Metabolism of $C^{14}$-acetate in the Ehrlich ascites tumor (에르릿히 복수암에 있어서 $C^{14}$-초산염 대사)

  • Tissue homogenates of Ehrlich ascites tumor tissues and several normal tissue of mice were incubated separately in medium maintaining $C^{14}$_acetate concentrations of 5, 10, 20, 30, 40, 50 and 60 mg%, in order to determine maximum oxidative rates of acetate. In every incubation experiments, respiratory $CO_2$ samples rapped by alkaline which was placed in the center well of the incubation blask were analyzed for total $CO_2$ Production rates and their radoactivies. The fractions of $CO_2$ from medium acetate to total $CO_2$ production rate were obtained with relative specific activities (RSA) which were calculated by ratio between specific activities (SA) of $CO_2$ and medium $CO^{14}$_acetate and $CO_2$ production rates from medium acetate were calculated from RSA and total $CO_2$ production rates. Maximum plateau values of oxidative rates described above were determined at incubation experiments of various concentrations of medium acetate and compared the oxidative rates of acetate of tumor with those of normal tissues such as kidney, brain and liver. Maximum plateau values of total $CO_{2}$ Production rates were obtained at acetate concentration of 20 mg% and represent $25.0{\pm}0.54\;{\mu}M/hr/gm$ in the brain, $16.3{\pm}2.5$ in the kidney, $9.1{\pm}1.78$ in the liver and $11.5{\pm}3.2\;{\mu}M/hr/gm$ in the ascites tuners. Substancial $CO_2$ yield was observed in the tumor tissues as in the normal tissues. On the other hand, plateau values of RSA were $25.7{\pm}1.04%$ in thee brain, $9.1{\pm}0.72%$ in the kidney, $2.5{\pm}0.73%$ in the liver and $0.51{\pm}0.12%$ in the tumor tissues. $CO_2$ yields from the medium acetate, were 4.19 in the kidney, 2.28 in the brain, 0.228 in the liter and $0.059\;{\mu}M/hr/gm$ in the tumor tissue. These show wide range even in the normal tissue but remarkable decrease in the tumor tissue. This fact means that further oxidation of acetate was inhibited remarkably in the tumor tissue.

  • Development of liver cancers is driven largely by genomic alterations that deregulate signaling pathways, influencing growth and survival of cancer cells. Because of the hundreds or thousands of genomic/epigenomic alterations that have accumulated in the cancer genome, it is very challenging to find and test candidate genes driving tumor development and progression. Systematic studies of the liver cancer genome have become available in recent years. These studies have uncovered new potential driver genes, including those not previously known to be involved in the development of liver cancer. Novel approaches combining multiple datasets from patient tissues have created an unparalleled opportunity to uncover potential new therapeutic targets and prognostic/predictive biomarkers for personalized therapy that can improve clinical outcomes of the patients with liver cancer.

Five-Year Survival of Alpha-Fetoprotein-Producing Gastric Cancer with Synchronous Liver Metastasis: A Case Report

  • Alpha-fetoprotein-producing gastric cancer is associated with poor prognosis because of frequent liver and lymph node metastasis. We present a case with synchronous liver metastasis who survived for 5 years. A 69-year-old man with upper abdominal pain was referred to our hospital. Gastrointestinal endoscopy revealed a Borrmann II-like tumor in the lower part of the stomach. Computed tomography revealed a tumor in the left lobe of the liver. Serum alpha-fetoprotein levels were markedly increased. We performed distal gastrectomy after administering oral tegafur/gimeracil/oteracil potassium and administered hepatic intra-arterial cisplatin injection. Liver metastasis showed partial response on computed tomography. Despite left hepatic lobectomy, further metastases to the liver and mediastinal lymph nodes became difficult to control. After sorafenib tosylate administration, stabilization of the disease was observed for 4 months. We conclude that hepatic intra-arterial chemotherapy and oral administration of sorafenib tosylate may potentially improve the prognosis in such cases.

