Factors affecting the rate of oral examination in the elderly in local communities (지역사회 노인의 구강검진 수검률에 미치는 영향요인)

  • Objectives: This study was conducted to investigate the factors affect the oral health examination of local community elder for 13,344 using the data of the community health survey. Methods: The dependent variable included answers yes or no about receiving oral examination while the independent variable included general characteristics, health behavior and oral health behavior. The IBM SPSS Statistics 20.0 program was used for frequency analysis, Chi-square test and logistic regression analysis. Results: Factors influencing affect receiving the oral examination was significantly higher in the answer yes group with the score of 1.18 (95% Cl 1.04-1.34) compared to the group that answered no to the question of marriage, significantly higher in the group that answered yes with the score of 1.36 (95% Cl 1.18-1.56) compared to the group that answered no to exposure to anti-smoking campaigns, significantly higher in the group that answered yes with the score of 3.53 (95% Cl 3.05-4.10) compared to the group that answered yes about health examination experience, significantly higher in the group that answered yes with the score of 1.50 (95% Cl 1.19-1.87) compared to the group that answered no about tooth brushing after breakfast and significantly higher in the group that answered yes with the score of 4.00 (95% Cl 3.55-4.37) compared to the group that answered no to whether they have had scaling experience. Conclusions: The study results show that the elderly had a low rate of oral examination. It is necessary to increase preventive treatments and continue studies on oral health examination in the elderly.

Machine Learning Based Model Development and Optimization for Predicting Radiation (방사선량률 예측을 위한 기계학습 기반 모델 개발 및 최적화 연구)

  • In recent years, radiation has become a socially important issue, increasing the need for accurate prediction of radiation levels. In this study, machine learning-based models such as Multiple Linear Regression (MLR), Random Forest (RF), XGBoost, and LightGBM, which predict the dose rate by time(nSv h-1) by selecting only important variables, were used, and the correlation between temperature, humidity, cumulative precipitation, wind direction, wind speed, local air pressure, sea pressure, solar radiation, and radiation dose rate (nSv h-1) was analyzed by collecting weather data and radiation dose rate for about 6 months in Jangseong, Jeollanam-do. As a result of the evaluation based on the RMSE (Root Mean Squared Error) and R-Squared (R-Squared coefficient of determination) scores, the RMSE of the XGBoost model was 22.92 and the R-Squared was 0.73, showing the best performance among the models used. As a result of optimizing hyperparameters of all models using the GridSearch method and comparing them by adding variables inside the measuring instrument, it was confirmed that the performance improved to 2.39 for RMSE and 0.99 for R-Squared in both XGBoost and LightGBM.

Some Factors Affecting Profitability of Local Public Hospitals (지방의료원의 재무성과 영향요인)

  • This paper aims at suggesting several ways lo change financial vulnerability and to improve managerial capability of local public hospitals (LPHs) in Korea through the identification of factors affecting profitability. Several findings of the research are as follows: To begin with, LPHs exhibited a statistically significant difference in their profitability from one another, according to tile analyses of their profitable margins from tile general characteristics. It depends on the number of hospitals in the area, the population of the hospital-built area, the number of competing hospitals, the number of staff per 100 beds, the opening of special clinic, the educational function, and the capacity of rooms. However, there was no variable in the managerial characteristics, presenting a significant difference, in contrast with hospitals which have been managed by private companies and made a great amount of profits. Second, according to the analyses of profit differences in behavioral effort-characteristics, a statistically significant difference was revealed upon the basis of the efforts to improve the clinic service, invite special patients, and shorten the period of being hospitalized. Third, the result of analyses about the difference of profitability from medical care and finance is statistically significant in the rate of labor cost, the rate of management cost, bed-occupancy rate, and the period of being hospitalized. Fourth, according to the analyses of the factors influencing the net profit ratio of the entire capital, Adjusted explanatory power(Adjusted $R^2$) was shown up to 65.2%, which is high. To compare the adjusted explanatory power stage by stage, the first stage model applying only two variables such as structural and strategic characteristics exhibited 23.8%, and the second stage model adding financial characteristics showed 51.5%. The explanatory power was much improved up to 65.2% when the third stage model incorporated the outcome of medical care performance. When the return on investment(ROI) was examined by using the multi-variate linear regression analysis at the final model of third stage, it was found that ROI had a positive relationship with the increase rate of patients, labor costs per doctor, and medical care rate of socially protected inpatients. However, it revealed that ROI had a negative relationship with the ratio of labor costs, the number of patients per managerial staff, and occupancy rate of rooms, respectively. The research suggests that in order for LPHs to increase profitability, LPH, should make efforts not only to attract patients to the hospitals without any discrimination of the patients depending on their financial status, but also to develop efficient management methods to reduce labor costs.

