Ob/ob Mouse에서 비탐-에스의 비 알코올성 지방간 개선 활성 (Bitam-S Improves the Non-alcoholic Fatty Liver Disease in C57BL/6J ob/ob Mice)

  • Semisulospira libertine (SL) has been used as a folk medicine for quenching a thirst, hepatic fever and inflammation in oriental countries. Although SL has been anecdotally ascertained to ameliorate the hepatic diseases, there are no sufficient experimental evidences. The purpose of this study was to examine the effect of Bitam-S, in which SL is a main component, on non-alcoholic fatty liver disease manifested in C57BL/6J ob/ob mice. At 6 week old, the ob/ob mice were randomly divided into four groups; control and three treatment groups. The control mice was to receive a regular diet, and the treatment groups were fed a regular diet with either 250mg/kg, 500mg/kg of Bitam-S (BS250 and BS500) or 300mg/kg of metformin (MT300) for a 8-week period. Bitam-S exerted beneficial effects on lipid homeostasis in ob/ob mice that are not necessarily due to its ability to decrease food intake but its specific effects on hepatic lipogenesis related genes (SREBP1a, FAS and SCD-1). The combined effects of Bitam-S to reduce body weight and lipogenic gene expressions, and reduce the deposition of triglyceride in the liver are indicative of a marked improvement in obesity-related non-alcoholic fatty liver disease. Taken together, Bitam-S has potential as a treatment agent for non-alcoholic fatty liver disease and deserves clinical trial in the near future.

Diagnostic Value of Fecal Calprotectin as a Screening Biomarker for Gastrointestinal Malignancies

  • Background: Calprotectin in feces seems to be a more sensitive marker for gastrointestinal (GI) cancers than fecal occult blood, but its specificity may be too low for screening average risk populations. This study aims at evaluating the diagnostic value of fecal calprotectin as a screening biomarker for GI malignancies. Materials and Methods: In a case-control study, 100 patients with GI malignancies (50 patients with colorectal cancer and 50 patients with gastric cancer) and 50 controls were recruited in Tabriz Imam Reza and Sina hospitals during a 24-month period. One to two weeks after the last endoscopy/colonoscopy, fecal specimens were collected by the patients and examined by ELISA method for quantitative measurement of calprotectin content. The results were compared between the three groups. Results: The mean fecal calprotectin level was $109.1{\pm}105.3$ (2.3-454.3, median:74), $241.1{\pm}205.2$ (3.4-610.0, median:19.3) and $45.9{\pm}55.1{\mu}g/g$ (1.3-257.1, median:19.3) in gastric cancer, colorectal cancer and control group, respectively, the differences being significant (p<0.001) and remaining after adjustment for age. The optimal cut-off point for fecal calprotectin was ${\geq}75.8{\mu}g/g$ for distinguishing colorectal cancer from normal cases (sensitivity and specificity of 80% and 84%, respectively). This value was ${\geq}41.9{\mu}g/g$ for distinguishing gastric cancer from normal cases (sensitivity and specificity of 62%). Conclusions: Our results revealed that fecal calprotectin might be a useful and non-invasive biomarker for distinguishing colorectal cancer from non-malignant GI conditions. However, due to low sensitivity and specificity, this biomarker may not help physicians distinguishing gastric cancer cases from healthy subjects.

Role of Soluble Adiponectin Receptor 2 in Non-Alcoholic Fatty Liver Disease in Children

  • Purpose: The incidence of non-alcoholic fatty liver disease (NAFLD) in children is gradually increasing. The aim of this study was to investigate the use of serum adiponectin and soluble adiponectin receptor 2 (soluble Adipo R2) levels for the diagnosis of fatty liver disease in obese and overweight children. Methods: The study included 51 obese and overweight children between the ages of 6 and 18 years diagnosed with NAFLD using ultrasonography and 20 children without fatty liver disease. Patients whose alanine transaminase level was two times higher than normal (${\geq}80U/L$) were included in the non-alcoholic steatohepatitis (NASH) group. Results: NASH was observed in 11 (21.6%) of the patients with NAFLD. The incidence of obesity was higher in patients with NASH (80% and 45%, p=0.021). While the adiponectin levels were similar in patients with NAFLD and those without, they were below the normal level in the whole study group. Adiponectin and soluble Adipo R2 levels of patients with NASH were lower than those in patients without NASH; however, this difference was not statistically significant (p=0.064 and p=0.463). Soluble Adipo R2 levels in obese patients with NAFLD were higher than those in obese children without NAFLD (p<0.001). Conclusion: Soluble adiponectin receptor 2 level is a noninvasive marker that can be used for the diagnosis of NAFLD in obese children.

