  • We'd experienced a case of primary hepatoma at medical department, Dae han Kyoyuk Ins. Company January 1985. As it was found very early stage, we made out the case long term-survival possibility unexpectedly. The conclusions we gained, making comparative-analysis of ultrasonic view and operation's view are as follows; 1. By ultrasonography, echogenicity of tumor was increased type as compared with liver parenchyme. 2. The margin of the primary hepatoma by ultrasonography encapsulated well and the extraction after operation was same. 3. The boundary of tumor was clear. 4. The space-occupying lesion in liver was solitary. 5. The affected site was left lobe of the liver. 6. We couldn't discover a little fluid echo in upper abdomen on the ultrasonography, but could see in the cul-de-sac area and it accorded with the gross finding during laparotomy. 7. The case was cholangiocarcinoma in view of pathologic histology. 8. After the operation of lobectomy. the prognosis which was passed a months was very satisfactory and she exists until present.

Late Occurrence of Multiple Bone Metastasis in Patient with Well Controlled Advanced Pancreatic Cancer

  • A 67-year-old male was admitted due to abdominal pain. Abdominal CT scan performed in a local clinic showed about 2 cm sized pancreatic tail mass with extensive liver and multiple regional lymph node metastasis. Histology of liver biopsy revealed poorly differentiated adenocarcinoma. He underwent chemotherapy with gemcitabine and erlotinib for 5 cycles followed by 8 cycles of second line chemotherapy with 5-fluorouracil and cisplatin. At 12 months after diagnosis, follow-up abdominal CT scan revealed marked reduction of tumor mass in the liver and pancreas with small residual tumor. After one month of last chemotherapy, he complained radiating pain along left leg. Blood chemistry revealed isolated elevation of alkaline phosphatase (ALP) and multiple bone metastasis were demonstrated in bone scan. Palliative radiation therapy to pelvic bone was performed for the relief of bone pain. The prognosis of advanced pancreatic cancer is extremely poor. We report late occurrence of multiple bone metastasis in a patient with well controlled advanced pancreatic cancer with chemotherapy.

  • Cholangiocarcinoma is a heterogeneous group of aggressive tumors that correspond to the second most common primary liver tumor. They can be classified according to their anatomical position concerning the biliary tree, and each subtype demonstrates different behavior and treatment. A 38-year-old male patient presenting solely right lumbar pain was diagnosed with a 7 cm hepatic tumor involving segments I, Iva, and VIII associated with involvement of the hepatic veins. He underwent a bloc resection of hepatic segments I, II, III, IV, partial V, partial VII, and VIII; right, middle, and left hepatic veins; and inferior vena cava segment, with perfusion of the remaining liver in situ with a preservation solution. As the patient had a large accessory inferior right hepatic vein draining the remaining liver, no reimplantation of hepatic veins was necessary. He remained clinically stable in outpatient follow-up, with excellent performance status-current survival of 2 years 6 months after surgical treatment.

The evaluation of useful on the additional PET/CT Liver scan (PET/CT 검사에서 Gastrointestinal Cancer 환자의 Liver 추가촬영에 대한 유용성 평가)

  • Purpose The liver one of the most common site for distant metastasis for a variety of tumor, especially of gastrointestinal origin. the purpose of this study was to analyze image quality between standard scan and additional liver scan. Materials and Methods From September 2015 to February 2016. 152 patients were examined undergo gastrointestinal cancer. 32 patients confirmed liver metastasis analyzed same liver ROI level and check the SNR, SUV and T/N ratio Results The $SNR_{mean}$ of standard was $17.7{\pm}10.3$; addition was $22.3{\pm}9.7$ (p<0.05). In $SUV_{max}$ of standard was $6.7{\pm}2.8$; addition was $7.6{\pm}3.2$ (P<0.05). and the T/N ratio of standard was $2.1{\pm}0.6$; addition was $2.5{\pm}0.8$ (P<0.05). Conclusion The $SNR_{mean}$, $SUV_{max}$ and T/N ratio were higher than those on the first scan (P<0.05). The SNRmean showed the highest change rate among the parameters. A additional liver scan is more favorable for the detection of gastrointestinal cancer patients.

