Increased Frequency of Foxp3+ Regulatory T Cells in Mice with Hepatocellular Carcinoma

  • The CD4+CD25+ regulatory T cell (Treg) is a special kind of T cell subset. Studies have showed that Treg cells are involved in a number of physiological processes and pathologic conditions such as autoimmune diseases, transplantation tolerance and cancer. Tregs with unique capacity for immune inhibition can impair anti-tumour immunity and help tumor cells to escape from immune surveillance. The aim of our study was to investigate whether Tregs are involved in hepatocellular carcinoma (HCC). A BABL/C mouse with HCC in situ model was established to evaluate the Treg existence in carcinoma tissues and the changes of Tregs in spleen using flow cytometry and immunohistochemistry methods. Granzyme B expression in carcinoma tissues was analyzed by immunohistochemistry to investigate the tumor local immune status.The proportion of CD4+CD25+/CD4+ spleen lymphocytes of tumor bearing mice ($18.8%{\pm}1.26%$) was found to be significantly higher than that in normal mice ($9.99%{\pm}1.90%$) (P<0.01 ). Immunohistochemistry of spleen tissue also confirmed that there was an increase in Treg in tumor-bearing mice, while in carcinomas it showed Treg cells to be present in tumor infiltrating lymphocyte areas while Granzyme B was rarely observed. Anti-tumour immunity was suppressed, and this might be associated with the increase of Tregs. Our observations suggest that the CD4+CD25+Treg/CD4+ proportion in spleen lymphocytes can be a sensitive index to evaluate the change of Tregs in hepatocellular carcinoma mice and the Treg may be a promising therapeutic target for cancer.

Effect of Bone Cement Volume and Stiffness on Occurrences of Adjacent Vertebral Fractures after Vertebroplasty

  • Objective : The purpose of this study is to find the optimal stiffness and volume of bone cement and their biomechanical effects on the adjacent vertebrae to determine a better strategy for conducting vertebroplasty. Methods : A three-dimensional finite-element model of a functional spinal unit was developed using computed tomography scans of a normal motion segment, comprising the T11, T12 and L1 vertebrae. Volumes of bone cement, with appropriate mechanical properties, were inserted into the trabecular core of the T12 vertebra. Parametric studies were done by varying the volume and stiffness of the bone cement. Results : When the bone cement filling volume reached 30% of the volume of a vertebral body, the level of stiffness was restored to that of normal bone, and when higher bone cement exceeded 30% of the volume, the result was stiffness in excess of that of normal bone. When the bone cement volume was varied, local stress in the bony structures (cortical shell, trabecular bone and endplate) of each vertebra monotonically increased. Low-modulus bone cement has the effect of reducing strain in the augmented body, but only in cases of relatively high volumes of bone cement (>50%). Furthermore, varying the stiffness of bone cement has a negligible effect on the stress distribution of vertebral bodies. Conclusion : The volume of cement was considered to be the most important determinant in endplate fracture. Changing the stiffness of bone cement has a negligible effect on the stress distribution of vertebral bodies.

  • Langerhans cell histiocytosis (LCH) is a pleomorphic disease entity characterized by local or disseminated atypical Langerhans cells (LCs) found most commonly in bone, lungs, mucocutaneous structures and endocrine organs. Among different sites, unifocal disease confined to a lymph node (LN) is rare. We report a case of LCH confined to a LN in a 38-year-old male who presented with a 2 cm-sized cervical mass. The fine needle aspiration (FNA) smears of cervical LN showed high cellularity having isolated LCs with contorted nuclei and nuclear grooves mixed in multinucleated giant cells, small lymphocytes and eosinophils. Charcot-Leyden crystals were also seen, as were a few dendritic-like cells and intranuclear inclusions. Confirmation of LCH was made by histopathologic studies, positive reactions for S-100 protein and CD1a immunohistochemical staining and by the demonstration of Birbeck granules on electron microscopy. The differentials to be considered include dermatopathic lymphadenitis, sinus histiocytosis with massive lymphadenopathy, Hodgkin's lymphoma and malignant histiocytosis. The characteristic cytomorphologic pattern of LCH in a LN FNA smear plays an important role in suggesting the diagnosis of LCH.