Cilostazol Decreases Ethanol-Mediated TNFalpha Expression in RAW264.7 Murine Macrophage and in Liver from Binge Drinking Mice

  • Alcoholic hepatitis is a leading cause of liver failure in which the increased production of tumor necrosis factor ${\alpha}$ (TNF${\alpha}$) plays a critical role in progression of alcoholic liver disease. In the present study, we investigated the effects of cilostazol, a selective inhibitor of type III phosphodiesterase on ethanol-mediated TNF${\alpha}$ production in vitro and $in$ $vivo$, and the effect of cilostazol was compared with that of pentoxifylline, which is currently used in clinical trial. RAW264.7 murine macrophages were pretreated with ethanol in the presence or absence of cilostazol then, stimulated with lipopolysacchride (LPS). Cilostazol significantly suppressed the level of LPS-stimulated TNF${\alpha}$ mRNA and protein with a similar degree to that by pentoxifylline. Cilostazol increased the basal AMP- activated protein kinase (AMPK) activity as well as normalized the decreased AMPK by LPS. AICAR, an AMPK activator and db-cAMP also significantly decreased TNF${\alpha}$ production in RAW264.7 cells, but cilostazol did not affect the levels of intracellular cAMP and reactive oxygen species (ROS) production. The $in$ $vivo$ effect of cilostazol was examined using ethanol binge drinking (6 g/kg) mice model. TNF${\alpha}$ mRNA and protein decreased in liver from ethanol gavaged mice compared to that from control mice. Pretreatment of mice with cilostazol or pentoxifylline further reduced the TNF${\alpha}$ production in liver. These results demonstrated that cilostazol effectively decrease the ethanol-mediated TNF${\alpha}$ production both in murine macrophage and in liver from binge drinking mice and AMPK may be responsible for the inhibition of TNF${\alpha}$ production by cilostazol.

Preparation and Biodistribution of Re-188-Sulfur Colloid Suspension in Lipiodol (Re-188이 표지된 황 교질(Sulfur Colloid)/리피오돌(Lipiodol)의 제조와 생체내 분포)

  • Purpose: Lipiodol is used for targeting liver cancers by administrating through the hepatic artery. In the present study, feasibility of Re-188-sulfur colloid suspension in lipiodol as a liver cancer targeting agent was investigated. Materials and Methods: Re-188-sulfur colloid was prepared, harvested by centrifugation, washed with organic solvent and then suspended into lipiodol. Biodistribution of Re-188-sulfur colloid in normal saline and its suspension in lipiodol in mice after 1 hr of injection through the tail vein were investigated. Biodistribution and autoradiography of tumor-hearing liver was acquired after 5 min post-injection into left ventricle of the tumor-inoculated rats. Results: After 1 hr of injection with Re-188-sulfur colloid suspensiob in lipiodol through the tail vein in normal mice (n=3), the uptakes in the liver and lung were $5.2{\pm}0.7\;and\;91.0{\pm}1.7%$ ID/organ, respectively. After 5 min of injection with Re-188-sulfur colloid suspention in lipiodol through the left ventricle in the tumor-inoculated rats (n=4), uptakes in the normal liver, hepatoma, and lung were $0.41{\pm}0.28,\;1.88{\pm}1.57,\;and\;1.65{\pm}1.54%$ ID/organ, respectively. And autoradiography of hepatoma showed increased uptake than normal liver tissues. Conclusion: Re-188-sulfur colloid suspension in lipiodol injected through the artery shows higher uptake in the hepatoma than normal liver tissue that indicates the feasibility as a new radiopharmaceutical for therapy of hepatoma.