Gene Regulations in HBV-Related Liver Cirrhosis Closely Correlate with Disease Severity

  • Liver cirrhosis (LC) is defined as comprising diffuse fibrosis and regenerating nodules of the liver. The biochemical and anatomical dysfunction in LC results from both reduced liver cell number and portal vascular derangement. Although several studies have investigated dysregulated genes in cirrhotic nodules, little is known about the genes implicated in the pathophysiologic change of LC or about their relationship with the degree of decompensation. Here, we applied cDNA microarray analysis using 38 HBsAg-positive LC specimens to identify the genes dysregulated in HBV-associated LC and to evaluate their relation to disease severity. Among 1063 known cancer- and apoptosis-related genes, we identified 104 genes that were significantly up- (44) or down- (60) regulated in LC. Interestingly, this subset of 104 genes was characteristically correlated with the degree of decompensation, called the Pugh-Child classification (20 Pugh-Child A, 10 Pugh-Child B, and 8 Pugh-Child C). Patient samples from Pugh-Child C exhibited a distinct pattern of gene expression relative to those of Pugh-Child A and B. Especially in Pugh-Child C, genes encoding hepatic proteins and metabolizing enzymes were significantly down-regulated, while genes encoding various molecules related to cell replication were up-regulated. Our results suggest that subsets of genes in liver cells correspond to the pathophysiologic change of LC according to disease severity and possibly to hepatocarcinogenesis.

  • 현대사회에 있어서 질병은 매우 다양하게 발견되고 있다. 또한 한번 발병하면 아니면 적절한 검진시기를 놓쳐 돌이킬 수 없는 상황을 접하게 되는 경우가 종종 있다. 사회가 발전하면서 과도한 비즈니스와 음주 흡연 등으로 우리의 간은 매우 피곤한 상태에 놓이게 되며 질병 발생율도 매우 높다. 여러 질병 중 특히 간에 관련된 질병은 회복이 불가하여 장기이식수술에 의존하는 편이다. 이와 관련하여 본 연구는 MDCT로 얻어진 영상을 3차원 영상 기법으로 간의 형상을 렌더링 하여 체적을 구하고 간 이식 수술시 혈관과의 분리 작업을 통해 절개 라인을 결정할 수 있도록 미리 시물레이션 할 수 있는 영상처리 기법을 개발하고자 간 영역 자동 추출 알고리즘을 적용하여 시물레이션 한 결과 수술을 용이하게 집도할 수 있도록 새로운 장을 열게 되었다.

사암침법 간정격과 간승격의 최근 연구 동향 분석 (Recent Study Trends of the Liver-tonification and Liver-sedation of Saam Acupuncture)

  • Objectives : This study was aimed to investigate current status researches and to provide source in setting development strategy on Liver-tonification acupuncture(LTA) and Liver-sedation acupuncture(LSA). Methods : We searched databases of Pubmed, KISS, OASIS, Google scholar and CNKI up to February 2017. We included articles that using LTA or LSA as main treatment, using the partial acupoints combination of LTA or LSA, or using each acupoint of LTA or LSA. We analyzed disease, details of acupuncture and control treatment and clinical outcomes from included studies. Results and Conclusions : Among the included clinical studies, case report was the most used methodology. LTA used to treat obesity, myopia, bartholinitis, tinnitus, hiccup and sleep disorder, prostate cancer and tremor. LSA used to treat panic disorder, gastroesophageal reflux disease and irritable bowl syndrome, herpes zoster and neuralgia. Obesity and tremor were the most studied diseases using LTA or LSA. Amyotrophic lateral sclerosis was the only disease which LTA or LSA have no therapeutic effect. Each acupoints was not used as a meaning of Saam acupuncture but as affiliated meridian acupoint. Since animal researches were not fully conducted, mechanism of LTA and LSA was hard to be proved. Thus, more clinical studies and basic animal research are needed.

