The petroleum refinery industry handles a variety of complex chemical substances and employs a large number of people around the world. According to previous research, diseases caused by exposure to chemicals were quite common among workers in refineries until the 1980s. More recently, it is unusual for oil refinery workers to suffer from these serious diseases. The objective of this study was to identify the occurrence level of general diseases and any differences in lifestyle habits of workers in refineries in Korea compared with the general population. In this study, we used the results of health examinations from 2014 for workers at a large oil refinery in South Korea. In addition, based on the results of KNHANES from 2014 as representative of the population, hypertension, diabetes, anemia, hyperlipidemia, liver function abnormality, and kidney function abnormality were calculated using the standardized incidence rate. Hypertension showed a low result with an SIR of less than 1, but a high result in the case of abnormal liver function. The workers' occurrence rates of hypertension, hyperlipidemia, kidney dysfunction, and anemia were lower than those of ordinary salaried employees. however, their occurrence rate for abnormal liver function was markedly higher. In lifestyle habits, the smoking rate of workers was low while the alcohol consumption rate was quite high. This study has improved the understanding of the health status of workers in a large oil refinery in Korea, and has shown the impact of lifestyle habits related to the work environment on chronic diseases.
Objective: To explore the molecular mechanisms of fatty liver hemorrhagic syndrome (FLHS) in laying hens, an experiment was conducted to reveal the differences in histopathological observation and gene expression between FLHS group and normal group. Methods: We compared the histopathological difference using hematoxylin and eosin staining and proceeded with RNA sequencing of adipose tissue to search differentially expressed genes and enriched biological processes and pathways. Then we validated the mRNA expression levels by real-time polymerase chain reaction and quantified protein levels in the circulation by enzyme-linked immunosorbent assay. Results: We identified 100 differentially expressed transcripts corresponding to 66 genes (DEGs) were identified between FLHS-affected group and normal group. Seven DEGs were significantly enriched in the immune response process and lipid metabolic process, including phospholipase A2 group V, WAP kunitz and netrin domain containing 2, delta 4-desaturase sphingolipid 2, perilipin 3, interleukin-6 (IL-6), ciliary neurotrophic factor (CNTF), and suppressor of cytokine signaling 3 (SOCS3). And these genes could be the targets of immune response and be involved in metabolic homeostasis during the process of FLHS in laying hens. Based on functional categories of the DEGs, we further proposed a model to explain the etiology and pathogenesis of FLHS. IL-6 and SOCS3 mediate inflammatory responses and the satiety hormone of leptin, induce dysfunction of Jak-STAT signaling pathway, leading to insulin resistance and lipid metabolic disorders. Conversely, CNTF may reduce tissue destruction during inflammatory attacks and confer protection from inflammation-induced insulin resistance in FLHS chickens. Conclusion: These findings highlight the therapeutic implications of targeting the JAK-STAT pathway. Inhibition of IL6 and SOCS3 and facilitation of CNTF could serve as a favorable strategy to enhance insulin action and improve glucose homoeostasis, which are of importance for treating obesity-related disorders for chickens.
Elevated plasma homocysteine (Hcy) is a risk factor for cognitive dysfunction and Alzheimer disease, although the mechanism is still unknown. Both folate and betaine, a choline metabolite, play essential roles in the remethylation of Hcy to methionine. Choline deficiency may be associated with low folate status and high plasma Hcy. Alterations in DNA methylation also have established critical roles for methylation in development of the nervous system. This study was undertaken to assess the effect of choline and folate deficiency on Hcy metabolism and genomic DNA methylation status of the liver and brain. Groups of adult male Sprague Dawley rats were fed on a control, choline-deficient (CD), folate-deficient (FD) or choline/folate-deficient (CFD) diets for 8 weeks. FD resulted in a significantly lower hepatic folate (23%) (p<0.001) and brain folate (69%) (p<0.05) compared to the control group. However, plasma and brain folate remained unaltered by CD and hepatic folate reduced to 85% of the control by CD (p<0.05). Plasma Hcy was significantly increased by FD $(18.34{\pm}1.62{\mu}M)$ and CFD $(19.35{\pm}3.62{\mu}M)$ compared to the control $(6.29{\pm}0.60{\mu}M)$ (p<0.001), but remained unaltered by CD. FD depressed S-adenosylmethionine (SAM) by 59% (p<0.001) and elevated S-adenosylhomocysteine (SAM) by 47% in liver compared to the control group (p<0.001). In contrast, brain SAM levels remained unaltered in CD, FD and CFD rats. Genomic DNA methylation status was reduced by FD in liver (p<0.05) Genomic DNA hypomethylation was also observed in brain by CD, FD and CFD although it was not significantly different from the control group. Genomic DNA methylation status was correlated with folate stores in liver (r=-0.397, p<0.05) and brain (r = -0.390, p<0.05), respectively. In conclusion, our data demonsoated that genomic DNA methylation and SAM level were reduced by folate deficiency in liver, but not in brain, and correlated with folate concentration in the tissue. The fact that folate deficiency had differential effects on SAM, SAH and genomic DNA methylation in liver and brain suggests that the Hcy metabolism and DNA methylation are regulated in tissue-specific ways.