?Effects of Duchesnea indica on Several Kinds of Cancer Cells (사매가 수종(數種)의 암세포(癌細胞)에 미치는 영향(影響))

  • Objectives: The aim of the experiment is to identify any anti-tumor effects of Duchesnea indica(Andr.) Focke on stomach, liver, urinary bladder, prostate and kidney cancer cells. Materials & Methods: For cancer cells, AGS stomach, Hep3B and Hep3G2 liver, HT-1197, HT-1376 urinary bladder, PC3 prostate, and A-704 kidney cancer cells, all obtained from Korean Ce 11 Line Bank, were used. The boiled extract of Duchesnea indica(Andr.) Focke (10 and 20 microliters) was injected into cultures, and the cultures were observed at 0, 6 and 12 hours, and from then on at 12 hours intervals up to 72 hours. The destruction of stomach, liver, urinary bladder, prostate and kidney cancer cells were measured through Trypan blue exclusion testing. The suppresion on viability of stomach, liver, urinary bladder, prostate and kidney cancer cells was measured via MTT assay. Anti-cancer mechanisms were assessed by analyzing the cell cycle. Results: In morphologic change, AGS, Hep3B, HepG2 showed the withdrawn and floating appearance that is typical in cellular impairment. The destruction of AGS, HT-1197, HT-1376, A-704, PC-3, Hep3B and HepG2 cancer cells in each test group was greater than that in the control group to a statistically significant degree. The suppression on viability of AGS, HT-1197 and Hep3G in each test group was greater than that in the control group to a statistically significant degree. Analysis of the cell cycle after injection of D... Focke showed inhibition of cell division in all test groups(AGS, Hep3B, HepG2, HT-1197, HT-1376, PC3, A-704). Conclusions: The results of this experiment suggest that Duchesnea indica(Andr.) Focke has statistically significant anti-tumor effects on stomach, urinary bladder, kidney, prostate and liver cancer, of which stomach and liver cancer are prominently significant. This in vitro experiment supports a role for Duchesnea indica(Andr.) Focke as a potential cancer treatment, but progressive research on Duchesnea indica(Andr.) Focke and its anti-tumor effects is needed to develop a practical application for it in cancer treatment.

Neuroendocrine Carcinoma of the Stomach -A Clinicopathologic Study of 18 Cases- (위에 발생한 신경내분비 암종 -18예의 위신경내분비 암종의 임상병리학적 고찰-)

  • Purpose: Neuroendocrine carcinomas of the stomach account for only about $0.3\%$ of all gastric tumors. The prognosis of this disease is very poor compared with the common type of gastric adenocarcinoma. The purpose of this retrospective study was to review the clinicopathologic features of 18 cases of this unusual gastric tumor and to establish a treatment strategy for this tumor. Materials and Methods: Excluding 2 cases of non-curative resection and 1 case of operative mortality, 18 cases of typical neuroendocrine carcinoma who had curative resection from January 1991 to December 2000 at Asan Medical Center were analyzed; 6841 gastric cancer patient were treated surgically during the same period. Results: The mean age at the time of diagnosis was 58.6 years (range: $35\∼75$ yr). Sixteen patients were male, and two were female. Eleven tumors ($61.1\%$) developed in the lower part of the stomach, three ($16.7\%$) in the middle part, and three ($16.7\%$) in the upper part. One tumor involved the entire stomach. Eight cases ($44.4\%$) were Borrmann type 2, and six case ($33.3\%$) were Borrmann type 3. The mean tumor size was 6.94 cm (range: $0.6\∼15$ cm). Nine cases ($50\%$) showed recurrence of the disease, and eight of them died within 20 months. Of the nine recurred cases, 7 cases ($77.8\%$) showed liver metastasis. The mean disease-free interval was 6.8 months (range: $2.5\∼11$ months) after surgical resection, and the mean survival was 17.9 months (range: $8\∼40$ months) for recurrence cases. One patient with liver metastasis was treated with a liver-wedge resection just after diagnosis and was still alive for 37.5 months postoperatively. There were 9 deaths after the median follow- up period of 40 months (range: $8\∼72$ months). Conclusion: Gastric neuroendocrine carcinomas frequently recur at the liver, even in early stage cancer, and have a poor prognosis. We experienced a case of successful control of hepatic metastasis by surgical resection and a case of a small cell carcinoma which was successfully controlled with systemic chemotherapy.

