간질증 초기 침습기에 발견된 간농양 예 (A human case of invasive fascioliasis associated with liver abscess)

  • 강원도 홍천에 거주하는 56세 구부가 갑자기 우상부 복통 고열 오한을 주소로 내원하였고 복 꾸 초음파검사와 콤퓨터단층촬영 결과 2-3 cm 직경의 불규칙한 꼴의 동공이 간 우엽에서 여러개 관찰되었다. 간생검상 샤르코-라이덴 결정을 포함한 심한 중심성 괴사 주위로 조직구. 호산구등 염 증세포가 침윤되어 주위 조직과 명확히 구분되었다. 효소면역측정법을 이총한 간질 특이 항체 혈청 걸사에서 양성이었다. 간 병변은 프라지관텔 투약 6개웍 후에 모두 사라졌으나. 특이 항체가와 호산구 증가는 투약 6개월 16개월 후에도 정상화 되지 않았다. 방사선학적 소견 조직병리학 소견 및 혈청학 검사 결과를 종합할 때. 초기 침습성 간질증으로 진단하였으며 프라지콴텔에 의하여 치료되지 않은 예이었다.

Procedural outcomes of laparoscopic caudate lobe resection: A systematic review and meta-analysis

  • A systematic review was conducted in compliance with PRISMA statement standards to identify all studies reporting outcomes of laparoscopic resection of benign or malignant lesions located in caudate lobe of liver. Pooled outcome data were calculated using random-effects models. A total of 196 patients from 12 studies were included. Mean operative time, volume of intraoperative blood loss, and length of hospital stay were 225 minutes (95% confidence interval [CI], 181-269 minutes), 134 mL (95% CI, 85-184 mL), and 7 days (95% CI, 5-9 days), respectively. The pooled risk of need for intraoperative transfusion was 2% (95% CI, 0%-5%). It was 3% (95% CI, 1%-6%) for conversion to open surgery, 6% (95% CI, 0%-19%) for need for intra-abdominal drain, 1% (95% CI, 0%-3%) for postoperative mortality, 2% (95% CI, 0%-4%) for biliary leakage, 2% (95% CI, 0%-4%) for intra-abdominal abscess, 1% (95% CI, 0%-4%) for biliary stenosis, 1% (95% CI, 0%-3%) for postoperative bleeding, 1% (95% CI, 0%-4%) for pancreatic fistula, 2% (95% CI, 1%-5%) for pulmonary complications, 1% (95% CI, 0%-4%) for paralytic ileus, and 1% (95% CI, 0%-4%) for need for reoperation. Although the available evidence is limited, the findings of the current study might be utilized for hypothesis synthesis in future studies. They can be used to inform surgeons and patients about estimated risks of perioperative complications until a higher level of evidence is available.

간 병변 특이성 조영제 자기공명영상에 대한 연구: Ferucarbotran과 Gd-EOB-DTPA 조영제의 비교 (A Study on Usefulness of Specific Agents with Liver Disease at MRI Imaging: Comparison with Ferucarbotran and Gd-EOB-DTPA Contrast Agents)

  • 다양한 간 질환에 Ferucarbotran (SPIO)과 Gd-EOB-DTPA (Primovist) 조영제를 사용하여 자기공명영상 상에서 비교하여 간 질환의 특성 및 검출 과 그 질환에 따른 조영제 흡수관계를 알아보고자 한다. 실험대상은 국소 간 병변을 가진 50명의 환자 중 남자 25명, 여자 25명으로 평균나이는 50세이었다. 사용된 장비와 코일은 3.0T (General Electric Medical System, Excite HD) 자기공명영상장치와 8채널 채부코일이었다. 질환과 조영제 흡수관계를 비교하기 위하여 사용된 펄스 시퀀스는 Liver aquisition with volume acceleration (LAVA)와 Multi-Gradient rapid Echo (MGRE)이었다. 모든 검사기법은 일정한 데이터를 획득하기 위하여 Ferucarbotran (SPIO) 및 Gd-EOB-DTPA (Primovist) 조영제를 주입 전 후에 같은 위치를 선정하여 검사를 시행했다(p<0.05). 검사 결과는 Ferucarbotran와 Gd-EOB-DTPA의 대조도대 잡음비는 간세포암(HCC, 16 cases)이 $3.08{\pm}0.12$$7.00{\pm}0.27$이었다. 과다형성결절(Hyperplastic Nodule, 7 cases) 질환은 $3.62{\pm}0.13$, $2.60{\pm}0.23$, 전이(Metastasis, 13 cases)는 $1.70{\pm}0.09$, $2.60{\pm}0.23$, 초점성 결절성 과증식(Focal Nodular Hyperplasia, 6 cases)질환은 $2.12{\pm}0.28$, $5.86{\pm}0.28$, 농양(Abscess, 8 cases)은 $4.45{\pm}0.28$, $1.73{\pm}0.02$이었다. 또한, 각 질환에 대한 진단적 수행을 위해 ANOVA 검증을 하였다(p<0.05). 결론적으로, 두 조영제 효과로 2개 검사기법 모두가 간질환의 특성 및 검출과 그 질환에 따른 조영제 특성을 잘 보여주었다.