  • 휴대폰 망과 같은 이동망이나 와이파이망, 주파수공용통신(Trunked Radio System)과 같은 망은 시설기반(Infra Structure)망이어서 핸드오버같은 것은 문제가 되지 않는다. 그러나 무전기 같이 수명에서 수십 명 내외의 그룹간 이동을 하면서 서로 통신을 해야 하는 경우는 망을 구성하고 있는 노드들이 실시간으로 이동하고, 토폴로지가 늘 변하고 있으며, 특히 무선으로 통신할 경우는 전파환경이 좋지 않아서 망을 구성하기란 쉽지 않다. 특히 특수목적망은 인프라 시설이 없어서 구성원들 간에 핸드오버(handover)가 필요한 경우가 많으며, 핸드오버하는 대상노드를 결정하는 것은 반드시 필요하며 중요하다. 어느 노드가 핸드오버 역할을 할지에 따라 다양한 토폴리지의 통달거리, 수신감도, 음질 등에 영향을 주게 된다. 본 고에서는 토폴로지 경우의 수를 다양하게 분석하여 전파간섭 및 핸드오버 프로토콜에 따라 데이터 전송을 원활하게 할 모델을 제시하고, 시뮬레이션을 통해 분석한 결과를 제시한다.

Development of Enzymatic Recombinase Amplification Assays for the Rapid Visual Detection of HPV16/18

  • Human papillomavirus (HPV) types 16 and 18 are the major causes of cervical lesions and are associated with 71% of cervical cancer cases globally. However, public health infrastructures to support cervical cancer screening may be unavailable to women in low-resource areas. Therefore, sensitive, convenient, and cost-efficient diagnostic methods are required for the detection of HPV16/18. Here, we designed two novel methods, real-time ERA and ERA-LFD, based on enzymatic recombinase amplification (ERA) for quick point-of-care identification of the HPV E6/E7 genes. The entire detection process could be completed within 25 min at a constant low temperature (35-43℃), and the results of the combined methods could be present as the amplification curves or the bands presented on dipsticks and directly interpreted with the naked eye. The ERA assays evaluated using standard plasmids carrying the E6/E7 genes and clinical samples exhibited excellent specificity, as no cross-reaction with other common HPV types was observed. The detection limits of our ERA assays were 100 and 101 copies/µl for HPV16 and 18 respectively, which were comparable to those of the real-time PCR assay. Assessment of the clinical performance of the ERA assays using 114 cervical tissue samples demonstrated that they are highly consistent with real-time PCR, the gold standard for HPV detection. This study demonstrated that ERA-based assays possess excellent sensitivity, specificity, and repeatability for HPV16 and HPV18 detection with great potential to become robust diagnostic tools in local hospitals and field studies.

11년간 경험한 피부의 악성종양에 대한 임상적 고찰 (A Retrospective Clinical Study of Skin Cancer: A Single Institution's Experience on 370 Cases)

  • Purpose: The incidence of skin cancer is increasing throughout the world including Asian countries such as Korea. Still there are only limited reports of the clinical features of skin cancer in Korea, especially in the fields of plastic surgery. This study is to demonstrate the recent clinical trends of skin cancer and the treatment of skin cancer in Korea by analyzing a single institution's experience. Methods: During a 11-year period, 370 patients visited our department for the excision of skin cancer. Data of the patients such as histopathologic diagnosis, primary site of the tumor, age and sex of the patient, operative methods were reviewed retrospectively. Results: We had 370 patients, 215 men and 155 women. The ages ranged from 10 to 95 years with a mean of 58.8 years. A total of 143 patients were diagnosed as basal cell carcinoma, while 100 were malignant melanoma, 80 were squamous cell carcinoma, 17 were dermatofibrosarcoma protuberans, and 30 were other miscellaneous skin cancers. Local flaps were the most frequently used reconstructive method after wide excision of the cancer, which was followed by primary closure, split-thickness skin graft, free flap and full-thickness skin graft. Conclusion: There was a gradual increase in the incidence of skin cancer after the year 2000. Basal cell carcinoma had the highest incidence which frequently involved the sun-exposed regions in the old ages. Malignant melanoma showed a relatively high incidence compared with other previous studies in Korea. The extremities were the most frequent location for malignant melanoma. Clinical features of other various skin cancers were also reported in this study. The reconstructive method highly depends on the primary site of the cancer, while local flaps were the mostly used operative technique.

