%}$ in stage C1+C2+C3. Survival rates decreased with increasing penetration of the bowel wall by tumor and the presence of regional lymph node metastasis. Those patients survival who underwent an abdominoperineal resection also experienced a significant decrease in compared to low anterior resection ($23.1{\%}$ vs. $63.8{\%}$ in 5-year survival, p <0.05). Local failure occurred in 15 ($19.5{\%}$) out of the 77 patients overall, 1($5.3{\%}$) of 19 in stage B2+B3, and 14 ($24.1{\%}$,) of 58 in stage C1+C2+C3. Presacral area was most common site of local failufre (8/17, $47.1{\%}$). Distant failure occurred in 13 ($16.9{\%}$) of 77 patients. The most frequent site of distant failure was the lung followed by the liver, the bone, and the brain. Combined locoregional and distant failure occurred in 2 ($2.6{\%}$) of 77 patients. Pathological confirmation of perirectal fat and/or regional lymph node involvement resulted in a singificant decrease in survival and local control.

Effects of marbling on physical and sensory characteristics of ribeye steaks from four different cattle breeds

  • Objective: Marbling or intramuscular fat (IMF) has been widely reported to directly impact the sensory acceptance of meat. This study was carried out to determine the physical and sensory characteristics of ribeye, Longissimus dorsi steaks obtained from four different cattle breeds namely Wagyu, Angus, Brahman, and Malaysian local beef, the Kedah-Kelantan (KK). Methods: The degree of marbling was determined by using an established combined camera-image analysis technique while instrumental texture determination was carried out by using Warner-Bratzler shear force analysis. Sensory evaluation of the beef steaks was performed following a quantitative descriptive analysis incorporating 10 trained consumer panelists. Results: Wagyu was found to possess the highest (p<0.05) percentage of IMF at 33.90% and the lowest shear force (raw = 5.61 N/㎟; cooked = 14.72 N/㎟) followed by Angus (20.87%), Brahman (12.17%), and KK (p<0.05, 6.86%). The difference in sensory properties of the four steaks was evident, with Wagyu appearing to be highly correlated with most sensory attributes measured namely sustained buttery, tooth-packing, chewiness, juiciness, tenderness, mouthfeel, oiliness, and overall acceptability. The Malaysian local beef, KK was found to be less acceptable (p<0.05), although most of its sensory attributes were found similar (p>0.05) in appearance, aroma, texture, juiciness, and flavour to the cooked steak from Angus and Brahman. Conclusion: This present study demonstrated the role of IMF in determining the quality and sensory acceptance of beef from different cattle breeds. These data have provided new information and further understanding on the physical and sensory quality of Malaysian local beef.

The Usage of Regional Folk Plants in Jeollanam-do (전라남도 지역의 민속식물 이용현황)

  • This study was conducted during two separate periods, from April to December 2008 and from April to November 2013, to identify the folk plants growing in Jeollanam-do which are used daily. A total of 1,200 questionnaires were distributed to 120 residents living in 14 counties and cities. From the results of the survey, 172 taxa of folk plants (77 families, 153 genera, 154 species, 2 subspecies, 14 varieties, and 2 forma) were identified. By regions, the city of Suncheon yielded 82 taxa, followed by 67 taxa from Gurye county, 65 taxa from Hampyeong county, 64 taxa from Jangseong county, 62 taxa from city of Naju, and 61 taxa from Jangheung county. Uses of the identified folk plants were: as edibles (117 taxa), for medicinal purposes (66 taxa), tea (15 taxa), for starch (6 taxa) for vegetable fat and oil (3 taxa), for nectar (2 taxa), for sap (2 taxa), for spice (2 taxa), for aroma (1 taxa), for fiber (1 taxa), and others (14 taxa). Thus, edible and medicinal uses of the plants appeared higher than other uses. The consistency of the local names of plants with the standard botanical nomenclature was 23.2% (40 taxa), while the proportion of local plant names analogous to the standard names was 18.6% (32 taxa). The consistency of the local plant names with the same plants in different cities and counties was 28.5% (49 taxa) and the proportion of local plant names analogous to each other in this category was 25.6% (44 taxa). Therefore, 54% of the folk plants growing in Jeollanam-do are known by location specific names. The usage types of the local folk plants were mostly collected through verbal communications.