  • 저자들은 출생 직후부터 심한 혈변과 구토 증상을 보인 신생아의 위액과 변에서 아메바를 확인하여 자궁 내에서 감염된 것으로 판단되는 신생아 아메바증을 경험하고 치료하였기에 문헌 고찰과 함께 보고하는 바이다.

농흉의 임상적 고찰 (114례) (A Clinical Study for the Empyema (114 cases))

  • During the past 10 years 114 patients with empyema have been treated in hospital of Chonnam University. There were 87 males and 27 females ranging from 20 days to 70 years of age. The etiology was pyogenic pneumonia in 36.7%, tuberculosis in 22.7%, paragonimiasis in 8.8%, post-thoracotomy in 5.4%, post-trauma in 4.4%, lung abscess in 3.5%, malignancy in 3.5%, post-esophageal operation in 1.8%, and sterile in 10.5%. The over-all mortality rate was 2% [3 patients]. The majority of deaths occurred in patients with associated systemic illness [liver cirrhosis in I, and renal tuberculosis in I] and resistant tuberculosis for anti-tuberculosis drugs in one patient. Adequate drainage and obliteration of the pleural space continues to be the most important aspect of treatment and can frequently be achieved by closed chest tube thoracostomy in acute empyema especially in children. The more chronic thick-walled or loculated cavities require open drainage [open window therapy], decortication, thoracoplasty, sterilization, and myoplasty for closure of tracheobronchial fistula.

Phylogenetic Characteristics of Fasciola hepatica Isolated from a Korean Patient

  • Fascioliasis is a parasitic infection caused by liver flukes. Although several cases have been reported in Korea, phylogenetic analysis of isolates is lacking. In this study, a 66-year-old woman with right upper quadrant (RUQ) abdominal pain was diagnosed as fascioliasis involving abdominal muscle by imaging study. She received praziquantel treatment, but symptoms were not improved. Lateral movement of the abscess lesion was followed. Trematode parasite was surgically removed from the patient's rectus abdominis muscle. The fluke was identified as Fasciola hepatica based on sequence analysis of 18S rDNA. To determine the phylogenetic position of this Fasciola strain (named Korean Fasciola 1; KF1), the cox1 gene (273 bp) was analyzed and compared with the genes of 17 F. hepatica strains isolated from cows, sheep, goats, and humans from various countries. Phylogenetic analysis showed that KF1 was closely related with the isolates from China goat.

  • We report here a human case probably mixed-infected with Clonorchis sinensis and Fasciola sp. who was diagnosed by computed tomography (Cn scan, serological findings, and/or fecal examination. The patient was a 43-year-old Korean female and was admitted to Kyung Hee University Hospital with the complaints of fever and abdominal pain. On admission, marked eosinophilia was noted in her peripheral blood. CT scan showed specific lesions for clonorchiasis and fascioliasis in the liver, along with lesions suggestive of amebic abscess. Micro-ELISA revealed positive results for the 2 helminthic infections. Eggs of C. sinensis and trophozoites of Entamoeba histo/ytica were observed in the stool. Treatment with praziquantel followed by metronidazole and tinidazole reduced abnormalities in the liver and eosinophilia. This is the first case report of a possible co-infection with 2 kinds of liver flukes in the Republic of Korea.