Low Counts of γδ T Cells in Peritumoral Liver Tissue are Related to More Frequent Recurrence in Patients with Hepatocellular Carcinoma after Curative Resection

  • Objectives: TCR-gamma-delta+T cells (${\gamma}{\delta}$ T cells) are non-conventional T lymphocytes that can recognize and eradicate tumor cells. Our previous studies showed that infiltration and function of ${\gamma}{\delta}$ T cells were substantially attenuated in hepatocellular carcinoma (HCC). However, their prognostic value was not clarified. Methods: The association between ${\gamma}{\delta}$ T cells and the clinical outcomes was determined by immunohistochemistry (IHC) in a HCC patient cohort (n = 342). Results:Immunohistochemistry showed decreased infiltration of ${\gamma}{\delta}$ T cells in tumoral tissues compared with paired peritumoral tissues. The counts of ${\gamma}{\delta}$ T cells in peritumoral tissues were negatively correlated with tumor size (P = 0.005). Survival analysis showed that the levels of peritumoral ${\gamma}{\delta}$T cells were related to both time to recurrence (TTR) and overall survival (OS) (P = 0.010 and P = 0.036, respectively) in univariate analysis, and related to TTR in multivariate analysis (P = 0.014, H.R. [95% CI] = 0.682 [0.502-0.927]). Furthermore, the level of peritumoral ${\gamma}{\delta}$ T cells showed independent prognostic value for TTR in Barcelona Clinic Liver Cancer (BCLC) stage A patients (P = 0.038, H.R. [95% CI] = 0.727 [0.537-0.984]). However, tumoral ${\gamma}{\delta}$ T cells did not show independent prognostic value for either TTR or OS in HCC patients. Conclusions: Low counts of ${\gamma}{\delta}$ T cells in peritumoral liver tissue are related to a higher incidence of recurrence in HCC and can predict postoperative recurrence, especially in those with early-stage HCC.

  • This study is concerned with assessment of diethylnitrosamine(DEN) induced liver cell carcinogenesis by measurement of changes preceding the development of neoplasms. Therefore, it was undertaken to investigate the changes of liver-specific enzyme activities in rats (Sprague-Dawley) by ad libitum feeding of DEN. And also, as another objective index in urine, the level of urinary biopterin was measured by high performance liquid chromatography(HPLC) method. The results were as follows ; 1. Minor behavioral change, brittleness of hair and decreased amount of water and diet intake were observed in rats 7 weeks after DEN administration. 2. The body and liver weights were significantly(p<0.05) decreased in rats 11 weeks after DEN administration. 3. The ratio of liver weight to body weight was rather stable and not significantly decreased in the all treatment groups. 4. The liver specific enzyme activities(AST, ALT,$\gamma$-GTP) were significantly(p<0.05) increased in all treatment groups compared to control group. 5. Compared to normal level, urine biopterin level was significantly (p<0.05) increased in all treatment groups(p<0.05). In conclusion, this result confirmed that the DEN was one of the potent hepatocarcinogens. In experimental model of rats exposed to DEN, the results indicated that values of liver specific enzyme activities(AST, ALT, $\gamma$-GTP) and urine biopterin level could be useful complementary tumor indices.