장애인서비스 전달체계 개편방안에 대한 비판적 고찰 (A Critical Perspective on the Central Governmental Reorganization of the Social Service Delivery System for People with Disabilities)

  • 장애인서비스 전달체계는 욕구평가체계의 결여, 사례관리체계의 미비, 전문인력의 부족 및 지방복지행정기관의 미비라는 문제를 안고 있다. 보건복지가족부는 장애인서비스 전달체계를 개편하기 위해 2007년부터 장애인복지인프라개선방안을 추진하고 있다. 개선방안에서는 장애판정체계의 개편과 장애서비스판정센터의 설립을 제안하고 있으며, 장애판정체계는 기존의 의학적 판정도구 외에 근로활동능력평가도구 및 복지욕구사정도구를 개발하여 장애인의 욕구평가체계를 도입하려고 시도하고 있다. 또한 장애서비스판정센터의 설립 모형으로 독립형, 지자체형, 공단형의 세 가지 모델을 제시하고 2008년 하반기에 시범사업을 실시하고 있다. 본 연구에서는 장애인서비스 전달체계를 개선하기 위한 핵심적인 요소가 보건복지가족부가 통제할 수 있고, 욕구평가와 사례관리를 수행할 수 있는 전문 인력을 갖춘 지방장애인복지행정기관을 120여개 설립하는 것이며, 현실적으로 한국장애인개발원을 정점으로 전국의 장애인복지관을 지방장애인복지행정기관으로 활용할 것을 제안하고 있다.

  • This study attempts to provide implications for developing more efficient and effective community-based support system with AIP perspective for the elderly in Korea. The main purpose of this study is to analyze Japan's community-based integrated care system that respond to the concept of aging in place (AIP) and its cases. In Japan, they have offered Community-Based Service since 2005, and the advanced system which include integrated support categories and consolidated community/local resources will started in 2015 by The revision of Long-term Care Insurance policy, 2012. The result of policy analysis and case studies are as follows: 1) The suggestion for ideal support system model promoted a relationship of multiple agents include private sectors even resident and senior and specified responsibility sharing, 2) the system proposed Not only health and medical care support, living care and residence are also addressed as a comprehensive support. and 3) the amount of available community resource is different by each local government, but the effort to get the understanding of community residents and to connect with a potential community resource is also essential aspect to set effective community-based support system.

Effects of Lumbar Position Sense on Swing Chair in Healthy Adults

  • Purpose: To recognize the change in the proprioception under conditions that activate the local muscles, this study found the difference in the proprioception on a fixed and swing chair. Methods: Healthy adults (7 males, 13 females) in their twenties were evaluated lumbar position sense on fixed and swing chair. And the difference of the position sense between the fixed and swing chair were analyzed. Results: The joint position senses were compared between the fixed and swing chair. As a result, there was a significant difference from all motions. Conclusion: This study found that the improvement of proprioception using swing chair means that the local muscle is activated. Therefore, to use a swing chair, which was designed in this study, is able to replace the general chair either at work or during studies. Thus, this method is easily accessible for busy modern people who are exposed to low back pain or spinal disorders due to a pattern of inactivity.

First Record of Paramphistomes Fischoederius cobboldi and Paramphistomum epiclitum Detected in Bovine Rumen from a Local Market of Savannakhet Province, Lao PDR

  • In the present study, we report on the occurrence of paramphistomes, Fischoederius cobboldi and Paramphistomum epiclitum, in Lao PDR with the basis of molecular data. Parasite materials were collected from bovines bred in Ban Lahanam area, Savannakhet Province, Lao PDR at Lahanam public market. Morphological observations indicated 2 different species of paramphistomes. The mitochondrial gene cox1 of the specimens was successfully amplified by PCR and DNA sequencing was carried out for diagnosis of 11 specimens. Pairwise alignment of cox1 sequences were performed and confirmed F. cobboldi and P. epiclitum infecting bovines in Laos. Although there were many limiting points, as the small number of worm samples, and the restricted access of the animal host materials, we confirmed for the first time that 2 species of paramphistomes, F. cobboldi and P. epiclitum, are distributed in Lao PDR. More studies are needed to confirm the paramphistome species present in Savannakhet and its hosts to clear the natural history of these parasites of ruminants in the region and measure the impact of this parasite infection in the life and health of the local people.