간경변증 환자의 자가간호 이행에 미치는 영향요인 (Factors Influencing Self-care Compliance among Patients with Liver Cirrhosis)

  • Purpose: The purposes of this study were to examine self-efficacy, severity, social support, and self-care compliance of patients with liver cirrhosis and to identify the factors that affect their self-care compliance. Methods: A total of 160 outpatients with liver cirrhosis visiting the gastroenterology clinic at an university hospital located in Busan were enrolled in this study. Data were collected via self-report questionnaires including self-efficacy, severity of disease, social support, and self-care compliance measures. The collected data were analyzed using t-test, ANOVA, $Scheff\acute{e}-test$, Pearson correlation coefficients, and multiple linear regression analysis. Results: The factors affecting self-care compliance were self-efficacy, severity of disease, age, and sex. These factors explained 28.4% of the variance in self-care compliance of patients with liver cirrhosis. Conclusion: Therefore, nursing interventions that enhance patients' self-efficacy are needed to promote self-care compliance in patients with liver cirrhosis. In addition, patients must be taught of the importance of self-care compliance so that even young patients and those with relatively mild disease. That would also be motivated to comply with self-care instructions.

간동맥 화학색전술을 받은 간암환자의 영양상태에 미치는 영향요인 (Influencing Factors of Nutritional Status among Liver Cancer Patients Receiving Transcatheter Arterial Chemoembolization(TACE))

  • Objectives : The purpose of this descriptive observational study was to identify the factors influencing the nutritional status of patients with liver cancer receiving transarterial chemoembolization(TACE) Methods : A total of 100 participants were enrolled in this study. Data were collected and descriptive statistics and logistic regression were performed. Results : Among participants, 41.0% had a risk of malnutrition and 59.0% had a normal nutritional status. Statistically significant differences between the normal and risk groups were found for the following factors: occupation; economical status; regular exercise; underlying disease; adverse events right after TACE; current adverse events; duration after TACE; depression; and self-care performance. Underlying disease(OR=5.134, p=.005) and self-care performance(OR=0.931, p=.032) had statistically associated with nutritional status. Conclusions : The findings suggest that underlying disease and self-care performance influence the nutritional status among liver cancer patients receiving transcatheter arterial chemoembolization(TACE).

Overview of RCT for Non-Alcoholic Fatty Liver Disease and Non-Alcoholic Steatohepatitis

  • Objective: This study aimed to get information on the current status of therapies to date for non-alcoholic fatty liver disease (NAFLD), including non-alcoholic steatohepatitis (NASH). Methods: All randomized clinical controlled trial (RCT)-derived papers for NAFLD or NASH were reviewed via PubMed Database. Results: 39 RCTs met the review criteria, of which 15 and 24 papers were for NAFLD and NASH, respectively. 83% of the papers were released since 2006, and 30 studies were conducted for western medicines, antioxidants and lifestyle intervention whereas nine trials were done using herbal medicine or acupuncture which showed positive outcome. Conclusions: NAFLD and NASH are new epidemic disorders which can be a target of traditional Oriental medicine. This study will be helpful for the Oriental medicine-based strategies or therapeutic development for them.

Noninvasive diagnosis of pediatric nonalcoholic fatty liver disease

  • Because nonalcoholic steatohepatitis can progress towards cirrhosis even in children, early detection of hepatic fibrosis and accurate diagnosis of nonalcoholic fatty liver disease (NAFLD) are important. Although liver biopsy is regarded as the gold standard of diagnosis, its clinical application is somewhat limited in children due to its invasiveness. Noninvasive diagnostic methods, including imaging studies, biomarkers of inflammation, oxidative stress, hepatic apoptosis, hepatic fibrosis, and noninvasive hepatic fibrosis scores have recently been developed for diagnosing the spectrum of NAFLD, particularly the severity of hepatic fibrosis. Although data and validation are still lacking for these noninvasive modalities in the pediatric population, these methods may be applicable for pediatric NAFLD. Therefore, noninvasive imaging studies, biomarkers, and hepatic fibrosis scoring systems may be useful in the detection of hepatic steatosis and the prediction of hepatic fibrosis, even in children with NAFLD.