강원도 사육 한우의 간질 감염실태 (Prevalence of Fascioliasis of Korean Native Cattle in Kangwon Province in Korea)

  • A field survey of fascioliasis of Korean native cattle raising and raised in specialized commercial breeding farms and local farms in Kangwon province using both intradermal test and sedimentation technique for feces was carried out from November to December, 1996. Fecal samples were taken from fascioliasis positive cattle by the intradermal test for the fecal examination. Liver tissues were randomly collected from an abattoir for histopathological examination of liver fluke infection in cattle. The results are as follows. 1. By the intradermal test for a total of 211 cattle raising in both Wonju and Wheongsung, Kangwon province, 60 heads(28.4%) showed positive reaction. Among 60 positive cattle, eggs of Fasciola hepatica were found from 51 heads(85.0%) by sedimentation technique. 2. According to the cattle raising areas, the positive rates by the intradermal test were 26.7%(20 out of 75 heads) in Wonju and 29.4%(40 out of 136 heads) in Wheongsung. 3. According to the age of cattle examined, the positive rates by the intradermal test in 1~3, 4~6 and 7~10 years old were 11.7%(7 out of 100 heads), 68.3%(41 out of 93 heads) and 20.0%(12 out of 18 heads), respectively. 4. The overall infection rates of fluke larvae from the slaughtered cattle at an abattoir in Wonju was 24.7%(37 out of 150 heads). In histopathology, liver lesions were observed such as inflammation with infiltration of eosinophils, polymorphonuclear cells, mononuclear cells and multinucleated giant cells, proliferation of connective tissues, calcification and abscess formation.

더러브렛 말의 Streptococcus equi subsp. zooepidemicus 감염에 의한 복막염 (Peritonitis associated with Streptococcus equi subsp. zooepidemicus in a thoroughbred horse)

  • Fibrinopurulent peritonitis and abdominal abscesses associated with Streptococcus (S.) equi subsp. zooepidemicus is reported in a 1-year-old female thoroughbred horse. The horse died after showing 5-day history of the cold, severe abdominal distention, abdominal breathing, hyperthermia, anorexia, and loss of intestinal sound. At necropsy, several liters of turbid yellowish green fluid were seen in the abdominal cavity. Yellowish creamy and fibrinous or bloody materials were presented on the serosa of various abdominal organs that were intensively adhered with each other. Spleen and mesenteric lymph node were remarkably enlarged. Affected lobes of the lung showed severe congestion, hemorrhage and doughy consistency. Histologically, the lung showed hemorrhagic pneumonia with diffuse congestion and edema. Severe diffuse fibrinopurulent peritonitis with Gram-positive bacterial cocci and adjacent fibrosis were showed in the serosa of various abdominal organs such as liver, spleen, stomach, and intestine. And multifocal abscess pouches were presented in the granulation tissue of abdominal viscera. S. equi subsp. zooepidemicus was isolated from the peritoneal swab, abdominal organs, and lung. Hematogenous dissemination of bacteria from hemorrhagic pneumonia is proposed as the route of infection in this case.

A Case of Ectopic Peritoneal Paragonimiasis Mimicking Diverticulitis or Abdominal Abscess

  • Paragonimiasis is a parasitic disease caused by Paragnonimus species. The primary site of infection is the lung, and extrapulmonary involvement is also reported. When infected with Paragonimus westermani, which is the dominant species in Korea, the central nervous system is frequently involved along with the liver, intestine, peritoneal cavity, retroperitoneum, and abdominal wall. Ectopic paragonimiasis raises diagnostic challenge since it is uncommon and may be confused with malignancy or other inflammatory diseases. Here, we report an ectopic paragonimiasis case initially presented with recurrent abdominal pain. The patient developed abdominal pain 3 times for the previous 3 years and the computed tomography (CT) of the abdomen revealed fluid collection with wall enhancement. Recurrent diverticulitis was initially suspected and part of the ascending colon was resected. However, the specimen showed intact colon wall without evidence of diverticulitis and multiple parasite eggs and granulomas were found instead. The size of about $70{\mu}m$, the presence of an operculum and relatively thick egg shell suggested eggs of Paragonimus species. With appropriate exposure history and a positive antibody test, the definitive diagnosis was made as peritoneal paragonimiasis.