Characteristics of non-emergent patients at emergency departments (응급실을 이용하는 비응급환자의 실태와 특성)

  • The objective of this paper is to examine the proportion and characteristics of non-emergent patients at emergency departments. The observational survey was conducted using a structured form used by emergency medicine specialists or senior residents on June 7-20, 2005. 1,526 patients at ten emergency centers took part in this study. The structural form contained type of insurance, route and means of emergency department (ED) visit, triage based on the Manchester Triage Scale(MTS)-modified criteria, emergency level based on the government defined rule, type of emergency centers (Regional Emergency Medical Center; REMC, Local Emergency Medical Center; LEMC, Local Emergency Agency; LEA), as well as patient's general information. Data were analyzed using SAS statistical program(V.8.2). Descriptive analysis was performed to describe the magnitude of non-emergent patients. ${\chi}^2-analysis$ and logistic regression analysis was performed to identify the nonurgent patients' characteristics. In the MTS-modified criteria, we found a 15.3% rate of non-emergent patients. This rate differed from that of non-emergent patients obtained using government's rule. In particular, there were inaccuracies in the definition of government rule on non-emergent patients, so it is necessary to apply the new government rule regarding classification of non-emergent patients. There were significant differences in the rate of non-emergent patients according to type of ED, means of ED visit, time to visit, and insurance. Non-emergent patients are more likely to visit a D-type ED(LEA having less than 20,000 patients annually), not to use ambulance, to have 'Automobile Insurance, Industrial Accident Compensation Insurance, or pay out-of-pocket'. Non-emergent patients tend to visit ED due to illness rather than injury. Further studies on the development' of triage scale and reexamination of the government's rule on emergency visits are required for future policy in this area.

A Study on the Prediction of the Construction Cost in Planning Stage of Local Housing Union Project (지역주택조합사업 기획단계의 공사비 예측에 관한 연구)

  • The accurate prediction of construction cost is a key factor in a project's success. However, it is hard to predict the construction costs in the planning stages rapidly and precisely when drawings, specifications, construction cost calculation statements are incomplete, among other factors. Accurate construction-cost prediction in the planning stage of a project is also important for project feasibility studies and successful completion. Therefore, various techniques have been applied to accurately predict construction costs at an early stage when project information is limited. There are many factors that affect the construction cost prediction. This paper presents a construction-cost prediction method as multiple regression model with seven construction factors as independent variables. The method was used to predict the construction cost of a local housing union project, and the error rate was 4.87%. It is not possible to compare the cost of the project at the planning stage of the local housing union project, but it has high prediction accuracy compared to the unit price of an existing unit area. It is likely to be applied in construction-cost calculation work and to contribute to the establishment of the budget for the local housing union project.

Analysis of Factors Influencing the Utilization Rate of Public Health Centers in Korea (한국의 보건소 이용률에 영향을 미치는 요인 분석)

  • This study was conducted to identify the utilization of public health centers, as well as the individual characteristics and regional characteristics that affect their utilization based on data from the 2016 Community Health Survey, National Statistical Portal, and National Institute of Environmental Research. Independent samples t-tests, variance analysis, and multiple logistic regression analysis were used for analysis. Hierarchical multiple regression was used to analyze individual and regional characteristics. The results of hierarchical multiple regressions revealed that aged regions, women, older age individuals, respondents with lower education level and income level, walking practitioners, nutrition label readers, individuals experiencing depression, those who have received health checkups, those who are not covered by essential care, those who have spouses, and basic livelihood beneficiaries have increased use of public health centers. However, the use of public health centers decreased in stressors, and regions in which the population per 1,000, number of health care workers, health and welfare budget, fiscal independence, and unemployment rate were above the national average. As above, the central government and local governments need to analyze not only individual characteristics such as health behavior and psychological factors, but also regional characteristics, when establishing local health care policy.

Actual Analysis of the Interrelationship between Evaluation Indicators of Communicable Disease Control and Prevention Activities and Communicable Disease Incidence Data (법정감염병 발생자료와 감염병관리사업 평가지표와의 관계 실증분석)

