The studies on the liver fluke of Korea have been reviewed. The fluke is Fasciola hepatica. Its intermediate host is Lymnaea biridis, whose principal habitats are rice paddies. The first fascioliasis of Korean native(beef) cattle was reported in 1915. Bovine fascioliasis is present throughout the conuntry, including north Korea, with the average prevalence of 30%. The prevalence of the disease is about the same in dairy cattle, which have been imported from other countries since 1960s. The disease was also found in Korea in other species of animals such as sheep, goats, deer of zoo and farms, and guinea pigs of a laboratory. Human fascioliasis was reported ; 35 cases by fecal examination and 11 by both parasitology and pathology. Of the latter 11 cases, 5 were ectopic parasitism. Economic loss due to the reduced carcass weight and milk production, and liver condemnation was great. Almost all flukicides developed in other countries are commercially available in Korea.
The livers of 300 cases of Korean native cattle which were collected at slaughted house in Pusan were examined by gross and histopathological means to study patterns of pathological changes of the organ. 1. The most dominated lesion of the liver was those of fascioliasis showing incidence rate as 907 per cent of inspected cattle. 2. On age distribution of the fascioliasis lesion in the liver, higher incidence was seen in animals that were from 6 to 9 years of age. 3. Significant changes of the hepaticparenchyma caused by immature flukes included thrombosis, migrational tracks such as hemorrhagic foci with massive eosinophilia and hepatic necrosis. The changes of chronically infected livers of cattle were recognized as proliferation of periductal eosinophilic granuloma of various degrees in each portal triad. And this easential cholangiohepatitis might develop into multiple focal cirrhosis or diffuse perilobular cirrhosis. 4. Most cases with gross lesion of swelled bile ducts on visceral surface of the liver showed histological evidences of fascioliasis lesion throughout hepatic lobes.

A total of 44 adult or juvenile nutrias were necropsied for disease survey. A large nodule was found in the liver of a nutria. The histopathological specimen of the hepatic nodule was microscopically examined, and sectional worms were found in the bile duct. The worms showed a tegument with spines, highly branches of vitelline glands and intestine. Finally, we histopathologically confirmed fascioliasis in a wild nutria. In the present study, a case of fascioliasis in a wild nutria is first confirmed in Korea.

A 56 year-old Korean housewife/farmerlgoat keeper suffered from right upper quadrant pain and fever with chills. In the abdominal sonogram and computerized tomography, multiple, 2-3 cm, irregular shaped cavities were observed in the right lobe of liver. A liver biopsy revealed extensive central necrosis with Characot-Leyden crystals surrounded by palisading histiocytes, eosinophil-rich inflammatory infiltration. Worm was not observed. However, the serologic test for Fusciola-specific IgG antibody by micro-ELISA was positive. Prior antibody levels did not differ and eosionophilia persisted 6 and 16 months after praziquantel treatment although the cavitaxy lesions in the liver disappeared 6 months after the treatment. Reported herein is a human case of invasive fascioliasis diagnosed clinically by a combination of radiological, histopathological and serological studies.
A field survey of fascioliasis of Korean native cattle raising and raised in specialized commercial breeding farms and local farms in Kangwon province using both intradermal test and sedimentation technique for feces was carried out from November to December, 1996. Fecal samples were taken from fascioliasis positive cattle by the intradermal test for the fecal examination. Liver tissues were randomly collected from an abattoir for histopathological examination of liver fluke infection in cattle. The results are as follows. 1. By the intradermal test for a total of 211 cattle raising in both Wonju and Wheongsung, Kangwon province, 60 heads(28.4%) showed positive reaction. Among 60 positive cattle, eggs of Fasciola hepatica were found from 51 heads(85.0%) by sedimentation technique. 2. According to the cattle raising areas, the positive rates by the intradermal test were 26.7%(20 out of 75 heads) in Wonju and 29.4%(40 out of 136 heads) in Wheongsung. 3. According to the age of cattle examined, the positive rates by the intradermal test in 1~3, 4~6 and 7~10 years old were 11.7%(7 out of 100 heads), 68.3%(41 out of 93 heads) and 20.0%(12 out of 18 heads), respectively. 4. The overall infection rates of fluke larvae from the slaughtered cattle at an abattoir in Wonju was 24.7%(37 out of 150 heads). In histopathology, liver lesions were observed such as inflammation with infiltration of eosinophils, polymorphonuclear cells, mononuclear cells and multinucleated giant cells, proliferation of connective tissues, calcification and abscess formation.