Roles of Fibroblast Growth Factor-inducible 14 in Hepatocellular Carcinoma

  • The prognostic value of the fibroblast growth factor-inducible 14 (Fn14) expression in hepatocellular carcinoma (HCC) is unknown. Real-time PCR (RT-PCR), western blot assays and immunohistochemistry analysis were here performed in order to compare Fn14 expressios in paired liver samples of HCC and normal liver tissue. Most of the tumor tissues expressed significantly higher levels of Fn14 compared to adjacent non-tumor tissues, with Fn14High accounting for 54.6% (142/260) of all patients. The Pearson ${\chi}^2$ test indicated that Fn14 expression was closely associated with serum alpha fetal protein (AFP) (P=0.002) and tumor number (p=0.019). Univariate and multivariate analyses revealed that along with tumor diameter and portal vein tumor thrombosis (PVTT ) type, Fn14 was an independent prognostic factor for both overall survival (OS) (HR=1.398, p=0.008) and recurrence (HR=1.541, p=0.001) rates. Fn14 overexpression HCC correlated with poor surgical outcome, and this molecule may be a candidate biomarker for prognosis as well as a target for therapy.

  • Non-islet cell tumor-induced hypoglycemia (NICTH) is associated with mesenchymal tumor types, including hemangiopericytoma, fibrosarcoma, mesothelioma, and neurofibroma, as well as carcinoma of the liver, adrenal glands, and kidneys. Non- islet cell tumors induce hypoglycemia by overproducing an abnormal form of insulin-like growth factor II (IGF m. Complete removal of the tumor or reduction of the tumor mass is a successful therapeutic strategy in cases of NICTH. However, if the tumor re-grows, curative resection is nearly impossible, and hypoglycemia occurs repeatedly. Glucocorticoids are effective in terms of long-term relief from hypoglycemia through promotion of gluconeogenesis in the liver, tumor suppression, production of 'big'-IGF-II, and correction of the attendant biochemical abnormalities involving the growth hormone (GH)-IGF axis. We found that administration of corticosteroid therapy to a patient suffering from NICTH resulted in improvement of hypoglycemia associated symptoms.

Intercostal Neuralgia and Spinal Cord Compression Symptom due to Spinal Tumor -A Case Report- (척추 종양에 의한 늑간 신경통 및 척수 압박 증상 -증례 보고-)

  • A 49 years old male patient was admitted to our neuro-pain clinic with symptoms of left 11th intercostal neuralgic pain and low back pain that developed 2 months prior to admission. Upon initial physical examination, motor weakness or sensory deficit were absent. Intercostal neuralgic pain improved significantly after we performed thoracic root thermocoagulation. However on the afternoon of the procedure the patient started to experience voiding difficulty, saddle anesthesia and rapidly progressing motor weakness and hypoesthesia that involved the lower back area and the lower extremities for three days. Based on these symptoms spinal cord compression was suspected and subsequently plain T-L spine X-rays and T-L spine MRI were performed. A spinal tumor that appeared metastatic in origin was seen at the T11 and T12 level. Liver ultrasonography demonstrated the presence of a $4{\times}4cm$ sized ill defined mass in the posterior segment of the right lobe. The patient was diagnosed to have hepatocellular carcinoma after needle aspiration biopsy and cytologic studies. Further orthopedic surgery was recommended but as the patient rejected any further treatment and examination, it was not possible to confirm the primary focus of the tumor. However as metastasis of a primary liver tumor to the spine is a rare occurrence, some other primary focus of metastasis or even a malignant primary tumor of the spine is more likely to explain this patient's condition.