Twenty-three patients with traumatic diaphragmatic injuries treated at the Department of Thoracic and Cardiovascular Surgery, Keimyung University Dongsan Medical Center from Aug. 1978 to Nov. 1990 were reviewed. There were 19 male and 4 female patients. The age distribution was ranged from 1.5 to 72 years, with a mean age of 34.3 years. Sixteen patients had blunt trauma[traffic accident 14, fall down 2], and 7 had penetrating injuries[stab wound 6, broken glass 1]. Sixteen [70 percent]of the injuries occurred on the left side and 7[30 percent] on the right side. Fifteen patients were operated on during the acute phase, 5 patients during the latent phase, 2 patients during the obstructive phase. The surgical approach in 20 patients was through a thoracotomy; in 2 patients, a thoracoabdominal incision was necessary, and in 1 patient, a laparotomy was performed. Herniated organs in thorax included stomach[10], colon[5], small bowel[5], spleen[4], liver[2]. Postoperative complications included wound infection, empyema, pneumonia, hepatitis and respiratory failure. There were 3 postoperative deaths, 2 with cerebral dysfunction and 1 with sepsis.
Objective & Method : By investigating physiological function of tri-energizer, symptoms, and organ coordination, we obtained following conclusion. Result & Conclusion : Therefore, dysfunction of the tri-energizer is caused by abnormal evaporation and metabolism. Upper-energizer regulates cardiopulmonary function, middle-energizer regulates spleen and stomach functions, and lower-energizer regulates liver, kidneys, small and large intestines, and bladder functions. Such a functional specialization is possible by receiving the source of qi through the wall wrapping around the internal organs. Tri-energizer represents the exterior and interior relationship by acting as the membrane structure supporting the five visceral organs and six hollow organs and at the same time, as the pericardium surrounding the heart.
37세의 여자 환자가 halothane 마취로 개심술후 14일에 고열, 황달등이 발생하여 급성 간염으로 진단을 받아 치료하던 중 19일째 사망한 예로 급성간염의 원인이 수혈에 의한 virus성 간염과 halothane에 의한 간염, 기타 약제나 수술의 특수조작에 의한 간염 인가를 대조 검토하였으나 정확한 원인을 규명할 수는 없었다.

Benign symmetric lipomatosis(BSL) is a lesion characterized by symmetrical and diffuse growth of adipose tissue. It is commonly found in the posterior neck or upper trunk, manifesting itself as an entity known as Madelung disease or Lanoid-Bensaude syndrome. Lipomatosis affecting the tongue is very unusual. The etiology of BSL is unknown, but It can be associated with alcoholism, liver dysfunction, hypothyroidism. Because of its diffuse histologic feature, complete resection of the lesion is often difficult. Seventy one years old male patient, whose chief complaint was multiple nodules of tongue was referred to our department. The lesion was diagnosed histopathologically as BSL and subsequently removed surgically. So we report this rare case of BSL with literature review.
An 8-year old castrated domestic long-hair cat was presented with a two week history of abdominal distension. Physical examination revealed a non-painful, fluctuant, palpable mass in the right craniodorsal abdomen, and unilaterally muffled heart sounds on the right thorax. Routine clinico-pathological values were unremarkable apart from mild azotemia with a concurrent urine specific gravity of 1.031, which reflect a degree of renal dysfunction. Radiographic and ultrasound examinations of the thorax revealed the cardiac enlargement to be due to the congenital peritoneo-pericardial diaphragmatic hernia with liver occupying the right half of the pericardial sac. There was also a mild gypertrophy of the heart. Radiography and ultrasonography of the abdomen showed the mass to be composed of a large fluid filled cystic structures surrounding the right and left kidneys, and the kidneys themselves were of increased echogenecity. A diagnosis of perinephric pseudocysts was made. The patient responded well to the surgical procedures. Perinephric pseudocysts and peritoneo-pericardial diaphragmatic hernia in the cat are rare, and a case is described and the literature is reviewed in this report.
Virus associated hemophagocytic syndrome(VAHS), a class II histiocytosis syndrome, is characterized by high fever, liver dysfunction, coagulation abnormalities, and generalized histiocytic proliferation with marked hemophagocytosis in bone marrow and lymph nodes. VAHS is associated with several viral infections including Epstein Barr virus which has a relatively high mortality rate. We report a fatal case of Epstein Barr virus associated hemophagocytic syndrome and its diagnosis by mRNA in situ hybridization and polymerase chain reaction. A brief review of related literaure is also presented.
Propionic acidemia is an inherited organic acid metabolic disorder. During chronic recurrent metabolic crisis, multiple blood transfusions can cause secondary hemochromatosis. We report a patient with propionic acidemia who had iron overload that resulted in liver dysfunction, cardiomyopathy and diabetes. When multiple blood transfusions are unavoidable, use of chelating agents for iron can prevent complications such as diabetes and hemochromatosis.