A research study was undertaken to quantify fascioliasis in both live and slaughtered zebu cattle at Savar, Bangladesh. Eggs of Fasciola gigantica per gram of feces (EPG) was determined in a total of 213 fasciola infested live cattle. The EPG per animal ranged from 100 to 400 (mean $138.03{\pm}4.27SE$). Counting of F. gigantica was made in a total of 63 fasciola infested livers of slaughtered cattle. Number of immature flukes per liver ranged from 0 to 37 (mean $8.74{\pm}0.85SE$) and mature flukes ranged from 2 to 121 (mean $20.54{\pm}2.23SE$). Total load of flukes recovered per liver varied from 4 to 132 (mean $29.28{\pm}2.42SE$). Significantly higher EPG (p < 0.05) and higher load of flukes in the livers (p < 0.01) were observed from September to December (post monsoon and winter). The EPG and fluke counts were found significantly higher (p < 0.01) in animals after one year of age and these were also higher in female animals (p < 0.05) than the males.
Fascioliasis is a parasitic infection caused by liver flukes. Although several cases have been reported in Korea, phylogenetic analysis of isolates is lacking. In this study, a 66-year-old woman with right upper quadrant (RUQ) abdominal pain was diagnosed as fascioliasis involving abdominal muscle by imaging study. She received praziquantel treatment, but symptoms were not improved. Lateral movement of the abscess lesion was followed. Trematode parasite was surgically removed from the patient's rectus abdominis muscle. The fluke was identified as Fasciola hepatica based on sequence analysis of 18S rDNA. To determine the phylogenetic position of this Fasciola strain (named Korean Fasciola 1; KF1), the cox1 gene (273 bp) was analyzed and compared with the genes of 17 F. hepatica strains isolated from cows, sheep, goats, and humans from various countries. Phylogenetic analysis showed that KF1 was closely related with the isolates from China goat.

Human Fasciola hepatica infection is a rare entity involving infestation of the liver and biliary tree with adult flukes. which can result in hepatitis. cirrhos is and biliary tract inflammation. obstruction and lithiasis. The patient had the typical diagnostic tetrad of fever. eosinophilic leukocytos is. tender hepatomegaly and fluke ova in the stools. Theatment consistes of Emetine hydrochloride administration for hepatic involvement and common bile duct exploration for removal of flukes. with cholecystectomy for associated cholelithiasis. The combination of medical and surgical therapy cal be expected to produce an arrest of this infection. The removed liver revealed eggs of the fasciols species in the intrahepatic bile duct. The clinical history. pathological findings and treatment of this case were described.
The incidences of liver abscess, fatty liver and fascioliasis were 11.9%, 23.2% and 36.0%, respectively in 211 Holstein cattle observed at the slaughter house. Blood chemical values of them were measured. The results were as follows : 1. Liver abscess group was significantly higher than normal group in the levels of SDH, AST, TP, globulin and fibrinogen, but it was significantly lower than normal group in the levels of albumin and A/G ratio (p<0.01). 2. Fatty liver group was significantly higher than normal group in the levels of SDH, AST, OCT, TP and globulin, but it was significantly lower than normal group in the levels of albumin and A/G ratio (p<0.01). 3. Fasciolisais group was significantly higher than normal group in the levels of AST. TP and globulin (p<0.01), but it was significantly lower than normal group in the levels of albumin and A/G ratio (p<0.01).
Fascioliasis is a foodborne zoonotic parasitic disease. We report 4 cases occurring in the same family, in whom diagnosis of acute fascioliasis was established after series of tests. One case was hospitalized with fever, eosinophilia, and hepatic lesions. MRI showed hypodense changes in both liver lobes. The remaining 3 cases presented with the symptom of stomachache only. Stool analysis was positive for Fasciola eggs in 2 adult patients. The immunological test and molecular identification of eggs were confirmed at the National Institute of Parasitic Diseases, Chinese Center for Disease Control and Prevention, Shanghai, China. The results of serological detection were positive in all the 4 patients. DNA sequencing of PCR products of the eggs demonstrated 100% homology with ITS and cox1 of Fasciola hepatica. The conditions of the patients were not improved by broad-spectrum anti-parasitic drugs until administration of triclabendazole.