The Antimicrobial Insect Peptide CopA3 Blocks Ethanol-Induced Liver Inflammation and Liver Cell Injury in Mice

  • Alcoholic liver disease (ALD), which encompasses alcoholic steatosis, alcoholic hepatitis, and alcoholic cirrhosis, is a major cause of morbidity and mortality worldwide. Although the economic and health impacts of ALD are clear, few advances have been made in its prevention or treatment. We recently demonstrated that the insect-derived antimicrobial peptide CopA3 exerts anti-apoptotic and anti-inflammatory activities in various cell systems, including neuronal cells and colonic epithelial cells. Here, we tested whether CopA3 inhibits ethanol-induced liver injury in mice. Mice were intraperitoneally injected with ethanol only or ethanol plus CopA3 for 24 h and then liver injury and inflammatory responses were measured. Ethanol enhanced the production of proinflammatory cytokines, tumor necrosis factor (TNF)-α, interleukin (IL)-1β, interferon (IFN)-γ, and IL-10. It also induced hepatocyte apoptosis and ballooning degeneration in hepatocytes. Notably, all these effects were eliminated or significantly reduced by CopA3 treatment. Collectively, our findings demonstrate that CopA3 ameliorates ethanol-induced liver cell damage and inflammation, suggesting the therapeutic potential of CopA3 for treating ethanol-induced liver injury.

  • Hepatocellular carcinoma is the most common primary tumor in the liver. FDG PET has been applied for staging and treatment planning of hepatocellular carcinoma. It could reflect tumor prognosis because glucose metabolism assessed by FDG PET is known to have correlations with the differentiation and aggressiveness of the tumor. Although the ability of FDG PET to detect well-differentiated or low grade tumors and intra-hepatic lesions is not good, it is expected to playa major role in pre-surgical assessments for liver transplantation because it is useful in detecting extra-hepatic lesions and unexpected distant metastases with a better diagnostic performance than other conventional imaging modalities. Additionally, FDG PET has an advantage to screen other cancers through whole body scanning. As a new tracer for PET, Acetate demonstrates higher sensitivity and specificity to FDG in evaluating hepatocellular carcinoma. It thus seems that simultaneous use of Acetate PET with FDG PET could be helpful in diagnosis, especially detecting extra-hepatic metastases.

  • Dichlorodiphenyltrichloroethane (DDT) is still used in certain areas of tropics and subtropics to control malaria and other insect-transmitted diseases. DDT and its metabolites have been extensively studied for their toxicity and carcinogenicity in animals and humans and shown to have an endocrine disrupting potential affecting reproductive system although the effects may vary among animal species in correlation with exposure levels. Epidemiologic studies revealed either positive or negative associations between exposure to DDT and tumor development, but there has been no clear evidence that DDT causes cancer in humans. In experimental animals, tumor induction by DDT has been shown in the liver, lung, and adrenals. The mechanisms of hepatic tumor development by DDT have been studied in rats and mice. DDT is known as a non-genotoxic hepatocarcinogen and has been shown to induce microsomal enzymes through activation of constitutive androstane receptor (CAR) and to inhibit gap junctional intercellular communication (GJIC) in the rodent liver. The results from our previously conducted 4-week and 2-year feeding studies of p,p'-DDT in F344 rats indicate that DDT may induce hepatocellular eosinophilic foci as a result of oxidative DNA damage and leads them to hepatic neoplasia in combination with its mitogenic activity and inhibitory effect on GJIC. Oxidative stress could be a key factor in hepatocarcinogenesis by DDT.

Malignant Metastatic Plasma Cell Tumor in a Dog

  • A primary metastatic malignant plasma cell tumor was diagnosed in a 7-year-old, female mixed breed dog. The dog was admitted to Veterinary Teaching Hospital at College of Veterinary Medicine, Seoul National University after suffering from depression, anorexia, and dyspnea. In radiographic examination, a large occupying mass was found at the right cranial thorax. Due to poor prognosis, the dog was euthanized. At surgery, numerous, firm, round, nodules, 1 to 2 cm in diameter were noted in the ribs. The nodules were often confluent and had cylindrical appearance. Osteolysis was associated with the nodules. A large amount of clear fluid was present in the thoracic cavity. The spleen and liver were markedly enlarged and congested. Histologically, the mass was diagnosed as plasma cell tumor which was believed to be originate from the rib. Evidence of metastasis was observed in the spleen, liver, and kidney.