  • This study examined the interrelationship between the evaluation indicators of communicable disease control and prevention activities, and the communicable disease incidence data. This study analyzed the incidence data of communicable disease in local governments of south Korea and evaluated the data of communicable disease control and prevention activities by the Ministry of Health of the central government in South Korea during 2004-2005. Frequency analysis was carried out to understand the character of the participant, t-test to compare the mean value between the two groups and stepwise multiple regression analysis to understand the significance between the dependent and independent variables. In this study, the finance related to communicable diseases (group I diseases in both city and rural center), keep rate of periodic reports on notifiable communicable diseases based on the law for communicable disease control and prevention (group II in city), the level of education on personal hygiene (group II in rural center), level of education on AIDS prevention and the reporting rate of cases of tuberculosis (group III in city), and reporting rate of incident cases of tuberculosis (tuberculosis and Hansen disease in both rural and city) were significant indicators. The level of education on AIDS prevention and the reporting rate of the cases of tuberculosis (in city), and number of adverse reactions after immunization (in rural area), reporting rate of cases of tuberculosis (in total center) were significant indicators in total communicable disease and all types of public health centers. The authors verified core evaluation indicators as actual proof. This study provides useful data for a summative evaluation, standardization, and guidelines on communicable disease control and prevention activities of public health centers and local government.

The Pattern of Failure after Definitive Radiotherapy for Squamous Cell Carcinoma of the Lung (편평상피 세포성 폐암의 방사선치료 실패 양상)

  • From Feb.1985 to Feb.1988,76 patients with squamous cell carcinoma of the lung treated at the Department of Therapeutic Radiology in Kyungpook National University Hospital were available for the analysis of this study. All patients received radiation of 4000cGy-6600cGy with curative aim. The overall rate of complete response was 25.0% and partial response was 452.6% The complete and partial regression of tumor was 14.3% in patients treated with dose below 5000cGy and 84.1% in the group treated with dose above 5000cGy (p<0.01). The complete response was seen only in the group of patients received radiation at least 6000cGy. The patterns of failure were as follows. The rate of initial intrathoracic recurrence was 52.6% in patients with complete response. The overall rate of failure was 68.8%. Distant metastasis was found in 47.4% of patients. Bone, contralateral lung, and brain were common metastatic sites in decreasing order All of the distant metastases and 80% of local recurrences were found within the first year after treatment.

Relationships among Recognition, Preference, and Purchasing Characteristics for Local Agricultural Products and Festival Satisfaction of Changnyeong Area - Compared by the Gender, Age, and Purchase Experience - (창녕지역 축제만족도와 농특산물 인지도, 선호도, 구매특성과의 관련성 - 성별과 연령, 구매경험을 중심으로 -)

  • The purpose of this study was to investigate the relationships among recognition, preference, and purchasing characteristics for Changnyeong onions and garlic as well as festival satisfaction among participants at agricultural product festivals in the Changnyeong region. Results showed that festival satisfaction of 'place of festival' was highest while 'convenience facility & event contents' earned the lowest scores. Most subjects (90.5%) had purchased Changnyeong agricultural and processed products. A major purchasing type was fresh agricultural products (66.7%). The pathways to recognize Changnyeong agricultural products were mostly 'promotion by related institutions' (22.0%), 'family relatives' (20.8%), 'mass media' (16.6%), and 'festivals and events' (16.1%). The most considered factors for purchasing regional products were 'geographical origin' and 'ingredients'. Changnyeong onion showed higher scores for recognition and preference and rate of purchase experience and intention than for garlic. The correlation coefficients of recognition and preference for onion and galic were 0.603 (p<0.001) and 0.598 (p<0.001), respectively. The explanation power ($R^2$) of related variables for purchase of Changnyeong onions was 0.258. The regression coefficients (${\beta}$) for 'recognition', 'preference' and 'convenient facility & event contents' were positive, whereas the regression coefficient for 'price' was negative. Recognition, preference, and convenient facility & event contents with garlic purchase showed a positive relationship ($R^2$=0.253). The most effective promotion method to increase sales of Changnyeong agricultural products was 'local festivals and events' (27.8%).

An Empirical Study to Identify Success Factors of B2B e-marketplaces: Focused on the Supply Companies (공급기업의 관점에서 본 B2B e-marketplace의 성공요인)

  • This study is intended to identify success factors which influence the performance of the B2B e-marketplaces.Thestudyisfocused on the supply companies and conducted to provide local small businesses with guidelines to improve their competitiveness and to develop appropriate business strategies in the digital economy age. Based on the literature review, four characteristics which influence the performance of B2B e-marketplaces are identified. Such characteristics as strategic, operational, organizational, and product/service properties are then utilized to develop the survey questionaire. The data collection is first performed through e-mail and, after finding out the response rate was too poor to analyze the results, followed by visits to the companies which participate in e-marketplaces. SAS 8.1 is used for the data analysis. Demographic analysis is conducted to generate the general information of the companies responded, and factor analysis is performed for the validity test. Finally, multiple regression is used for the hypothesis testing. This study is expected to contribute to find out critical factors for the success of B2B e-marketplaces and suggest guidelines for local small businesses to develop an appropriate e-business strategy to improve their competitive position.
